1.Glomus Tumor in the Hand.

Glomus tumor arises from the subcutaneous glomus body, which is an arteriovenous anastomosis involved in the regulation of cutaneous blood circulation and are widely distributed in the body, especially the hand. Twelve patients (male 3, female 9) with glomus tumors of the hand, who were operated from Jan. 1986 to Dec. 1994 were selected. The lesions were distributed in the distal phalanges of the thumb (6 cases), the index (3 cases), the ring finger (2 cases) and the little finger (1 case). All patients had the classic triad of symtoms: pain, tenderness and temperature sensitivity. Palpable mass (4 cases) and bluish purple discoloration in the skin (6 cases) were also presented. They had no trauma history and on the roentgenography, bone change detected in 2 cases. A simple excision with partial nail extraction was done in all cases. The symtoms were disappeared in all cases and no recurrence was found. For a complete cure, meticulous complete excision is the only recommended treatment.

2.A study to the workers exposed to organic solvents by neurobehavioral tests.

In order to evaluate the confounding factors of neurobehavioral tests and the neurobehavioral effects in the workers exposed to organic solvents, NCTB was carried out on 100 workers. 46 workers had never been exposed to neurotoxic substances, and the others were being exposed to the solvents, mainly toluene. Simple reaction time, digit symbol, Santa Ana dexterity test and persuit aiming were different with age in non exposure group. Simple reaction time was carried out well in males, and digit symbol and persuit aiming were in females. There was no difference at educational level when the subject was educated over 12 years. Santa Ana dexterity and Benton visual test differed according to exposure level to toluene, however simple reaction time didn't. The acute neurotoxic effect was not excluded in this study. But, NCTB could be used to evaluate and prevent neurobehavioral changes in workers exposed to neurotoxic solvents in Korea.

4.The Carpal Angle, Metacarpal Sign and Sesamoid Index of the Healthy Koreans

The authors estimated and analyzed the carpal angle, metacarpal sign and sesamoid index in the roentgenogram of both hands of healthy 180 male and 180 female Koreans. The results were as follows. 1. The mean value of carpal angle was 133.3°±5.8° in the right hand and 133.5°±5.1° in left hand. 2. The negative metacarpal sign were seen in 93.9 percent of normal subject. 3. The mean value of sesamoid index was 19.6±5.4 in male and 18.2±4.9 in female.

5.Telomerase Activity and Expression of MIB-1 and bcl-2 in Human Chorionic Villi from Early and Term Normal Pregnancy.

Telomerase is an enzyme that maintains telomeres and prevents telomere shortening, and may be linked with cellular proliferation or the aging process. The purpose was to examine telomerase activity in human chorionic villi from early and term normal pregnancies, and to analyze the correlation of telomerase activity (TA) with MIB-1 & bcl-2. A total of 37 placentae were obtained from 16 early and 21 term pregnancies. TA was assayed by telomeric repeat amplification protocol, and immunohistochemical staining was performed for MIB-1 & bcl-2 expression. TA & MIB-1 expression were strong in early placenta, but bcl-2 was highly expressed in term placentae. Thirteen (81.25%) of 16 early placentae showed TA, but only 2 (9.52%) of 21 term placentae expressed TA (p<0.01). MIB-1 was observed in nuclei of cytotrophoblast, and the expression rate was 16.09% in early placentae and 2.87% in term placentae (p<0.01). bcl-2 was observed only in the cytoplasm of syncytiotrophoblast. Term placenta demonstrated stronger expression of bcl-2 compared to early placentae (p<0.05). These findings suggest that TA, MIB-1 & bcl-2 expression are critically regulated over the course of gestation: cytotrophoblast, main cells of early chorionic villi, may be a common source of telomerase and proliferative activity. The TA showed good correlation with cellular proliferative activity. Syncytiotrophoblast, may be a main source of bcl-2 expression which is stronger in the term placentae.

6.Factors Affecting Supply of Mental Health Care Services in Korea.

OBJECTIVES: The purpose of the study is to investigate the factors affecting adequate provision of mental health care services to the psychiatric patients in mental hospitals and to suggest the introduction of a new reimbursement schedule for treatment of chronic Medicaid patients in order to avoid long-term hospitalization and to meet rising cost. METHOD: Five hundred and seventy-five psychiatric patients chosen by the stratified random sampling in six hospitals in Korea; three mental hospitals and three general hospitals, have been classified into three groups: two groups were selected from mental hospitals with the national health insurance system and Medicaid, while only one group from general hospitals with the national health insurance system. They were investigated about mental health care services provided for one week, which are transformed into financial cost by using Korean Resource Based Relative Value Scale points. The severity of their psychiatric symptoms were assessed with RAI-MH(Resident Assessment Instrument-Mental Health). RESULTS: When the Medicaid was compared to the health insurance in mental hospitals, sixty-seven percent was the relative ratio. Mental hospitals compared to general hospitals under health insurance, it was only 31%. The longer the hospitalization, the lesser services they received in all three groups. CONCLUSION: The significant factors affecting supply of mental health care services in Korean mental hospitals were duration of hospitalization, type of hospital, type of reimbursement. The introduction of Medicaid system, differentiated by duration of stay, may induce short-term hospitalization and prohibit deficient service care, especially in acute patients under Medicaid system.

7.Effect of Pantethine in the Treatment of Hyperlipidemia.

After pantethine in doses of 540 mg per day was administered for 8 weeks in 20 patients with hyperlipidemia, the serum levels of total cholesterol, triglyceride and HDL-cholesterol were compared with the pretreatment level. The serum level of total cholesterol decreased to 240+/-52 mg%[mean+/-standard deviation] after 4 weeks and 223+/-41 mg% after 8 weeks from the pretreatment level of 293+/-38 mg%(p<0.01). The serum triglyceride level also decreased to 254+/-109 mg% after 4 weeks and 239+/-114 mg% after 8 weeks from the pretreatment level of 375+/-126 mg%(p<0.01). 8.Clinical Correlation between the Serum Pepsinogen I/II Ratio and Gastric Cancer.

PURPOSE: In order to clarify the carcinogenesis mechanism from chronic atrophic gastritis toward gastric cancer, we measured the pepsinogen I and II and compared their ratio with several clinical findings. MATERIALS AND METHODS: We measured the preoperative serum pepsinogen I and II by using a radio-immunoassay and compared their ratio with several clinical findings, such as tumor size, mucinous vs non-mucinous tumor, cell differentiation, tumor location, depth of invasion, lymph-node status, Lauren's classification, and peritumoral atrophy in 103 consecutive patients with gastric adenocarcinomas who had received resections at Bundang CHA Hospital during the period from July 2003 to February 2005. RESULTS: There were significant differences in the serum pepsinogen I/II ratio between patients with mucinous vs non-mucinous tumors (n=4 vs 9 and mean pep I/II=1.29 vs. 2.99, P=0.0288), with tumor size more than and less than 10 cm2 (n=55 vs. 48 and mean pep I/II=2.64 vs. 3.24, P=0.0491), and with or without peritumoral atrophy (n=94 vs. 9 and mean pep I/II=2.83 vs. 3.89, P=0.0466). In patients with peritomoral atrophy, the pepsinogen I/II ratio was also lower in larger tumors (n=48 vs. 46 and mean pep I/II=2.44 vs. 3.23, P=0.0083). Well-differentiated carcinomas showed significantly lower serum pep I/II ratios than signet-ring-cell types. There was no correlation between serum pep I/II ratio and tumor location, depth of invasion, lymph-node status, or Lauren's classification. CONCLUSION: We proved the existence of a correlation between serum pepsinogen level and musosal atrophy, but these results are not sufficient for clinical application of serum pepsinogen level as a screening tool for gastric cancer.

9.Effects of Somatostatin Analogue Pretreatment on Ovarian Response to Ovulation Induction in Patients with Polycystic Ovarian Syndrome.

OBJECTIVE: To determine whether the somatostain analogue, octreotide, pretreatment before ovulation induction with human menopausal gonadotropin (hMG) affects ovarian response, and ovulation induction outcome in infertile patients with polycystic ovarian syndrome (PCOS) resistant to clomiphene citrate (CC) METHODS: From November 1998 to June 1999, 30 infertile patients with PCOS unresponsive to CC were randomly allocated either octreotide pretreatment (treatment group) (n = 15) or hMG alone (control group) (n = 15) groups. In the treatment group, 100 g of octreotide were administered daily for 7 days after progesterone injection for withdrawal bleeding, and then hMG was administered for ovulation induction. RESULTS: There were no differences in the total number of hMG ampules required and the duration of hMG administration between the two groups. The number of follicles of 10-14 mm diameter on the day of hCG injection was significantly less in the treatment group than that in the control group (4.3 +/- 2.5 vs. 9.6 +/- 4.4, p < 0.001). The serum estradiol (E2) level on the day of hCG injection was significantly lower in the treatment group, with 1579.2 +/- 421.0 pg/ml compared with 2120.3 +/- 512.7 pg/ml in the control group (p < 0.001). The hematocrit level on the day of hCG injection was also significantly lower in the treatment group than that in the control group (36.9 +/- 2.1% vs. 40.8 +/- 2.9%, p < 0.05). The incidence of severe ovarian hyperstimulation syndrome (OHSS) seemed to be lower in the treatment group, but the difference did not achieve significance (6.7% vs 20.0%). CONCLUSION: This study suggests that octreotide pretreatment before ovulation induction could improve hormonal milieu compared to hMG alone, and therefore may be effective in ovulation induction for patients with PCOS resistant to CC.

10.Prognostic Factors Affecting Postoperative Morbidity and Mortality in Destroyed Lung.

BACKGROUND: Postoperative morbidity and mortality in destroyed lung are relatively high. We tried to identify the prognostic factors affecting postoperative morbidity and mortality in destroyed lung through a retrospective study. MATERIAL AND METHOD: The retrospective study was undertaken in 112 patients who had undergone pneumonectomy or pleuropneumonectomy for destroyed lung at Severance Hospital from 1970 to 2000. We analyzed the correlation between postoperative morbidity and mortality and etiology, duration of disease, preoperative FEV1, presence or absence of preoperative empyema, operation timing, the side of operation, duration of operation, and operation type. RESULT: There were 55 men and 57 women, aged 20 to 81 years (mean 44 years). Etiologic diseases were tuberculosis in 86 patients(76.8%) including tuberculous empyema in 20 and tuberculous bronchiectasis in 4, pyogenic empyema in 12(10.7%), bronchiectasis in 12(10.7%), and lung abscess in 2(1.8%). Postoperative morbidity were 25%(n=28) and postoperative mortality was 6%(n=7). The presence of preoperative of empyema(p=0.016), pleuropneumonectomy(p=0.037) and preoperative FEV1 of less than 1.75 L(P=0.048) significantly increased the postoperative morbidity. If operation time was less than 300min, postoperative morbidity(p=0.002) and mortality(p=0.03) were significantly low. CONCLUSION: Postoperative morbidity and mortality in destroyed lung were acceptable. Postoperative morbidity and mortality were significantly low when operation time was less than 300 min. Preoperative existence of empyema, pleuropneumonectomy and preoperative FEV1 of less than 1.75 L significantly increased postoperative morbidity.
