1.Clinical Evaluation of the BTA TRAK assay and Comparison with Voided urine Cytology in Patients with Bladder Cancer.

PURPOSE: The purpose of this study was to evaluate the clinical performance of the BTA TRAK assay and to compare it with that of voided urine cytology in the detection of bladder cancer. MATERIALS AND METHODS: TRAK was evaluated two groups of patients. Bladder cancer group was comprised of patients wit histologically confirmed active bladder cancer. The second group of patients had a history of bladder cancers but were considered to have no evidence of disease on basis of cystoscopic evaluation of bladder and/or biopsy. Sensitivity was determined in urine samples from patients with histologically confirmed bladder cancer. Specificity was determined in samples from patients who had a history of bladder cancer but no current evidence of disease. RESULTS: BTA TRAK assay was positive in 21 of 24 samples from patients with diagnosed bladder cancer. The sensitivity of BTA TRAK assay (87.5%) was significantly higher than that of voided urine cytology (45.8%). According to grade, the sensitivity of BTA TRAK assay was significantly higher than that of voided urine cytology in urine samples from patients with grade 1 bladder cancer. The specificity of BTA TRAK assay was 80% in patients who had a history of bladder cancer but no current evidence of disease. CONCLUSIONS: The BTA TRAK assay is superior to voided urine cytology in the detection of bladder cancer. The difference of the sensitivity was statistically significant(p<0.05). The results of this study indicate that BTA TRAK assay is a useful adjunct to cystoscopy in the detection of bladder cancer and useful monitoring tool of bladder cancer.

5.Effect of Betamethasone on Pulmonary Surfactant Activity in Unilateral Pneumonectomized Rabbits.

Although it is well established that steroid is effective for treatment of neonatal respiratory distress syndrome (NRDS), the action mechanism of steroid on NRDS is not well known. Several authors have insisted that steroid increases secretion of pulmonary surfactant from type II pneumocyte, but others have insisted that steroid does not affect the secretory function of the type II pneumocyte. And some authors have suggested that steroid may cause compositional change of pulmonary surfactant phospholipid. From these aspects, it is desirable to confirm the effect of steroid on the secretory function of the type II pneumocyte. In order to know the effect of steroid on pulmonary surfactant activity, phospholipid phosphorus of lung lavage was measured and composition of pulmonary surfactant phospholipid of lung lavage was analyzed by thin layer chromatography (TLC) in control (c), pneumonectomized (PN), and pneumonectomized with betamethasone treated (PNS) rabbits. And lung weight and lung weight-body weight ratio were measured in each experimental group also. In PN group, right lung pneumonectomy was performed as PN group, and one day after the surgery, betamethasone was injected for four days intramusculary (4 mg/day) and rabbits were sacrificed. The experiment yielded following results. PNS group's lung weight was significantly (p<0.01) heavier than C group's, but in comparison with PN group's it showed no significant change. PNS group's L/B ratio was significantly (p<0.05) higher than C group's, but compared with PN group's it showed no significant change. The value of phospholipid phosphorus content of PNS group was significantly (p<0.01) higher than that of C group. Even if the value of phospholipid phosphorus content in PNS group was not significantly higher than that of PN group, it showed increasing tendency compared with that of PN group. And in an analysis of the thin layer chromatogram, quantity (µmol/gm of wet weight lung) of phosphatidylcholine in PNS group decreased significantly (p<0.05) compared with C and PN group. From these results, it may be suggested that though steroid inhibits cellular hyperplasia in the compensatory growing lung, it auguments the secretory function of type II pneumocyte and causes compositional change of pulmonary surfactant phospholipid.

6.Epizoological survey on infestation rate of helminths in Korean native cattle..

The authors made a study on the species and distribution of helminths in Korean native cattle. The feces and internal organs from 1,755 heads were examined in slaughter plants of Taejon district. The results are summarized as follows. Distribution and infestation rate of helminths among the collected samples are, Paramphistomum spp. 68.4 per cent, Fasciola spp. 48 per cent, Eurytrema spp. 23.5 per cent, Moniezia spp. 8 per cent, Dictyocaulus spp. 3.5 per cent, Trichostrongylus spp. 12.4 per cent, and other species 26.8 per cent, respectively. In 33.4 per cent of ths cases, single infestation was seen and in 31.5 per cent, 18.0 per cent, and 14.0 per cnet mixed infestation of double, triple, and quadruple or more species were seen, respectively. Geographical analysis of infested helminths in the native cattle indicates: Among the trematoda order, Paramphistomum spp. and Fasciola spp. showed high rate of infestation throughout all districts of the country, especially Cheju island with a significant higher rate. Central district (Taejon) was higher in number of native cattle infested with Fasciola spp. than any other district of the country investigated. The rate of infestation with cestoda also showed to be higher in Cheju island than any other district of main land, but the rate of infestation with other nematoda showed no geographical difference.

7.The Histological Change of the Fractured epiphyseal Plate of the Distal End of the Femur in the Growing Rabbit

The orthopaedic treatment regarding to the residual deformity secondary to epiphyseal plate injury is the most important thing. Author particularily interested in the microscopic finding upon the repair of damaged transepiphyseal fracture of the distal femoral end of growing rabbits. Experimentally this type of fracture is produced surgically and fixation is achieved by two Kirshner wires; one Kirshner wire is inserted obliquely through the metaphysis. A total of 10 growing rabbits were fed with normal diet, and each 5 rabbits were psacrified by 8 and 10 weeks after the surgical procedure respectively and histological change is examined. No rabbit had infection for those experiment. The results of observation were summarized as follows:1. The ruptured epiphyseal plate due to osteotomy through epiphyseal plate was found to be healed by new bone and fibrotic tissue, not by cartilage regeneration. 2. The epiphyseal plate in the small part of fracture fragments revealed as follows; a. The arrangement of cartilage zones revealed marked irregularity. b. The form and the character of cartilage cells revealed an abnormality such as the hyperophy or the atrophy of cells or both. c. The necrotic area revealed at the several places. d. The thickness of epiphyseal plate revealed more narrowing and irregularity. 3. The effect on the kirschner wires were seen as follows; a. There was no evidence of cartilage regeneration on the margin of the damaged epiphyseal plate. due to Kirschner-wire insertion b. The adjacent tissues around the both kirschner Wires revealed pretty heavy new bone formation.

8.Expression of p53 Protein and Proliferating Cell Nuclear Antigen in Epstein - Barr Virus-associated Gastric Adenocarcinoma.

PURPOSE: Recently, it has been reported that Epstein-Barr virus (EBV) is associated with some gastric cancers. But EBVs role in EBV-associated gastric carcinomas (EBVaGCs) has not been fully elucidated. This study was undertaken to evaluate the characteristics of EBVaGCs and to compare those with non-EBVaGCs. MATERIALS AND METHODS: EBV infection was studied using paraffin-embedded tissue blocks of 119 cases of gastric adenocarcinomas by in situ hybridization for EBV-encoded small RNAs (EBERs). In EBVaGCs and non-EBVaGCs, molecular characteristics were evaluated by immunohistochemical staining for latent membrane protein (LMP)-1, p53 protein, and proliferating cell nuclear antigen (PCNA). RESULTS: EBERs were detected in 12 cases (10.1%) of 119 gastric adenocarcinomas. LMP-1 was negative in all carcinomas tested, p53 protein was positive in 7 cases (58.3%) of 12 EBVaGCs and in 51 (47.7%) of 107 non-EBVaGCs, the difference between two groups being not significant. Mean PCNA index was 38.2+-26.1% in EBVaGCs and 22.8 +- 20.0% in non-EBVaGCs. The index was significantly higher in the former than in the latter. CONCLUSION: These results suggested that neoplastic progression in EBVaGCs was implicated with high expression of PCNA, but not consistently with overexpression of p53 protein or LMP-1.

9.Clinical Study of Intra-articular Fracture of Distal Radius

The treatment of intra-articular fractures of distal radius is difficult and has various problems. Maximum recovery of wrist function is dependent on accurate and stable reduction of the radial articular surface. We analysed one hundred and five cases of intra-articular fracture of distal radius followed for more than 6 months at Yonsei University from January 1980 to September 1988 and studied the result according to the type of treatment. The results of this study were as follows; 1. The incidence was high in female who aged over 6th decade, and in male who is active aged. 2. The main cause of this injury was fall down(34.3%). 3. In a group of closed reduction and cast immobilization, the functional result was satisfactory in 45.4%. 4. In a group of closed reduction and percutaneous pinning, the functional result was satisfactory in 50%. 5. In a group of open reduction and internal fixation, the functional result was satisfactory in 83. 3%. 6. In a group of external fixation, the functional result was satisfactory in 33.3%. 7. Overall result was mainly correlated with initial severity of the injury and anatomical reduction of the intra-articular surface.
