2.Prevalence and evaluation of asymptomatic microscopic hematuria in adults.

To knew the prevalence and nature of asymptomatic microscopic hematuria in a community. we evaluated 13,678 cases(M; 12.198 cases, F; 1,480 cases) that visited to the Department of Health Service. Kang Nam General Hospital from April to October, 1992 for annual physical examination. For the detection of hematuria urine dipstick test was done. and then urine microscopy was done in cases with positive dipstick test (262 cases). Hematuria was defined as 3 or more RBCs/HPF on microscopic examination of the urinary sediment(189 cases). Repeated urinalysis was done in cases with 3-4 RBCs/HPF (30 cases). And in cases with 5 or more RBCs/HPF on urine microscopy, excretory urography(105 cases) was primarily checked and in addition, renal ultrasonography. cystoscopy. RGP. urine culture. urine cytology. serum IgA, C3. C4 were performed for investigation ofThe results were as follows: I. Prevalence of asymptomatic microscopic hematuria was 1.38% and significantly increased with age (p<0.01). 2. Urologic abnormalities associated with hematuria were identified in 23 cases (21.9 %)-6 cases of renal stone. 4 cases of BPH. 4 cases of UPJ stricture, 3 cases or cystitis, 2 cases of ureteral stone. 1 case or ureteral stricture. nephroptosis. renal cyst. renal tuberculosis. S. In 82 cases, urologic abnormalities could not be found and then serum IgA. C3 C4 checked in 36 cases among these. IgA was increased in 21 cases (58.3%). From the above result. the causes of asymptomatic microscopic hematuria in adults could not be found in 78.1% and serious urologic diseases tumor and so on) were almost not found. We conclude that in order to obtain the detection of accurate prevalence and various urologic diseases. repeatedurine dipstick test and long-term followup in larger populations would be needed, and that screening test for the evaluation of hematuria would consist of performing excretory urography only in relative young adults and excretory urography with selective urine cytology after age 50 years.

3.Prostate specific antigen as a tumor marker for adenocarcinoma of the prostate.

Serum prostate specific antigen (PSA) and prostate acid phosphatase (PAP) were measured in 106 patients including 9 patients with prostate cancer, 63 patients with BPH. and 34 normal control group free of prostate disorders. The mean PSA value was 61.14+/-43.87 ng/ml for prostate cancer, 6.07+/-5.57 ng/ml for BPH and 1.42+/-0.98 ng/ml for normal control group, and the mean PAP value was 9.81+/-10.16 ng/ml for prostate cancer, 1.58+/-2.36ng/ml for BPH and 1.00+/-0.17 ng/ml for normal control group. The positive rates of PSA were 78% for prostate cancer, 22% for BPH and 0% for normal control group, and those of PAP were 67% for prostate cancer, 6% for BPH and 0% for normal control group. The sensitivity and specificity for PSA were 78%, respectively. We conclude that PSA may be a useful tool for detection and therapeutic monitoring of the prostatic cancer.

4.A case of true hermaphroditism with 46, XX/47, XXX karyotype.

5.The Refractive Errors and Axial Length in Amblyopia.

PURPOSE: It is the purpose of this study to investigate the distribution of the refractive error between amblyopic and nonamblyopic eye in the strabismic or anisometropic amblyopia and also evaluate the relationship among refractive error, axial length and the depth of amblyopia. METHODS: We prospectively reviewed the corrected best visual acuity, amount of deviation, cycloplegic refractive error and axial length in a total of 65 patients who are 29 strabismic and 36 anisometropic amblyopic patients from February 1999 to June 2001. RESULTS: Hyperopia was detected in as many as 20 of 29 strabismic (69%) and 22 of 36 anisometropic amblyopia (61%). In strabismic amblyopia the depth of amblyopia is correlated with the degree of hyperopia (r=0.535, p=0.022), not with axial length. In anisometropic amblyopia, the depth of amblyopia is correlated with the degree of hyperopia (r=0.689, p=0.000) and the degree of myopia (r=-0.691, p=0.013), and in especially hyperopic anisometropia, it is correlated with axial length (r=0.513, p=0.017). CONCLUSIONS: Hyperopia was the major refractive error in strabismic and anisometropic amblyopia and the depth of amblyopia is correlated with degree of hyperopia. In hyperopic anisometropic amblyopia, the greater the depth of amblyopia, the shorter the axial length.

The coronavirus disease 2019 (COVID-19) vaccines are categorized according to the manufacturing technique, including mRNA vaccines and adenovirus vector vaccines. According to previous studies, the reported efficacy of the COVID-19 vaccine is excellent regardless of the type of vaccine, and the majority of studies have shown similar results for safety. Most of the adverse reactions after vaccination were mild or moderate grade, and severe reactions were reported in a very small proportion. However, the adverse reactions that might occur after nationwide vaccinations can contribute to crowding of emergency departments, and this can further lead to significant obstacles to providing necessary treatment for life-threatening conditions. Therefore, as emergency physicians, we would like to present some concerns and suggestions to prevent these predictable problems.

8.A Case of Mania Associated with Hypericum Perforatum in a Patient with Major Depression.

Recently Korean Food and Drug Administration approved Hypericum perforatum preparations (St. John's wort) as an over-the-counter drug. The authors reported a case of Hypericum perforatum induced mania in a patient with major depressive disorder and reviewed. A seventy-five years old female patient was diagnosed as major depression 5 years ago, and had been admitted in the psychiatric ward twice and had received regular outpatient treatment. Since October 1998, she took paroxetine 20 mg, and bromazepam 1.5 mg constantly. But all of a sudden, she stopped taking the drugs on June 20th 2000, and took Hypericum perforatum 300 mg three times daily. Ten days after, she became abruptly talkative, irritable, hyperactive, and euphoric. She was diagnosed as Hypericum perforatum-induced mania. Manic symptoms came down after several days by discontinuation of taking Hypericum perforatum. Since then she has been got a continuous treatment with paroxetine 20 mg, and she is in full remission state now. Hypericum perforatum is one of herb extracts, which is known to have the antidepressant effect like traditional and newer antidepressants. The side effects of Hypericum perforatum are gastrointestinal disturbance, allergy reaction, fatigue, photosensitivity. And more serious side effect is drug-drug interaction due to it's inducing effect of cytochrome P450 and P-glycoprotein. It is possible to switch from depression to manic episode as was in this case. Although it was approved as an over-the-counter drug in Korea, the use of Hypericum perforatum need to be thoroughly examined to prevent misused if not fully understood.

9.A Case of Primary Ureteral Adenocarcinoma Associated with Long-standing Ureteral Stone.

Primary ureteral carcinoma is a rare disease comprising 1% of all urinary tract tumors. Primary adenocarcinoma of the ureter is extremely rare. We report a case of primary ureteral adenocarcinoma associated with long-standing ureteral stone in 40-year-old man, who was treated by nephroureterectomy with bladder cupping, with a brief review of the literatures.

10.Toxoplasma antibodies by indirect latex agglutination tests in National Seoul Mental Hospital patients.

A total of 573 patients hospitalized in National Seoul Mental Hospital and 76 of healthy persons as control were examined by indirect latex agglutination test in order to evaluate Toxoplasma antibody titers in mental patients. Throughout this survey, 1:32 or more titers of diluted sera were regarded as positive. The 573 samples of test sera showed negative in 386 cases (67.4 percent), 1:2 in 93 cases (16.2 percent), 1:4 in 57 cases (9.9 percent), 1:8 in 14 cases (2.4 percent), 1:16 in 12 cases (2.1 percent), 1:32 in 5 cases (0.9 percent), 1:64 in 1 case (0.2 percent), 1:128 in 3 cases (0.5 percent) and 1:256 in 2 cases (0.3 percent) respectively. Among total 573 mental patients, 11 cases (19 percent) showed positive, and they were 9 cases (1.8 percent) of schizophrenia and 2 cases (7.4 percent) of manic depression. One case (1.3 percent) out of 76 control sera showed positive result.
