1.Heterotopic Glial Nodule in the Lung of an Anencephaly Patient : An autopsy case.

The heterotopic and tissues may be divided into two categories: those that are found in the head and neck region, and those that arise elsewhere. The latter type is rare and most cases are found in the lungs of patients with neural tube defect, particularly anencephaly. Our report descrives anencephalic male infant with heterotopic glial nodules in both lungs. The largest nodule is 2x1.5x1 cm, locates in the lower lobe of the left lung, and has a round gray-white cut surface with cystic spaces. Microscopically, the nodules consist of irregularly arranged astrocytes and glial fibers, in which are embedded gland-like or cystically dilated bronchioles. The astrocytes and glial fibers are strongly positive for glial fibrillary acidic protein and show astrocytic filaments on electron microscopy. This will be an additional case supporting the amniotic fluid aspiration/implantation theory of pathogenetic mechanism of distal heerotopic glial tissue.

3.Free Digital Sensory Nerve Graft in Degloving Injuries of the Finger

Degloving injuries of the fingers receive digital sensory nerve damage as well as digital artery damage together with degloving of the finger skin at the time of injury. Although the finger is well covered with skin and the joint motion has completely recovered after the injury, if there is no sensation in the finger tip problems remain. From October, 1982 to December, 1985 Dong San Medical Center, Keimyung University did free digital sensory nerve grafts on 11 cases using the sural nerve. Follow up studies which lasted from 10 to 39 months showed good results. Therefore this free digital nerve graft operation is considered to be a favourable mothod in obtaining sensation for digital degloving injuries.

6.A Clinical Analysis on Operative Treatment of Displaced Proximal Humeral Fracture

Most proximal humeral fractures respond satisfactory to simple conservative treatment. But operative treatment is recornmended in the cases that poor results are anticipated by prolonged immobilization, or because of the severe displacement and comminution. After Neer. in 1970, emphasized the need for operative treatment in displaced proximal humeral fractures, many papers reported better results with the operative management. Twenty five cases of displaced fractures and fracture dislocations of the proximal humerus treated at the department of orthopaedic surgery, Keimyung University with open reduction and internal fixation were analyzed clinically and radiologically. The following results were obtained. 1. The prevalent age distribution was the second snd third decades(average 42.6 years) and ratio of male and female was 2.1: 1. 2. In 10 of the 25 cases, wire loop was used for internal fixation, and the result was satisfactory in 8 cases. However, the disadvantage was difficulty in wire removal after bony union because the loop was buried in the callus or cortex. 3. Over-all results were excellent or satisfactory in 64% of 25 cases. There were 4 excellent and 1 satisfsctory in 5 cases of type Il fracture, 7 excellent, 2 satisfactory and 3 unsatisfactory in 12 cases of type III fracture, 1 excellent and 4 unsatisfactory in 5 cases of type 1V fracture, and 1 excellent, 1 unsatisfactory and 1 failure in 3 cases of fracture dislocation. 4. In 6 patients with four part lesions treated with open reduction and internal fixation and followed up for an average of 23 months(from 18 months to 3 years and 2 months), only one case of fracture dislocation revealed avascular necrosis with resorption of the head.

8.Nephrotic syndrome under 2 years of age.

There had been total 20 patients with early onset(4 months~2 years) primary nephrotic syndrome in the Department of Pediatrics, Seoul National University Children's Hospital, during the period from March 1987 to February 1993. We analysed clinical courses, response to treatment, pathological findings and prognosis of the patients And the results were as follows; 1) The initial responders to steroid treatment were 10(50%), of whom 3 became late nonresponders. Of the 10 initial nonresponders, 8 revealed continuing nonresponsiveness. 2) Incidence of hypertension was significantly higher in the intial nonresponders than in the initial responders. 3) Renal biopsies were performed in 9 initial nonresponders and 2 late nonresponders. And the results were 6 with minimal change lesion, 4 with focal segmental glomerulosclerosis and 1 with mesangiocapillary glomerulonephritis. 4) Six continuing or late nonresponders received methylprednisolone pulse therapy or cyclophosphamide. And partial remission was induced in 4 of them. 5) The overall 3 year maintenance rate of normal renal function was 81%(100% in the initial responders and 64% in the initial nonresponders). In conclusion, the initial steroid responsiveness and prognosis were poorer in patients with early onset nephrotic syndrome than in patients with usual childhood nephrotic syndrome.

9.Trichofolliculoma in Korean Patients: A report of six cases.

Trichogenic tumors are a very rare group of primary cutaneous neoplasms and a few tumors have been recognized. Trichofolliculoma is a benign, papular, organoid adnexal tumor of the skin and presents as a solitary skin-colored nodule. We report six cases of trichofolliculoma occurred in Korean patients consisting of three male and three female patients. The duration of disease was variable from 3 months to 5 years. The site of tumor was limited to the face and included eyelid in two cases, nose in two cases, cheek and posterior aspect of ear in each one case. The size of the mass varied from 4 to 8 mm in diameter. Microscopically, all the cases showed large central cystic hair follicle with numerous secondary follicular buds, producing a radiating pattern. The stroma was fibrotic and well demarcated from the adjacent dermis.

10.Morphological Changes of Retinal Vessel Under Tissue Culture.

PURPOSE: This study was performed to investigate the growth pattern of the retinal vessel including retinal vascular endothelial cell and pericyte. METHODS: The sensory retina was detached from the eyecup obtained from donor's eye. The retinal vessel which was separated from the sensory retina was cultured in tissue culture media for 2, 3 and 4 weeks separately and examined by transmission electron microscopy. RESULTS: On the second week of tissue culture, both the retinal vascular endothelial cells and pericyte were intact in morphology in nuclear and cytoplasmic pattern. Both cells were partially detached from the surrounding basement membrane. On the third week of tissue culture, the cytoplasm of the retinal vascular endothelial cells and pericyte were degenerated, whereas the vascular endothelial cell were intact in nuclear and cytoplasmic profiles which were still partially surrounded by the basement membrane. CONCLUSIONS: These findings suggest that the survivability of the retinal vascular endothelial cells and pericyte may be limited for 2 and 3 weeks, separatedly under the ordinary culture medium.
