3.Characteristics of Gastric Cancer in Korea - with an Emphasis on the Increase of the Early Gastric Cancer (EGC).

Gastric cancer is the most common malignancy and the second leading cause of cancer-related death in Korea. Age-adjusted annual incidence of gastric cancer per 100,000 persons is 62.8 for male and 25.7 for female. The proportion of early gastric cancer among surgically treated gastric cancer patients was 28.6% in 1995, 32.8% in 1999, and 47.4% in 2004. Screening upper endoscopy is quite commonly performed in Korea, and the proportion of early gastric cancer in the screened population is almost 75%. Gastric adenoma is considered to be a very important precancerous lesion. Gastric adenomas are actively treated in Korea, and at least 1/3 of the gastric adenomas with high grade dysplasia show cancerous focus when endoscopically resected. The prevalence of Helicobacter pylori is decreasing in Korean population, which may lead to the decreased incidence of gastric cancer in the near future. Paradigms of the diagnosis and treatment of gastric cancer are rapidly changing in Korea. Efforts to establish robust scientific data for new approaches are strongly required. The proportion of early gastric cancer among all gastric cancers has been and will be more increased in Korea.

4.Usefulness of 99mTc-MDP Bone Scintigraphy for Assessing Vascular Ingrowth on Hydroxyapatite Ocular Implant.

PURPOSE: This study was to evaluate the clinical usefulness of 99mTc-MDP bone scintigraphy for assessing vascular ingrowth into the ocular implants after enucleation or evisceration. MATERIALS AND METHODS: Twenty-four patients (M:F=7:17, mean age: 36 years), who buried a coralline hydroxyapatite after uncomplicated enucleation or evisceration surgery were studied. Dynamic and static scintigraphy on the orbit fossa were obtained after injection of 740 MBq 99mTc-MDP to evaluate the status of vascularization. The study was performed from the 3 to 33 weeks after surgery. According to the visual analysis, activity greater than nasal bridge was graded as 4, equal to the nasal bridge as 3, less than nasal bridge but greater than normal orbit as 2, greater than normal orbit but less than grade 2 as 1. Uptake ratio was also calculated by measuring the implants activity (H) and contralateral orbit activity (N). Grading score and uptake ratio were compared with clinical outcome of vascularization. Additionally, we also analyzed the vascularization status as time lapse between primary surgery and scintigraphic study and surgical methods. RESULTS: Twenty-one patients who had bone scintigraphy at 11 weeks after surgery showed increased uptake above grade 2 and greater H/N ratio than 1.56. Of these, 19 patients who had drilling surgery for permanent peg application showed adequate bleeding during the procedure. The activity grade and uptake ratio were inversely correlated with vascular ingrowth. Higher than grade 2 or greater than 1.56 in H/N ratio seemed to be an indicator for better prognosis. Accomplishment of vascularization was not affected by the surgical way such as enucleation or evisceration. CONCLUSION: 99mTc-MDP bone scintigraphy can be a useful method to evaluate the vascularized status of implants. Adequate time for 99mTc-MDP bone scintigraphy may be 11-20 weeks after enucleation or evisceration.

6.Dupuytren's contracture: Report of 12 cases

We experienced 12 cases of 8 patients Duputren's contracture, which is a rare disease entity and characterized by proliferative fibroplasia of the subcutaneous palmar aponeurosis and contracture of the palmar fascia and result in the deformity of the hand and fingers. Among 12 cases, we operated 11 cases and the result was excellent in 2 cases and good in 6 cases, and there were no evidence of recurrence over the follow-up period, average 4.1 years.

7.Electrodiagnostic Study of Ulnar Nerve Entrapment at Elbow

The elbow region is the most common site at which the ulnar nerve is prone to a localized neuropathy. Diagnosis of ulnar nerve entrapment at elbow is made on the basis of history, lacal neurologic finding, nerve conduction studies and electromyography. Electrophysiologic test is simple, relatively quick, and accurate method for the diagnosis of peripheral nerve lesion. Therefore, electrodisgnosis makes it possible to identify and localize the early stage of entrapment lesion. Definitive diagnosis of ulnar nerve entrapment at elbow requires electromyographic demonstration of decreased ulnar nerve conduction velocity across the elbow and depressed sensory nerve action potentials. The purpose of this study were to analyze the electrophysiologic findings of patients with an established ulnar nerve entrapment at elbow compared to normal eonduction velocity of our laboratory. And we evaluate the correlation of clinical symptom with the electromyographic findings. Twenty five subjects were studied, 22 men and 3 women, aged from 5 to 62 years. The results were as follows . 1. The mean motor conduction velocity of ulnar nerve across elbow were 32.85±10.21 m/sec. The longer duration of symptom were, the slower conduction veloctity across elbow were. 2. Among the 25 patients, motor conduction velocity of ulnar nerve at forearm segment decreased in 15 patients(60%). As the motor conduction velocity of the ulnar nerve across elbow became slower, thst of forearm segment were slower. 3. In 19 patients (76%) among the 25 patients, sensory evoked potentials revealed prolonged distal latency or were not evoked. 4. On needle electrodiagnostic study, positive sharp wave or fibrillation potentials could be found in abductor digiti minimi first dorsal interosseus and/or flexor carpi ulnaris muscles in 22 patients(88%).

10.A Case of Allergic Contact Dermatitis to Resorcin in Skin Marking Solution Occurred During Radiation Therapy.

Resorcin is widely being used in the fields of the therapeutics, cosmetics, industry, but allergic contact dermatitis is an infrequent adverse reaction. We have experienced a case of allergic contact dermatitis to resorcin. A 59-year-old male with Kaposi's sarcoma on the right foot developed linear erythematous vesicular eruption along the marking areas 4 days after application of Castellanis paint, used as a skin marking solution for radiation therapy. He showed patch test positive reaction to Castellani's paint as well as its individual components, resorcin and phenol.
