1.Subtalar Dislocation: A Case Report

Subtalar dislocation of the foot is one in which there is simultaneous dislocation of the talonavicular joint and talocalcaneal joint while the tibiotalar relationship is unchanged. It was described first by Judey and Defourest in 1811. It incidence was 1% to 1.3% of all dislocations and 15% of injuries of the talus. We have experienced one case of a medial subtalar dislocation without fracture. In our case which was followed for 14 months, the head of the talus was palpable on the dorsum of the foot and the heel was displaced medially in relation to the leg. Radiographically, on the lateral view, the head of the talus was shown superior to the navicular and on the A-P view, the normal talonavicular relationship was disturbed with the calcaneus being displaced medially. The closed reduction was carried out successfully.

Multiple osteochondromatosis is a hereditary disorder affecting the endochondral skeleton during period of growth. We have studied a family which have multiple osteochondromatosis clinically and radiologically. In this family, eight of ten members could traceable, have the lesions of multiple osteochondromatosis. The most common site of the lesions was in femur, and the next common site was in tibia, especially, near the knee joint.

6.Two Cases of Rhabdomyosarcoma

Rhabdomyosarcoma is highly malignant soft tissue tumor which arise within skeletal muscles of the body. It makes up about 10 to 15 percent of sarcomas of the soft somatic tissues. Approximately two third of them originate in the muscles of the extremties, usually the legs. We have experienced two cases of rhabdomyosarcoma, the one arised within left forearm skeletal muscles and the other, within right calf muscles. The former was pleomorphic type, the latter was embryonal type in histological study.

7.Early Detection of Viability of the Femoral Head by 99mTc-MDP Bone Scan in Femoral Neck Fracture

After femur neck fracture, many techniques have been known for early detection of the viability of the femoral head which is very important to decide method of treatment. At the present time, bone scan with 99mTc-MDP is the best available radioistope for use in scanning, because it is very sensitive, non-invasive and simple, minimal radiation dose, easily reproducible. Bone scanning was carried out in 18 cases of femur neck fractures from July, 1981 to October, 1982 in Kyung Hee University Hospital. The results were as follows: l. In twelve cases in which radiological confirmation of viability of the femoral head was difficult to make, we could confirm the presence or absence of vascularity of femoral head using bone scan. 2. Six cases which were considered to have vascular impairment to the femoral head radiologically, were identified to have vascular impairment to the femoral head by bone scan and these were evidenced by operative findings and by histology of biopsy specimen. 3. In fresh fractures, we identified the vascular impairment by the bone scan minimally 5 days after injury. 4. In old fractures, we could confirm the viability of the femoral heads by bone scan. 5. Bone scan is considered to be the excellent technique for early detection of the vascular impairment to the femoral head after femur neck fractures.

8.Mechanism of Low K+-induced Depolarization in Mammalian Cardiac Muscle.

The membrane permeability to potassium at a resting state is greater than to any other ions and the maintenance of resting membrane potential is largely dependent on K+ concentration of outside medium (Hodgkin and Horowicz 1959), i.e. an increase of K+ concentration of medium induces a depolarization, vice versa. However, on the contrary to this prediction, in some mammalian heart muscle a reduction of external K+ concentration induces a depolarization of membrane potential rather than a hyperpolarization (Vassalle 1965). In this study it was aimed to elucidate the possible mechanism of spontaneous depolarization induced by low external K+ in canine Purkinje fibers. The membrane potential was constantly recorded while components of cations in the bathing medium were replaced one by one by equimolar sucrose until the low K+ induced depolarization was blocked. The results are summarized as follows; The membrane potential of canine Purkinje fibers was spontaneously depolarized by low external K+, and the magnitude of depolarization was not affected by verapamil TEA, and a partial replacement of external Na+ and Ca2+ with choline chloride. But the membrane potential was hyperpolarized only when the all external cations were substitued with sucrose; and this hyperpolarization was disappeared again by substitution of sucrose with choline chloride. From these results, it may be concluded that the depolarization induced by low external K+ in canine Purkinje fibers is due to the nonspecific increase of membrane permeability to external cations and/or combinations with decreased K+ conductance.

Majority of .hepatocellular carcinoma is evolved from a well differentiated cancerous condition such as hypetptastic lesions eg; adenomatous hyperplasia in cirrhotic liver or de no vo carcinogenesis and prolifenation along with dedifferentiation. Adenomatous hyperplasia is may be seen in severe acute hepatic injury, like svhmassive hepatic necrosis, or in chronic liver diseases, particularly liver cirrhosis and it has recently attracted much interest from both clinicians and pathologists because it is regarded as a precursor lesion of hepatocObdar carcinoma. Hepatic. denomatous hyperplasia resembling focal nodular hyperplasia might have developed from localized vascular changes associated with chronic liver disease, pre-existing arterial malformation and early stage of angiogenesis in hepatocarcinogenesis. .fie present a patient who developed hepatocellular carcinoma after hepatic artery ligation.
