5.Treatment of Intertrochanteric Fracture of Femur: A Randomized Prospective Comparative Analysis of the Internal Fixation of Gamma Nail and Compression Hip Screw

We made a randomized prospective comparison of the compression hip screw and the Gamma nail for the internal fixation of 60 intertrochanteric fractures to compare perioperative details and to analyze radiographic and clinical results in elderly patients. The operation time for Gamma nail group was shorter than for CHS group and also the intraoperative blood loss was lower in the Gamma nail group. And the Gamma nail enabled earlier ambulation. We found no significant difference of the union time and the sliding length of lag screw between two groups. The decrease of neck shaft angle in the Gamma nail group was significantly smaller than the CHS group. There was no significant mechanical complication in the Gamma nail group such as a fracture of the femoral shaft or fixation failure. We obtained satisfactory results on treatment of elderly patients with intertrochanteric fracture of femur using Gamma nail. So we consider that in the treatment of intertrochanteric fractures of geriatric patients, the Gamma nail would be more efficient than the CHS.

7.Improving Arterial Oxygenation with Continuous Positive Airway Pressure to the Nonventilated Lung during One Lung Ventilation.

The authors studied the effect of insufflation of oxygen at 5 cmH2O into the nonventilated lung on the arterial oxygenation in the thoracotomy patients(n=14) who showed PaO2 below 100 mmHg or oxygen saturation below 95% during one-lung ventilation. When applying CPAP 5 cmH2O to the collapsed lung, we inflated the collapsed lung with reservoir bag that is one of the equipment of self made CPAP device. Hemodynamic parameter(mean arterial pressure and heart rate), arterial blood gas analysis and calculated shunt fraction was collected 10, 20, 30min after applying CPAP. PaO2 was significantly increased from 76+/-20mmHg to 180+/-66 (mean+/-SD) and shunt fraction was significantly decreased from 43.5+/-5.6% to 31.5+/-4.9% at CPAP 10 min. There was no significant difference in the PaO2 and shunt fraction between CPAP 10, 20 and 30 min. Heart rate was also significantly decreased with CPAP compared to pre-CPAP value. Other parameter(mean arterial pressure, PaCO2) was not changed with CPAP. The authors conclude that CPAP 5 cmH2O to the nonventilated lung is effective to improve oxygenation when hypoxemia occurs during one-lung ventilation.

8.A Case of Nonischemic Central Retinal Vein Occlusion Developed in the course of Ulcerative Colits.

Ulcerative colitis is a chronic, inflammatory disease of colon which is rare in Korea. In the course of ulcerative colitis numerous extraintestinal complication may develop on the joints, skin, liver, kidney, vessel, oral cavity and eyes. The most common ocular lesions complicated in patients with ulcerative colitis are episcleritis and iritis. And the cases of retinal vascular disease in associated with ulcerative colitis are extremely rare. We experienced nonischemic central retinal vein occlusion complicated in a patient with ulcerative colitis, who is 32-year-old male and complained decreased visual acuity and metamorphosis.
