Congenital dislocation of the hip has been known as one of the most common congenital anomalies in the orthopedic field since the time of Hippocrates (460-357 B.C.). The occurence of C.D.H. in twin babies is extremely rare thus few if any papers have been reported. During the period of January 1971 through June 1976 the authors have treated a total of 49 cases as inpatients. Two of these were twin babies. In both cases the hip was surgically reduced and followed by open reduction and femoral derotation osteotomy. Clinically and radiologically the results were good during the duration of follow-up seven months after operations.

Background: Fluvastatin is the first entirely synthetic 3-hydroxy-3-methylglutaryl-coenzyme A(HMG-CoA) reductase inhibitor. Clinical data indicate that this agent exhibits the proven efficacy of its class and also has some theoretical advantages in safety for long-term use because of its unique pharmacololgic property consistent with hepatoselectivity(i.e., low systemic exposure). This study is to evaluate efficacy and safety of fluvastatin in hypercholesterolemic patients in Korea. Methods: An open clinical trial with fluvastatin was conducted in 31 subjects who continued to have high blood cholesterol levels of 6.21 mmol/L(240 mg/dl) or greater after 1 month of lipid-lowering diet plus single blind placebo period. Fluvastatin was administered for 8 weeks with the initial dose of 20 mg per day and if serum cholesterol levels did not fall below 5.20 mmol/L(200 mg/dl) after 4 weeks the dose was increased to 40 mg per day for the second 4 weeks. Plexiform schwannoma is a rare benign tumor arising from the peripheral nerve sheath and characterized by a multinodular and plexiform growth pattern. This tumor usually arises sporadically. In rare cases, plexiform schwannomas have been associated with neurofibromatosis type 2. Plexiform schwannoma should be differentiated from plexiform neurofibroma, because the latter is pathognomonic tumor of neurofibromatosis type 1 and has a potential of malignant transformation. We report a case of multiple plexiform schwannomas associated with bilateral acoustic neuromas and meningioma.

Teramethylammonium(TMA) in one of the synthetic compounds of nicotine that act at ganglionic site. The major action of TMA consists of initial stimulation followed by a more persistent depression of all autonomic ganglia by binding to a cholinergic receptor. It is well believed that the level of membrane potential in arterial smooth muscle is an important regulator of tension development. Depolarization and hyperpolarization by only few millivolts results in significant changes in tension. In general, the agents of vascular smooth muscle induce vascular relaxaion. The present study was undertaken to elucidate the effect of TMA on vascular contractility in the isolated rabbit thoracic aorta with or without endothelial cell, and mechanisms involved in the change of vascular contractility by TMA. The results obtained are summarized as follows ; 1) In the presence of endothelial cell, TMA induced a relaxtion of the aorta precontracted with norepinephrine but induced a contraction in the aorta without endothelial cells, indicating that in the rabbit aorta, relaxations produced by TMA were the endothelium-dependent. 2) The addition of inhibitor such as methylene blue, hemoglobin, hydroquinone and p-bromophenacyl bromide during the TMA-induced relaxation reversed the contractile tension to a level similar to or higher than that before the addition of TMA in rabbit thoracic aorta.This relaxation effect of TMA suggest that the TMA-inducdd relaxation in rabbit aorta is due to the release of endotheline derived relaxing factor(EDRF). 3) Relaxation induced by TMA was antagonized by atropine and thus the TMA does seem to act on the muscarinic receptors. 4) TMA reduced the norepinephrine-induced Ca++ influx into rabbit smooth muscle membrane. From the above results, it may be concluded that TMA-induced vacular relaxation in rabbit aorta is due to the release of EDRF. Mechanism involved in the relaxation induced by TMA may be the stimulation of soluble guanylate cyclase and increased tissue cGMP concentrations.

OBJECTIVES: The purposes of this clinical study were 1) to assess 5-year survival rates in 195 patients with locally invasive cervical cancer(stage Ia-IIb) treated by surgery, neoadjuvant chemotherapy+surgery and postoperative radiation in selected cases, 2) to evaluate the prognostic factors affecting the survival rate, and 3) to get helpful information for the better treatment. METHODS: A retrospective analysis was conducted of 195 patients. They were diagnosed and operated on during the period of Jan. 1988 - Dec. 1993 in Dept. of OB/Gyn, Pusan Paik Hospital, Inje Medical School. The 5-year survival rates were estimated using the Kaplan-Meier method, and prognostic factors were assessed using Wilcoxon test. RESULTS: The 5-year survival rates for FIGO stages Ia, Ib, IIa, and IIb were 100, 86.1, 76.9, and 81.1%, respectively. Factors that affect the 5-year survival rates were clinical stage(P=0.0001), cell type(small cell vs other, P=0.0001), depth of invasion(< or = 5 mm vs >5 mm, P=0.0013), tumor size(< or = 3 cm vs >3cm, P=0.0035), and lymph node metastasis(0 vs 1 vs more than 2, P=0.0001). There was no difference in 5-year survival rates between without neoadjuvant chemotherapy group and with neoadjuvant chemotherapy group which had poor prognostic factors. The predicted recurrence rate of the postoperative radiation group showed a lower 5-year survival rate than the no postoperative radiation group(P=0.0001). CONCLUSIONS: We conclude that the factors affecting the prognosis were FIGO stage, cell type, depth of invasion, tumor size, and lymph node metastasis, and speculated that the survival rates could be improved by using neoadjuvant chemotherapy for more advanced locally invasive cases properly.

Steroid cell tumor of ovary, first described as lipid cell tumor, is rare lesions composed entirely of cells resembling typical steroid hormone - secreting cells, that is lutein cells, Leydig cells, and adrenal cortical cells. Steroid cell tumors oftcn secret androgen and manifest themselves with symptoms of virilization. Other presentations include abdominal swelling or pain, menstrual dysfunction, postmenopausal bleeding, or rarely ascites. We experienced a case of right ovarian steroid cell tumor, not otherwise specified(NOS), manifested hirsuitism and amenorrhea in 49 - year - old patient. The tumor was about 5 cm in size, and associated with huge ascites (l3,000 ml), both pleural effusion, and elevated serum CA 125. We present a case of Meigs syndrome associated with benign ovarian steroid cell tumor with a brief review of the literature.
