1.Expression and significance of NF-κB and VEGF in the prostatic cancer

Objective To explore the relationship between the expression of NF-κB and VEGF and the biological behaviors of prostatic carcinoma.Methods Immunohistochemical method was used to examine the expression of NF-κB and VEGF in 40 cases of benign prostatic hyperplasia (BPH) and 53 cases of prostatic carcinoma.Results The positive rates of NF-κB and VEGF in prostatic carcinoma and BPH were 64.2% and 69.8%,12.5% and 37.5% respectively.There are more cases with positive NF-κB and VEGF expression in prostatic carcinoma group than in the BPH group( x2 =24.976,9.655,P < 0.01 ).The expression of VEGF and NF-κB in prostatic carcinomas increased significantly with the decrease of tumor differentiation and advance of the TNM stages(x2 =15.936,18.459;4.316,14.205,P <0.01 or P <0.05);The expression of VEGF in prostatic carcinoma was positively correlated with NF-κB ( r =0.297,P =0.027 ).Conclusion The expression of VEGF and NF-κB in prostatic carcinoma are highly correlated with each other and may be suggestive to understand the biological behavior of prostatic carcinoma.

4.Vertex technique for treatment of atlantoaxial instability:Clinical outcomes and implant features

This paper is aimed to explore the correlation of clinical outcomes with material science features of implant in the treatment of atlantoaxial instability by the Vertex technique.A total of 26 cases with atlantoaxial instability and dislocation were enrolled from the Affiliated Hospital of Luzhou Medical College between January 2004 and January 2006.They all underwent the internal fixation via posterior cervical pathway and atlantoaxial fusion in Vertex system that was produced by American Sofamor Danek Company.The intraoperative complications and postoperative neurological recovery of each case were documented.Dynamic radiograph and digital tomography were used to evaluate the extent of the atlantoaxial vertebrae stability.①The result of X-ray showed that bony fusion was successful in 22 patients during 3-month follow-ups,3 patients during 6-month follow-ups,and 1 case after implantation following bony resorption.②There was no deterioration of spinal cord injury and the JOA scores of neurofunction increased from 3-17 points(mean 6.8 points) to 5-17 points(mean 14.8 points) ,with the rate of improvement as 83%.③All 26 patients were normal in the movements of expansion and flexion,but the rotation was partly limited.There were 18 cases rotate in 120?,6 cases in 100? and 1 case in 60?.Vertex system is effective in dealing with internal fixation via posterior cervical pathway.Due to the connection with stick by multiaxial screw,Vertex system presents superiority in clinic,such as secure fixation,random length and convenient connection.Vertex system internal fixation is effective in dealing with atlantoaxial instability and dislocation.
5.Experimental Study on Effect of Homoharringtonine on SP,IL-1? and TNF-? in Rats of Adjuvant Arthritis

[Objective]To observe the effect of Homoharringtonine on Adjuvant Arthritis Rats.[Methods]Observe the effect of Homoharringtonine on Adjuvant Arthritis Rats’ SP,IL-1? and TNF-?;we take the Adjuvant Arthritis Rats as model and Tripterygium wilfordii polycoside was the positive group.[Results]The Homoharringtonine and Tripterygium wilfordii polycoside group’s level of TNF-?,IL-1? in the serum and synovial membrane decreased instructively,and their dose of SP in the plasma and synovial membrane was suppressed obviously too.[Conclusion]Homoharringtonine has the effect of anti-inflammation,it may treat the Adjuvant Arthritis Rats through reducing the dose of TNF-?,IL-1? in the serum and synovial membrane,inhibiting the secretion and releasing SP in the plasma and synovial membrane.
6.Anatomic observation of the atlanto-axial joint in functional positions by CT three-dimensional imaging

BACKGROUND: Clinical diagnosis criteria for rotary subluxation of atlanto-axial joint have not been identified. Many clinical diagnoses are suspicious. Therefore, investigating a new diagnostic method and diagnostic evidence is beneficial to enhancing diagnosis accuracy, and is a study hotspot in the current field.OBJECTIVE: This study aimed to observe the relationship between anatomy of normal human atlanto-axial joint and functional positions, and to provide anatomical basis for the diagnosis of atlanto-axial rotary subluxation.DESIGN: Single sample self-control study.SETTING: Department of Imageology, Zhongshan Hospital Affiliated to Xiamen University.PARTICIPANTS: This study was carried out in the CT Room, Department of Imageology, Zhongshan Hospital Affiliated to Xiamen University between June 2003 and November 2004. Fifty-three volunteers, 27 males and 26 females, with an average age of 31.2 years (range 4.5-75 years), without cervical trauma or history of cervical diseases, were randomly recruited in to this study. Written informed consents were obtained from each subject or their guardians.METHODS:CT helical scanning and three-dimensional reconstruction imaging of atlanto-axial joint in the neutral position were performed in all the volunteers and were supplemented in the right and left rotary position in 21 subjects on a helical CT scanner (somatom plus 4, Siemens, Germany), SUN magic view 1000 workstation and three-dimensional reconstruction software.MAIN OUTCOME MEASURES: ① Lateral atlanto-axial articular facet contraposition. ② Lateral atlanto dental space③ Atlanto dental interval. ④ The largest extent of articular facets dislocation in the right and left rotary positions. ⑤ Head's rotary maximal angle in the right and left rotary positions.RESULTS: ① Three-dimensional images with atlanto-axial joint in the functional position of 53 patients showed that the articular surfaces of lateral atlanto-axial joint had basic contraposition, the width of atlanto-dental interval ranged from 1.0 to 3.3 mm, and the lateral atlanto-dental distance devirated in 24 patients (45.28%).②Three-dimensional images with atlanto-axial joint in the rotary position of 21 patients showed that the width of atlanto dental interval had no change, the articular facets of lateral atlanto-axial joint had rotary displacement with the width ranging from 5.76 to 8.18 mm, and anastomotic area of articular facet was decreased by 1/2 to 2/3. The condyles had deflected to the rotary direction in the patients with symmetrical lateral atlanto dental space, and for 15 patients with asymmetrical lateral atlanto dental space, the condyle deflected slightly or had no changes.CONCLUSION: CT-three dimensional imaging can clearly show the anatomical structure of atlanto-axial joint in functional position and the changing rule of atlanto-axial joint movement, providing theoretical evidence for the clinical diagnosis and the treatment of atlanto-axial subluxation.
7.A Comparison of SVR and tb-ABR Response Threshold and PTA Threshold in Normal -hearing Young People

Objective To study the relationship among the slow vertex response (SVR) ,tone burst ABR threshold and pure tone thresholds in young people with normal hearing ,and to discuss the values of SVR and tb -ABR assessing hearing thresholds .Methods A total of 30 normal-hearing young people aged 20 to 24 were indud-ed in this study .The thresholds of SVR ,tb -ABR and threshold of PTA were tested at each verbal frequency (0 .5 ,1 ,2 ,4 kHz) .The differences and the relationship among SVR with PTA ,tb-ABR ,and PTA were statistical-ly studied .Results The differences between SVR and tb -ABR and PTA at various frequencies were statistically significant (P<0 .05) .The minimum difference between SVR and PTA was (3 .59 ± 7 .32) dB at 0 .5 kHz .The differences of 1 ,2 ,and 4 kHz were 6 .92 ± 5 .07 ,8 .33 ± 6 .71 ,and 9 .83 ± 9 .41 dB ,respectively .The minimum differences between tb -ABR and PTA was 14 .58 ± 7 .24 dB at 4 kHz ,and the differences of 0 .5 ,1 ,and 2 kHz were 29 .42 ± 6 .83 ,23 .25 ± 7 .47 ,and 16 .50 ± 6 .43 dB ,respectively .SVR and PTA had a good correlation at 1 and 2 kHz (r=0 .51 ,P<0 .01 and r=0 .44 ,P<0 .05 ,respectively) .tb-ABR and PTA had a good correlation at 4 kHz (r=0 .53 ,P<0 .01) .Conclusion In normal hearing young population ,SVR and tb-ABR can estimate PTA well , and the former is closer to PTA threshold .

The paper reviews the background and current situation of medical disputes and puts forward the concept of the reaction chain of medical disputes in view of their high occurrence rate and the difficulty in handling them. It discusses the framework, chief content, basic principles and major features of the reaction chain of medical disputes and argues that as a commonly used tool for handling medical disputes, the reaction chain renders it possible to check from the angje of the interests of the patients whether each party involved in the dispute has fulfilled its duties. Intended to improve the professional ethics of the physicians and the quality of medical care and calling on the patients to rebuild their confidence in the physicians, the reaction chain indicates that it is necessary to scientifically analyze the entire process and various influencing factors of medical disputes and bring into play the advantages of integration so as to successfully resolve medical disputes.

10.Aegisy recyclable filter formed in the treatment of deep venous thrombosis of lower extremity

Objective To investigate the therapeutic efficacy of Aegisy retrievable filter in inferior vena cava interventional treatment for acute deep venous thrombosis of lower extremity. Methods Acute or sub acute unilateral lower extremity deep vein thrombosis ( deep vein thrombosis, DVT) patients in our hospital were selected, and Aegisy filter was put into the inferior vena cava through the femoral, and thrombolytic therapy was carried out in the next day. Limb conditions was observation after 10 to 15 days, confirmed by vascular ultrasound and angiography of lower limb vein after no fresh or free thrombus. The filter was removed with protector, anticoagulant therapy after review the venography of lower limb postoperative. Results All Aegisy filter were put into patients, and there were no obvious forward jumping and drawing back occurred during the release of the filters, and all Aegisy filter had a better position. Placement of a filter everage time was (16. 8 ± 1. 3) days, 89 cases were successfully recovery filter. The effct of Aegisy filter on the patients with lower extremity deep venous thrombosis after treatment is obvious effect, no pain, and no symptom of pulmonary embolism. Conclusion Aegisy retrievable inferior vena cava filter has many advantages in clinical use. The filter can be placed to the scheduled position accurately,and it is very powerful in captu-ring the thrombus,it can be hold in the inferior vona cava for long time before it is planed to be retrieved,and its retrieval success is very high. Moreover,with high technieal successful rate the implantation of Aegisy retrievable inferior vena cava filter is very safe and effective for the interventional treatment of acute deep venous thrombosis of lower extremity.
