1.Establishment of a rabbit model of facial skin lifting and application of the absorbable soft tissue plastic belt

Objective To establish a rabbit model of facial skin lifting and to evaluate the skin lifting effects of absorbable soft tissue plastic belt, and to provide experimental basis for clinical application of this plastic belt.Methods 36 Japanese white rabbits were randomly divided into model control group (n=6) and plastic belt lifting group (n=30). The model control group received only facial skin resection surgery, while the plastic belt lifting group had facial skin resection and plastic belt implantation.The changes of rabbit general status, skin lifting effects, performance of plastic belt and pathological changes of subcutaneous tissue after implantation were determined during the 4 weeks after surgery. Results The rabbit skin wound was healed within 3-7 days after surgery without infection.Meanwhile, the plastic belts did not show shifting or rupture, and only whitening around the perforations was observed in the two groups.Compared with the model control group, the MA and MB distances in the plastic belt lifting group were significantly lower ( P<0.01 ) , while the biological tension of plastic belts in the facial skin lifting rabbit models was significantly increased with the extension of time after implantation (P<0.01), and the biological tension was 18.62 N at 4 weeks after transplantation. In addition, the tensile intensities of perforations and plate in the two groups were significantly reduced at 4 weeks postoperatively (P<0.01), the tensile intensity of perforations and plate in the two groups were maintained at 35.07 N and 53.31 N, respectively, and the perforations/plate tensile intensity ratio of the two groups remained unchanged during 4 weeks after transplantation ( P >0.05 ) .Moreover, the molecular weight ( Mw ) , peak molecular weight ( Mp ) , Z molecular weight (Mz) and viscosity were gradually decreased along with the time passing after implantation (P<0.01), and its dispersion Mz/Mw ratio was also gradually decreased from 2 weeks after implantation (P<0.01), and no obvious pathological changes were found after subcutaneous implantation of the plastic belts.Conclusions We have successfully established a facial skin lifting rabbit model, and the plastic belt can obviously lift the facial skin fascia system and keep intact more than 4 weeks in the body.Therefore, this plastic belt can be applied for anti-wrinkle facial soft tissue lifting against the skin damage caused by aging.

3.Preliminary curative effect of interspinous blocking injection guiding by X-ray for the diagnosis and treatment of lumbar Baastrup's disease.

OBJECTIVETo evaluate the preliminary curative effect of interspinous injections for the diagnosis and treatment of back pain caused by lumbar kissing spine (Baastrup's disease) under fluoroscopically guiding.
METHODSFrom November 2011 to March 2013,17 patients with back pain caused by Baastrup's disease were treated with fluoroscopically-guided interspinous injections, including 7 males and 10 females with an average age of 49.6 years old ranging from 40 to 71 years old; the duration of the disease ranged from 2 to 5 years with a mean of 3.7 years. The visual analogue scale (VAS) and the lumbar segments range of motion (ROM) was analyzed at pre-operation, 2 days, 3 months and final followed-up after operation, the effects were evaluated with modified Macnab standard.
RESULTSAll patients were follow-up from 6 to 10 months with an average of 7.6 months. The pre-operative VAS was 6.41 +/- 0.94, the postoperative VAS at different time points improved significantly comparing with pre-operation,and the differences were statistically significant (P < 0.01). There was no significant difference in VAS at different time points after operation (P > 0.05). The ROM of operated segment and adjacent segment was (4.88 +/- 0.86) degrees and (6.82 +/- 0.73) degrees respectively at pre-operation. The postoperative operated segment ROM at different time points improved significantly comparing with pre-operation, and the differences were statistically significant (P < 0.05). Compared with pre-operation, there was no significant difference in adjacent segment ROM at different time points after operation (P > 0.05). According to modified Macnab, the result was excellent in 6 cases, good in 7 cases, fair in 3 cases and poor in 1 case.
CONCLUSIONFluoroscopically-guided interspinous injections is an effective method for the diagnosis and treatment of Baastrup's disease. The method has advantages of simple operation, minimal invasion and safety, satisfactory short-term and medium-term therapeutic effect; it can also effectively lessen the pain of lumbar and back.

4.Preliminary application of diffusion-weighted imaging with 3.0 T magnetic resonance scanner for nasopharyngeal carcinoma.

OBJECTIVETo evaluate the clinical value of diffusion-weighted imaging (DWI) with 3.0 T magnetic resonance scanner for nasopharyngeal carcinoma (NPC) and to explore the appropriate b value during this procedure.
METHODSTotally 50 patients with histopathologically confirmed NPC and 20 volunteers underwent DWI using single-shot echo-planar imaging technique were enrolled in this study. Four groups of b value (600, 800, 1000 and 1200) were applied. The apparent diffusion coefficients (ADC) between the primary tumor of NPC and the metastatic lymph nodes were compared in each group. ADC between the metastatic lymph nodes and benign nodes were also compared.
RESULTSAlthough the mean ADC were not significantly different between NPC and those of metastatic cervical lymph nodes in each group, the mean ADC of the metastatic lymph nodes were significantly lower than those of benign nodes (P<0.05). When b value was 800, both satisfying images and accurate measurements were acquired.
CONCLUSIONDWI is helpful in the diagnosis of primary lesions and their lymph node metastases of NPC.

5.Relationship between the expression of HBV DNA, HBV cccDNA in human ovary tissues and the HBV intrauterine infection

Objective To investigate the relationship between hepatitis B virus (HBV)deoxyribonucleic acid (DNA) and HBV covalently closed circular DNA (cccDNA) in the ovary and HBV intrauterine infection.Methods HBV DNA and HBV cccDNA were assayed in the ovaries of 33 pregnant women who were positive for HBV DNA,tested by Fluorescent quantitative polymerase chain reaction (FQ-PCR).The level of HBV mark (HBVM) and the content of HBV DNA in peripheral blood of infants were measured by chemoluminescence and FQ-PCR methods respectively.Results The overall positive rate for both HBV DNA and HBV cccDNA in ovarian samples was 51.52% (17/33).The rate on intrauterine infection among infants was 12.12% (4/33) and all the 4 infected infants were delivered from mothers with normal hepatic function.When HBV DNA and HBV cccDNA were both positive,the rate of intrauterine infection in infants was significantly higher than those who were with both negative results (P<0.05).Levels of HBV cccDNA and the rate of positive samples were significantly higher in mothers with infants who appeared to have had intrauterine infection than those did not (P<0.01 and <0.05,respectively).Conclusion HBV infection could be discovered in the human ovary and might be transmitted to the filial generation via ovum.

Syndrome of resistance to thyroid hormone(RTH)is a rare hereditary thyroid disease with various clinical manifestations and laboratory findings. RTH could be misdiagnosed and mistreated, resulting in aggravation of the disease. We reviewed the medical records of a patient with RTH over the past six years. In addition, we provided a summary of latest progress for RTH to help the clinicians to improve the understanding of the disease.
7.False positive rate and false negative rate of layered mammography in diagnosis of mammary gland diseases

Objective:To analyze the false positive rate and false negative rate of layered mammography in diagnosis of mammary gland diseases.Methods:120 patients with malignant breast diseases and 120 patients with benign breast diseases treated in Huadong Hospital Affiliated to Fudan University from February 2015 to October 2016 were selected.All patients were examined by both traditional mammography and layered mammography.Taking surgical pathology as the gold standard,the diagnostic value of the two methods on patients with different ages,different lesion sizes,and different gland texture were compared.Results:The false positive rate and false negative rate of patients with mammary gland diseases in layered mammography group aged less than 45 years,45-60 years,and more than 60 years were lower than those of the conventional group,and the differences were statistically significant (P<0.05).The false positive rate and false negative rate of patients with mammary gland diseases in layered mammography group with lesion sizes less than 1 cm,1 2 cm,and more than 2 cm were significantly lower than those of the conventional group,and the differences were statistically significant (P<0.05).As for the patients with different gland texture,the false positive rate and false negative rate of the two groups were not statistically different.Conclusions:The false positive rate and false negative rate of layered mammography in diagnosis of breast diseases are lower than the conventional mammography,so it has higher clinical diagnostic value.
8.Pleiotropic effect of tatC mutation on metabolism of pathogen Yersinia enterocolitica.

OBJECTIVETo analyze the impact of depletion of the twin arginine translocation (TAT) system on virulence and physiology of Yersinia enterocolitica for a better understanding of its pathogenicity.
METHODSWe constructed a DeltatatC::SpR mutant of Yersinia enterocolitica by P1 phage mediated transduction using Escherichia coli K-12 DeltatatC::SpR strain as a donor.
RESULTSA P1-mediated genetic material transfer was found between the two species of enterobacteria, indicating a great potential of acquisition of antibiotic resistance in emergency of a new threatening pathogen by genetic material exchanges. Periplasmic trimethylamine N-oxidase reductase activity was detected in the wild type Y. enterocolitica strain and translocation of this enzyme was completely abolished by the DeltatatC::SpR mutation. In addition, the DeltatatC::SpR mutation showed a pleiotropic effect on the metabolism of Y. enterocolitica. However, the tat mutation did not seem to affect the mobility and virulence of Y. enterocolitica under the conditions used.
CONCLUSIONUnlike other pathogenic bacteria studied, the TAT system of Y. enterocolitica might play an important role in the pathogenic process, which is distinct from other pathogens, such as Pseudomonas aeruginosa and enterohemorrhagic E. coli O157:H7.

9.Experiment study of alendronate in the prevention and treatment of aseptic loosening of prosthesis.

OBJECTIVETo investigate the effect of alendronate on the prevention and treatment of aseptic loosening of prosthesis.
METHODSA rat model of particle-induced osteolysis was used. Thirty-xis SD rats were divided into three groups: negative control group, positive control group and experiment group. Alendronate was administered by ig in experiment group. Positive control group and experiment group received intro-articular injections of ultrahigh molecular weight polyethylene (UHMWPE) particles at 4, 6, 8, 10 weeks postoperatively. Negative control group was received injection with mixture solution of mouse serum and PBS only. All animals were sacrificed at 12 weeks after operation for histologic examination. In vitro human peripheral blood mononuclear (PBMC)were separated and cultured and divided into five groups as group A: PBMC group, group B: PBMC and particles,group C:PBMC and particles with 10(-4) mol/L alendronate, group D:PBMC and particles with 10(-5) mol/L alendronate, group E: PBMC and particles with 10(-6) mol/L alendronate. The production of IL-1beta, IL-6, TNF-alpha in each group were tested.
RESULTSAlendronate could prevent particle-induced osteolysis. The production of IL-1beta, IL-6, TNF-alpha was inhibited when alendronate was used.
CONCLUSIONAlendronate can inhibit bone absorptive factors expression induced by wear particles and may be used in the prevention and treatment of aseptic loosening of prosthesis.

10.A novel mutation in the myocilin gene identified in a Chinese primary open angle glaucoma family.

OBJECTIVETo determine the possible myocilin molecular genetic defect underlying POAG in China and to identify the pathogenic mutation causing the disease.
METHODSThe majority of 1 branch of a large Chinese POAG family were personally examined by two senior ophthalmologists. The diagnoses were made by both doctors according to the signs of elevated intraocular pressure, glaucomatous optic neuropathy and glaucomatous visual field defect. All coding sequences of the myocilin gene plus the flanking sites were amplified by polymerase chain reaction (PCR) using genomic DNA from all examined family members followed by sequencing of the PCR products. One hundred normal control subjects were screened by single strand confirmational polymorphism analysis for the mutation.
RESULTSThis Chinese pedigree exhibited autosomal dominant mode of inheritance. The onset age ranged from 26 to 59 years. A novel disease-causing missense mutation T455K in the third exon of the myocilin gene was identified in all affected family members, all glaucoma suspects and 4 individuals who have not shown apparently signs of glaucoma. None of the subjects without the mutation had glaucoma. Affected individuals with the T455K mutation showed variable onset between 26 and 59 years of age. Filtering surgery was performed on all of 7 affected family members. The T455K mutation in myocilin gene was not found in the normal controls. A previously reported polymorphism IVS2+35(A to G)was detected in 4 individuals.
CONCLUSIONThe novel myocilin sequence alteration T455K that is highly associated with the development of glaucoma and locates in a very conserved residue is proven to be a disease-causing missence mutation. All affected individuals and all POAG suspects in this family are identified to have this mutation. The mutation in this family is associated with a phenotype characterized by mix-onset open angle glaucoma and associated with a high penetrance. It is important for the mutation screening and periodical checkups of presymptomatic individuals belonging to the family of a POAG patient with T455K mutation.
