1.Correlation analysis of serum adipokines and cytokines levels in obese patients with colon cancer

Objective To explore the correlation between serum adipokines and cytokines levels in obese patients with colon cancer.Methods 53 patients with obese colon cancer treated in our hospital from January 2011 to June 2013 were selected as the observation group and at the same time 51 cases of non-obese colon cancer were selected as the control group.TNF-α,IGF-1,VEGF and adipokines levels were measured in the two groups.Results The IGF-1 level in the stage A-B and the stage C-D of the ob-servation group was significantly increased compared with the same stages of the control group (P <0.05).The TNF-α,IGF-1 and VEGF levels in the stage C-D of the observation group were significantly higher than those in the stage A-B (P <0.05 ),the TNF-αand VEGF levels in the stage C-D of the control group were significantly increased compared with the stage A-B (P <0.05).The visfatin and leptin levels in the stage A-B of the observation group were significantly increased compared with the same stages of the control group (P <0.05),the visfatin,resistin and leptin levels in the stage stage C-D of the observation group were significantly increased compared with the same stages of the control group (P <0.05).Visfatin was positively correlated with IGF-1 and VEGF (P <0.05),resistin was positively correlated with IGF-1 (P <0.05 ),leptin was positively correlated with TNF-α, IGF-1 and VEGF (P <0.05).Conclusion Obvious lipid metabolism and cellular cytokines disorders exist in obese colon cancer, their disorder level could promote the progression of colon cancer.
2.Effects of cefodizime on the T-lymphocyte subsets of peripheral blood in mice with immunological liver injury

Objective To study the regulatory effect of cefodizime on the T-lymphocyte subsets of peripheral blood in mice with immunological liver injury.Methods The mouse model of immunological liver injury was induced by Bacillus Calmette Guerin and Lipoposaccharide.The study was conducted by using completely random design.The mice with immunological liver injury were divided into thymosin group,cefodizime high-,medium-,low-dose groups,ceftriaxone group and the normal saline group.The six groups were continuously administered agents respectively for 7 days,and T-lymphocyte subsets of peripheral blood in mice were determined and contrasted with those of the normal mice treated with normal saline on the 7th day.Flow cytomytry was used to determine the effects of cefodizime on T-lymphocyte subsets of peripheral blood in mice by using immuno-fluorescence technique.Results The immunological liver injury mice were deficient because their CD3+(%),CD4+(%),CD8+(%) and the ratio of CD4+CD8+ were lower than those of the normal mice.Cefodizime effectively increased CD3+(%),CD4+(%) and the ratio of CD4+CD8+ of the mice with immunological liver injury.Conclusion Cefodizime effectively improves the immune function of the host by regulating the balance between CD4+ cell and CD8+ cell.
3.Pre-mRNA splicing and retinitis pigmentosa

Retinitis pigmentosa (RP) is a large group of common hereditary eye diseases with highlyheterogeneous genetic background. Over forty genes with diverse functionalities are associated with RP and they include a set of ubiquitously expressed genes. These include five genes involved in the precursor messenger RNA( premRNA) splicing. Recent progress in disease gene identification for RP has established the involvement of pre-mRNA splicing as one important mechanism in the disease etiology and has shed light on the splicing process itself, a fundamental biological process. To this date, studies in this field have been focused on two major issues. First, how do the mutations of the adRP associated splicing factors (adRP-SF) affect the splicing function? Second, how do the mutations in these ubiquitously expressed genes lead to specific retinopathy? The two topics fit with the two continuous important steps of the disease pathogenesis. Recently, researchers have made a dramatic progress in the first topic. The identification of the SNRNP200 gene,the fifth adRP-SF and its relevant functional study has shown significance to the progress in the study of RP. Numerous investigations are also being carried out in addressing the second issue.Generation of a variety of models led to a better description of the pathological process of the disease. However, in respect to the key pathogenic mechanism,researchers are still puzzled with a number of confusing questions. In this commentary,the results from the latest investigations were summarized, and in particular,the difficulties in studying the molecular mechanism by which the pre-mRNA splicing deficiency causes RP were detailed.
4.Percutaneous vertebroplasty for fresh osteoporotic vertebral compression fractures

Objective To evaluate the clinical efficacy of percutaneous vertebroplasty (PVP) for patients with fresh osteoporotic vertebral compression fracture (OVCF).Methods From February 2010 to November 2011,103 patients with fresh OVCF were assigned to receive either PVP or conservative treatment according to their choice or the choice of surgeons.There were no significant differences between the 2 groups regarding general clinical data (P > 0.05).The pain was scored at pre-treatment,1,4,12,24 and 48 weeks post-treatment in all the patients.The vertebral re-fractures were recorded in both groups at follow-ups.The 2 groups were compared in terms of postoperative pain relief and risk of re-fracture.Results The overall pain relief at 1,4 and 12 post-treatment in the PVP group was significantly greater than in the conservative treatment group (P < 0.05).The pain relief at night at 1,4 and 12 post-treatment was significantly greater in the PVP group than in the conservative treatment group (P < 0.05).At one year post-treatment,7 vertebral re-fractures were observed in the PVP group (18.4%,7/38) and 12 ones in the conservative treatment group (20.3%,12/59),with no significant difference between the 2 groups in the risk of re-fracture (Hazard ratio =0.909,95% CI:O.36 ~ 2.29,P =0.841).Conclusion Compared with conservative treatment,PVP may significantly reduce the pain for patients with fresh OVCF but does not increase the risk of vertebral re-fracture.
5.Anesthesia methods during artificial femoral head replacement in elderly patients

Artificial femoral head replacement is commonly performed in elderly patients with femoral neck fracture. However, the patients are always complicated by other diseases and their major organ functions are low, which increases risks of anesthesia and replacement. The anesthesia methods for artificial femoral head replacement include epidural anesthesia, combined spinal-epidural anesthesia, and general anesthesia. Respiratory system and nervous system complications, bleeding and transfusion complications, deep venous thrombosis and lung embolism, and bone cement-associate complications are common complications in anesthesia for femoral head replacement. Therefore, appropriate anesthesia pattern, rigid monitoring, and proper treatment can reduce anesthesia risks.
6.Biomechanical analysis of the failure of the interlocking nail of femal

Objective To evaluate the reason of failure of the interlocking nail.Methods From1998-2001, 72 patients with femur fractures were treated with the interlocking nail.Results 61 cases were followed-up,there were 5 breakages in the locking nail. The failure occurred after operation from 4~9 months.And 3 cases were at the middle of the nail.2 cases were at the first screw of the distal of the fracture.Conclusion The reason of breakage concluded open operation,too much separation of the periosteum,too early full weight bearing and unfit function exercise.
7.Clinical observation of sufentanil and midazolam assisted epidural anesthesia for cholecystectomy

Objective To investigate the effect of sufentanil and midazolam assisted epidural anesthesia for cholecystectomy on sedative effects,visceral pulling reaction and the functions of respiratory and circulatory system.MethodsFourty patients underwent cholecystectomy received informed consent, were randomly allocated to two groups:group A and group B( n =20 each). When the level of epidural block is appropriate,auxiliary medicine infusion was given 5min before the skin incision. In group A,first injection was sufentanil 0. lμg · kg-1 and midazolam 0. 02mg · kg-1 in 1-2 min, then continuous infusion of sufentanil and midazolam was given at the rate of 0. 2μg ·kg-1 · h-1 and 0. 04μg · kg-1 · h-1 respectively,and the infusion was stopped at the time of wound closure. Group B received slow intravenous injection of pethidine 1mg · kg-1 and droperidol 0. 5mg · kg-1. We recorded the differences of SBP、DBP、SpO2 、HR、Ransay score and visceral pulling reaction classification. Results SBP、DBP、HR after anesthesis,were decreased in different degree in the above groups,but there was no statistically significant difference( P >0. 05). According to Ramsay score,in group A there were 3 cases in grade Ⅱ、8 cases in Ⅲ and 9 cases in Ⅳ ,while in group B there were respectively 4 cases in Ⅱ、13 cases in Ⅲ and 3 cases inⅣ. By comparision,the differences had statistical significance ( u = 3.75, P < 0. 05 ); Group A according to visceral pulling reaction classification (0- Ⅲ ), were 9 cases in grade 0,9 cases in Ⅰ , 1 cases in Ⅱ and 1 cases in Ⅲ, while in group B there were respectively 4,5,7 and 4 cases. By comparision, the differences had statistical significance(u = 4. 01,P < 0. 01 ). Conclusion In the cholecystectomy,sufentanil and midazolam assisted epidural anesthesia,could improve the level of sedation ,prevent visceral pulling reaction ,and had a minor interference to respiratory and circulatory function.
8.The role of cyclophilin A in the development of abdominal aortic aneurysm

Abdominal aortic aneurysms (AAA) is one kind of common vascular disease. Inflammation and matrix degradation in the vasculature are crucial for AAA formation, key mechanism of which include vascular smooth muscle cell(VSMC) senescence, oxidative stress, increased local production of proinflammatory cytokines, and increased activities of matrix metalloproteinase (MMPs). Cyp A, the member of cyclophilins,has a variety of physiological functions. It is highly expressed in VSMC, and plays an important role in the development of AAA, making it a new therapeutic target for such disease.
9.Design on the performance evaluation framework of district and county health system in Shanghai

Shanghai district and county health system performance evaluation conceptual frameworkwas proposed based on health services system performance assessment frameworks raised by some countries and international organizations,and combined with Shanghai district and county health system characteristics.This framework contains two first grade indices and seven second grade indices:In the long term goals,'health','satisfaction' and 'disease risk protection' are the county's longterm effects of health system performance assessment dimensions; in the medium-term goals,the'accessibility','cost',' efficiency' and 'quality' are the county' s medium-term effects of health system performance assessment dimensions.This study aimed to evaluate Shanghai county health system's overall performance.Further study is needed in constructing its third grade indices.
10.Enhancing nerve regeneration by tubes filled with valproic acid on peripheral nerve defect rats

Objective To observe enhancing effect of nerve regeneration on peripheral nerve defect models bridged by silicone tube idled with valproic acid (VPA). Methods In present research we demon-strate the effect of VPA on peripheral nerve regeneration and recovery of motor function following sciatic nerve transaction in rats. An 8-mm sciatic nerve deficit was created in a rat mode land bridged by a 1-cm silicone tube.Then, 10 lad of 8% VPA were perfused into the silicone chamber in the VPA group. The same volume of normal saline was delivered in the control group. Results Each animal was observed sciatic nerve function index (SFI) at 2-week intervals and studied electrophysiology at 4-week intervals for 12 weeks. Histological and morphometrical analyses were performed at the end of the experiment, 12 weeks after operation. Using the digital image-analysis system, thickness of the myelin sheath was measured, and total numbers of regenerated axons were counted. There was a significant difference in SFI, electrophysiological index (motor-nerve conduct velocity, MCV), and morphometrical results (regenerated axon number and thickness of myelin sheath) in nerve regeneration between the VPA group and controls (P < 0.05). Conclusion The results demonstrated that VPA is able to enhance sciatic nerve regeneration in rats, suggesting the potential clinical application of VPA for the treatment of peripheral nerve injury in humans.
