1.The diagnostic value of procalcitonin combined with C-reactive protein in bacterial pneumonia in elderly patients

Objective To investigate the diagnostic value of procalcitonin(PCT) combined with C‐reactive protein (CRP) in the pulmonary infection in elderly patients .Methods 145 cases of inpatients in Affiliated Hospital of Luzhou Medical College from Mar .2013 to Mar .2014 were selected and divided into three groups [non‐infection group (43 cases) ,local infection group (68 ca‐ses) ,severe infection group (34 cases)] according to whether they have pulmonary bacterial infection and the severity of infection . Serum PCT level ,blood CRP level and white blood cell (WBC) counts were detected ,and the receiver operating characteristic curve (ROC curve) were evaluated .The diagnostic values of these inflammatory markers were compared .Results The serum PCT level in the serve infection group was significantly higher than those in the local infection group and non‐infection group (P<0 .05) ,area under the ROC curve (AUC) of PCT was 0 .960 ± 0 .033 ,CRP was 0 .717 ± 0 .084 ,WBC was 0 .582 ± 0 .094 ,respectively .When the threshold of PCT was 0 .2 μg/mL ,the diagnostic sensitivity was 95 .1% ,specificity was 88 .4% ,Youden index was 0 .835 ;when the threshold of CRP was 67 .72 mg/L ,the diagnostic sensitivity was 57 .8% ,specificity was 83 .7% ,Youden index was 0 .415 .The AUC of WBC had no diagnostic value .Conclusion PCT could be an ideal marker in diagnosis of bacterial pneumonia and assessing the severity of infection in elderly patients .PCT combined with CRP ,WBC could provide better auxiliary diagnosis of the pulmonary infection in elderly patients .
2.The Changes of Plasma Chemokines Levels in Patients with Congestive Heart Failure

Objective To explore the relationship between the plasma level of chemokines and clinical condition in patients with congestive heart failure (CHF). Methods The plasma levels of monocyte chemoattractant protein-1 (MCP-1) and interleukin-8 (IL-8) were measured in 62 patients with CHF (NYHA II~IV) and 18 healthy control subjects by ELISA and RIA, respectively. Results The levels of MCP-1 and IL-8 were significantly higher in CHF group than those in control group, and had a gradual increase along with the increasing of NYHA functional class. The increase of the chemokines levels was independent on the cause of the heart failure. 3.The analysis of laboratory indexes in type 2 diabetes mellitus patients with hypoglycemia

Objective To analyze the laboratory indexes related to type 2 diabetes mellitus(T2DM ) and to explore whether hy‐poglycemia would aggravate the injury of body and development of complication .Methods 27 cases of patients with T2DM from Jan .to Jun .2014 were enrolled in hypoglycemia group ,and 80 patients of diabetes mellitus without hypoglycemia were enrolled in non‐hypoglycemia group .The laboratory indexes were compared between the two groups ,and the relationship between incidence rate of hypoglycemia and levels of glycosylated hemoglobin (HbAc1) were also analyzed ..Results The age ,course of the disease , levels of alanine aminotransferase (ALT ) and serum creatinine (SCr) and urinary protein excretion (Upro ) in the hypoglycemia group were higher than those in the non‐hypoglycemia group ,there were statistical significant differences (P<0 .05);while there was no significant differences in levels of fasting plasma glucose(FPG) and HbA1c ,gender and body mass index(BMI) between the two groups(P>0 .05) .The levels of creatine kinase(CK) ,high sensitivity C‐reactive protein(hs‐CRP) and fibrinogen(FIB) in pa‐tients with T2DM after developing hypoglycemia were higher than those in patients before developing hypoglycemia ,while superox‐ide dismutase(SOD) was lower ,there were statistical significant differences (P<0 .05) .It showed an incomplete inverse correlation between risk of hypoglycemia in patients with T 2DM and levels of HbA1c(P>0 .05) ,at the same time the risk of hypoglycemia tended to be higher in patients with near‐normal level of HbA1c than patients with poor HbA1c control .Conclusion The older , long course of disease and near‐normal levels of HbA1c controled in patients with T2DM may be prone to develop hypoglycemia . Hypoglycemia could aggravate the body injury and speed up the development of complications of T 2DM .
4.Serum diagnosis of gastric cancer using surface-enhanced desorption ionization mass spectrometry and artificial neural network analyses

Objective To develop an artificial neural networks tool and use it to identify proteomic patterns in serum so as to distinguish gastric cancer from controls. Methods Serum samples from 84 gastric cancer patients and 75 controls were randomized into training set (106 samples) and test set (53 samples). At first, samples of the training set were detected using SELDI mass spectrometry and CMIO protein chips. Using a multi-layer ANN with a back propagation algorithm, a proteomic pattern that could distinguish cancer from control samples was identified in the training set. The discovered pattern was then used to determine the accuracy of the classification system in the test set. Results Totally 5 differentially expressed proteins between patients and controls were identified. The five proteins (P < 0.05, m/z at 7567,6742,5262,4869, 4256) were chosen to develop ANN based diagnostic model. The model was blindly tested in the test set for diagnosing gastric cancer. The sensitivity and specificity was 90.0% and 91.3% respectively. Conclusions Combination of SELDI with the artificial neural networks can get a high sensitivity and specificity approach to identify the gastric cancer from the controls. The method shows great potential for early diagnosis of gastric cancer and screening of new tumor biomarkers.
5.Distribution characteristics of subtypes of HPV cervical infection

Objective To investigate the female infection situation of human papilloma virus (HPV) and the distribution charac-teristics of HPV gene subtypes in the hospital ,so as to provide a theoretical basis for clinical prevention and treatment of HPV cer-vical infection and the study for developing the preventive HPV vaccine suitable for this hospital .Methods The PCR-reverse dot blot hybridization (PCR-RDB) was adopted to check the genotypes of HPV on the cervical samples in 625 gynecologic outpatients and inpatients .Results Among 625 cases ,210 cases were infected by HPV with the total HPV infection rate of 33 .6% .The single subtype infection was in 152 cases ,accounting for 72 .38% ,the mixed infection by more than or equal to 2 kinds of subtype was in 58 cases ,accounting for 27 .62% .The subtypes with the higher infection rate from high to low were HPV 58(7 .52% ) ,HPV16 (6 .72% ) ,HPV6(5 .76% ) ,HPV43(4 .80% ) ,HPV11(4 .64% ) ,HPV35(3 .36% ) ,HPV56(2 .56% ) ,HPV18(2 .40% ) ,HPV52 (2 .40% ) and HPV68(2 .40% ) .Conclusion The HPV infection rate in the gynecologic outpatients and inpatients in the hospital is high ,the subtype distribution is wider and is dominated by the high risk HPV infection .The most common subtype is HPV58 and multiple HPV infection rate is high .
6.Detection of differentially expressed microRNA profile in serum of patients with schizophrenia

Objective No uniform standards has been established for the clinical diagnosis of schizophrenia .The article was to investigate the differentially expressed microRNA ( miRNA) profile and explore its clinical significance . Methods Differentially expressed miRNAs were screened by FlashTag TM Biotin RNA chips and SAM software in serum of patients with schizophrenia and healthy controls , then the validation was performed by real-time fluorescent quantitative reverse transcription polymerase chain reaction ( RT-PCR) . Results Three differentially expressed miRNAs were screened , including up-regulated hsa-miR-1281 ( fold change =1.50881) and two down-regulated miRNAs:hsa-miR-2861(fold change=0.642) and hsa-miR-638 (fold change=0.516).The com-parative analysis of RT-PCR by SPSS 17.0 Kruskal Wallis H Test validated the expression levels of hsa-miR-2861 and hsa-miR-638 in patients with schizophrenia decreased significantly in comparison to healthy controls (χ2 =4.089,χ2 =4.083, P<0.001).While the expression level of hsa-miR-1281 in patients with schizophrenia was in higher expression level compared with control group (χ2 =5.333, P<0.001). Conclusion There are differentially expressed miRNA profile in serum of patients with schizophrenia , in which miR-1281 , miR-2861 and miR-638 could be new serum markers for diagnosis of schizophrenia .

Objective To examination the expression levels of plasma miR-31-5p and miR-134-5p in Schizophrenia,Primary discussion for their diagnostic value.Methods Collecting Schizophrenia group,BDAP group and healthy control group,with 90 cases in each group.Detection of plasma miR-31-5p and miR-134-5p expression levels by real-time fluorescent quantitative PCR,the statistical analysis were used to verify differentially expressed miRNA.The diagnostic value of miRNA were evaluated by made receiver operating characteristic(ROC) curves and stepwise Logistic regression analysis.Results The expressed level of miR-31-5p was significantly lower in Schizophrenia group than others (x2 =9.223,P < 0.05).The different expression of miR-3 1-5p compared Schizophrenia group with control group had statistical significance(q =7.85,P < 0.05).The expressed level of miR-31-5p was significantly lower in Schizophrenia group than others(x2 =29.111,P <0.05).The different expression of miR-134-5p had statistical significance compared Schizophrenia group with control group(x2 =20.40,P < 0.05),compared Schizophrenia group with BPAD group (x2 =23.18,P < 0.05).The AUCROC of miR-31-5p was 0.617,senstivity was 73.3%,and specificity was 54.4%.The AUCROC of miR-134-5p was 0.696,senstiivity was 84.4%,and specificity was 48.9%,The combined forecast variables of PRE's AUCROC was 0.695,sensitivity was 84.4%,specificity was 48.9%.Conclusion Signal miR-134-5p and it combine with miR-31-5p had the same diagnostic value in SZ,both of them had better diagnostic value than signal miR-31-5p.
