1.Paroxysmal hemicrania:clinical analysis of 8 cases

Objective To clinically analyze the feature of paroxysmal hemicrania in order to improve our cognition toward it.Methods Eight patients,3 men,5 women,aging 17 to 74 years old,were prospectively analyzed over the past 2 years in our hospital.Results Their age of onset was from 9 to 60years old(mean 42.5±16.3).Seven of the 8 cases were treated with indomethacin,out of whom 5 got an immediate and complete response and one of them remitted partially.Another stopped taking indomethacin because of gastroenteric side effects.She was treated with verapamil and prednisone and partial relief was gained.Conclusions Paroxysmal hemicranial is a rare benign disorder.which needs our improved understanding.The patient who is diagnosed with paroxysmal hemicranial should firsfly receive indomethacin.and standard anti-cluster headache medications or other non-steroid anti-inflammatory drugs is used if she/he can not get relief and (or)tolerate the adverse effects.
2.Comparative study of Topiramate and Sodium Valproate as prophylactic therapy for migraine

Objective To assess the efficacy,safety,and tolerability of Topiramate(TPM) and Sodium Valproate(SV) as prophylactic therapy for migraine.Methods Prospectively,total of 146 patients with episodic migraine were randomly divided into two groups:TPM group(74 cases) and SV group(72 cases).TPM was titrated by 25 mg per week for 2~4 weeks and the highest dose was 100 mg/d.SV was given at 200 mg twice daily during initial titration of therapy and the highest dose was 600 mg/d.The therapies lasted for 13 weeks,meanwhile the migrain frequency,days and the severity of headache were recorded in headache diaries.The severity of headache was rated on a 0~10 rating scale.Results Mean frequency of headache attack decreased from 7.01 to 1.89 times per month(P
3.Synthesis of a nano-antibacterial inorganic filler containing a quaternary ammonium salt with long chain alkyl and its effect on dental resin composites.

OBJECTIVEThis study aimed to synthesize a novel nano-antibacterial inorganic filler that contains a quaternary ammonium salt with long chain alkyl and to report the antibacterial property of dental resin composites.
METHODSA novel nano-antibacterial inorganic filler that contains a quaternary ammonium salt with long chain alkyl was synthesized based on previous research. The antibacterial property of the filler was measured. The surface of the novel nano-antibacterial inorganic filler was modified by a coupling agent to achieve a good interfacial bonding between the filler and the resin matrix. Infrared spectrum analysis was carried out. The modified novel nano-antibacterial inorganic fillers were then incorporated into the dental resin matrix. The dispersion of the fillers was observed and compared with those incorporated into Tetric N-Ceram, a commercial resin composite, under a scanning electron microscope. Streptococcus mutans was used in testing the antibacterial property of the dental resin composites.
RESULTSA quaternary ammonium salt with a long chain alkyl was successfully grafted onto the surface of nano-silica particles. The novel nano-antibacterial inorganic filler that contains quaternary ammonium salt with a long chain alkyl showed stronger antibacterial efficacy than the antibacterial inorganic filler that contains quaternary ammonium salt with a short chain alkyl. The modified novel antibacterial inorganic fillers displayed a homogeneous dispersion in the resin composite bulk and combined closely with the resin matrix, similar to the Tetric N-Ceram. The resin composites that contain novel antibacterial inorganic fillers showed stronger antibacterial effect on Streptococcus mutans compared with the control group.
CONCLUSIONThe novel nano-antibacterial inorganic filler that contains a quaternary ammonium salt with long chain alkyl showed a strong antibacterial property. It also exhibited good compatibility with the dental resin matrix after undergoing coupling treatment.

4.Accuracy of serum S-100β protein and neuron specific enolase level in prediction of postoperative delirium in different age patients

Objective To investigate the accuracy of serum S-100β protein and neuron specific enolase (NSE) level in predication of postoperative delirium (PD) in patients of different ages.Methods Four hundred ASA Ⅰ -Ⅳ patients of both sexes weighing 40-82 kg undergoing abdominal surgery performed under general anesthesia were divided into 4 age groups:group Ⅰ 18-44 yr; group Ⅱ 45-59 yr; group Ⅲ 60-74 yr and group Ⅳ ≥75 yr.The diagnosis of PD was made by using confusion assessment method.The incidence of PD was recorded within 72 h after operation.Each group was further divided into PD and non-PD subgroups.Blood samples were taken at 1 day before operation (T1),during their stay in PACU (T2) and at 24 and 72 h after operation (T3,4 ) for determination of serum S-100β protein and NSE concentrations.The receiver operating characteristic (ROC) curve for serum S-100β protein concentration in determining the PD efficacy was plotted.Results The incidence of PD was significantly higher and the duration was significantly longer in groups Ⅲ and Ⅳ than in groups Ⅰ and Ⅱ,and in group Ⅳ than in group Ⅲ (P < 0.05).There was no significant difference in the serum S-100β protein concentration between PD subgroup and non-PD subgroup in groups Ⅰ - Ⅲ ( P > 0.05).Compared with that at T1 and in nonPD subgroup,the serum S-100β protein concentration was significantly increased in PD subgroup in group Ⅳ,and the serum NSE concentration was significantly decreased at T2,3 in PD subgroup in group Ⅰ (P < 0.05).There was no significant difference in the serum NSE concentration between PD subgroup and non-PD subgroup in groups Ⅱ -Ⅳ.The analysis results of the ROC curve showed that:the area under the curve for the serum S-100β protein concentration and 95% confidence interval were 0.329 (0.127-0.531),0.352 (0.168-0.536),0.619 (0.466- 0.772) and 0.921 (0.846-0.995),the sensitivity was 50%,50%,56% and 88%,and the specificity was 29%,22%,46% and 86% in groups Ⅰ-Ⅳ respectively.Conclusion Increase in the serum S-100β protein concentration can be used in predicting the development of PD in patients ≥75 yr,but the serum NSE protein concentration can not be used.
5.Comparison of the three kind of criteria to evaluate the incidences of postoperative cognitive dysfunction in non-cardiac surgery in non-cardiac surgery

Objective Compare the three kind of criteria in evaluating the incidences of postop-erative cognitive dysfunction in non-cardiac surgery.Methods Four hundred and sixty one non-cardiac surgery patients were randomly enrolled in this study group and two hundred forty four non-surgery patients as control group.Patients??cognitive state was measured on preoperative and postoperative 1 and 3 d by mini-mental state examination(MMSE).POCD was assessed by patients??education level, one standard deviation and Z-score scale,respectively.Results On the first and third day after sur-gery,Z-score scale assess POCD result showed the highest relevance ratio and 95%CI as well as the lowest education level.The education scale showed the lowest incidence of POCD.Education level criteria was positive on postoperative 1 and 3 day,while one case on postoperative 1 day and four cases on postoperative 3 day were negative by Z-score scale.Conclusion The incidence of POCD in same pa-tients by three kind of criteria are different.The Z-score scale is recommended to evaluate the POCD in order to avoid misdiagnosis.
6.Effects of isoflurane or sevoflurane combined with remifentanil anesthesia on blood amyloid beta protein in elderly patients undergoing abdominal surgery

Objective To compare the effects of isoflurane or sevoflurane in combination with remifentanil anesthesia on blood amyloid beta protein (Aβ) in the elderly patients undergoing abdominal surgery.Methods Two hundred patients of both sexes,aged 65-75 yr,weighing 51-76 kg,of ASA physical status Ⅰ or Ⅱ,scheduled for elective abdominal surgery under general anesthesia,were randomly divided into 2 groups (n =100 each) using a random number table:isoflurane combined with remifentanil anesthesia group (IR group) and sevoflurane combined with remifentanil anesthesia group (SR group).Fifty healthy elderly subjects served as control group (group C).After anesthesia was induced with iv penehyclidine,sufentanil,propofol and vecuronium,the patients were endotracheally intubated and mechanically ventilated.In group IR,anesthesia was maintained with inhalation of isoflurane (end-tidal concentration 1.68 %,in IR group) or sevoflurane (end-tidal concentration 1.71%,in SR group),and target-controlled infusion of remifentanil (target plasma concentration 2-6 ng/ml).At l day before surgery and 3 days after surgery,the patients' cognitive function was assessed using Mini-Mental State Examination (MMSE),the development of postoperative cognitive dysfunction (POCD) was recorded,and blood samples were taken for determination of serum Aβ40 and Aβ42 concentrations.Results The incidence of POCD was 5% (in C group),56% (in IR group) or 22% (in SR group),and there was no significant difference among the three groups.There were no significant differences in the serum Aβ42 and Aβ40 concentrations after surgery among the three groups.Conclusion The mechanism by which sevoflurane or isoflurane in combination with remifentanil anesthesia results in POCD is not related to the levels of blood Aβ40 or Aβ42 in the elderly patients undergoing abdominal surgery.
7.Comparison of effects of different methods of general anesthesia on postoperative cognitive function in patients undergoing non-cardiac surgery

Objective To compare the effects of different methods of general anesthesia on postoperative cognitive function in patients undergoing non-cardiac surgery.Methods One thousand ASA Ⅰ or Ⅱ patients,aged 18-60 years and undergoing non-cardiac surgery,were randomly divided into five groups (n=200 each):isoflurane + propofol + fentanyl group (group IPF),isoflurane + remifentanil group (group IR),sevoflurane + propofol + fentanyl group (group SPF),sevoflurane + remifentanil group (group SR),and propofol + remifentanil group (group PR).Two hundred patients receiving non-operative treatment served as control group (group C).In groups IPF and SPF,anesthesia was maintained with inhalation of 1.68% isoflurane or 1.71% sevoflurane,target controlled infusion (TCI) of propofol with the target plasma concentration of 2-5 μg/ml,and intermittent intravenous boluses of fentanyl.In groups IR,SR and PR,anesthesia was maintained with inhalation of 1.68% isoflurane or 1.71 % sevoflurane,or TCI of propofol with the target plasma concentration of 2-5 μg/ml,and TCI of remifentanil with the target plasma concentration of 2-6 ng/ml.The patients' cognitive function was assessed with minimental state examination (MMSE) 1 day before operation,when leaving the post-anesthetic care unit (PACU),and 1 and 3 days after operation,respectively.Z score was used to identify the cognitive dysfunction as recommended by Moiler when leaving the PACU,and 1 and 3 days after operation.Results Compared with group C,the MMSE score was significantly decreased when leaving the PACU,and the incidence of cognitive dysfunction increased when leaving the PACU and 1 day after operation in the other groups (P < 0.05).Compared with groups IPF,IR,SPF and PR,the incidence of cognitive dysfunction was significantly increased in group SR (P<0.05).Conclusion General anesthesia with sevoflurane combined remifentanil exerts fewer effects on the postoperative cognitive function in patients undergoing non-cardiac surgery.

The Z score was used to identify the cognitive dysfunction as recommended by Moller while leaving PACU, and at 1 and 3 d after operation. Results Compared with group C, the MMSE score was significantly decreased while leaving PACU , and the incidence of cognitive dysfunction increased while leaving PACU and at 1 d after operation in the other groups ( P < 0.05). Compared with groups IPF,IR,SPF and PR, the incidence of cognitive dysfunction was significantly increased in group SR ( P < 0.05) . Conclusion General anesthesia with sevoflurane combined remifentanil exerts less effect on the postoperative cognitive function in patients undergoing non-cardiac surgery.
9.Effect of quality management activities on transferring security of critical patients

Objective To explore the effect of quality management activities on transferring security of emergency patients, in order to make sure the safety of patients′transfer.Methods We founded the group carrying out the quality management activities from March to October 2013.Checking the scheme for emergency patients transferring, tracking of emergency patients transferring, summarized the problems during emergency patients transferring and implemented seven reforming measures, we improved 7 aspects.Results In the study group, 78 patients (91.76%) were qualified before transfer, and 77 patients (90.59%) were qualified during transfer;75 patients ( 88.23%) were qualified at the hand-over section and 84 patients ( 98.82%) were qualified in the nursing documents;84 patients in the study group (98.82%) were qualified in the total score. There were significant differences between the study group and the control group (χ2 =4.85,5.80,6.12,4.49, 4.84, respectively;P <0.05 ) .Conclusions After the quality management activities, the purpose of safe transferring was achieved, which make sure the patients′transferring security, improve the working efficiency, reduce the chance of medical misunderstanding, decrease the danger of transferring and assure patients life safety.
10.Application of response to name in early identification of infants with autism spectrum disorders

Objective To explore the behavioral characteristics of response to name in 16-30 months old infants with autism spectrum disorders (ASD),in order to provide a theoretical basis for the early identification and early diagnosis.Methods Two professionals,according to the scoring criteria and using video analysis methods,evaluated the response score (RS),reaction time (RT),duration time (DT) and the rate (RR) of response to their names among ASD infants (ASD group,61 cases),who were diagnosed at Child Developmental and Behavioral Center,the Third Affiliated Hospital of Sun Yat-Sen University from April to December 2017.Then they were compared with infants with developmental delays (DD group,32 cases) and neuro-typical (NT group,33 cases) infants.Finally,researchers predicted the diagnosis for ASD infants according to the behavioral indicators,which had significant differences compared with other groups.Results Compared with DD group [RS 2 (1) score,RT 1.32 (4.65) s,DT 2.69(1.84) s] and NT group [RS 2 (1) score,RT 1.37 (4.37) s,DT 2.90 (2.23) s],RS was significantly lower [1 (1)score],RT was significantly longer [5.87 (4.64) s],and DT was significantly shorter [0.77 (1.88) s] in ASD group,and the differences were statistically significant (H =-4.91,-5.94;5.36,5.41;-4.47,-5.78;all P <0.05);while the differences between DD group and NT group were not significant(all P >0.05).The RR was significantly lower in ASD group [0.25 (0.50)] and DD group [0.50 (0.25)],compared with NT group [0.75 (0.50)],and the differences were statistically significant (H =-6.39,-4.45,all P < 0.01);while the differences between DDgroup and ASD group were not significant(P >0.05).When detecting ASD from ASD and NT infants,the area under the receiver operating characteristic curve (AUC) was 0.889 (P <0.01);when detecting ASD from ASD and DD infants,AUC was 0.924 (P < 0.01);when detecting ASD from all infants,AUC was 0.868 (P < 0.01),according to all indicators of response to name.Conclusions There are significant differences between ASD infants and DD and NT infants in response to name domain.Behavioral characteristics in the procedure of response to name can predict ASDwell.Response to name as an early social behavioral indicator,being tested at 2 years old,is still of importance for the early identification and early diagnosis of ASD.
