1.The relationship between colorectal neoplasia and nonalcoholic fatty liver disease

ObjectiveTo investigate the relationship between colorectal neoplasia and nonalcoholic fatty liver disease (NAFLD).MethodsData of 809 patients who had undergone colonoscopy and epigastric CT scan in the same period were reviewed for gender,age,history of diabetes,hypertension,hyperlipidemia,NAFLD,colonoscopy and pathology results.Correlation of colorectal adenoma (CRA),colorectal carcinoma (CRC) and NAFLD were studied with multivariate Logistic analysis.Results NAFLD (P <0.0001,OR 2.06,95% CI 1.53-2.76) was the independent risk factor for CRA,where the incidence of CRA was higher than that in the control group.ConclusionPatients with NAFLD is the risk population of colorectal adenomas.
2.Diagnostic value of T-SPOT.TB in patients with fever

Objective To investigate the diagnostic value of T-SPOT.TB in patients with fever.Methods T-SPOT.TB was carried out in 456 febrile patients who had no determined reason for fever or who could not be excluded from infection with Mycobacterium tuberculosis.These patients were followed up for their final clinical diagnosis and the diagnostic value of T-SPOT.TB was evaluated using the receiver operating characteristic (ROC) curve.Results In the present study,85 patients were clinically diagnosed with tuberculosis (TB) (including 34 cases of pulmonary TB,24 cases of TB infection defined as T-SPOT.TB positive without TB lesion,but effective with anti-TB treatment,10 cases of tuberculous pleurisy and 17 cases of other extrapulmonary TB) and 371 patients had no definite evidence of Mycobacterium tuberculosis infection (including 42 cases of suspected TB and 329 cases of other diseases).T-SPOT.TB had a high sensitivity of 84.3% in TB patients and a high negative predictive value of 94.2 %,but the specificity was 68.1% and the positive predictive value was 41.4 %.The Kappa value was 0.380 and the area under ROC curve was 0.762 (95%CI:0.707-0.817).T-SPOT.TB had a higher sensitivity in extrapulmonary TB than in pulmonary TB (88.2 % vs 78.1 %),with the negative predictive values of 95.3 % and 92.6 %,Kappa values of 0.438 and 0.299,and the areas under ROC of 0.791 (95 % CI:0.724-0.857) and 0.719 (95%CI:0.623-0.815),respectively.Ninety-nine cases were T-SPOT.TB positive out of 329 patients diagnosed with other diseases.Conclusions T-SPOT.TB test can be used as a method for TB diagnosis in patients without obvious TB lesions.Due to its relatively high negative predictive value,T-SPOT.TB can be used to rule out infection with Mycobacterium tuberculosis.
3.Role of natural antiox-LDL subclass IgM antidodies in formation of atherclerosis

Objective To study the role of natural antiox-LDL subclass IgM antibodies in formation of atherclerosis.Methods The macrophages from mice were divded into LDL group,Cuox-LDL group and control group.The macrophages in LDL group and Cuox-LDL group were precultured with LDL and Cuox-LDL.Adhesion experiment was performed in control group.3A6,5G8 and 2H7 were incubated.The macrophages were divided into 3A6 group,5G8 group and 2H7 group.Adhesion experiment was performed by adding the pretreated 125I Cuox-LDL.The binding parameters of macrophages and 125I Cuox-LDL in different groups were recorded.Foam cells were analyzed by cell lipid analysis with oil red O staining.Results The bound 125I Cuox-LDL level was significantly lower in Cuox-LDL group than in control group and LDL group (P<0.01).No significant difference was found in bound 125I Cuox-LDL level between 5G3 group and 2H7 group (P>0.05).The bound 125I Cuox-LDL level was significantly lower in 3A6 group than in control group,5G8 group and 2H7 group (P<0.01).Oil red O staining showed no oil drop in macrophages,but a small number of oil drops in 3A6 group and a large number of oil drops in control group.The cholesterol and TG in macrophages reduced 45% and the TG reduced 170% in 3A6 group,which were significantly lower than those in control group (P<0.01).Conclusion Antiox-LDL IgM subclass antibody 3A6 can effectively prevent the binding of unactivated macrophages to ox-LDL,reduce the occurrence and progression of atherosclerosis.
4.Association between tooth missing and non-alcoholic fatty liver disease

Objective:To evaluate the association between the number of teeth missed and the prevalence of non-alcoholic fatty liver disease (NAFLD) in adults.Methods:A cross-sectional study was carried out in 26 983 adults from Tianjin Chronic Low-grade Systemic Inflammation and Health Cohort Study. The number of teeth missed (excluding third molars) was recorded and classified into four categories: 0, 1-, 3- and ≥6. NAFLD was diagnosed by at least two liver ultrasonography examinations. Adjusted multivariable logistic regression analysis was used to access the association between the number of missed teeth and NAFLD in adults.Results:The overall prevalence of NAFLD was 7 270 and the number of the subjects with at least one tooth missed was 9 667. The multivariable-adjusted ORs (95% CI) of NAFLD across the categories of tooth missing (0, 1-, 3- and ≥6) were as follows: 1.00, 1.04 (0.93-1.15), 1.08 (0.93-1.26) and 1.38 (1.09-1.76) (trend test P=0.030) in males; 1.00, 0.96 (0.82-1.12), 1.11 (0.91-1.35) and 1.22 (0.90-1.64) (trend test P=0.450) in females. Conclusion:The number of missed teeth was positively associated with a higher prevalence rate of NAFLD in males with over 6 teeth missed, but not in females.
5.Correlations between DHT/AR and ATAD2 expression in hepatocellular carcinoma

Objective To investigate the expression of androgen receptor(AR),ATAD2 in hepatocellular carcinoma(HCC) and the correlations with clinicopathological features,and the role of DHT/AR and ATAD2 in proliferation of HCC cells.Methods The samples of 75 patients with HCC in the First Affiliated Hospital of China Medical University from February 2012 to December 2012 were collected.LM3 and Huh7 cells were divided into control group,DHT group,DHT + CDX (bicalutamide) group and CDX group;and also divided into Ri-ATAD2 group (adding interference fragments) and Ri-C group (adding control vector sequence).Immunohistochemistry was used to detect the expression of AR and ATAD2,and to analyze the correlations between clinical features and survival of patients.Real-time PCR and Western Blot were used to detect the expression of AR and ATAD2,and CCK-8 was used to detect cell proliferation.Results HCC patient samples were grouped according to AR and ATAD2 expression.Compared with low AR expression group (n =31),the ratio of tumor <5 cm in high expression group (n =44) was higher,and the ratio of TNM stage Ⅰ + Ⅱ was lower.Compared with low ATAD2 expression group (n=35),the ratio of metastasis and tumor differentiation grade Ⅲ + Ⅳ was higher in high expression group (n=40),and the difference was statistically significant (P < 0.05).The overall survival rate of patients with high expression of ATAD2 was lower than other patients,and the differences were statistically significant (P<0.05).Multivariate Cox regression analysis showed that ATAD2 expression (HR=1.935,95% CI:1.066~3.515) and metastasis (HR=2.212,95% CI:1.059~4.619) were independent predictors of poor prognosis.Compared with LO2 cells,the mRNA and protein level of AR and ATAD2 in LM3 and Huh7 cells were significantly higher,and the differences were statistically significant (P<0.05).And the proliferation rate of HCC cells increased significantly after 48 and 72 hours compared with the control group,and the differences were statistically significant (P<0.05).After adding CDX,the proliferation of LM3 and Huh7 induced by DHT was inhibited.DHT enhanced the expression of ATAD2,while CDX inhibited the expression of ATAD2.The expression of ATAD2 protein decreased when LM3 and Huh7 cells were interfered.Compared with Ri-C group,the proliferation of HCC cells in Ri-ATAD2 group decreased significantly after the DHT treatment 48 and 72 hours,and the difference was statistically significant (P<0.05).Conclusions DHT/AR promoted the proliferation of HCC cells by inducing ATAD2 expression.Modulating ATAD2 expression may be the potential mechanism of DHT/AR in HCC proliferation.
6.Effect analysis of vertebroplasty combined with posterior decompression and internal fixation in the treatment of spinal metastases with neurological symptoms

Objective:To analyze the efficacy of vertebroplasty combined with posterior decompression and internal fixation in the treatment of spinal metastases with neurological symptoms.Methods:This study was a retrospective analysis of 32 cases with neurological symptoms caused by spinal metastatic tumor in the Department of Orthopedics, Beijing Friendship Hospital, Capital Medical University from June 2016 to January 2022. All 32 patients (20 males, 12 females) aged between 39 to 85 years were enrolled in this study, with a mean age of (64.8±11.9) years. Surgery time, bleeding volume, modified Tokuhashi score, spinal instability neoplastic score (SINS) were collected. The visual analogue score (VAS), Oswestry disability index (ODI), Karnofsky performance scale (KPS) and American spinal cord injury association (ASIA) score before surgery, 1 month after surgery, 3 months after surgery and 6 months after surgery were collected and analyzed. Measurement data with normal distribution were represented as mean±standard deviation( ± s), and the comparison before and after surgery was conducted using repeated-measures analysis of variance test. Measurement data with skewed distribution were represented as M( Q1, Q3). Results:All 32 patients were followed up for 6 months. The operation time was (209.6±49.0) min, and the blood loss was (462.9±298.3) mL. 14 patients(43.8%) were treated with blood transfusion. The modified Tokuhashi score was 8.09±2.89, and the SINS was 8.81±1.97. The VAS score before surgery, postoperative 1, 3, 6 months were 7.47±0.98, 3.87±0.87, 2.91±0.73 and 2.34±0.60, respectively. ODI score before surgery, postoperative 1, 3, 6 months were 79.13±9.50, 39.14±6.31, 34.43±6.42 and 31.08±4.80, respectively. KPS score before surgery, postoperative 1, 3, 6 months were 49.69±14.70, 64.68±15.02, 71.88±12.81 and 75.63±10.76, respectively. The ASIA grading at 6 months follow-up was improved compared to preoperative baseline. Postoperative complications occurred in 4 cases, including nerve root injury (1 case), bone cement leakage in paravertebral soft tissue (1cases), bone cement leakage in paravertebral vein(1case) and acute thoracic epidural hematoma (1case).Conclusion:Vertebroplasty combined with posterior decompression and internal fixation can effectively relieve pain, relieve the compression of spinal cord and nerve, recover nerve functions, and improve the quality of life of patients with spinal metastatic tumor.
7.Current status of the application of artificial intelligence in the diagnosis and treatment of pancreatic cancer

Pancreatic cancer is a common malignant tumor of the digestive system,with a low early diagnosis rate,a high surgical mortality rate,a low cure rate,and a poor overall prognosis.In recent years,with the continuous development of artificial intelligence in the medical field,artificial intelligence techniques,such as machine learning and deep learning,have been widely used in medical research.This article reviews the application of artificial intelligence techniques in the screening,diagnosis,treatment,complications,and prognosis prediction of pancreatic cancer,so as to provide a basis and new ideas for the application of artificial intelligence in the diagnosis and treatment of pancreatic cancer.
8.Expression and clinical significance of miRNA identified by high-throughput sequencing in left ventricular hypertrophy

Objective To investigate the expression levels and clinical significance of differentially expressed microRNAs(miRNAs)based on high-throughput sequencing in patients with left ventricular hypertrophy(LVH)and their possible regulatory mechanisms.Methods The miRNA sequencing was performed on plasma samples from 4 LVH patients and 4 healthy volunteers,and qRT-PCR validation was performed on plasma samples from 25 healthy volunteers and 35 LVH patients.The target genes of the obtained differentially expressed miRNA were predicted.The molecular function(MF),biological process(BP)and cellular components(CC)of these miRNAs were predicted by the Gene Ontology(GO)enrichment analysis,and their signal pathways were predicted by the Kyoto Encyclopedia of Genes and Genomes analysis.Meanwhile,the protein-protein interaction network was constructed.The diagnostic values of 2 differentially expressed miRNAs(hsa-miR-942-5p and hsa-miR-184)in LVH were analyzed by the area under the receiver operating characteristic(ROC)curve(AUCROC).Results A total of 945 differentially expressed miRNAs were identified,of which 1 and 9 were observed to be significantly upregulated and downregulated,respectively.The expression levels of hsa-miR-942-5p and hsa-miR-184 were verified by qRT-PCR to be significantly downregulated.The AUCROC of hsa-miR-942-5p and hsa-miR-184 were 0.694 6 and 0.880 4,respectively.Bioinformatics analysis revealed that the target genes were mainly enriched in the cell senescence,sphingolipid metabolism,and TGF-β,p53,and diabetic metabolism related signaling pathways.Conclusion The expression levels of hsa-miR-942-5p and hsa-miR-184 in plasma of LVH patients are significantly decreased,suggesting that they have good diagnostic potential.
