AIM: To study the suppressive effect of c hu ankezhi (CKZ) injection, a Chinese medicine, on murine allergic contact dermatit is (type IV hypersensitivity). METHODS: Mice were divided into 6 groups according to different medicine treatments: CKZ high, middle, low dose ( CKZⅠ,Ⅱ,Ⅲ) groups, dexamethasone(DEX), benadryl and saline groups. Murine alle rgic contact dermatitis was induced by intraperitoneal injection of 2, 4-dinitro fluorobenzene. Different medicines were administrated at 2 h before sensitizatio n on day 0 and day 1, day 2, 2 h before elicitation and 6 h after on day 5. The six experimental groups were compared according to left ear thickness difference (S1), left ear weight difference (S2), body weight difference (S3) and dermal i nflammatory infiltration cell number. [ ABSTRACT] AIM:To explore the mechanisms of fluctuant high blood glucose-induced apoptosis of hepatocytes. METHODS:SD rats were randomly divided into normal control group ( N) , stable high blood glucose group ( S) , fluctu-ant high blood glucose group ( F) and insulin group ( I) .Diabetic rats were induced by intraperitoneal injection of strepto-zotocin (65 mg/kg) , and the fluctuant high blood glucose animal model was induced by intraperitoneal injection of ordinary insulin and glucose at different time points every day.The blood glucose fluctuation patterns of the animals in F group with-in 12 weeks were similar every day and no significant difference of the HbA1c concentration was observed compared with S group, indicating that the fluctuant hyperglycemia was successfully established in F group.The activity of superoxide dis-mutase ( SOD) and glutathione peroxidase ( GSH-Px) , and the content of malondialdehyde ( MDA) and nitric oxide ( NO) in the homogenate of the liver tissues were detected by colorimetry.The mRNA and protein levels of JNK, p-JNK, Bax and Bcl-2 were examined by RT-PCR and Western blot.RESULTS:After 12 weeks, the increases in the intakes of food and water, the urine output, and the abnormal liver function were observed in S group, I group and F group.Compared with N group, the MDA level was increased, the content of NO and the activity of SOD and GSH-Px were decreased, and up-regu-lation of JNK mRNA and p-JNK and Bax proteins, and down-regulation of Bcl-2 were also found in S group, I group and F group.The above effects were more obviously showed in F group.CONCLUSION:Oxidative stress activates JNK-MAPK signaling pathway, which is involved in fluctuant high glucose-induced apoptosis of hepatocytes.
3.Effects of climbing exercise intervention on gross motor development in children aged 8 to 9 years

Objective:
The purpose of this study was to determine the effect of climbing exercise intervention on gross motor function in children aged 8 to 9 years, and to provide a theoretical basis and scientific reference for the application of climbing exercise in the improvement of children health.
Methods:
Random sampling method was used to select 53 children aged 8-9 from a public school in Haidian District in Beijing, and children were randomly divided into control group (n=30) and exercise group (n=23), children in the exercise group received climbing exercises for 8 weeks . KTK test was used to assess gross motor development.
Results:
After intervention, the gross motor level of male and female children in Exercise group were significantly higher than before (in male group, before intervention, KTK score:217.5±32.3,after intervention, KTK score:245.1±24.2,P<0.01;in female group, before intervention, KTK score: 219.6±30.3, after intervention, KTK score: 246.9±23.2,P<0.01); while the gross motor level in control group had no significant change compared to the intervention state before(in male group, before intervention, KTK score:219.9±26.8,after intervention, KTK score:224.8±30.2,P>0.05;in female group, before intervention, KTK score: 216.6±18.1,after intervention, KTK score:214.8±28.6,P>0.05). There were no differences of gross motor level between sham and exercise group at baseline (P>0.05). After intervention, except the MS scores (P>0.05), other test program scores were higher than those of control group (in exercise group, WB: 52.1±6.8; HH: 61.7±8.7; JS: 73.7±7.6; in control group, WB: 43.1±9.9; HH: 54.5±15.2; JS: 64.9±6.8) (P<0.01).
Conclusion
Our research shows that climbing exercise intervention helps promoting the children’s physical quality, and establishes substantial foundation for the formation and promotion of sports cognition and complex motor skills.
4.Correlation between waist circumference and psychological sub health of middle school students in Xinjiang

Objective:
To understand the association between waist circumference with psychological sub health among middle school students in Xinjiang, and to provide evidence for the management of waist circumference and effective prevention of mental health problems.
Methods:
By using the stratified cluster sampling, 6 191 middle school students aged 13-15 in Urumqi, Karamay, Yining, and Kashgar of Xinjiang were surveyed on demographic information, lifestyle, waist circumference, and psychological sub health symptoms. Chi square test, one way ANOVA, and Logistic regression analysis were used to analyze the association between waist circumference and mental sub health.
Results:
The detection rate of psychological subhealth among secondary school students in Xinjiang was 21.4%. The detection rates of emotional problems, behavioral problems, and social adjustment difficulties were 28.4 %, 28.1%, and 17.7%, respectively. Significant differences in the detection rate of emotional problems, social adjustment difficulties, and psychological sub health scores were observed among students with different waist circumference levels ( F= 4.03 , 10.90 , 5.81, P <0.05). Logistic regression analysis showed that secondary school students with high waist circumference had a higher risk of social adjustment difficulties ( OR=1.45, 95%CI =1.20-1.75) and psychological sub health problems ( OR=1.46, 95%CI =1.22-1.74) compared to those with normal waist circumference ( P <0.01).
Conclusion
Psychological sub health problems of middle school students in Xinjiang are prominent, and the waist circumference is positively related to the psychological sub health problems. Schools should pay attention to the mental health education of middle school students, ensure reasonable diet, sleep and living habits, and promote their physical and mental health development.
5.Application effect of discharge preparation service based on theory of goal attainment on patients with cervical spinal cord injury

Objective:To investigate the application effect of discharge preparation service based on theory of goal attainment on patients with cervical spinal cord injury.Methods:A retrospective cohort study was conducted to analyze the clinic data of 60 patients with cervical spinal cord injury admitted to Zhengzhou Orthopedics Hospital from January 2017 to December 2022, including 49 males and 11 females, aged 23-79 years [(52.2±13.5)years]. Patients were all treated with cervical decompression fusion and internal fixation. Patients admitted from January 2017 to December 2019 were treated with conventional nursing intervention (conventional nursing group, n=30) and patients admitted from January 2020 to December 2022 were treated with discharge preparation service based on theory of goal attainment (discharge preparation service group, n=30). The readiness for hospital discharge of the two groups was compared using the Chinese version of Readiness for Hospital Discharge Scale (RHDS) at 4 hours before discharge. The degree of cervical spinal cord dysfunction of the two groups were compared using Japanese Orthopedic Association (JOA) score before intervention, at discharge and at 6 months after discharge. The complication and unplanned readmission rates of the two groups were compared at 6 months after discharge. Results:All the patients were followed up for 6 months. At 4 hours before discharge, the scores of the three parameters of RHDS containing personal status, adaptability and anticipatory support and the total score of the discharge preparation service group were (20.9±3.5)points, (35.9±2.2)points, (30.4±3.0)points and (87.1±7.8)points respectively, higher than those of the conventional nursing group [(16.2±1.7)points, (32.5±2.2)points, (26.3±2.1)points and (75.0±5.6)points respectively] ( P<0.01). There was no statistically significant difference in the JOA score of the two groups before intervention ( P>0.05). The JOA scores of the discharge preparation service group at discharge and at 6 months after discharge were (11.8±1.7)points and (13.8±1.5)points respectively, higher than those of the conventional nursing group [(10.3±1.8)points and (11.6±1.9)points respectively] ( P<0.01). At 6 months after discharge, the complication rate of the discharge preparation service group was 6.7% (2/30), lower than that of the conventional nursing group [36.7% (11/30)] ( P<0.05). The unplanned readmission rate of the discharge preparation service group was 3.3% (1/30), lower than that of the conventional nursing group [23.3% (7/30)] ( P<0.05). Conclusion:For patients with cervical spinal cord injury, discharge preparation service based on theory of goal attainment can improve the discharge readiness, promote spinal functional recovery and reduce the complication and unplanned readmission rates.
6.Detection of circulating tumor cells in patients with hepatocellular carcinoma.

OBJECTIVETo explore the detection efficiency of circulating tumor cells (CTCs) in patients with hepatocellular carcinoma (HCC).
METHODSImmunomagnetic negative enrichment by nanometer magnetic beads and label-free capture with Captor(TM) system were used to isolate and enrich CTCs from peripheral blood of HCC patients, and epithelial and HCC markers were applied to identify CTCs by immunofluorescence staining. CTCs were detected in 50 HCC patients before and after hepatectomy to test the method for isolation, enrichment and identification. The dynamic changes of pre- and post-operative CTCs' numbers were compared. The clinical data were analyzed using SPSS 19.0 software.
RESULTSNegative enrichment methods by nanometer magnetic beads and label-free capture using Captor(TM) system were both suitable for CTCs isolation and enrichment in HCC patients. The positive detection rate of CTCs in HCC patients via negative enrichment was 96.0% (48/50), the preoperative median number of CTCs was 16 per 7.5 ml blood, and the postoperative median number was 17 per 7.5 ml blood.
CONCLUSIONSBoth negative enrichment and Captor(TM) system are suitable for isolation and enrichment of CTCs in HCC patients. There is a significant difference in the numbers of CTCs before and after operation, and dynamic detection of CTCs will provide helpful prognostic information for HCC patients in clinics.

7.Elimination of Gibbs artifact based on local subpixel shift and interlaced local variation.

OBJECTIVE:
To extend the application of Gibbs artifact reduction method that exploits local subvoxel- shifts (LSS) to zero- padded k-space magnetic resonance imaging (MRI) data.
METHODS:
We investigated two approaches to extending the application of LSS-based method to under-sampled data. The first approach, namely LSS+ interpolation, utilized the original LSS-based method to minimize the local variation on nonzero-padding reconstructed images, followed by image interpolation to obtain the final images. The second approach, interlaced local variation, used zero-padded Fourier transformation followed by elimination of Gibbs artifacts by minimizing a novel interlaced local variations (iLV) term. We compared the two methods with the original LSS and Hamming window filter algorithms, and verified their feasibility and robustness in phantom and data.
RESULTS:
The two methods proposed showed better performance than the original LSS and Hamming window filters and effectively eliminated Gibbs artifacts while preserving the image details. Compared to LSS + interpolation method, iLV method better preserved the details of the images.
CONCLUSIONS
The iLV and LSS+interpolation methods proposed herein both extend the application of the original LSS method and can eliminate Gibbs artifacts in zero-filled k-space data reconstruction images, and iLV method shows a more prominent advantage in retaining the image details.

8. Induced membrane technique and microsurgery for open leg fractures of Gustilo types ⅢB-C

Objective:
To evaluate the induced membrane technique combined with microsurgery for repair of open leg fractures of Gustilo types ⅢB-C.
Methods:
This retrospective study reviewed 15 patients who had been treated for open leg fractures of Gustilo types ⅢB and ⅢC by the induced membrane technique and microsurgery between January 2015 and January 2017 at Institute of Orthopedics, 920 Hospital, The Joint Logistic Service of The People’s Liberation Army. They were 10 men and 5 women, aged from 18 to 41 years(average 32 years). There were 9 cases of Gustilo type IIIB and 6 ones of Gustilo type IIIC. After thorough debridement, the fractures were reduced and fixated temporarily using external frames. The bone defects were filled with antibiotic bone cement to induce biofilm formation. After necessary reconstruction of limb structures, including neurovascular repair and transposition of tendon and nerve, crucial soft-tissue wounds were covered with surgical flaps. In the secondary surgery 6 to 8 weeks later, with the external frames replaced by internal fixation or not, the bone cement was removed without damaging the biofilm before graft reconstruction with autogenous cancellous bone.
Results:
The 15 legs were all salvaged successfully. The in-hospital time ranged from 21 to 39 days (mean, 29 days). Crucial wounds were repaired primarily without any severe or persistent infection. Follow-ups ranged from 12 to 24 months (average, 15 months). Bone union time ranged from 10 to 17 months (average, 11.2 months) with satisfactory aesthetic and functional recovery of the leg.
Conclusion
Reconstruction of open leg fractures of Gustilo types Ⅲ B-C with induced membrane technique and microsurgery can result in decreased therapeutic duration, reduced complications and positive outcomes.
9. Effects of human amniotic epithelial stem cells-derived exosomes on healing of wound with full-thickness skin defect in rats

Objective:
To investigate the effects of human amniotic epithelial stem cells-derived exosomes on healing of wound with full-thickness skin defect in rats.
Methods:
(1) Human amniotic epithelial stem cells were isolated from the amnion tissue of 5 full-term pregnant women in Department of Obstetrics of our hospital by the method of trypsin digestion, and their morphology was observed. The third passage of cells were stained with rhodamine-phalloidin for cytoskeleton observation. The third passage of cells were identified with flow cytometry through the detection of expressions of cell surface markers CD29, CD31, CD34, CD90, CD105, SSEA3, SSEA4 and immunity-related marker human leukocyte antigen-D related site (HLA-DR). The third passage of cells were also assessed the ability of adipogenic and osteogenic differentiation. (2) The third passage of human amniotic epithelial stem cells were cultured in DMEM medium supplemented with 10% exosome-free fetal bovine serum. Exosomes were isolated from culture supernatant by the method of ultracentrifugation and represented with scanning electron microscope for morphologic observation. (3) Six adult SD rats were anesthetized, and four 1 cm×1 cm sized wounds with full-thickness skin defect were made on the back of each rat. The wounds on the back of each rat were divided into control group, 25 μg/mL exosomes group, 50 μg/mL exosomes group, and 100 μg/mL exosomes group according to the random number table (with 6 wounds in each group), and a total volume of 100 μL phosphate buffered saline, 25 μg/mL exosomes, 50 μg/mL exosomes, and 100 μg/mL exosomes were evenly injected around the wound through multiple subcutaneous sites, respectively. The wound healing rate was calculated based on measurement on post injury day (PID) 7, 14, and 21. On PID 21, the healed wound tissue of each group was collected and stained with HE to observe and count skin accessories, and the arrangement of collagen fibers was observed with Masson staining. Data were processed with analysis of variance for repeated measurement, analysis of variance of randomized block design, one-way analysis of variance, and Bonferroni test.
Results:
(1) The cells, which were isolated and cultured, displayed typical cobblestone morphology with many microvilli on cell surface. Among the cells, the positive expression rates of CD29, CD90, SSEA3, and SSEA4 were above 50.0%, and the rate of CD105 was 8.0%, while the rates of CD31, CD34, and HLA-DR were almost 0. The cells could differentiate into adipocytes and osteoblasts. The above results revealed that the cells cultured were human amniotic epithelial stem cells. 10.Analysis of the construction of county-level clinical key specialized talents in Henan province

Objective To study the development of the first county-level key clinical specialized talent teams in Henan province. Methods Data on the first 30 county-level key clinical specialized talent teams were collected through questionnaire in 2014 and 2017 respectively, and signed rank sum test was conducted to analyze such facts as the changes of medical service volume, academic titles and scientific researches of discipline leaders before and after such construction. McNemar′s test and chi-square test were conducted to study the age structure, seniority, education and technical title makeup of these discipline leaders. Such tests were also conducted to study the age structure, seniority, education and technical title makeup of the talents. These efforts aim to discern the construction achievement of the key clinical specialties. Results The indexes of healthcare capacities of the first batch of clinical key specialties were observed as improved ( P <0. 05 ) in terms of the medical service volume. Of the discipline leaders′ age structure, the 51 -60 age groups accounted for 40. 0% and 66. 7% respectively, before and after the construction, scoring a difference of statistical significance ( P <0. 05 ); the number of senior physicians increased by 80 persons, the number of physicians having master or above titles increased by 40 persons;the number of physicians participating in continuing education held by these specialties grew from 146. 50 to 262. 50(P<0. 05). Conclusions The volume of medical service of the first batch of county-level clinical key specialties has been elevated, and a group of talents of high education and senior titles have been recruited. Whereas, the title structure needs to be improved, and more young academic leaders are expected, while more opportunities of further education and training are also expected for the physicians, in order to enhance their professional ability.
