1.Progress of cancer-associated fibroblasts in breast cancer

Tumor tissue is a complex of tumor cells, stromal cells, and extracellular matrix, and they constitute a disordered and aggressive microenvironment, which plays an indispensable role in the occurrence and development of tumors. In breast cancer, cancer-associated fibroblasts (CAF) not only promote the occurrence, proliferation, invasion, metastasis, and drug resistance of tumor, but also participate in events including angiogenesis, lymph angiogenesis, extracellular matrix remodeling, and reconstruction of the microenvironment, which are known to induce cancer. Therefore, a new strategy for tumor therapy is provided by targeting CAF. This article reviews the research progress of CAF in breast cancer.
2.Clinical Application of Platelet-Rich Plasma in Treatment of Deep GradeⅡBurn Wounds

Objective To evaluate the clinical effects of platelet-rich plasma in treatment of DeepⅡburn wounds. Methods Sixty eight cases of inpatients with Deep Ⅱburns were selected, whose age were from 1 to 65 years and their burn areas were between 5%~62%of total body surface area (TBSA). Deep gradeⅡburn of each sample was divided into two parts. Part A was the treatment group and part B was the control group.The burn wounds in the treatment group were treated with platelet-rich plasma and the counterpart in the control group were treated with SD-Ag. Healing time ,recovering rate and the frequency of dressing changes,frequency of changing the most innerlayer gauze and the rate of wound infection were also analyzed. Finally the laboratory abnormalities and adverse effect were monitored regularly. Results The healing time of the treatment group (16.5±3.1 ) d was shorter than that of the control group (19.5±3.8 ) d. The recovering rate of the treatment group was higher than that of the control group on the 14th and 17th day after treatment. There were statistically sig?nificant differences between these two groups (P<0.05). After two weeks’treatment, the internal and external dressing per?meability into wet gauze layers were 20.6 ± 1.7, which were significantly decreased than that in the control group 23.3 ± 5.9. The frequency of dressing changes was(7.2±1.1)times in treatment group versus(9.2±1.4)times in control group and the frequency of changing the inner most layer gauze was( 2.3±0.6)times in the treatment group versus(5.3±1.3)times in con?trol group. There were 5 inflammation reaction cases in the treatment group, but 13 cases in the control group. However, there was no statistic significance between the two group in the outcomes of bacterial culture, the laboratory abnormalities and the adverse effect. Conclusion Platelet-rich plasma can remarkably shorten the healing time,improve healing rate,re?duce frequency of dressing change and promote wound healing for deepⅡburn wound. PRP is a potential safety reagent in treating deepⅡburn wound.
3.Research Progress of Pseudogenes in Hepatocellular Carcinoma

Pseudogenes were initially thought to have no function and were called by aliases, such as "junk genes." With the emergence of large-scale genomics projects and more and more experimental studies, pseudogenes have been shown to play an important role in the occurrence and development of solid tumors, especially playing an important regulatory role in the occurrence and develepment of liver cancer, such as regulating the proliferation, apoptosis, invasion, metastasis, and immunity of liver cancer cells. Recent studies showed that pseudogenes can act as regulators of oncogenes and tumor suppressors in hepatocellular carcinoma (HCC) and can thus serve as prognostic markers and even therapeutic targets for this cancer type. In this review, we systematically summarize the mechanisms and functions of different pseudogenes in HCC and present their future prospects as therapeutic targets.
4.Discussion on the mechanism of Guizhi Fuling Pills in the treatment of atherosclerosis based on network pharmacology and molecular docking technology

Objective:To explore the active components, targets and mechanism of Guizhi Fuling Pills in the treatment of atherosclerosis (AS) based on network pharmacology and molecular docking technology.Methods:The active components and potential target information of Guizhi Fuling Pills in the treatment of AS was obtained using Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP), SwissTargetPrediction database and Genecards database. The target protein interaction network was constructed by using STRING database. The DAVID database was used to perform the Gene Ontology (GO) function and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment on potential targets. AutoDockVina and PyMOL software were used to verify the molecular docking of the main active components and key targets of Guizhi Fuling Pills.Results:A total of 74 active components, 239 potential targets and 4 710 AS-related disease targets were screened, and 182 intersection targets were obtained. A total of 484 biological process items, 132 molecular function items and 74 cellular component items were obtained by GO functional enrichment analysis, and 116 signal pathways were screened by KEGG enrichment analysis. The results of molecular docking suggested that the active components of Guizhi Fuling Pills have good binding activity to the key intersection targets.Conclusion:The active components of Guizhi Fuling Pills, such as sitosterol and paeoniflorin, mainly treat AS by regulating estrogen signal pathway and inflammatory signal pathway through TNF, VEGFA and other targets.
5.MobileNetV3 network-based diagnosis of caries and periapical periodontitis from periapical films

Objective:
To research the effectiveness of deep learning techniques in intelligently diagnosing dental caries and periapical periodontitis and to explore the preliminary application value of deep learning in the diagnosis of oral diseases
Methods:
A dataset containing 2 298 periapical films, including healthy teeth, dental caries, and periapical periodontitis, was used for the study. The dataset was randomly divided into 1 573 training images, 233 validation images, and 492 test images. By comparing various neural network models, the MobileNetV3 network model with better performance was selected for dental disease diagnosis, and the model was optimized by tuning the network hyperparameters. The accuracy, precision, recall, and F1 score were used to evaluate the model's ability to recognize dental caries and periapical periodontitis. Class activation map was used to visualization analyze the performance of the network model
Results:
The algorithm achieved a relatively ideal intelligent diagnostic effect with precision, recall, and accuracy of 99.42%, 99.73%, and 99.60%, respectively, and the F1 score was 99.57% for classifying healthy teeth, dental caries, and periapical periodontitis. The visualization of the class activation maps also showed that the network model can accurately extract features of dental diseases.
Conclusion
The tooth lesion detection algorithm based on the MobileNetV3 network model can eliminate interference from image quality and human factors and has high diagnostic accuracy, which can meet the needs of dental medicine teaching and clinical applications.
6.Study on Quality Evaluation of Honeysuckle Oral Liquid Based on Quality Markers of Anti-Inflammatory Effect

Objective To establish a quality traceability evaluation method for the whole honeysuckle oral solution process by identifying and screening its anti-inflammatory quality markers.Methods UPLC/-TOF-MS was used to analyze the iridoids and phenolic acids in oral solution,and the correlations were constructed by molecular network technology.The HPLC fingerprints of multiple batches of oral solution were established,and similarity analyses were performed to identify key pharmacodynamic molecules.The key anti-inflammatory quality markers were confirmed by the NF-κB dual luciferase assay system.Further,the quantification of 12 quality markers of iridoids and phenolic acids in oral solution was established separately based on the dual-wavelength HPLC technique.The quality of the oral solution was evaluated by examining the extraction and transfer rate of quality markers during the processing of raw materials and preparations and thermal stability.Results A total of 9 iridoids and 6 phenolic acids were identified in the oral solution,and the possible conversion relationships between their components were depicted.Fingerprint analysis of 11 batches of oral liquids showed that the composition of their main peaks was the same,with a similarity of more than 90%.Among them,6 iridoids(loganic acid,secologanoside,secologanic acid,sweroside,secoxyloganin,secologanin)and 6 phenolic acids(neochlorogenic acid,chlorogenic acid,cryptochlorogenic acid,isochlorogenic B,isochlorogenic A,isochlorogenic C)exhibited NF-κB inhibitory activity,which were the main pharmacological components and could be used as quality markers.The traceability of the above 12 quality markers was investigated in a multi-batch process based on the dual-wavelength HPLC method.The thermal stability studies of the raw materials revealed that the contents of their total iridoids and phenolic acids remained stable.Still,some of them would be transformed between components.The production process of the oral solution was stable,and the transfer rates of the iridoids and phenolic acids during the extraction,concentration and preparation were over 76%and 63%,respectively.Conclusion The method is stable,reliable,easy to operate and can evaluate the full honeysuckle oral solution process,which provides an effective means for the quality control of honeysuckle herbs and preparations.

ObjectiveTo evaluate the local failure and the impact on survival by prospectively comparing involved field radiotherapy (IFRT) and elective nodal irradiation (ENI) in combination with concurrent chemotherapy for locally advanced non-small cell lung cancer ( LA-NSCLC ).Methods LANSCLC patients were treated with 2 cycles of carboplatin ( AUC =5 - 6,d1 ) combined with paclitaxel ( 175mg/m2 ),followed assessment without distant metastasis,then randomized into IFRT (45 patients) or ENI (54 patients) arm.IFRT included primary tumor,ipsilateral hilar and positive mediastinal lymph nodes;ENI included the primary lesion,ipsilateral hilar,hilateral mediastinal lymph node drainage and bilateral supraclavicular area.The prescription dose was given as high as possible with V20 ≤35% and spinal cord dose ≤50 Gy,combined weekly paclitaxel 40 mg/m2 concurrent chemotherapy.The Kaplan-Meier method was used to estimate survival data and the log-rank method was used to test distribution of survival time between arms.ResultsThe follow-up rate was 99%.49,29 and 17 patients were followed-up for 1-,2-and 3-year,respectively.More patients from group IFRT received >60 Gy than ENI (49% vs.26%,x2 =5.59,P =0.018 ).The local failure rates were 29% and 36%,respectively ( x2 =0.46,P =0.497 ).The 1-,2-and 3-year local tumor progression-free survival rates were 76%,69%,65% and 80%,53%,49% ( x2 =0.74,P =0.389),respectively; the 1-,3-and 5-year overall survival rates were 80%,41%,33% and 69%,32%,13% (x2 =3.97,P =0.046),respectively.There were no significant differences in acute and late toxicities between the arms ( x2 =3.910 - 0.155,P =0.142 - 0.925 ).ConclusionsIFRT improved radiation dose and survival rate and did not increase the failure of elective lymph node region compared with ENI.The toxicities were no differences between IFRT and ENL Further investigation with big size sample is warranted.
8.Effects of Modified Xiaoyao Powder (逍遥散) on the Programmed Cell Death of Hypothalamic Dopaminergic Neurons in Rat Model of Hyperprolactinemia with Liver Depression and Spleen Deficiency

ObjectiveTo investigate the possible mechanisms of modified Xiaoyao Powder (逍遥散) in the treatment of hyperprolactinemia (HPRL) with liver constraint and spleen deficiency. MethodsNinety-six female SD rats were randomly divided into a normal group (n=16) and a modeling group (n=80). In the modeling group, rats were subjected to chronic unpredictable stress combined with intraperitoneal injection of metoclopramide to establish a rat model of HPRL with liver constraint and spleen deficiency. The 80 successfully modeled rats were randomly divided into a model group, a high, medium, and low-dose group of modified Xiaoyao Powder, and a bromocriptine group, with 16 rats in each group. The high, medium, and low-dose groups of modified Xiaoyao Powder were orally administered doses of 60, 30, and 15 g/(kg·d) respectively, the bromocriptine group was orally administered bromocriptine tablets at a dose of 1 mg/(kg·d), and the normal group and model group were orally administered 10 ml/(kg·d) of normal saline for 14 consecutive days. ELISA was used to detect serum prolactin (PRL) level; immunohistochemistry was used to determine the expression of tumor necrosis factor-alpha (TNF-α) and tyrosine hydroxylase (TH) in the hypothalamus; Western blot was used to detect the protein expression of tumor necrosis factor receptor 1 (TNFR1) in the hypothalamus; Real-time PCR was used to detect the mRNA expression of receptor interacting protein kinase 3 (RIP3) in the hypothalamus; immunofluorescence was used to detect the co-expression of RIP3 and dopamine neurons in the hypothalamus. ResultsCompared with the normal group, the serum PRL levels were increased in the model group, and the expression of hypothalamic TNF-α, TNFR1, RIP3 mRNA, and the co-expression of RIP3 with dopamine neurons were significantly increased, while TH expression was decreased (P<0.05 or P<0.01). Compared with the model group, the expression of hypothalamic TNF-α was decreased in the bromocriptine group and low-dose group of modified Xiaoyao Powder, and the expression of TH was significantly increased in the medium and high-dose groups of modified Xiaoyao Powder and the bromocriptine group. The serum PRL levels, hypothalamic TNFR1 and RIP3 mRNA expression, and the co-expression of RIP3 with dopamine neurons were significantly decreased in all dose groups of modified Xiaoyao Powder and the bromocriptine group (P<0.05 or P<0.01). Compared with the bromocriptine group, the serum PRL level were significantly increased in the high and low-dose groups of modified Xiaoyao Powder, TH expression was significantly increased in the medium-dose group of modified Xiaoyao Powder, hypothalamic RIP3 mRNA expression was decreased in the low-dose group of modified Xiaoyao Powder, and the co-expression of RIP3 with dopamine neurons was significantly increased in the high-dose group of modified Xiaoyao Powder (P<0.01). ConclusionModified Xiaoyao Powder can regulate the programmed cell death of hypothalamic dopamine neurons, affect DA expression, and regulate PRL levels, which may be one of its mechanisms in the treatment of HPRL with liver constraint and spleen deficiency.
9. Clinical analysis of endovascular repair with covered stent in the treatment of 40 patients with Stanford B type aortic dissection

Objective:
To evaluate the efficacy and safety of endovascular repair with covered stent in the treatment of Stanford B-type aortic dissection.
Methods:
The clinical data of 40 patients with Stanford B-type aortic dissection who underwent endovascular repair with covered stent in the People's Hospital of Suqian, Nanjing Drum-Tower Hospital Group from January 2012 to December 2017 were retrospectively analyzed.The incidence of complications before and after treatment, and its influence on the serum levels of IL-1β, INF-γ, VEGF were analyzed.After one year follow-up, the changes of quality of life score before and after surgery, and the mortality, reoperation or interventional treatment within one year after operation were compared.
Results:
Forty patients were successfully implanted with stent grafts, 5 cases of which had a small amount of endoleak, 1 case had postoperative cerebral infarction, and no complications such as paraplegia or renal failure were observed.Compared with before treatment, the serum levels of IL-1β, INF-γ and VEGF were significantly down-regulated after treatment[(3.24±0.19)ng/L vs.(2.02±1.16)ng/L, (2.65±0.23)ng/L vs.(1.14±1.27)ng/L, (1.51±0.17)ng/mL vs.(0.73±0.34)ng/mL], the differences were statistically significant(
10. Design and preliminary application of regional control and prevention auxiliary information system under the attack of COVID-19 infectious disease

Objective:
To propose the concept of a novel regional control and prevention (RCP) system for the outbreak of COVID-19 infectious disease, design an emergency epidemic prevention information system based on the existing network architecture and information system in the region, and a remote intelligent medical consultation and remote office platform, research and develop the technology of risk assessment and early warning for people in the region, and improve the regions’prevention and control ability facing emergency of major infectious diseases.
Methods:
Taking colleges, affiliated (teaching) hospitals, and cloud applications as typical RCP regional units, the existing local area network interaction methods between the cloud and universities and affiliated (teaching) hospitals are established to realize remote work in the network environment, remote medical imaging, psychological and ethical consultation and interaction; applying multi-agent propagation model based on complex network, combining Global Positioning System (GPS), Radio Frequency Identification (RFID), and electronic fence technology, to realize the risk classification and early warning of units and personnel in the area.
Results:
In the RCP, a system architecture combining campus network, affiliated (teaching) hospital intranet, and the Internet is used. Dynamic connection is made using distributed technology and cloud storage. The data buffer mechanism of the intermediary database in the network realized telemedicine consultation and telecommuting. Relying on the platform, multi-agent propagation model based on complex network and cellular automaton model are used to realize the score and early warning of population exposure risk in the region by using GPS, RFID and electronic fence technology.
Conclusions
In the epidemic phase of major infectious diseases, the construction of RCP can improve the response speed of wartime epidemic prevention, provide reasonable data-based warnings and risk ratings, and reduce the exposure risk of susceptible people. The design and development of RCP is a systematic project that needs to combine regional structural and functional characteristics, and the foundation of the early informatization work in the region and the level of the emergency development team determine the development progress, maintenance, and actual application effects. It is recommended to establish a peacetime and wartime combined RCP mode and incorporate it into the government's disease control system to improve the national and regional level of prevention and control of major infectious diseases.
