1.Therapeutic effects of tension-free vaginal tape and tension-free vaginal tape obturator in women with stress urinary incontinence:meta analysis

Objective To assess the effectiveness of tension-free vaginal tape obturator tape(TVT-O)as treatment of SUI by means of a systematic review and meta analysis.Methods Using the search terms"TVT,TVT-O,SUI,RCT,TOT",the literature in Chinese and English from January 2001 tO March 2007 on the difference of TVT and TVT-O was searched from MEDLINE,PUBMED,EMBASE,Google Scholar,CNKI,WAN FANG DATA,and criteria randomized controlled trials(RCTs)were studied by Meta-analysis in RevMan 4.2.At the same time,Ors of randomized model and fixed model were calculated tO evaluate the sensitivity.Results There were six RCTs that compared TVT-O with TVT.When compared by Subjective cure,TVT-O at 1-17 months were no better than TVT(OR 0.67;95% CI 0.40,1.13).Adverse events such as bladder injuries (OR 0.15;95% CI 0.03,0.66)was less common,where as groin/thigh pain(OR 8.61;95% CI3.03,24.52)was more common;but there were rio significant difference in de novo urgency(OR=1.16;95% CI 0.54,2.47),urinary retention(OR=0.54;95% CI 0.24,1.20)or urinary tract infection(OR=1.07;95% CI 0.61,1.87)between the tWO groups.Conclusions There is no significant difference between TVT and TVT-O.TVT-O group had no bladder injuries complications,but groin/thigh pain was more common.
2.Comparison of efficacy and safety between flexible ureteroscope lithtripsy and percutaneous nephrolithotomy for renal calculus

Objective To compare the safety and efficacy between flexible ureteroscope lithtripsy (FURL) and percutaneous nephrolithotomy (PCNL) in treating the renal calculus.Methods From Jan.2012 to Oct.2013,120 male and 87 female patients with renal calculus were accepted FURL (n =108)or PCNL (n=99) in our hospital.In FURL group,108 cases were classified according to the diameter of the stone,including 33 patients with the diameter of renal stone more than 20mm (range 20-39 mm,mean 29.2±5.6 mm)and 75 patients with the diameter of renal stone less than 20 mm (range 13-19 mm,mean 16.8± 1.3 mm).In PCNL group,99 cases were also classified according to the diameter of the stone,including 51 patients with the diameter of renal stone more than 20 mm (range 20-45 mm,mean 30.4±6.6 mm)and 48 patients with the diameter of renal stone less than 20 mm (range 14-19 mm,mean 17.2±1.4mm).There were no significant differences between the groups FURL and PCNL in the stone size(P>0.05).Demographic data,operative duration,postoperative hospital stay,complication rate,and stone-free rate were recorded and compared.Results The overall stone free rate in group FURL was significantly lower 80.6% (87/108) than that in group PCNL 91.9% (91/99) (P<0.05).In those patients with the diameter of stones less than 20 mm,the stone free rate was 88.0% (66/75) in group FURL vs 93.8% (45/48) in group PCNL (P>0.05).The operative time was 30-65 (mean 49.5±6.9) min and 30-65 (mean 46.9±7.2) min in FURL and PCNL group,respectively (P>0.05).In those patients with the diameter of stones more than 20 mm,the stone free rate was 63.6% (21/33) in group FURL,which was lower than that in group PCNL 90.2% (46/51) (P<0.05).The operative time was 60-115 min (85.0±16.3) min and 30-95 min (68.3± 16.7) min in FURL and PCNL group,respectively (P<0.05).The overall complication rate was significantly lower in group FURL 2.8% (3/108) compared to group PCNL 8.1% (8/99) (P<0.05).Postoperative hospitalization stay were 2-6 d (3.5±1.4) d and 3-9 d (5.8±1.9) d in FURL and PCNL group,respectively (P<0.05).Conclusions Both PCNL and FURS are efficacy and safe surgical alteration for patients with renal calculus.As to the FURS,its merits were faster recovery,less invasiveness than those in PCNL.Therefore,it can be considered as the first-line treatment for renal calculus <20 mm.However,for renal calculus ≥20 mm stones,FURS is still effective.
3.Effect of castration on miR-301a expression in a mouse xenograft model of human prostate cancer

Objective To investigate the changes of miR-301a and its host gene expression SKA2 in LNCaP prostate cancer xenografts in the castrated nude mice.Methods LNCaP cells were inoculated subcutaneously in nude mice to establish xenograft models of human prostate cancer.When the tumor volume grew to 200 mm3,the nude mice were randomly divided into the following 4 groups(n =6): 2 groups of nude mice to surgical castration,the other 2 groups of mice as control groups.The growth of those xnografts in nude mice was observed weekly and a growth curve of the xenografts was drawn.A point time during the process of tumor-regressing was selected when a group of castrated nude mice and a group of control mice were killed(the first time).The other 2 groups nude mice were continued to be observed.Another point time in the process of tumor re-growth,the rest castrated nude mice and control mice were killed(the second time).The tumors were weighted and the inhibitory rate was calculated.MiR-301a and SKA2 expression were detected by real-time PCR.Results The growth of the xenografts gradually decreased in LNCaP xenografts in nude mice after castration.After 13 days,the xenografls sizes decreased to(62.5 ±21.5)mm3 and tumor inhibitory rate was 59.8%(t =-3.895,P =0.018)in castration group of nude mice.At the 17th day after castration,tumor volume reached the minimum,and then gradually increased.At the 41st day after castration,tumor volumes in castration group increased to(364.5 ±97.3)mm3 and the tumor inhibitory rate was 62.2%(t =-7.017,P =0.002).MiR-301a and SKA2 in the first time of xenografts from the castrated group were both significantly lower than those of the control group(0.65-fold and 0.50-fold,respectively).However,their expressions in the second time of xenografts from the castrated group increased and were consistent with the control group(P > 0.05).Conclusions Castration could turn prostate cancer xenografts from androgen-dependent into androgen-independent.There could be a close correlation between the characteristic of prostate cancer androgen-dependent and the expressions of miR-301 a and host gene SKA2.
4.Effect of sarcopenia on skeletal muscle and cardiac function in elderly patients with chronic heart failure

Objective To investigate the effect of sarcopenia on the skeletal muscle and cardiac function in elderly patients with chronic heart failure (CHF).Methods Sixty patients with CHF and sarcopenia and 60 sex and age-matched CHF patients without sarcopenia were enrolled from September 2014 to December 2015.The skeletal mass was evaluated by fat-free mass index (FFMI) and muscle function was evaluated by gait speed (GS),hand strength (HS) and the simple physical performance battery (SPPB).The cardiac function was accessed by a 6-min walk distance (6-MWD) and left ventricular ejection fraction (LVEF).Furthermore,the serum inflammation cytokines IL-6,TNF-α,and skeletal muscle biomarker C 1q were measured.Results The CHF patients with sarcopenia had lower values for skeletal muscle mass:FFMI [(17.68±0.74) vs.(18.34±0.54)kg/m2,F=33.696,P<0.05] and lower muscle function:HS [(17.26±4.20)vs.(28.85±6.43)kg,F=136.54,P<0.05],GS [(0.65±0.11) vs.(0.90±0.10)m/s,F=-12.922,P<0.05],SPPB [(6.45±2.07) vs.(7.65± 1.76),t=-3.452,P<0.05].And the cardiac function decreased significantly in patients with sarcopenia:6-MWD [(253.76 ± 72.62) vs.(340.91 ± 55.78)m,F=54.350,P<0.05],LVEF [(39.12 ± 7.02)vs.(43.83±5.81)%,t=16.060,P<0.05].Serum IL-6/TNF-α/C1q levels were significantly elevated:IL-6[(14.12± 1.40) vs.(13.46±1.06) ng/L,F=8.513,P<0.05],TNF-α [(443.43±28.06) vs.(299.37±21.53)ng/L,t=31.556,P<0.05],C1q[(578.92±23.63) vs.(504.1 1±41.77)ng/L,F=145.78,P<0.05].Conclusion The CHF patients with sarcopenia present less skeletal muscle mass,poorer skeletal function and reduced cardiac function,and higher inflammation levels.
5.Clinical study of venous phase CT value in predicting central group lymph node metastasis of papillary thyroid carcinoma

Objective:To investigate the clinical value of imaging features of primary lesions combined with venous phase CT value in predicting central group lymph node (LN) metastasis in patients with papillary thyroid carcinoma (PTC) .Methods:Clinical data of 170 PTC patients who underwent central group LN dissection in the First People's Hospital of Handan City of Hebei Province from January 2017 to June 2020 were retrospectively analyzed. All patients were divided into different groups according to whether central group LN metastasis or not, and there were 89 patients with central group LN metastasis and 81 patients without central group LN metastasis. The CT value and imaging features of primary lesions in different periods were analyzed, and the imaging features of primary lesions combined with venous phase CT values to predict the central group LN metastasis were evaluated by the receiver operating characteristic (ROC) curve.Results:There were no statistically significant differences in CT value in plain scan phase and CT value, net increased CT value, standardized CT value in arterial phase between patients with and without central group LN metastasis (all P>0.05) . The CT value, net increased CT value and standardized CT value in venous phase of patients with central group LN metastasis were (113.84±22.95) HU, (59.05±12.10) HU and 0.72±0.14 respectively, which were significantly higher than those of patients without central group LN metastasis [ (103.99±17.67) HU, (51.29±14.45) HU and 0.59±0.10] ( t=3.26, P<0.001; t=3.81, P<0.001; t=3.67, P<0.001) . ROC curve analysis showed that the area under the curve for diagnosing central group LN metastasis of PTC patients was 0.75, 0.70 and 0.76 when the cut-off values of CT value, net increased CT value and standardized CT value in venous phase were 115.78 HU, 62.37 HU and 0.75 respectively. There were statistically significant differences in the diameter of primary focus and the contact area of thyroid capsule between patients with and without central group LN metastasis ( Z=-2.34, P=0.019; Z=-2.08, P=0.037) . There were no statistically significant differences between calcification and primary lesion location (both P>0.05) . Lesion diameter >2 cm (87.73%) and capsule contact range ≥1/2 (92.17%) had the highest specificity in predicting central group LN metastasis. The imaging features of primary lesion combined with standardized CT value in venous phase was in good agreement with histopathological diagnosis results in predicting central group LN metastasis (Kappa=0.475) , and the sensitivity and specificity were 73.12% and 82.75% respectively. Conclusion:The imaging features of the primary lesion combined with CT value in venous phase have a good clinical value in predicting central group LN metastasis in PTC patients. Patients with primary lesion diameter >2 cm, capsule contact range ≥1/2 and the standardized CT value in venous phase >0.75 are more likely to have central group LN metastasis.
6.Single-cell transcriptome analysis of multigrade glioma heterogeneity and immune microenvironment revealed potential prognostic biomarkers.

Glioma, the most common intrinsic tumor of the central nervous system, is characterized by its high incidence and poor prognosis. The aim of this study was to identify differentially expressed genes (DEGs) between glioblastoma multiforme (GBM) and low-grade glioma (LGG) to explore prognostic factors of different grades of gliomas. Single-cell transcriptome sequencing data of gliomas were collected from the NCBI Gene Expression Omnibus (GEO), which included a total of 29 097 cell samples from three datasets. For the analysis of human gliomas of different grades, 21 071 cells were obtained by filtering, and 70 genes were screened from differentially expressed genes by gene ontology (GO) analysis, Kyoto encyclopedia of genes and genomes (KEGG) pathway analysis, from which the gene DLL3 was focused by reviewing the literature. The TCGA-based gene expression profiling interactive analysis (GEPIA) database was used to explore the survival curves of genes in LGG and GBM, and the gene expression profiling interactive analysis and tumor immune estimation resource (TIMER) database was used to study the expression of key genes in gliomas of different grades, predicting biomarkers that were closely related to immunotherapy. The cBioPortal database was used to explore the relationship between DLL3 expression and 25 immune checkpoints. Gene set enrichment analysis (GSEA) further identified pathways associated with central genes. Finally, the efficacy of biomarkers in prognosis and prediction was validated in the Chinese glioma genome atlas (CGGA). These results demonstrated that prognostic genes are associated with tumor proliferation and progression. Analysis of biological information and survival suggested that these genes might serve as a promising prognostic biomarker and as new targets for selecting therapeutic strategies.

7.Analysis of progress characteristics of retinoblastoma based on single cell transcriptome sequencing.

Retinoblastoma (RB) is the most common intraocular malignant tumor in infants and young children. The key causative factors in the progression of RB remain unclear. Therefore, identifying genes closely associated with RB progression may provide important clues for disease diagnosis and gene therapy. However, tumor tissues have strong cellular heterogeneity. There may be significant differences in cell function and gene expression among cells in different pathological states. In this study, we downloaded single-cell transcriptome sequencing data of RB tumors and adjacent tissues from the GEO public database. Subsequently, we analyzed RB tumor transcriptional profiles with different disease duration at the single-cell level and identified cell groups and gene sets potentially associated with RB progression. The results showed that the tumor tissue and the adjacent tissues had overall consistency in the single-cell transcriptional map, but there were obvious differences in the distribution proportions of G1 phase cells, G2 phase cells, and microglia cells of cone precursors in RB tumor and the adjacent tissues. Furthermore, the role of three cell populations in the progression of RB tumors was emphatically analyzed. We found that in the early stage of RB tumors, cone precursor cells proliferated abnormally in G1 phase. With the progression of RB tumors, the proportion of cone precursor cells in G2 phase increased significantly. Meanwhile, the results of differential analysis of microglial populations during RB progression showed that the key genes mainly involved in immune response include RPL23, B2M, and HLA superfamily genes. This study provides new perspectives and data resources for the research of RB pathogenesis and progress.

8.A new class of potent liver injury protective compounds: Structural elucidation, total synthesis and bioactivity study.

A new class of potent liver injury protective compounds, phychetins A-D ( 1- 4) featuring an unique 6/6/5/6/5 pentacyclic framework, were isolated and structurally characterized from a Chinese medicinal plant Phyllanthus franchetianus. Compounds 2- 4 are three pairs of enantiomers that were initially obtained in a racemic manner, and were further separated by chiral HPLC preparation. Compounds 1- 4 were proposed to be originated biosynthetically from a coexisting lignan via an intramolecular Friedel-Crafts reaction as the key step. A bioinspired total synthesis strategy was thus designated, and allowed the effective syntheses of compounds 2- 4 in high yields. Some of compounds exhibited significant anti-inflammatory activities in vitro via suppressing the production of pro-inflammatory cytokine IL-1β. Notably, compound 4, the most active enantiomeric pair in vitro, displayed prominent potent protecting activity against liver injury at a low dose of 3 mg/kg in mice, which could serve as a promising lead for the development of acute liver injury therapeutic agent.
9.Innovative insights into extrachromosomal circular DNAs in gynecologic tumors and reproduction.

Originating but free from chromosomal DNA, extrachromosomal circular DNAs (eccDNAs) are organized in circular form and have long been found in unicellular and multicellular eukaryotes. Their biogenesis and function are poorly understood as they are characterized by sequence homology with linear DNA, for which few detection methods are available. Recent advances in high-throughput sequencing technologies have revealed that eccDNAs play crucial roles in tumor formation, evolution, and drug resistance as well as aging, genomic diversity, and other biological processes, bringing it back to the research hotspot. Several mechanisms of eccDNA formation have been proposed, including the breakage-fusion-bridge (BFB) and translocation-deletion-amplification models. Gynecologic tumors and disorders of embryonic and fetal development are major threats to human reproductive health. The roles of eccDNAs in these pathological processes have been partially elucidated since the first discovery of eccDNA in pig sperm and the double minutes in ovarian cancer ascites. The present review summarized the research history, biogenesis, and currently available detection and analytical methods for eccDNAs and clarified their functions in gynecologic tumors and reproduction. We also proposed the application of eccDNAs as drug targets and liquid biopsy markers for prenatal diagnosis and the early detection, prognosis, and treatment of gynecologic tumors. This review lays theoretical foundations for future investigations into the complex regulatory networks of eccDNAs in vital physiological and pathological processes.

10.Pharmacodynamic study of sinapine thiocyanate dissoluble microneedle for acupoint administration against bronchial asthma

OBJECTIVE To study the efficacy of sinapine thiocyanate dissoluble microneedle (ST-DMN) for acupoint administration against bronchial asthma (BA). METHODS The network pharmacology and molecular docking techniques were used to screen the core targets of sinapine thiocyanate (ST) against BA, and the pharmacodynamics of the top 3 core targets was studied. Firstly, ST-DMN was prepared (drug loading of ST was 1 mg/tablet); secondly, 30 rats were divided into blank control group, model control group, blank microneedle group, Sinapine powder plaster group (positive control group) and ST-DMN group. Except for the blank control group, rats of other groups were sensitized with 10% ovalbumin (containing aluminum hydroxide adjuvant) and nebulized with 1% ovalbumin to induce the BA model. After modeling, blank control group did not receive any intervention; normal saline was applied to the Feishu acupoint and Dazhui acupoint of the rats in the model control group, while the blank microneedle group, Sinapine powder plaster group and ST-DMN group were given blank microneedle, Sinapis alba powder (plaster, 1.5 g) and ST-DMN (3 tablets at 2 acupoints) at same acupoint, once a day, for 28 consecutive days. After administration, the general symptoms were observed and the body mass of the rats was measured.pathological changes of lung tissues in rats was observed; the levels of prostaglandin endoperoxide synthase 2 (PTGS2), GNYL matrix metalloproteinase-9 (MMP-9) and interleukin-2 (IL-2)in serum, bronchoalveolar lavage fluid (BALF) and lung tissues were determined. RESULTS Results of network pharmacology and molecular docking showed that the key targets of ST against BA were identified as PTGS2, MMP-9, IL-2, epidermal growth factor receptor, heat shock protein90AA1, etc. Pharmacodynamic experiments showed that compared with model control group, relieved cough, restored hair color, sensitive behavior, stable respiration and increased body weight were all found in ST-DMN group; the histopathological changes as the structure of lung tissue, infiltration of alveolar epithelial cells and pulmonary interstitial inflammatory cells were improved to different extent; the levels of PTGS2, MMP-9 and IL-2 in serum, BALF and lung tissue were significantly reduced (P<0.05 or P<0.01). CONCLUSIONS The anti-BA effect of ST-DMN acupoint administration is good, the mechanism of which may be associated with decreasing the levels of PTGS2, MMP-9 and IL-2 in serum, BALF and lung tissue.
