1.Clinical study of dexmedetomidine combined with propofol on double-balloon enteroscopy

Objective To explore the efficacy and safety of dexmedetomidine combined with propofol on double-balloon enteroscopy.Methods Forty cases of patients ASA Ⅰ-Ⅱ grade who underwent double-balloon enteroscopy were divided into dexmedetomidine combined with propofol group (combine group) and propofol group by random digits table with 20 cases each group.Combine group was given load 0.7 μg/kg (intravenous infusion for 10 min) before induction and 0.2 μg/ (kg·h) continuous infusion during the surgery.All the patients were used propofol by target concentration with 2.5-4.0 mg/L for target-controlled infusion heart rate (H R),mean arterial pressure (MAP),respiratory rate (RR),peripheral oxygen saturation (SpO2) were recorded before examination(T0),eye lash reflex time(T1),during the perform (T2),the end of check (T3),sedation score,induction time,recovery time,total propofol amount,rates of adverse reaction and satisfaition were recorded.Results The induction time,recovery time was no statistically significant difference between two groups (P > 0.05).The RR,SpO2 was no statistically significant difference between two groups at different time points (P >0.05).The HR at T1,T2 in combine group was significantly lower than that at T0 and in propofol group same period[(65.8 ± 7.3),(68.6 ± 8.2) times/min vs.(84.6 ± 7.1) and (77.6 ± 7.2),(89.6 ± 8.4) times/min,P < 0.05].The MAP at T1 in combine group was significantly higher than that at T0 and in propofol group same period [(88.9 ± 9.4) mm Hg (1 mm Hg =0.133 kPa) vs.(81.3 ± 4.5) and (73.5 ± 6.8) mm Hg,P < 0.05],at T2 was significantly lower than that in propofol group [(80.6±6.6) mm Hgvs.(88.5 ±7.6) mm Hg,P<0.05].Sedation score 1 score 18 cases,2 scores 2 cases in combine group; 1 score 3 cases,2 scores 17 cases in propofol group,the difference was statistically significant (P < 0.05).Combine group apnea two cases,moving,choking three cases,propofol group were eight cases and seven cases,the difference was statistically significant (P < 0.05).The total propofol amount in combine group was significantly lower than that in propofol group [(421 ± 76) mg vs.(638 ± 89) mg,P < 0.05].Conclusion Dexmedetomidine composite target controlled infusion of propofol used for double-balloon enteroscopy can produce good narcotic analgesic which is safe and effective anesthesia.
2.Measurement of Urinary S100B Protein Concentrations for the Early Evaluation of Brain Damage in Preterm Infants

Objective: To investigate the changes of urinary S100B protein concentrations and their relationship with brain damage in preterm infants there of . Methods: The urinary S100B protein of 84 preterm infants and 26 full term infants, which were used as control, were measured at 24 h and 120 h after birth. At the same time, routine clinical observations, neurologic patterns and ultrasound screens were recorded. The value of urinary S100B protein and brain damage were evaluated in preterm infants with different gestational age. Results: The differences of urinary S100B protein were statistical significance between the different gestations. The levels of urinary S100B protein were higher in preterm infants, whose gestations were lower than 32 W, than those of other groups. The levels of S100B protein were significantly higher in samples of 27 peri-intraventricular hemorrhage (PIVH) and 3 peri-ventricular leukomalacia(PVL) than those in samples without brain damage( P < 0.05). The S100B levels were significantly higher in urine of 10 preterm infants with polycethemia than those in infants without brain damages. In addition, the S100B levels were different in urine of preterm infants with different prognosis. The S100B levels were significantly higher in urine of infants who died or deteriorated than those of others(P < 0.05). Conclusion: There is an evident trend of decrease in urinary S100B protein concentration with increasing gestational age. It will be helpful to identify preterm infants with PIVH,PVL and high risk of brain damages by measurement of S100B protein in urine early after birth, which indicates further inspection, provides protective treatment and enhances follow up.

4.Temperature and humidity monitoring system of imaging equipment room based on wireless network.

This paper presents a wireless temperature and humidity control system for hospital's video room. The system realizes one to multiple communication using wireless communication module CC1020 and SHT11 as sensors, and then sets up the communication between system and the central station with serial communication controller MSCOMM. The system uses VISUAL C++ programming to realize the video room temperature and humidity alarm control. It is wireless, efficacious and manpower-efficient.

5.Enhanced recovery after surgery in total laparoscopic radical gastrectomy

Objective To investigate the effect of enhanced recovery after surgery (ERAS) on immune function and postoperative recovery in gastric cancer patients undergoing total laparoscopic radical gastrectomy.Methods Patients were randomly divided into ERAS group and control group.Blood CD4 +,CD8+,CD4 + CD25 +,C-reactive protein,postoperative recovery and complications were compared between the two groups.Results On day1,CD4 +,CD8 +,CD4 + CD25 + in the two groups were significantly lower than those before surgery (t =9.070,7.297,5.830,12.870,3.529,10.547,all P<0.05).The ERAS group had higher CD8 +,CD4 + CD25 + levels than the control group (t =2.163,2.203,P < 0.05).On day3,CD4 + CD25 + in ERAS group was not different from that before surgery (t =1.062,P > 0.05) while the other indexes in the two groups raised but still were lower than preoperative level (t =3.322,5.015,3.418,9.912,all P <0.05);CD4 +,CDs +,CD4 + CD25 + in ERAS group were higher than control group (t =2.804,2.040,2.210,all P<0.05).On day5,CD4+,CD4 + CD25+ in the two groups and CDs+ in ERAS group returned to the preoperative level,while CDs + of the control group was still lower than the preoperative level (t =6.862,P <0.05).On day1,3 and 5,the C-reactive protein levels of the two groups were higher than those before surgery(t=-13.338,-13.715,-11.319,-12.286,-13.182,-15.076,all P < 0.05),and ERAS group were lower than the control group (t =-3.246,-2.100,-2.211,all P<0.05).There was no mortality in neither groups.The time of passage gas by anus,defecation,getting out of bed,oral feeding,and postoperative hospital stay in the ERAS group were less than thoseinthecontrolgroup[(2.8±1.0)dvs.(3.9±0.9)d,t=-5.974;(3.8± 0.9)d vs.(4.3±1.0)d,t=-2.700;(19.1 ±4.0)hvs.(35.9±6.6)h,t=-16.045;(9.9 ±1.6)d vs.(11.5±2.0) d,t =-4.479,all P < 0.05].Conclusions ERAS in the perioperative period of patients with total laparoscopic radical gastrectomy mitigates the stress on the cellular immune system,reduces inflammatory response,and help fast recover the postoperative gastrointestinal function.
6.Influence of citalopram on anxiety and depression of patient with functional dyspepsia

Objective To observe the influence of citalopram on anxiety and depression of patient with functional dyspepsia (FD).Methods One hundred and thirty-two FD patients were randomly divided into treatment group (n =68)and control group (n =64).The patients in con-trol group were given omeprazole and mosapride,while the patients in the treatment group were added citalopram based on the treatment in the control group,and meanwhile given corresponding psychology guidance.The anxiety and depression disorders were evaluated respectively by Self-rat-ing Depression Scale (SDS)and Self-rating Anxiety Scale (SAS).The symptomatic scores before treatment and on the second,fourth and eighth week of treatment,clinical efficiency and adverse reactions were observed in the two groups.Results Among 132 patients,there were 60 with de-pression and 53 patients suffering from anxiety.SDS and SAS scores on on the second,fourth and eighth week of treatment and FD symptomatic scores on the fourth and eighth weeks of treatment were obviously lower,while the overall response rate was significantly higher in treatment group than that in the control group.So the difference was statistical significant.Before and after treat-ment,there were no abnormal patients in two groups.Conclusion FD patients are usually accom-panied by anxiety and depression.The application of citalopram on the basis of routine treatment could significantly ameliorate the patients′anxiety and depression,alleviate symptoms,improve the efficiency and reduce adverse reactions.

8.Epidemiological characteristics and spatial clustering analysis of brucellosis in Shandong Province from 2015 to 2020

Objective:To learn about the epidemiological and spatial clustering characteristics of brucellosis in Shandong Province, and to provide a reference for scientific prevention and control of brucellosis.Methods:The epidemic data of human brucellosis in Shandong Province from January 2015 to December 2020 were collected from the Infectious Disease Reporting Information Management System of China Disease Control and Prevention Information System, and the data were analyzed by descriptive epidemiology and spatial clustering analysis.Results:A total of 18 811 cases of human brucellosis were reported in Shandong Province from 2015 to 2020, and the average annual incidence rate was 3.16/100 000. Human brucellosis occurred in every month of the year, and the peak incidence was from March to August, accounting for 66.31% (12 474/18 811). The top 5 counties (districts) with average annual incidence rates were Lijin County (32.39/100 000), Kenli District (11.02/100 000), Wudi County (10.35/100 000), Zhanhua District (9.59/100 000) and Shanghe County (8.80/100 000). There were 13 436 males and 5 375 females, with a male-female sex ratio of 2.50 ∶ 1.00; the age was mainly concentrated in 30-69 years old, accounting for 83.23% (15 656/18 811); farmer was the main occupation, accounting for 85.82% (16 144/18 811). The results of global spatial autocorrelation analysis showed that the annual incidence rates of brucellosis in Shandong Province showed a spatial clustering distribution from 2015 to 2020; and the local spatial autocorrelation analysis showed that the high incidence of human brucellosis was mainly concentrated in the north of Shandong Province.Conclusions:The incidence of brucellosis in Shandong Province is mainly concentrated in spring and summer, most of them are farmers, and the high incidence areas have spatial clustering. Key prevention and control measures should be taken for high incidence seasons, high-risk population and northern high clustering areas to reduce the incidence of brucellosis.
9.Application evaluation of detection system based on microfluidic technology in initial screening of blood donors

【Objective】 To evaluate the screening efficacy and practical value of the portable microfluidic biochemical analyzer in the detection of blood donors before blood donation. 【Methods】 Blood donor samples, clinical blood samples and constant quality control products were collected. Referring to the documents of ISO15189 and National Health Industry Standard, the precision and accuracy of hemoglobin (Hb) and alanine aminotransferase (ALT) were verified and compared with other detection systems. 【Results】 The MS200 biochemistry instrument has an intra-batch precision of 1.40% to 1.46%, inter-batch precision of 1.91% to 1.94%, and correctness bias of -0.9% to -1.3% for Hb test, and an intra-batch precision of 3.77% to 4.86%, inter-batch precision of 4.92% to 6.02%, and correctness bias of -3.0% to -4.8% for ALT test, which were within the range of quality requirements of industry standard. Comparison of Hb test results between MS200 biochemistry and Hb201 analyser on 1 189 peripheral blood samples from donors showed no statistically significant difference (P>0.05). 65 samples showed positive correlation between MS200 biochemistry and XS-900i automated hematology analyzer on Hb test results (R2=0.986, P=0.000). Correlation analysis of all the results of ALT detection by MS200 biochemical analyzer and AU480 biochemical analyzer in 1 065 samples showed a positive correlation (R2=0.965, P=0.000). The elevated ALT samples did not affect the Hb test results, and the samples with abnormal Hb value did not affect the ALT test results, with no interference between the two items in the detection. 【Conclusion】 The MS200 biochemical analyzer based on microfluidic technology has reliable methodological performance and can meet the need of pre-donation testing.
