Objective To construct the assessment index system for general nursing goals management based on the performance structure theory. Methods Expert interviews and Delphi expert enquiry were used to complete the construction of index system, and SPSS17.0 was used to do data statistics and reliability analysis. Results The assessment index system for general nursing goals management was constructed which contained 6 level-Ⅰindicators such as nursing quality, nursing safety, nursing service, teaching and training, the contribution of department and the communication skills and 22 level- Ⅱ indicators. The department monthly examination scores calculation method and feedback mechanism were also established. The expert positive coefficients were 97.6%and 98.8%respectively, the authority coefficients were 0.85 and 0.86, the importance and feasibility mean scores of every index were 3.86-4.98 and 3.98-4.88, and coefficients of variation were 0.03-0.27 and 0.05-0.25 in 2 rounds of enquiry. The importance of level-Ⅰor level-Ⅱ indicators and operability coordination coefficients were 0.10-0.36, the significance test showed P<0.05. Conclusions The design of each dimension of the index system gives full consideration to 3 dimensions of performance: task-adaptive-relationship. Representative of key indicators is strong. The Delphi method used in building is scientific and the results are reliable. The maneuverability of calculate Method is strong, easy to quantify. The feedback mechanism has a strong incentive by combining the assessment results closely with the individual interests. The establishment of the index system can provide scientific and objective basis for the full implementation of the general nursing goals management.

Objective To investigate the relationships between different dietary patterns and serum uric acid in Tianjin residents. Method By cross-sectional study design, 11282 subjects were studied via lifestyle questionnaires (including a valid food frequency questionnaire), physical examination and laboratory tests. Using factor analysis method to establish the dietary patterns, and relationships between different dietary patterns and serum uric acid were analyzed. Results Factor analysis showed 3 kinds of dietary patterns, including eastern dietary pattern, vegetable diets and Tianjin dietary pattern. There were more males in hyperuricemic population, who have a higher BMI, waist circumference, blood lipids, blood pressure and blood sugar levels, and also higher proportion of metabolic syndrome, smokers and alcohol drinkers. In addition, hyperuricemia patients had a higher level of social status and income. After adjustments for age, sex, BMI, smoking habits, alcohol consumption, hypertension, hyperlipidemia, education level, occupational status, energy intake, physical activity and metabolic syndrome, logistic regression analysis showed the odds ratio (95%confidence interval) for hyperuricemia across the quartiles of 3 dietary patterns as follows: 1.00, 1.07 (0.91, 1.25), 0.96 (0.82, 1.14), 1.12 (0.94, 1.33);1.00, 1.03 (0.88, 1.21), 1.04 (0.88, 1.23), 1.00 (0.84, 1.19); 1.00, 1.28 (1.08, 1.52), 1.40 (1.22, 1.71), 1.37 (1.15, 1.63), respectively. Significantly positive correlation was shown between Tianjin dietary pattern and hyperuricemia. Conclusion Tianjin residents' Tianjin dietary pattern is closely related to serum uric acid levels. Tianjin dietary patterns have characteristics of high calorie, high fat, high protein. Healthy eating habits have significant importance to prevent occurrence of hyperuricemia.

Coffee administration has been shown to increase maximum voluntary contraction and muscular endurance. However, no study has addressed the association between daily consumption of coffee with muscular function. This cross sectional study aimed to investigate the association between frequency of coffee consumption (CC) in daily life and muscle strength among a population of working adult men. This cross sectional study included men (n = 516, mean age: 47.1) who participated in an annual health examination conducted in Sendai. The monthly frequency of coffee consumed was reported by a brief-type self-administered dietary history questionnaire. Leg extension power (LEP) was measured as an index of muscle strength. Subjects were divided into tertile levels according to CC. Analysis of covariance was used to examine the relation between the tertile levels of CC and LEP. After adjustment for potential confounders (including all lifestyle factors), the adjusted means (95% confidence interval) of LEP across the tertiles of CC were 17.4 (16.6 - 18.1) for the lowest tertile, 17.9 (17.4 - 18.5) for the mid tertile, and 18.9 (18.3 - 19.5) for the highest tertile (p for trend = 0.007). Higher frequency of monthly coffee consumption was associated with higher muscle strength.

A DNA fragment encoding human interleukin 4 was obtained by PCR from pORF-hIL4 plasmid. The amplified fragment was inserted into prokaryotic expression vector PQE60 and recombinant protein was expressed in E. Coli M15 by adding isopropyl-beta-D-thiogalactoside (IPTG). The hIL-4 protein was present as insoluble inclusion bodies in the bacterial extract. After denaturation of inclusion bodies with 5 mol/L guanidine hydrochloride, the supernate was diluted to get renaturized. Then dialysis and Ni chelating chromatography were used for purification. TF-1 proliferation assay of recombinant human interleukin 4 was performed, and then rhIL-4 was fit to be used for proliferation of human dendritic cells from monocyte in vitro.

In order to effectively manage the atrial fibrillation patients, West China Hospital has established an integrated general-specialty management model within medical consortium. This model takes the atrial fibrillation-stroke integrated management outpatient clinic as the platform, combines general practice and specialty to provide standardized care for atrial fibrillation patients. The model was characterized by primary diagnosis, two-way referral, up and down linkage, and differential management for acute and chronic conditions. This article, taking West China Hospital as an example, introduces the integrated team of cardiologists and general practitioners in the tertiary hospital with general practitioners in the community, and the preliminary accomplishment in the management of atrial fibrillation within the medical consortium. It would provide a reference for the long-range management of atrial fibrillation in other provinces and cities.

Objective:To investigate the awareness of digestive system injury caused by coronavirus disease 2019 (COVID-19) in gastroenterologists. Methods:From February 21 to 23 in 2020, the electronic questionnaire was used to learn about the condition of the basic knowledge of COVID-19 and awareness of digestive system injury caused by COVID-19 among the gastroenterologists across the country. Chi-square test was used for statistical analysis. Results:A total of 2 216 gastroenterologists from 31 provinces, autonomous regions and municipalities nationwide completed the survey. 99.7% (2 209/2 216) of gastroenterologists stated that they had read the COVID-19 diagnosis and treatment guidelines. The percentages of physicians who knew clearly about the diagnostic criteria of suspected and confirmed cases of COVID-19 was 34.9% (774/2 216) and 39.4% (874/2 216), respectively. The percentages of physician who gave the right answers of COVID-19 detectable methods and pulmonary imaging was 68.4% (1 516/2 216) and 71.6% (1 586/2 216), respectively. The percentages of correct answers of digestive system injury caused by COVID-19 were physicians and chief physicians was 30.9% (134/433) in residents, 33.9% (234/691) attending physicians, 32.4% (213/657) associated chief physicians and 34.9% (152/435) chief physicians, respectively, however there were no statistically significant differences among physicians with different professional titles ( χ2=6.60, P> 0.05). 95.6% (2 119/2 216) of gastroenterologists believed that probiotics could effectively improve bowel function, and 94.0% (2 082/2 216) of gastroenterologists considered that enteral nutrition support could improve patients’ prognosis. Conclusions:The knowledge and dynamic progress of the digestive system injury caused by COVID-19 are still insufficiently grasped by gastroenterologists in China. So it is necessary to carry out systematic and pertinent training programmes for them.
