1.Effect of computer navigation gap balance technology on lower limb recovery after total knee arthroplasty.

OBJECTIVE:
To investigate the effect of computer navigation gap balance technology on the recovery of lower limb function after total knee arthroplasty.
METHODS:
The clinical data of 106 patients with knee osteoarthritis (OA) who underwent total knee arthroplasty from July 2018 to June 2019 were analyzed retrospectively. They were divided into measurement osteotomy group and space balance group according to different osteotomy techniques during total knee arthroplasty. There were 61 cases in osteotomy group, 24 males and 37 females;The age ranged from 45 to 77(63.35±4.26) years;According to K-L classification, 41 cases were grade Ⅲ and 20 cases were grade Ⅳ. intraoperative measurement osteotomy was performed. There were 45 cases in the gap balance group, 17 males and 28 females;Age 45 to 78(64.03±4.31) years;According to K-L classification, 29 cases were classified as grade Ⅲ and 16 cases as grade Ⅳ. computer navigation gap balance technology was implemented. The amount of intraoperative bleeding, operation time, incision length, hospital stay and postoperative complications were compared between two groups. The clinical efficacy was evaluated by Knee Society score(KSS) before operation and 12 months after operation.
RESULTS:
Total of 106 patients were followed up for 12 to 18(20.38±3.25) months. There were significant differences in intraoperative bleeding and operation time between two groups(P<0.05). There was no significant difference in incision length and hospital stay between the two groups(P>0.05). At 12 months after operation, the total score of KSS in the gap balance group (173.59±14.50) was better than that in the osteotomy group (164.95±12.10)(P<0.05). There were no serious complications of poor prosthesis loosening between two groups during follow-up, and there was no significant difference in the incidence of other complications between two groups(P>0.05).
CONCLUSION
The application of computer navigation gap balance technology in total knee arthroplasty is conducive to the recovery of lower limb function in patients with OA, and there are no serious adverse complications and high safety.

2.Investigation on polycyclic aromatic hydrocarbons (PAH4) in domestic edible vegetable oil sold in Henan Province in 2019 - 2020

Objective To investigate the contamination of four polycyclic aromatic hydrocarbons (PAHs) in domestic pre-packaged edible vegetable oil sold in Henan Province. Methods A total of 212 domestic edible vegetable oil samples from Henan Province were collected. The contents of four polycyclic aromatic hydrocarbons (benzo[a]anthracene, chrysene, benzo[b]fluoranthene, and benzo[a]pyrene, defined as PAH4) were determined by HPLC and gas chromatography-mass spectrometry in 2019 and 2020, respectively. Results The content of PAH4 in domestic pre-packaged edible vegetable oils ranged from 0.34 μg/kg to 26.6 μg/kg, the detection rate was 83.49%, and the over-standard rate was 3.30% according to EU standard. In terms of food category, the peanut oil samples were most seriously contaminated with PAH4. From the point of view of product grade and production process, with the decrease of sample quality index, the risk of PAH4 contamination in sesame oil increased. According to GB 2762-2017, the content of benzo (a) pyrene in all samples was qualified, while the peanut oil had the highest rate exceeding the EU standard, with the over-standard rate of 38.46%. Peanut oil had a higher risk of being contaminated by benzo [a] pyrene. Conclusion Peanut oil in domestic pre-packaged edible vegetable oil sold in Henan Province has a high risk of PAH4 contamination. With the decrease of sesame oil quality index, the risk of PAH4 contamination increases.
3.Significance of Troponin-I in patients with septic shock and left ventricular diastolic dysfunction

Objective To investigatethe clinical value of troponin-I(cTnI)in patientswith septic shocka-nd left ventricular diastolic dysfunction. Methods As a retrospective analysis ,38 patients with left ventricular di-astolic dysfunction and septic shock(Sa group),as well as 20 patients with normal cardiac function(Sn group) were enrolled in this study. Moreover ,20 patients with left ventricular diastolic dysfunction and without septic shock were used as control group(Ca group). The ratio of early diastolic mitral inflow velocity to early diastolic mi-tral annulus velocity(E/e′)was measured as the evaluation index of left ventricular diastolicfunction by echocar-diography within 72 hours after admission to ICU. Level of cTnI was detected in all cases and the relationship was evaluated by E/e′. Receiver operating characteristic curve(ROC)was constructed to indicate the predictable value of left ventricular diastolic dysfunction in patients with septic shock. Results The level of cTnI was significantly elevated in both Sa group and Sn group(P<0.05),while the level of cTnI and E/e′in Sa group were significantly higher than those in Sn group(P < 0.05). cTnI was positively correlated with E/e′(r = 0.367 ,P = 0.004). The area under the curve(AUC)of cTnI was 0.834,with the cut-off value of 0.49 ng/mL(sensibility=77.6,specificity=80.7). Conclusion The level of cTnI was significantly higher in patients with septic shock. cTnI was significantly correlated to left ventricular diastolic dysfunction in patients with septic shock. cTnI ≥ 0.49 ng/mL could be an available predictor for left ventricular diastolic dysfunction in patients with septic shock.
4.Effects of chronic alcohol exposure on hippocampus and cerebral cortex neurons in mice

This study was performed to investigate the changes of the number, morphology and ultrastructure of the central nervous system of mice during the long-term alcohol exposure. Mice at 60 days in age were used to establish the long-term alcohol exposure model. The structure of the central nervous system, such as nuclear antigen, dendritic spines and synapses, were labeled by the methods of immunocytochemistry and DiI (1, 1'-dioctadecyl-3, 3, 3', 3'-tetramethy lindocarbocyanine perchlorate) scattering. The results showed that prolonged alcohol exposure could promote apoptosis of nerve cells in the central nervous system, and inhibit the proliferation of neural stem cells, which reduced the number of nerve cells in the central nervous system. Long-term ethanol exposure can also lead to a decrease in the density of dendritic spines of neuron, a smaller number of synapses (connections between nerve cells), and some changes in synaptic ultrastructure. The density of nerve cells and their dendritic spines, as well as the changes of synaptic ultrastructure, suggest that the function of nerve cells may be low.

7.Related factors and countermeasures of surgical site infections for department of general surgery in Chengde Medical Hospital in 2015

Objective To assess the impact factors of surgical site infection(SSI) in the department of general surgery,the improve the quality of the target of monitoring,provide clinical theoretical basis for reducing the incidence of SSI.Methods In 2015,920 patients who underwent general surgery was took in the targeted monitoring of SSI.SPSS19.0 software was used to analyzing the data.Results The infection rate was 4.35%;Surgical site infection rate was rising,with the increase of NNIS.17 pathogens were isolated,including 11 Escherichia colis which was the most.The incidence of the SSI was 2.40% between two groups in the patients who underwent the elective surgeries 10.85%,in the patients who underwent emergency surgery.there was significant difference between two groups(x2 =27.997,P<0.05).The type Ⅱ surgical incision was smain type in the department of general surgery,the incidence of the typeⅡ surgical incision was 2.27%,the incidence of the typeⅢ surgical incision was 21.90%,no SSI occurred in the type Ⅰ surgical incision;SSI incidence of surgery time which was more than 3 h was 7.27%,less than 3 h was 3.71 %,there was significant difference between two groups(x2 =4.136,P<0.05);the SSI incidence of the incision length ≥10 cm was 13.11 %,less than 10 cm was 1.82%,the difference was statistically significant (x2=48.966,P<0.05).Conclusion NNIS score,wound type,type of surgery,duration of surgery may become the risk factors SSI.
8.Relationship among cardioversion,cerebral infarction and NT-proBNP level in patients with heart fail-ure complicated atrial fibrillation

Objective:To explore the relationship among cardioversion ,cerebral infarction (CI) and N terminal pro brain natriuretic peptide (NT‐proBNP) level in patients with heart failure (HF) complicated atrial fibrillation (AF) . Methods :A total of 150 HF + AF patients received intravenous drip of amiodarone for cardioversion therapy .Ac‐cording to cardioversion results ,they were divided into cardioversion group (n=100 ) and non‐cardioversion group (n=50) ,NT‐proBNP level change was observed in two groups before and after cardioversion .According to CI on‐set or not ,patients were divided into CI group (n=20) and non‐CI group (n= 130) ,NT‐proBNP level was com‐pared between two groups before and after onset .Results :Within 48h after administration ,a total of 100 patients (66.67% ) recovered to sinus rhythm .Compared with before cardioversion ,NT‐proBNP level significantly reduced [(967.04 ± 366.16) pg/ml vs .(496.21 ± 142.54) pg/ml] after cardioversion in cardioversion group ,and was signifi‐cantly lower than that of non‐cardioversion group (996.76 ± 351.28) pg/ml , P<0.01 all . In CI group ,compared with small size CI group ,there were significant rise in NT‐proBNP level [ (784.21 ± 231.26) pg/ml vs .(1983.24 ± 32.96) pg/ml ,(3562.19 ± 1468.32) pg/ml] in medium and large size CI group , P< 0.05 or <0.01 .Conclusion:NT‐proBNP level at hospitalization possesses predictive value for drug cardioversion effect in HF + AF patients . NT‐proBNP level is related with CI onset .After acute CI ,the higher NT‐proBNP level is ,the larger infarct size is , the poorer prognosis is .
9.Simultaneous determination of quercitrin, phloridzin and 3-hydroxyl phloridzin in leaves of Malus halliana by ultrasound-assisted ionic liquid-reversed phase liquid chromatography.

To establish a method for the simultaneous determination of phloridzin, 3-hydroxy phloridzin and quercitrin in leaves of Malus halliana by ultrasonic-assisted ionic liquid coupled with RP-HPLC. An Agilent TC-C₁₈ (4.6 mm×250 mm, 5 μm) column was used, with the mobile phase of acetonitrile and 1% phosphoric acid-water (20∶80) by gradient elution at the detection wavelength of 270 nm. The flow rate was 0.8 mL•min⁻¹, and chromatographic column temperature was controlled at the room temperature. Under the optimized conditions, the linear ranges for phloridzin, 3-hydroxy phloridzin and quercitrin were 0.9-112.5 μg (r = 0.999 6), 0.093 2-11.65 μg (r = 0.999 1) and 0.097 2-12.15 μg (r = 0.999 8), respectively. The average recoveries of the three constituents were 99.35%, 98.80% and 98.19%, respectively. The method was environmental friendly, rapid, accurate and highly reproducible, and so suitable for the quantitative analysis of phloridzin, 3-hydroxy phloridzin and quercitrin in leaves of M. halliana.
10.Effect of Hypoxia on the Proliferation and Hypoxia Inducible Factor-1α Expression in Human Leukemia K562 Cells.

OBJECTIVETo explore the effect of hypoxia on the cell proliferation and expression of hypoxia-inducible factor-1α(HIF-1α) of human leukemia K562 cells.
METHODSThe leukemia cells were divided into 2 groups: hypoxia-treated group and conventional oxygen group as control. The K562 cells in hypoxia-treated group were treated with 50, 200, 400 and 800 µmol/L of CoCl, while the K562 cells in control group were cultured in conventional oxygen condition, then the K562 cells in 2 groups were colleted after treatment for 24, 48 and 72 hours. The morphological changes of cells were observed by the inverted phase-contrast microscopy, the proliferation-inhibitory rates of cells was detected by MTT method, the expression of HIF-1α at transcription level was detected by real-time fluorescent quantitative PCR.
RESULTSThe cell morphology was changed with increase of CoClconcentration and prolonging of treatment time of CoCl. The MTT assay showed that the inhibitory rate of cell proliferation was enhanced also with increase of CoClconcentration and prolonging of treatment time of CoCl(r=0.435, r=0.389, P<0.05). The real-time fluorescent quantitative PCR showed that the mRNA expression of HIF-1α in K562 cells of hypoxia group was up-regulated in concentration- and time- dependant manners. and displayed the positive correlation with concentration of CoCland treatment time (r=0.954, r=0895).
CONCLUSIONThe hypoxia can inhibit the proliferation of K562 cells and up-requlate the expression of HIF-1α in cells, but does not induce cell differentiation.
