AIM: To observe the therapeutic effect of intravitreal injection of ranibizumab combined with compound Xueshuantong capsule on patients with central exudative chorioretinopathy(CEC).

METHODS: A total of 98 consecutive patients(98 eyes)with CEC treated in our hospital from September 2013 to May 2015 were enrolled in this study and randomly divided into 2 groups. The experimental group received intravitreal injection of ranibizumab treatment, were treated with compound Xueshuantong capsule, 3 consecutive months of treatment; the control group only treated with compound Xueshuantong capsule treatment, the same dosage and course of treatment with the experimental group. Two groups of patients were reviewed monthly, the follow-up time was 6mo. Before and after treatment, visual acuity, foveal retinal thickness and leakage area were measured.

RESULTS: Visual acuity(LogMAR): the visual acuity of the two groups before and after surgery was significantly different(P<0.05); the visual acuity of the experimental group was significantly better than that of the control group, the difference was statistically significant(P<0.05). There was significant difference between experimental group and control group on central macular thickness macular(CMT)(P<0.05), which of experimental group decreased more than that of control group. The improved leakage of the two groups was significant different by χ2 test(P<0.05).

CONCLUSION: Ranibizumab combined with compound Xueshuantong capsule in the treatment of CEC can reduce vascular leakage, reduce macular edema, shorten the course, improve eyesight, which is a safe and effective method for the treatment of CEC. Multiple intravitreal injection of ranibizumab to CEC can improve the vision of patients and keep it stable.

AIM: To study the effect of ocular surface and meibomian gland dysfunction of adolescents before and after wearing overnight orthokeratology, and to explore its effects on adolescent tear film.

METHODS: A prospective clinical study was conducted. 55 cases of 8 to 16-year-old adolescents who met the selection criteria from January 2017 to November 2017 were selected for orthokeratology. Before lens wear and after lens wear 1wk, 1mo, 3mo, 6mo, and 12mo were followed up for Ocular Surface Disease Index(OSDI)questionnaire, slit-lamp examination, and Keratograph 5M.

RESULTS: During the study period, no patients developed infectious keratitis. The OSDI scores were 7.38±1.71 points, 9.21±1.39 points, 10.19±1.02 points, 10.28±1.18 points, 10.29±1.85 points after wearing at 1wk, 1, 3, 6, and 12mo respectively, which were higher than those before wearing(4.80±1.63 points)were increased, and the difference was statistically significant at different times before and after wearing(P<0.05). 1wk, 1, 3, 6, 12mo NIBUT(f)values were 12.39±4.76s, 9.95±5.23s, 11.30±4.58s, 11.21±4.34s, 11.63±5.32s, and NIBUT(av)values were respectively 11.26±6.77s, 11.16±6.48s, 13.09±5.79s, 13.13±5.41s, 13.19±5.21s, NIBUT(f)1mo after wearing compared with before wearing, the difference was statistically significant(P<0.05), NIBUT(av)at 1wk and 1mo after wearing was compared with before wearing, the difference was statistically significant(all P<0.05), and the difference was both at 3, 6, 12mo after wearing and before wearing No statistical significance(all P>0.05).There was no significant change in TMH at 1wk, 1, 3, 6, 12mo after wearing, and the difference was not statistically significant(F=2.168, P>0.05). Corneal fluorescein staining scores were 3.51±1.67 points, 3.54±1.62 points, 4.05±1.52 points, 4.14±1.32 points, 4.50±1.43 points, respectively, 1wk after wearing, 1, 3, 6, and 12mo, which were higher than those before wearing(P<0.05). The meibomian gland lipid secretion score and the meibomian gland loss score did not change significantly at different times before and after wearing(P>0.05).

CONCLUSION: The tear film function in the early period of wearing the orthokeratology is reduced, but it gradually returns to the level before wearing the lens after 6mo, and tends to increase steadily. After wearing the orthokerato-scope, the symptoms of eye discomfort increase and corneal fluorescein stain increased after wearing, no effect on meibomian gland function.

Objective:To explore the value and significance of transcranial color-coded duplex (TCCD) parameters, serum insulin-like growth factor-1 (IGF-1), neuron-specific enolase (NSE) levels in the evaluation of the condition of cerebral infarction.Methods:One hundred and ten patients with cerebral infarction in Anhui Wanbei Coal-Electricity Group General Hosptial were selected from April 2018 to February 2020, and TCCD examination was performed after admission. Blood samples were taken to determine the levels of serum IGF-1 and NSE, and corresponding treatment was given. The TCCD parameters and serum IGF-1 and NSE levels were compared in patients with different disease severity, plaque nature and different prognosis after 3 months′ follow-up. The correlation between the above indicators and the condition and plaque nature and the predictive value of the prognosis of patients with cerebral infarction were analyzed.Results:The level of IGF-1 were negatively correlated with the severity of disease ( r=- 0.650) and the nature of plaques ( r=- 0.711); and the level of NSE and resistance index (RI), pulsatility index (PI) were positively correlated with the severity of the disease ( r=0.609, 0.613, 0.645) and the nature of plaques ( r=0.589, 0.579, 0.608), and the differences were statistically significant ( P<0.05). After 3 months′ follow-up, the level of IGF-1 in the favourable prognosispatients was higher than that in the unfavourable prognosis patients, the levels of NSE, RI and PI in the favourable prognosis patients were lower than those in the unfavourable prognosis patients: (9.01 ± 2.64) μg/L vs. (25.13 ± 3.82) μg/L, 1.05 ± 0.19 vs. 1.32 ± 0.22, 0.69 ± 0.06 vs. 0.89 ± 0.07, and the differences were statistically significant ( P<0.05). The area under the curve (AUC) of RI, PI, IGF-1 and NSE in predicting the prognosis of cerebral infarction was less than that of combined diagnosis. Conclusions:The early use of TCCD parameters, serum IGF-1 and NSE combined detection can provide evidence-based guidance for evaluating the condition and prognosis of cerebral infarction.

OBJECTIVE: To investigate the effect of computer navigation gap balance technology on the recovery of lower limb function after total knee arthroplasty. METHODS: The clinical data of 106 patients with knee osteoarthritis (OA) who underwent total knee arthroplasty from July 2018 to June 2019 were analyzed retrospectively. They were divided into measurement osteotomy group and space balance group according to different osteotomy techniques during total knee arthroplasty. There were 61 cases in osteotomy group, 24 males and 37 females;The age ranged from 45 to 77(63.35±4.26) years;According to K-L classification, 41 cases were grade Ⅲ and 20 cases were grade Ⅳ. intraoperative measurement osteotomy was performed. There were 45 cases in the gap balance group, 17 males and 28 females;Age 45 to 78(64.03±4.31) years;According to K-L classification, 29 cases were classified as grade Ⅲ and 16 cases as grade Ⅳ. computer navigation gap balance technology was implemented. The amount of intraoperative bleeding, operation time, incision length, hospital stay and postoperative complications were compared between two groups. The clinical efficacy was evaluated by Knee Society score(KSS) before operation and 12 months after operation. RESULTS: Total of 106 patients were followed up for 12 to 18(20.38±3.25) months. There were significant differences in intraoperative bleeding and operation time between two groups(P<0.05). There was no significant difference in incision length and hospital stay between the two groups(P>0.05). At 12 months after operation, the total score of KSS in the gap balance group (173.59±14.50) was better than that in the osteotomy group (164.95±12.10)(P<0.05). There were no serious complications of poor prosthesis loosening between two groups during follow-up, and there was no significant difference in the incidence of other complications between two groups(P>0.05). CONCLUSION The application of computer navigation gap balance technology in total knee arthroplasty is conducive to the recovery of lower limb function in patients with OA, and there are no serious adverse complications and high safety.

OBJECTIVETo understand childhood asthma from age 0 to 14 in Kaifeng city and the relevant risk factors, effects of therapy and costs.

METHODSEleven thousand children aged 0 to 14 were chosen in Gulou area, Shunhe area using cluster sampling.

RESULTS(1) The overall prevalence of childhood asthma was 1.16% sex ratio 1.72:1. Significant difference was found among every age group (P < 0.001), with the highest from 0 to 3, the prevalence rate in industrial area was significantly higher than that of residential area, with a ratio of 2.13:1 (P < 0.001). (2) Major relevant factors were found to be: history of hypersensitivity, upper respiratory infection and family history; while nationalities, history of contact with pets were not found to be related to childhood asthma (P > 0.05). (3) The expenditure was significantly different between non-specific therapy and specified therapy (P < 0.001). Among those children with asthma, 89.8% did not get specified treatment and the average expenditure was 2,375.2 Yuan per year, which was 10.2% of accepted specified therapy, namely under GINA program, with average expenditure 653.68 Yuan every year.

CONCLUSIONThe result of this study provided scientific basis for child asthma prevention and cure in this area.

Objective: To explore the effect of comprehensive yoga intervention program on cardiopulmonary function and physical fitness among female college students. Methods: Twohundred and forty female college students were invited to participate in the program and were randomly divided into intervention (n=120) and control group (n=120) by random number table method. The intervention group received regular yoga exercise, while the control group had no exercise intervention. Body shape, cardiopulmonary function, body flexibility, muscle strength and mental health of the two groups were compared before and after intervention. Results: The cardiopulmonary function indices except PP of the intervention group improved significantly after intervention. There were no significant changes in cardiopulmonary function before and after the intervention for control group. The HR values of the observation group were significantly lower, and the values of SV, CO, VC, MVV and FEV1/FEV were significantly higher compared with those of the control group(P<0.05). Body mass index(BMI), waisttohip ratio and body fat percentage of observation group were significantly lower than that of control group(P<0.05). The performance on the sitandreach and situp test of observation group were significantly improved after intervention and were significantly higher than the control group(P<0.05). Somatization, sensitivity, depression and anxiety in the observation group decreased significantly after intervention and were lower than that of the control group(P<0.05). Conclusion The comprehensive yoga intervention program can effectively improve body shape, body flexibility and muscle strength, as well as improve cardiopulmonary function. It also plays a key role in improving mental health among female college students.

Objective To evaluate the clinical effect of endovascular stent placement to the treatment of intracranial internal carotid artery dissection.Methods Two patients with intracranial internal carotid artery dissection received the treatment of stent placement,and 1 patient with a dissection of the supra clinoid internal carotid artery received conventional anticoagulation treatment.Results Two patients with intracranial internal carotid artery dissection were given treatment of Apollo stent placement,of which 1 patient had improvement of left limb paresis,the score of NIHSS from 3 before operation to 2 after operation; the other one with episodic left limb weakness was not seen any attack after stent placement.Another one patient without stent placement receiving conventional anticoagulation treatment had some improvement of right limb paralysis.Conclusion The treatment of endovascular stent placement to the intracranial internal carotid artery dissection has better clinical efficacy and especially used for those patients with no effect to the conventional anticoagulation treatment.

Objective To evaluate the protective effects elicited by ClpP fusion protein in animal protection tests. Methods Pneumococcal antigens were purified from recombinant Escherichia coli express-ing CIpP cloned gene. 6- to 8-week-old female BALB/c mice were immunized intraperitoneally with either ClpP or PBS plus alum. Every mouse received three doses of 20 μg antigen or PBS in 100 mg of alum adju-vant at 14 d intervals. Sera were collected from mice and analyzed by ELISA 1 week after the third immuni-zation, Intraperitoneal-challenge experiments with 12 different serotypes of Streptococcus pneumoniae were carried out 2 weeks after the third immunization, and we compared their median survival times and survival rates respectively by Mann Whitney U test and Fisher exact test. Results ELISA analysis demonstrated high titer specific antibody responses to ClpP. The median survival times for mice immunized with ClpP pro-tein antigens in adjuvant were significantly longer than those for mice that received the adjuvants alone. Con-clusion A highly expressed recombinant ClpP protein has been successfully obtained and proved to exert the protection against invasive pneumococcal infection without relation of serotype, suggesting ClpP can be a promising candidate vaccine.

Objective To explore the clinical feature,diagnosis,therapy and prognosis of primary cardiac lymphoma.Methods One case of primary diffuse large B-cell lymphoma of the heart was studied.The clinical feature,pathogenesis,diagnosis,therapy and prognosis of primary cardiac lymphoma were further investigated through literatures review.Results Patient,female,64 years old,mainly clinical feature with the activities of chest tightness,gas tight,echocardiogram showed right atrial lesions,ECG of sick sinus syndrome.Right atrial tumor excision by pathological examination confirmed the diagnosis of diffuse large B cell lymphoma(DLBCL),treated with R-CHOP (rituximab,cyclophosphamide,vinorelbine,pirarubicin,prednisone) chemotherapy,cardiac pacemaker implantation,the patient had received 3 cycles of chemotherapy currently,and was at the intermission of chemotherapy.Conclusion Primary cardiac lymphoma is extremely rare.Its pathogenesis remained to be unclear.With non-specific clinical manifestations,the diagnosis is mainly confirmed by histopathological and immunohistochemical staining.It is sensitive to chemotherapy generally,but the special location of the tumor leads to poor prognosis.
