1.Riboflavin metabolism involved into comparison of therapeutic action of wild-simulated and transplanted Astragali Radix to treat chronic atrophic gastritis rats based on endogenous-xenobiotics metabolism.

OBJECTIVE:
Astragali Radix (AR, Huangqi in Chinese) has been utilized generally as a bulk drug for the treatment of chronic atrophic gastritis (CAG) in China. The efficacy of wild-simulated AR (WAR) and transplanted AR (TAR) commercially available is unclear. This study aimed to further clarify the therapeutic action of WAR and TAR to treat CAG rats based on endogenous-xenobiotics metabolism.
METHODS:
Ultra-high performance liquid chromatography coupled with quadrupole-time of flight mass spectrometer (UHPLC-Q-TOF/MS) based metabolomics had been used to analyze the cecal contents metabolic features and metabolic process differences of two ARs in the treatment of CAG.
RESULTS:
Apparent pharmacodynamic indicator examination revealed that the WAR group had a more substantial curative effect. Metabolomics studies revealed that seven endogenous metabolites were significant differences in two ARs. Furthermore, following treatment, 77 and 65 xenobiotics metabolites were identified in the WAR and TAR groups, respectively. Finally, the most critical riboflavin metabolic route in the formation of CAG was chosen for molecular docking with the relevant exogenous components, and WAR scored higher than TAR.
CONCLUSION
In this work, we analyzed the efficacy differences of AR from diverse cultivation forms by combining metabolomics and medicinal chemistry technologies, and it gave a fresh perspective for TCM efficacy evaluation and quality control research.
2.Research progress on the relationship between transporters in the stria vascularis and hearing loss

Stria vascularis(SV), located in the lateral wall of the cochlea, is a highly vascularized tissue that plays a crucial role in maintaining the homeostasis of endolymph and normal auditory function, with its core function relying on the transporter system distributed within it. These transporters in the SV precisely mediate the transport of ions, nutrients, and metabolites, constituting the cornerstone of the homeostasis of endolymph. Abnormal expression or dysfunction of transporters in the SV can lead to an imbalance in the inner ear microenvironment, which can lead to hearing loss. However, the effects of transporters in SV on hearing loss have not been fully clarified. In this paper, the function and distribution of different types of transporters in SV and their relationship with hearing loss are reviewed, providing a new perspective and prospect for exploring the molecular basis of hearing loss and targeted therapies of transporters in the future.
3.Effect of endovascular treatment on thrombosis of autogenous arteriovenous fistula

Objective:To investigate the effectiveness and safety of ultrasound-guided endovascular therapy for autogenous arteriovenous fistula (AVF) thrombosis.Methods:It was a single-center retrospective cohort study. Data of patients undergoing ultrasound-guided intravascular therapy due to AVF thrombosis in the First Hospital of Hebei Medical University from August 2018 to June 2021 were analyzed. According to different surgical procedures, the patients were divided into two groups. Patients treated with percutaneous transluminal angioplasty (PTA) + drilling thrombectomy were in group A, and patients treated with PTA only were in group B. After 1 year of follow-up, the surgical technique success rate, primary patency rate, secondary patency rate and complications were compared between the two groups.Results:A total of 152 patients were enrolled, including 74 in group A and 78 in group B. There were no significant differences in gender, age, proportion of patients with diabetes and hypertension, and thrombosis time of AVF between the two groups (all P>0.05). Compared with group B, the diameter and length of thrombus in group A were larger [13.0(9.0, 16.0) mm vs. 6.0(5.0, 6.5) mm, Z=-9.362, P<0.001; 12(8, 15) cm vs. 3(3, 4) cm, Z=-10.061, P<0.001], and the establishment time of AVF was longer [5(2, 7) years vs. 2(1, 5) years, Z=-2.698, P=0.007]. Among the overall patients, the success rate of surgery was 96.7% (147/152), and the success rate of surgery was 95.9% (71/74) in group A and 97.4% (76/78) in group B respectively, with no statistical difference ( χ2=0.004, P=0.952). Kaplan-Meier survival analysis showed that, overall, the primary patency rate at 3rd, 6th and 12th month after operation was 87.1%, 71.4% and 56.6%, and the secondary patency rate was 97.1%, 96.4% and 94.1%, respectively. The primary patency rate of group A at 3rd, 6th and 12th month was 82.4%, 66.7% and 53.6%, and the secondary patency rate was 95.7%, 94.2% and 89.7%, respectively. The primary patency rate of group B at 3rd, 6th and 12th month was 91.5%, 73.2% and 59.7%, and the secondary patency rate was 98.6%, 98.6% and 98.5%, respectively. There was no significant difference in the primary and secondary patency rate between group A and group B at 3rd, 6th and 12th month (all P>0.05). The duration of operation in group A was longer than that in group B [2.0(1.9, 2.0) h vs. 2.0(1.0, 2.0) h, Z=-5.181, P<0.001], but no serious complications occurred in both groups. Conclusion:The two surgical methods are effective, safe and reliable in the treatment of AVF thrombosis, and have high clinical application value.
4.Association of T helper cells and cytokines with abdominal obesity in Hashimoto's thyroiditis

Objective To investigate the correlation between abdominal obesity and autoimmune thyroid disease in the view point of helper T cells and cytokines.Methods Clinical and laboratory data were collected from 108 pa-tients with Hashimoto's thyroiditis(HT)plus abdominal obesity and 122 patients of Hashimoto's thyroiditis without abdominal obesity who visited the People's Hospital of Xinjiang Uygur Autonomous Region and also from the control population.Abdominal circumference was measured,and patients in the HT patients were grouped according to whether they were abdominally obese or not.The thyroglobulin antibody(TgAb)and thyroid peroxidase antibody(TPOAb)were detected,and the ratio of helper T cells and related cytokines were detected by flow cytometry and enzyme-linked immunosorbent assay.Results The abdominal circumference of the TgAb-positive group was higher than that of the TgAb-negative group(P<0.05).Correlation analysis suggested that abdominal circumference was significantly and positively correlated with TgAb and IL-4 but negatively correlated with Th1.After correcting for gender and age,and abdominal obesity and IL-4 were risk factors for TgAb antibody positivity OR=3.080(95%CI:1.022-9.284)and OR=1.296(95%CI:1.022-9.284),both with P<0.05.Conclusions Abdominal obesity may be an influential factor in TgAb antibody positivity,with larger abdominal circumference having higher TgAb antibody titers,lower Th1 levels,and higher IL-4 levels.Visceral adiposity may exacerbate autoimmune dam-age of thyroid tissue by disruption of helper T cell pathway.
5.Correlation between choroidal thickness and WMH in patients with recent small subcortical infarct based on optical coherence tomography

Objective To investigate the relationship between choroidal thickness and white matter hyperintensity(WMH)in patients with recent subcortical small infarct(RSSI).Methods A total of 83 RSSI patients hospitalized in our hospital from November 2021 to May 2023 were enrolled,and according to paraventricular WMH,they were divided into no to mild lesion group 1(46 cases)and moderate to severe lesion group 1(37 cases),and based on deep WMH,40 of them were assigned into no to mild lesion group 2 and 43 cases into moderate to severe lesion group 2.All patients underwent cranial MRI for paraventricular WMH and deep WMH,and optical coher-ence tomography to measure choroid thickness by semi-automatic segmentation.The relationship between choroidal thickness and WMH at paraventricular and deep parts was analyzed.Results Significant advanced age,higher proportions of smoking and hypertension,and larger volume of deep WMH,while obviously thinner choroid thickness were observed in the moderate to severe lesion group 2 than the no to mild lesion group 2(P<0.05,P<0.01).While the moder-ate to severe lesion group 1 had older age,higher smoking and hypertension ratios,and higher volume of paraventricular WMH when compared with the no to mild group 1(P<0.05,P<0.01).Multivariate logistic regression analysis showed that age(OR=1.170,95%CI:1.064-1.287,P=0.001),hypertension(OR=4.533,95%CI:1.215-16.914,P=0.024),and choroidal thickness(OR=0.953,95%CI:0.931-0.975,P=0.001)were independent risk factors for deep WMH grade.Conclusion Changes in choroidal thickness in RSSI patients are independent influ-encing factors for severity of deep WMH lesions.The more severe the deep WMH lesion is,the thinner the choroidal thickness.
6.Effect of denosumab on glycolipids and cardiovascular indices in patients with postmenopausal osteoporosis

Objective To investigate the effects of denosumab on glucose-lipid metabolism and cardiovascular in patients with postmenopausal osteoporosis(POMP).Methods Patients diagnosed with POMP in Xinjiang Uygur Autonomous Region People's Hospital from 1 January 2017 to 31 August 2022 were selected,and the patients were divided into the desutumomab and atorvastatin calcium group(group A),the desutumomab group(group B),and the control group(group C)according to the drugs used,and the general data and laboratory examinations of the patients in the three groups were collected.General information and laboratory tests were collected from the three groups of patients,and basic information,bone metabolism,glucose metabolism,lipid metabolism,and myocardial injury indexes were compared among the three groups of patients.Results After 1 year of treatment,the alkaline phosphatase level of the three groups of patients decreased significantly compared with that before treatment(P<0.05),and the differences in lactate dehydrogenase(LDH)and fasting blood glucose levels of the three groups of patients after drug treatment were statistically significant(P<0.05),and compared with group C,the reduction of fasting blood glucose in group B was more significant,and the increase in LDH was more significant(P<0.05)after treatment.Comparison of creatine kinase(CK)level in group C and LDH level in group A before and after treatment within the group,the difference was statistically significant(P<0.05).Conclusion The effect of denosumab treatment on glucose metabolism in patients with POMP may be beneficial,and the use of denosumab may be a better choice for patients with diabetes combined with osteoporosis.The present study provides preliminary evidence that denosumab in combination with statin in patients with POMP resulted in lower LDH levels,whereas CK was elevated in patients treated with atorvastatin only,suggesting that denosumab may be more advantageous in regulating glucose metabolism,lipid metabolism,and the prevention of cardiovascular events.
7.Correlation between optic nerve tissue thickness and basal ganglia-enlarged perivascular space in patients with recent small subcortical infarct

Objective To explore the relationship between optic nerve tissue thickness and enlarged perivascular space(EPVS)in patients with recent small subcortical infarct(RSSI).Methods A total of 72 RSSI patients admitted to Department of Neurology of Changzhou Second People's Hospital affiliated to Nanjing Medical University from November 2021 to December 2022 were re-cruited in this study.All patients underwent cranial MRI,optical coherence tomography(OCT)and fundus photography.The thickness of retinal nerve fiber layer(RNFL)and ganglion cell-inner plexiform layer(GCL-IPL)were measured by software semi-automatic segmentation,and fundus lesions such as fundus hemorrhage and exudation were observed through fundus photography.The severity of EPVS was graded by visual quantitative evaluation,and then the patients were divided into no to mild EPVS group(32 cases)and moderate to severe EPVS group(40 cases).Basal gan-glia EPVS(BG-EPVS)and semioval center EPVS(CSO-EPVS)were further graded and grouped into no to mild BG-EPVS group(31 cases)and moderate and severe BG-EPVS group(41 cases),and no to mild CSO-EPVS group(39 cases)and moderate and severe CSO-EPVS group(33 cases).The clinical data of each group were compared separately.Results There were statistical differences in age,ratio of hypertension and thicknesses of RNFL and GCL-IPL between the no to mild EPVS group and the moderate to severe EPVS group(P<0.05,P<0.01),between the no to mild BG-EPVS and the moderate to severe BG-EPVS group(P<0.01).Multivariate logistic re-gression analysis showed that age(OR=1.207,95%CI:1.028-1.416,P=0.021),hypertension(OR=7.264,95%CI:1.079-11.929,P=0.042),RNFL thickness(OR=0.753,95%CI:0.617-0.915,P=0.004)and GCL-IPL thickness(OR=0.848,95%CI:0.758-0.949,P=0.004)were independent factors of BG-EPVS grade.Spearman correlation analysis indicated that the thicknes-ses of RNFL and GCL-IPL were negatively correlated with BG-EPVS grade(r=-0.571,r=-0.649,P<0.01).Conclusion RNFL and GCL-IPL thicknesses are independent factors for BG-EPVS grade and negatively correlated with BG-EPVS grade.OCT has important clinical value in assessing the severity of EPVS lesions in RSSI patients.
8.Internalized and externalized behavioral problems in children with leukemia and influencing factors

Objective:To investigate internalized and externalized behavioral problems in children with leukemia and to analyze the influencing factors.Methods:A total of 121 children match the inclusion and exclusion criteria with leukemia hospitalized in Hematology and Oncology Department of 3 hospitals in Zhengzhou city from November 2021 to March 2022 were recruited through a cluster sampling method.The general information questionnaire, Strengths and Difficulties Questionnaire (parent version), Psychological Adaptation Scale, Perceived Social Support Questionnaire and Zarit Burden Interview were used for investigation.Multiple linear regression was used to analyze the influencing factors of internalized and externalized behavioral problems in children with leukemia.Results:Among 121 children with leukemia, there were 72 males and 49 females, with the age of (7.16 ± 3.18) years.The total score of interna-lized and externalized behavioral problems in 121 children with leukemia was (6.12±3.13) points and (5.49±2.92) points, respectively.Multiple linear regression analysis showed that burden of care ( β=0.412, P<0.001), family support ( β=-0.242, P=0.003) and monthly household income per capita ( β=-0.167, P=0.036) were the influencing factors of internalized behavioral problems in children with leukemia.Burden of care ( β=0.360, P<0.001), social integration ( β=-0.223, P=0.008) and caregiver age ( β=-0.176, P=0.035) were the influencing factors of externalized behavioral problems in children with leukemia. Conclusions:Children with leukemia suffer severe internalized and externalized behavioral problems.Burden of care of the caregiver, family support and social integration are predictors of adverse emotional and behavioral problems in children.
9.Unrelated cord blood stem cell transplantation for high-risk/refractory childhood acute myeloid leukemia: a clinical analysis of 160 cases

Objective:To retrospectively analyze the clinical outcomes of single unrelated cord blood transplantation (UCBT) in children with high risk and refractory acute myeloid leukemia (AML) .Methods:Between June 2008 and December 2018, a total of 160 consecutive pediatric patients with AML received single UCBT (excluding acute promyelocytic leukemia) . Myeloablative conditioning (MAC) regimen were applied. All patients received a combination of cyclosporine A (CsA) and mycophenolate mofetil (MMF) for the prophylaxis of graft -versus- host disease (GVHD) .Results:The cumulative incidence of neutrophil cells engraftment at day +42 and platelet recovery at day +120 was 95.0% (95% CI 90.0%-97.5%) at a median of 16 days after transplantation (range, 11-38 days) and 85.5% (95% CI 83.3%-93.4%) with a median time to recovery of 35 days (range, 13-158) , respectively. Incidence of grades Ⅱ-Ⅳ and Ⅲ-Ⅳ acute GVHD and chronic GVHD were 37.3% (95%CI 29.3%-45.2%) , 27.3% (95% CI 20.0%-35.0%) and 22.4% (95% CI 15.5%-28.7%) , respectively. The transplant-related mortality (TRM) at 360 day was 13.1% (95% CI 8.4%-18.9%) . The 5-year cumulative incidence of relapse was 13.8% (95% CI 8.5%-20.3%) . The 5-year disease-free survival (DFS) and overall survival (OS) were 71.7% (95% CI 62.7%-77.8%) and 72.2% (95% CI 64.1%-78.7%) , respectively. The 5-year GVHD and relapse free survival (GRFS) was 56.1% (95% CI 46.1%-64.9%) . The 5-year cumulative recurrence rates of CR1, CR2, and NR groups were 5.3%, 19.9%, and 30.9% ( P=0.001) , and the 5-year OS rates were 79.9% (95% CI 70.3%-86.7%) , 71.1% (95% CI 50.4%-84.4%) and 52.9% (95% CI 33.0%-69.3%) ( χ2=7.552, P=0.020) , respectively. Conclusions:For pediatric patients with high risk and refractory AML, UCBT is a safe and effective treatment option, and it is favorable to improve the survival rate in CR1 stage.
10.The influence of coping style of caregivers and family cohesion on emotional behavior of children with hematological tumor

Objective:To explore the current situation of emotional and behavioral problems in children with hematological tumors, and the relationship with caregivers′ coping styles and family cohesion.Methods:Using cross-sectional study design and cluster sampling method, caregivers of children with hematological tumors hospitalized in hematology department of five third-class hospitals in Zhengzhou from November 2020 to January 2021 were selected as the respondents. The strengths and difficulties questionnaire (parent version), simple coping style questionnaire, family cohesion scale and self-made general information questionnaire were used for questionnaire survey. Statistical analysis was performed by SPSS 24.0 software using Spearman correlation analysis and multivariate Logistic regression analysis.Results:A total of 237 questionnaires were distributed and 214 valid questionnaires were collected. The abnormal detection rates of total difficulty score, emotional symptoms, conduct problems, hyperactivity, peer interaction problems and prosocial behavior were 17.8%, 22.9%, 10.7%, 8.4%, 39.7% and 13.6% respectively. The results of correlation analysis showed that caregivers′ positive coping was negatively correlated with the total score of difficulties, hyperactivity and peer interaction problems ( r=-0.186, -0.153, -0.174, all P<0.05), and positively correlated with the score of prosocial behavior ( r=0.214, P<0.05). Caregivers′ negative coping was positively correlated with the total score of difficulties, emotional symptoms and hyperactivity ( r=0.203, 0.204, 0.170, all P<0.05). Family cohesion was negatively correlated with the total score of difficulties, emotional symptoms, conduct problems, hyperactivity and peer interaction problems ( r=-0.254, -0.225, -0.183, -0.137, -0.195, all P<0.05), and positively correlated with prosocial behavior ( r=0.235, P<0.01). Multivariate Logistic regression analysis showed that positive coping of caregivers was a protective factor of peer interaction in children ( β=-0.050, OR=0.951, 95% CI=0.907-0.996), while family intimacy was also a protective factor for children with prosocial behavior ( β=-0.045, OR=0.956, 95% CI=0.923-0.991). Conclusion:The emotional and behavior problems of children with hematological tumors are serious, and the positive coping of caregivers and family cohesion have certain predictive value for the occurrence of negative emotional behavior problems in children, so medical staff should take corresponding measures to reduce the occurrence of emotional and behavior problems in children.
