Journal of Forensic Medicine 2014;30(4):270-272
OBJECTIVE:
To study the causes of false-positive hyoid fractures and forensic identification.
METHODS:
Twelve cases of false-positive hyoid fractures were collected and analyzed.
RESULTS:
Improper dissection technique (4 cases) and congenital separation (8 cases) were the main reasons for false-positive hyoid fractures.
CONCLUSION
True fractures can be differentiated from false-positive hyoid fractures. False-positive hyoid fracture caused by improper dissection technique can be identified through examination of peripheral muscle, soft tissue hemorrhage, and the characteristics of fracture end.

Journal of Forensic Medicine 2015;31(6):422-424
OBJECTIVE:
To investigate the pathological features and significance of myocardial contraction band necrosis in sudden cardiac death.
METHODS:
Using HE and PTAH staining, the distribution sites and pathological features of myocardial contraction band necrosis were observed. The data were analyzed according to the extent of necrosis.
RESULTS:
The locations, pathological features and the extent of necrosis were similar in different sudden death cases. The locations were related with sites of myocardial damage. The papillary muscles of left ventricular were most occurred, followed by the anterior wall of left ventricular. In the sudden death cases caused by stress cardiomyopathy, the extent of myocardial contraction band necrosis was lighter than the others. Most cases were predominantly level 1, the others were level 2.
CONCLUSION
Used as the diagnosis basis of acute myocardial ischemia, the myocardial contraction band necrosis has important significance to determination of death.

BACKGROUND: Most of the patients suffered from degenerative lumbar instability are treated by exposure both sides and bilateral pedicle screw fixation, which bring highly operative risk, large blood loss and great medical expenditure to patients.OBJECTIVE: To explore the clinical efficacy of single cage plus unilateral pedicle screw placement for treating lumbar degenerative instability.METHODS: Totally 51 cases with lumbar degenerative instability underwent single cage plus unilateral pedicle screw placement were selected, including 32 males and 19 females, aged ranging from 41 to 72 years.47 cases had single segment involved and 4cases had two segments involved. All cases experienced unilateral laminectomy and transforamenal lumbar interbody fusion. The therapeutic effect was assessed by Japanese Orthopaedic Association(JOA)score system.RESULTS AND CONCLUSION: The blood loss was 90-430 mL.The surgical time was 100 minutes(85-120 minutes)for single segment and 150 minutes(120-170 minutes)for double segments. The patients were allowed to early ambulation at 2-3 days after operation. Two cases did not get improvement on back-leg pain, but there was no abnormality from CT and MRI recheck, one case felt pain relieved after anti-symptom treatment for 3 months while the other did not relieve. The average JOA scores at pre-operation and 1 year follow-up was 11(7-13 scores)and 25(18-27 scores),respectively. The total improvement rate of JOA was larger than 50%.44 cases were evaluated as fusion and 7 cases as possible fusion. The average fusion time was 5.4 months(4.3-7.1 months).Postoperative X-ray showed no evidence of pedicle screw loosening, broken,or cage displacement.Single cage plus unilateral pedicle screw placement is characterized by simple operation, small blood loss, short operation and few interference to spine, which is a better method for treating lumbar degenerative instability.

Journal of Forensic Medicine 2014;30(4):245-249
OBJECTIVE:
To explore the correlation between postmortem interval (PMI) and five RNA markers of rat's skin--β-actin, glyceraldehyde-3-phosphate dehydrogenase (GAPDH), 18S ribosomal RNA(18S rRNA), 5S ribosomal RNA (5S rRNA), and microRNA-203 (miR-203), at different temperatures.
METHODS:
Eighteen SD rats were randomly divided into three environmental temperature groups: 4 °C, 15 °C and 35 °C, respectively. Skin samples were taken at 11 time points from 0 h to 120 h post-mortem. The total RNA was extracted from the skin samples and the five RNA levels were detected by real-time fluorescent quantitative PCR. Proper internal reference was selected by geNorm software. Regression analysis of the RNA markers was conducted by GraphPad software.
RESULTS:
5S rRNA and miR-203 were most suitable internal references. A good linear relationship between PMI and RNA levels (β-actin and GAPDH) was observed in two groups (4 °C and 15 °C), whereas the S type curve relationship between the expression levels of the two markers (β-actin and GAPDH) and PMI was observed in the 35 °C group. The partial linear relationship between 18S rRNA and PMI was observed in the groups (15 °C and 35 °C).
CONCLUSION
Skin could be a suitable material for extracting RNA. The RNA expression levels of β-actin and GAPDH correlate well with PMI, and these RNA markers of skin tissue could be additional indice for the estimation of PMI.

Chinese Journal of Orthopaedics 2012;32(5):393-397
ObjectiveTo observe in vivo segmental lumbar motion in patients with lumbar disc herniation (LDH) during functional weight-bearing activities.MethodsFifteen patients with LDH at L4-5 were studied as experimental group.Ten healthy volunteers were recruited as control group.Three-dimension(3D) lumbar spine models of L3,L4 and L5 were reconstructed from thin section CT scans.Spine motions were then reproduced by matching lumbar spine models and images got from dual fluoroscopic imaging system (DFIS)under different motion state (standing,flexion-extension,left-right twisting and left-right bending). From local coordinate systems at the end plates, the motion of the cephalad vertebrae relative to the caudal vertebrae was calculated for vertebrae levels:L4-5 and L3-4.ResultsThe motion pattern at L4-5 was found to be altered.During flexion-extension,the migrations of the affected segments along the frontal axis, sagittal axis, vertical axis were similar with that of the control group, but the rotation angle along the frontal axis was significantly larger than that of the control group (P<0.05).During left-right bending and left-right twisting, the migration and rotation angle along the frontal axis were significantly larger than those of control group.During flexionextension, the migrations of the neighboring segments (L3-4) along the three axes were larger than those of the control group, but there were no statistical significances.During left-right bending and left-right twisting, the migrations of the neighboring segments (L3-4) along the vertical axis were significantly larger than those of the control group (P<0.05).ConclusionThe 3D lumbar motion pattern in LDH patient is different with that of normal people. For the affected segment, compared with the normal people, the range of flexion-extension motion and the translocation in left-right direction were significantly larger, but the rotation range along the vertical axis was smaller.
