Objective To compare the curative effect of tension-free hernioplasty and transabdominal preperitoneal herniorrhaphy(TAPP)in adult patients with inguinal hernia.Methods A prospective randomized controlled study.A total of 240 patients with inguinal hernia were randomly divided into TAPP group and tension-free hernioplasty group(open group),120 patients in each.Results In both the groups,the surgical treatments were completed.The mean operation time in TAPP group was significantly longer than that in open group [(77?15)min vs(48?9)min,t=18.147,P=0.000],and the average cost for surgery in TAPP group was significant higher than that in open group [(7363?351)yuan vs(4635?242)yuan,t=72.663,P=0.000].However,the patients in TAPP group returned to activity and was discharged earlier than those in the open group [(6?1)h vs(10?2)h,t=-19.385,P=0.000;and(3?1)d vs(5?1)d,t=-15.300,P=0.000;respectively].No significant difference was detected in the severity of postoperative pain,complications,and recurrence rate between the two groups.Conclusion Both TAPP and tension-free hernioplasty are worth being wildly used.

Compressed oxygen in diaphragm-type compresesor currently used can easily cause fire,ex- plosion or damage.The reason for the causes was analysed and requirment for the oxygen was put forward. Proceed from the three aspect:selection of lubricating oil;replace of material quality and alarm for the di- aphragm broken;the problems of diaphragm-type compressor were finally settled.

Objective To observe the morphological characteristics of congenital optic disc pit with maculopathy, the natural de-velopment, and changes after laser photocoagulation. Design Retrospective case series. Participants Twelve cases with congenital optic disc pit. Methods Records of 12 patients with congenital optic disc pit with maculopathy were reviewed. Clinical examination includes optical coherence tomography (OCT), color fundus photography, and fluorescein angiography (FA). The data was analyzed with the exist-ing theory of pathogenesis of the disease. Main Outcome Measures Visual acuity and morphology of macupopathy. Results All the patients were noted to have serous maculopathy associated with optic disc pit. Serous detachment of neuro-retina was found in two pa-tients, schisis of neuro-retina in two patients, and both serous detachment and schisis of nearo-retina were observed in other patients. Two patients were associated with choroidal coloboma. Four patients were treated with laser photocoagulation, in which 3 patients had vision improved. Conclusions Schisis and detachment of neuro-retina are the important morphologic changes of congenital optic disc pit with maculopthy. Proper understanding of the relationship between the development of the disease and these changes will be helpful to study its pathogenesis. Patients may benefit in part from laser photocoagulation. Objective To explore the management of benign prostatic hyperplasia(BPH)by photoselective vaporization of prostate(PVP)and curative effect in the near future. Methods Clinical data of 60 BPH patients underwent PVP were retrospctively aralyzed.The changes of IPSS and QOL and the max urine flow rate and residual urine volume(RUV)perioperatively were compared through observations of the operation time and complication. Results The therapeutic efficacy was favourable.No transurethral resection of prostate(TURP)syndrome and uroclepsia happens.IPSS(t=2.471,P <0.05)and QOL(t=2.392,P <0.05)and the max urine flow rate(t=2.287,P ＜0.05)and residual urine volume(t=2.513,P＜0.05)obviously improved(P ＜0.05). Conclusion PVP had many advantages such as operated simply and no serious complications.It was a good operation method to treat BPH.

Objective To explore the management,curative effect and safe of superficial bladder tumor(BT)by greenlight photoselective vapontion per urethra(GPVPU). Methods 32 patients of BT were treated by GPVPU,and operation time,postoperative complications,relapse rate and so on were observed. Results All cases were completed successfully. No complication happened in operations. Urinary canal was detained 2 ～4 days and bladder washout(BW) did not need after operation. Follow-up average time was 16 months. Irrigation chems of bladder and cystoscopy were applied routinely. Conclusion GPVPU was safe and effective in treatment of superficial bladder tumor.

Objective To explore the application of single-person operated mammotome minimally inva-sive surgery in treatment of benign breast lesions. Methods Clinical information of 403 patients were collected and retrospectively analyzed. 262 patients receiving ultrasound guided single-person operated mammotome mini-mally invasive surgery for benign breast masses were selected as the treatment group,141 patients undergoing open surgery for treatment of benign breast masses were selected as the control group.Operative time, introopera-tive blood loss,length of stay,postoperative healing time, as well as the occurrence of complications were com-pared between the two groups. Result Compared with the control group, the operative time was shorter, the in traoperative blood loss was less, the scar size and the postoperative healing time were shorter for the treatment group. The difference had statistical significance (P<0.05).The rate of postoperative pain, local ecchymosis, local infection and breast deformity for the treatment group was significantly lower than those of the control group (P<0.05). The difference had statistical significance (P<0.05). The recurrence rate between the two groups had no sta-tistical significance (P>0.05). Conclusions Single-person operated Mammotome minimally invasive surgery has the advantages of good operative time,good cosmetic effects, small surgical trauma, less blood loss, fast postoper-ative healing and few complications, which is worthy of clinical application.

OBJECTIVETo investigate the expression of zinc finger E-box binding homebox 1 (ZEB1) in the prepuce of hypospadias children and its relationship to the incidence of hypospadias.

METHODSPrepuce tissues were collected from 37 children aged 6-15 months undergoing hypospadias repair and 11 age-matched controls receiving circumcision. Based on the position of the urethral meatus, the hypospadias cases were classified as severe (n = 13) and mild-moderate (n = 24). The mRNA and protein expressions of ZEB1 were determined by immunohistochemistry and RT-PCR.

RESULTSThe expression of the ZEB1 protein was remarkably higher in the severe (100% [13/13]) and mild-moderate hypospadias patients (75.0% [18/24]) than in the controls (9.1% [1/11]), with statistically significant differences between any two groups (P < 0.05). RT-PCR showed the integrated density value (IDV) of the ZEB1 mRNA expression to be (0.67 ± 0.21), (0.81 ± 0.24), and (1.55 ± 0.29) in the control, mild-moderate, and severe hypospadias patients, respectively, significantly higher in the severe hypospadias than in the control and mild-moderate hypospadias groups (P < 0.05), but with no significant difference between the latter two (P = 0.64).

CONCLUSIONThe expression of ZEB1 is significantly increased in hypospadias patients, and its upregulation is positively correlated with the severity of hypospadias, which suggests that the overexpression of ZEB1 may contribute to the development of hypospadias.

With the continuous development of bone tissue engineering, a variety of emerging bone graft materials provided various methods for repairing bone defects. Early and rapid accomplishment of revascularization of materials interior after implantation of bone transplantation materials is a difficulty faced to bone tissue engineering. Blood vessels ingrowth provides the requisite netritional support for the regeneration reconstruction of bone tissue, for this reason, vascularization plays a significant role in bone tissue engineering. However,there is not a golden standard strategy of vascularization at present. Scaffold materials, cells and growth factors still are three indispensable elements in tissue engineering, and are cardinal points of the promoting vascularization strategies. Multiple growth factors or multiple cells combined with scaffolds, which are hot spots, have obtained excellent vascularization. This review focused on the comprehensive strategies for promoting the successful vascularization of tissue engineered scaffolds.

Objective To discuss the operative difficulties and management in laparoscopic thyroidectomy.Method The clinical data of 169 cases undergoing laparoscopic thyroidectomy were retrospectively analyzed.Result Among 169 cases,only 2 cases were switched to open surgery.Among the other 167 cases local thyroidectomy for thyroid adenoma Was performed in 37 cases,one side subtotal thyroidectomy in 63 cases,two side subtotal thyroidectomy in 42 cases,laparoscopic subtotal thyroideetomy was done in 20 cases for Grave's disease,and radical thyroidectomy was undeRaken in 5 cases of thyroid carcinoma.Postoperatively fat colliquation occurred in 2 cases,pneumohypoderma,skin bruise and temporary vocal dullness or hoarsness in 1 each case.There was no postoperative recurrent laryngeal nerve injury,nor that of the parathyroid gland. Conclusions Laparoscopie thyroidectomy has its own intraoperative difficulties. Accurate operative skills and careful perioperative management can decrease the occurrence of complications.
