1.A novel protein peptide associated with ischemic heart disease: dermcidin

Dermcidin (DCD) was found in isolated human skin sweat glands with antimicrobial effect, and was defined as a kind of new small molecule antimicrobial peptide. It was a part of human sweat glands in the skin as the immune system's innate defense. With the studies of DCD, its extensive biological functions are gradually discovered. Since 2010, a number of studies have shown that DCD may be a new risk factor for atherosclerosis. And the role of DCD in ischemic heart disease has drawn increasing attention in particular its relationship with insulin secretion and glycemic control, nitric oxide (NO) synthesis and hypertension, platelet aggregation and acute myocardial infarction (AMI). In those experiments, it was also confirmed that aspirin had antagonistic and reverse effects on various biological functions of DCD. Further research on the role of DCD in cardiovascular and cerebrovascular diseases may lead to the prevention, early warning, prognosis evaluation and treatment breakthrough of cardiovascular and cerebrovascular events.
3.Rehabilitative therapy for the elderly chronic somatic disease complicated with depression

BACKGROUND: There is a complicated relation between somatic disease and mental disorder. Somatic disease may induce functional disorder of brain, and generate psychiatric symptoms due to the change of psychological and social environment. The proportion of depression in elder people based on the chronic somatic disease is much higher, and thus the fatality of them increases.OBJECTIVE: To investigate the influence of single antidepressant, and therapeutic alliance of psychological and drug treatment on the rehabilitation of patients with elderly chronic somatic disease complicated with depression. DESIGN: Randomized and controlled grouping experiment.SETTING: Department of Senile Disease of Tongji Hospital Affiliated to Huazhong University of Science and Technology.PARTICIPANTS: Totally 102 patients were hospitalized in the Department of Senile Disease of Tongji Hospital Affiliated to Huazhong University of Science and Technology from March 2000 to January 2003, and all the patients had sleep disorder to a different extent, and suffered from symptoms of heart and alimentary tract which could not be explained as organic diseases, and chronic pain or systemic symptom. Their score of Zung scale was > 40 points with average of (48.83±4.15)points. The patients were randomly divided into the drug group (n=46) and the combined treatment group (n=56). Each patient had more than two somatic symptoms in the two groups and more than 81.0% of them had three somatic symptoms. METHODS: According to the different primary diseases in patients of the two groups, regular treatment was given routinely. Oral thymoleptics fluoxetine of 20 mg/d 5-hydroxyltryptamine reuptake restrainer with 20 mg/d was administered, the dose of which could be added to 40 mg/d according to the pathogenetic conditions. At the beginning of the administration of oral antidepressant drugs, the patients in the combined treatment group were The focal point of depression and fear was identified, and the comfort and edge related to the disease was supplied, and the pathogenetic condition and the prognosis were told to the patients objectively. The family and society were encouraged to learn the rehabilitative skill and improve their possible, and the family and society were encouraged to give the patients adequate psychological support and life support. The patient and the family were assisted to establish favorable communication. The feeling of loneliness and isolation was reduced, and the sense of safety was increased. The depression status and the quality of life of the patients in the two groups were evaluated before the treatment and at 2 and 8 weeks after treatment with the Zung scale and the Spitzer quality of life index score (QLI), including 5 aspects: Activity, daily life, health, support and perspective. The highest score was 10 points. The lowest was 0, and the higher the score, the better the quality of life is.MAIN OUTCOME MEASURES: The results of the Zung scale and the QLI score evaluated before the treatment and at 2 and 8 weeks after the treatment.RESULTS: The treatment of the patients in the two groups was all finZung scale in the drug group and the combined treatment group at 2 weeks after the treatment indicated that there was significant difference in comparison with those before the treatment (41.12±2.60,48.83±4.15,t =10.67,P < 0.05); (40.88±3.43,49.03±4.21 ,t=11.24,P < 0.05). There was not significant difference in group comparison. The total points of Zung scale at 8 weeks were significantly decreased (36.11 ±2.87 ), (30.71±3.62),and there was significant difference in comparison with that before the treatment. The combined treatment group was better than the drug group in difference of QLI score at 2 weeks between the drug group and combined treatment group before and after the treatment (5.47 ± 1.33,5.32 ± 1.04,5.55±1.18,5.27±1.35). The QLI score of drug group and combined treat ment group at 8 weeks after treatment began to rise 7.21±0.13 and 8.83 ±0.32 ,respectively, and there was significant difference in comparison withthose before the treatment, and there was also significant difference in group comparison.CONCLUSION: The positive and effective combined therapy of anti-depression significantly improved the somatic symptoms of patients, no complications appeared, and most patients presented optimistic attitude to the prognosis of the primary disease. To identify the depression of elder people in earlier period is very important to promote rehabilitation of the primary disease.
4.Observation of the efficacy of Aidi Injection combined with chemotherapy in the treatment of malignant advanced tumor

AIM: To observe the clinical effecacy of Aidi Injection (Radix Ginseng, Radix Astragali, Mylabris, Radix et Caulis Acanthopanacis Senticosi) combined with chemotherapy in the treatment of malignant advanced tumor. METHODS: Patientes (106 cases) with advanced malignant tumor were randomized into two groups: therapeutic group (56 cases) and control group (50 cases). The therapeutic group:the chemotherapy combined with Aidi Injection (60~80mL), once a day, for 10 days. The control group:the chemotherapy alone. Two groups mainly used PTX、DDP、EPI、5 FU、VP16 etc for chemotherapy. 5.Analysis of atrioventricular function and its efficacy in patients with AVNRT undergoing atrioventricular junction ablation of slow conduction pathway

Objective The electrophysiological parameters of atrioventricular conduction function in patients with atrioventricular nodal reentry tachycardia (AVNRT)were analyzed,and to explore the effect of different ablation endpoints on atrioventricular conduction function in patients with AVNRT.Methods 96 cases with AVNRT under-went radiofrequency catheter ablation (RFCA)of slow conduction pathway.According to whether the slow pathway eliminated,the patients were divided into the slow pathway disappeared group and slow tracks remaining group.Preop-erative and postoperative intracavitary electrophysiological examination and atrioventricular function data were recor-ded,including before and after radiofrequency ablation of bundle of HIS,PA,AH,HV interval,atrioventricular prequel venturi point(AVN -WKB),ventriculoatrial retrograde venturi point(VAN -WKB),atrioventricular node prequel effective refractory period(AVB -WKB).Patients were followed up by telephone for a year.Then,a comparative anal-ysis of preoperative and postoperative was done.Results RFCA of AVNRT patients with atrioventricular node pre-quel time effect:RFCA and preoperative ratio,after his bundle electrogram PA,ah,HV interval had no significant changes(P >0.05).Effect of RFCA surgery on patients with AVNRT refractory atrioventricular node prequel:com-pared with RFCA before surgery,postoperative slow pathway disappear fast pathway shortening of effective refractory period[preoperative(287.5 ±46.2)ms,postoperative(260.2 ±55.6)ms,t =2.901,P =0.005],slow pathway effec-tive refractory period[disappear preoperative(243.3 ±43.2),postoperative(0.0 ±0.0)ms,t =43.290,P =0.000], AV node Wenckebach point in advance before operation[(261.3 ±44.3)ms,postoperative(293.2 ±46.2)ms,t =3.828,P =0.000];group after slow pathway to improve fast pathway effective refractory period without obvious change (P >0.05 ),the slow pathway effective refractory period in high concentration [preoperative (242.2 ± 42.8)ms,postoperative(281.2 ±41.3 )ms,t =3.879,P =0.000〗,atrioventricular node Wenckebach point in advance before operation[(261.5 ±43.5)ms,postoperative(291.3 ±46.5)ms,t =2.769,P =0.007〗.Comparison between groups,after slow pathway disappeared group fast pathway effective refractory period was significantly shorter in the slow diameter improvement group,but between the two groups in the atrioventricular node Wenckebach point differences was not statistically significant(P >0.05).There was no recurrence in the follow -up after a year of slow path loss and slow pathway.Conclusion RFCA caused by slow pathway to disappear or modified two slow pathway ablation right AVNRT patients atrioventricular time had no effect,but all the atrioventricular junction the prequel's point advance.The atrioventricular node slow pathway disappear fast and effective pathway refractory period short-ened,slow pathway improved the slow pathway effective refractory period.RFCA surgery done by the slow pathway dis-appeared or slow pathway ablation is effective in patients with AVNRT,and there was no significant recurrence rate in both groups within 1 year.
6.Comparison and experience of two vacuum assisted biopsy systems in benign breast lesions: EnCor and Mammotome

Objective To investigate application value of two vacuum assisted biopsy systems in benign breast lesions and compare the advantages and disadvantages of EnCor and Mammotome system.Methods Vacuum assisted excision was performed on 346 benign breast lesions in 227 patients (EnCor with 103 cases, Mammotome with 124 cases) from November 2012 to May 2013, including 162 cases using EnCor system and 184 cases using Mammotome system.Results Duration of operation in EnCor group was (14.18± 4.21) minutes,in Mammotome group was (18.77 ± 7.06) minutes, and the difference was significant (P < 0.001) .Weight of tissues in Mammotome group was (6.87± 3.43) g, in EnCor group was (8.32± 1.18) g, and the difference was significant (P < 0.001).No significant differences were found in postoperative hematoma (P =0.58), postoperative pain (P =0.35), incidence of residual tumor (P =0.15).Conclusion Compared Mammotome group, EnCor group had significantly shorter duration.But using Mammotome system, the specimens can be observed directly.Mammotome is more suitable for women of childbearing age, it can preserve lactation function.There is no significant difference in complication and residual tumor of two vacuum assisted biopsy system in benign breast lesions.
7.Mechanism of effect of Ginkgolide B on lipopolysaccharide induced microvascular endothelial cells

Objective To investigate the effect of Ginkgolide B (BN52021) on lipopolysaccharide (LPS) induced pancreas microvascular endothelialv (MS1) cells, and to explore its molecular mechanism.Methods The optimal concentration and best time point of LPS inhibing MS1 cell survival and the optimal concentration of BN52021 increasing survival of LPS induced MS1 cells were determined by MTT.The mRNA and protein expression of adenylate cyclase (AC), phospholipase A2 (PLA2), phospholipase Cβ (PLCβ),protein tyrosine kinase (PTK) and G protein coupled receptor kinase (GRK) in platelet activating factor receptor(PAFR) signal pathway in MS1 cells were determined by real-time PCR and Western blot.Results It was showed that 10 μg/ml LPS for 24 h was the optimal concentration and best time point to induce the decrease of MS1 cells.50 mmol/L of BN52021 was the optimal concentration of increacing survival of LPS induced MS1 cells.After LPS induction, AC, GRK, PLA2, PLCβ, PTK mRNA expressions of MS1 cells were 4.02 ±0.14, 2.63 ± 0.03, 3.31 ± 0.12, 2.09 ± 0.08, 1.85 ± 0.07, which were significantly higher than those in control group (P < 0.01).After BN52021 treatment, AC, GRK, PLA2, PLCβ mRNA expressions of LPS induced MS1 cells were 2.35 ±0.13, 1.17 ±0.14, 1.87 ±0.11, 1.65 ±0.10, which were significantly lower than those in LPS induction group (P < 0.01).The expression of PTK mRNA was 1.83 ± 0.13, which was not significantly different from that in LPS induction group.Western blot showed that the levels of protein expression were consistent with those of mRNA expression.Conclusions BN52021 can down-regulate the up-regulated genes expression of AC, GRK, PLA2 and PLCβ in the PAFR signal pathway in LPS induced MS1 cells.
8.Analysis of Technical Keypoints of The Second-Phase Diaplasis Treatment after Hartmann Operation

Objective To discuss the technical keypoints of the second-phase diaplasis treatment after Hartmann operation. Methods Twenty-one cases of the second-phase diaplasis operation of Hartmann operation in this hospital from January 2003 to December 2007 were analysed retrospectively. Results In this group, stapler technique was used in 15 cases, anastomotic ring was used in 3 cases, and one-layer suture was used in 3 cases. The time of these operations was between 118 min and 240 min (mean 164 min). Neither stomal leak nor stomal stenosis occurred, acute ileus occurred in 2 cases, and both recovered after the treatments of gastrointestinal decompression, inhibition of secretion digestive juice and reoperation. Acute retension of urine occurred in 1 case after urine catheter removed. The symptoms disappeared after excises of bladder function. These cases were followed up for 3-36 months (mean 20 months). All the patients recovered to normal function of defecation. Conclusion The technical keypoints of the second-phase diaplasis treatment after Hartmann operation are how to find and liberate the end of the distal colon. It will affect the prognosis and the occurrence of complications after this operation.
9.Relationship between human papilloma virus-DNA and cytomegalovirus-DNA virus content and risk factors of endometrial cancer and prediction model

Objective:To explore the relationship between human papilloma virus (HPV)-DNA and cytomegalovirus (CMV)-DNA virus content and the risk factors of endometrial cancer (EC) and their prediction models.Methods:Fifty-eight patients with EC from January 2017 to June 2020 in Suzhou Hospital Affiliated to Anhui Medical University were selected as the observation group, and 58 patients with endometrial hyperplasia (EH) during the same period were selected as the control group according to the 1∶1 matching case control principle. The clinical data, HPV-DNA, CMV-DNA positive rate and virus content of the two groups were compared, and the independent variables with P<0.05 were included in the Logistic regression model through single factor analysis. The risk factors of EC were analyzed, and the correlation between HPV-DNA, CMV-DNA and EC risk factors were evaluated. Logistic regression model was evaluated using the likelihood ratio chi-square and goodness-of-fit test, and the receiver operating characteristic (ROC) curve was used to analyze the predictive value of the model to EC. Results:The proportion of patients with age ≥ 55 years old, body mass index (BMI)≥ 24 kg/m 2, a history of hypertension and diabetes, malignant tumor family history and hormone replacement therapy history in the observation group were higher than those in the control group: 72.41%(42/58) vs. 39.66%(23/58),70.69% (41/58) vs. 43.10%(25/58), 36.21%(21/58) vs. 10.34%(6/58), 31.03%(18/58) vs. 8.62%(5/58),29.31%(17/58) vs. 5.17%(3/58), 27.93%(17/58) vs. 8.62%(5/58), and the differences were statistically significant ( P<0.05). The positive rates of HPV and CMV, the contents of HPV-DNA and CMV-DNA in the observation group were higher than those in the control group: 62.07%(36/58) vs. 29.31%(17/58),81.03%(47/58) vs. 41.38%(24/58), (471.16 ± 33.58) copies/ml vs. (240.08 ± 17.41) copies/ml, (256.19 ± 24.77) copies/ml vs. (132.27 ± 13.20) copies/ml, the differences were statistically significant ( P<0.05). Age ≥ 55 years, BMI ≥ 24 kg/m 2, history of hypertension and diabetes, family history of malignant tumors, history of hormone replacement therapy, HPV positive, CMV positive were risk factors for EC ( P<0.05). HPV-DNA virus content was positively correlated with the age of EC patients, family history of malignant tumors, and history of diabetes ( P<0.05), the viral content of CMV-DNA was positively correlated with age, family history of malignant tumors, and history of hypertension ( P<0.05). According to the ROC curve drawn by the predicted value and the true value of the Logistic regression model, the area under the curve was 0.930 (95% CI 0.885 - 0.976), when the predictive probability of individual nosocomial infection>0.528, the prediction sensitivity was 84.48%, and the specificity was 89.66%. Conclusions:The occurrence of EC is related to many factors, and the content of HPV-DNA and CMV-DNA viruses are the risk factors of its occurrence. The overall predictive value of a Logistic regression model constructed based on EC pathological characteristics and the virus content of HPV-DNA and CMV-DNA is high, and it has clinical application prospects.
10.Early evaluation of exercise tolerance test on diastolic functional changes of left ventricle in patients with type 2 diabetes

BACKGROUND: Color Doppler flow imaging can exam the early myocardial disorder in type 2 diabetic patients. What does exercise tolerance test work for the examination of such disorder in combination with color Doppler flow imaging (CDFI) ?OBJECTIVE: To analyze in comparison the early evaluation on reduced diastolic function in left ventricle in patients with type 2 diabetes between the examinations of exercise tolerance test combined with color Doppler flow image and simple color Doppler flow imaging.DESIGN: Cases-controlled comparison and self-comparison.SETTING: Department of electrodiagnosis and department of Endocrinology in a municipal hospital.PARTICIPANTS: Thirty-six cases of inpatients with type 2 diabetes were selected from Department of Endocrinology of Shenyang Red Cross Hospital from March to December in 2004, of which, 25 cases were males and 11cases females. The diabetic patients included had no cardiac vascular complications and participated in the study in volunteer. Thirty-two patients who received annual routine health check at the same period were selected as the control, of which, 20 cases were males and 12 cases females.METHODS: Metronics treadmill exercise test equipment was used for exercise tolerance in two groups. Before exercise(at quiescent state) and after exercise tolerance, Vivid 4 CDFI was used to determine flow velocity at E and A peak values respectively, ratio between flow velocities at E peak value and A peak value as well as isovolumtric relaxation time (IRT).MAIN OUTCOME MEASURES: Comparison of cardiac functional indexes before and after exercise tolerance in two groups.RESULTS: Thirty-four diabetic patients accomplished exercise tolerance test, of which, 1 case presented frequent ventricular extrasystole, another one presented precordial pain and stopped the test. In the control, all of 32 cases ratio between flow rates of E peak value and A peak value was remarkably lower than that in the control (0. 90 ± 0. 25, 1.40 ± 0.30, P < 0.05 ); IRT was remarkably longer than that in the control [ (112. 07 ± 20. 16),imental group, the ratio between flow rates of E peak value and A peak value was remarkably lower than that in the control (0.62 ±0. 12, 1.28 ±0.87, P< 0.01 ); IRT was remarkably longer than that in the control[ (138. 10± 19.21), (97.37±9.61) ms, P <0.01].CONCLUSION: CDFI can supervise and evaluate at early stage the cardiac functional changes in type 2 diabetic patients. Due to the induction of exercise tolerance, the combination of exercise tolerance test and CDFI provides more accurate, objective and valuable conclusions at early stage.
