1.Analysis of the relationship between serum HBV-DNA And hepatic fibrosis

Objective To analyze the relationship between serum HBV-DNA and hepatic fibrosis in patients with hepatitis B. Methods Serum HBV-DNA load and hepatic fibrosis hyaluronic acid (HA),Ⅲ procollagen N terminal peptide(PⅢNP),laminin (LN),Ⅳ collagen(Ⅳ-C)of 154 cases were detected by real-time fluorescence quantitative PCR and chemiluminescence method. Statistical analysis of the correlation between HBV-DNA load and hepatic fibrosis indicator was performed.Results The data of HBV-DNA load after taking lg value was 5.49±1.39;The serum concentration of hepatic fibrosis HA was (166.95±148.20)ng/mL,PⅢNP was (12.14±7.37)ng/mL,Ⅳ-C was (65.29±45.48)ng/mL,LN was (59.52±35.67)ng/mL.The difference between experimental group and control group in hepatic fibrosis was significant(P <0.05).Serum HBV-DNA only correlated with LN(r=0.284,P =0.015).Conclusion There is no significant correlation between serum HBV-DNA and hepatic fibrosis in hepatitis B pa-tients.Anti-fibrosis treatment should also be strengthened for hepatitis B patients in the course of antiviral treatment.
2.Advance in Rehabilitation of Upper Limb Function in Hemiplegic Patients after Stroke (review)

The incidence of upper limb dysfunction of stroke patients was high, and the recovery was difficult, which seriously impact-ed their activities of daily living and quality of life. This article reviewed the new techniques and methods for hemiplegic patients after stroke. The main techniques and methods includeed bilateral isokine-matic training, mirror neuron system, gymnastics rods training, music therapy, motor imagery, upper limb rehabilitation robot, virtual reality, constraint induced movement therapy, functional electrostimulation, electromyographic biofeedback therapy, transcranial magnetic stimulation, neurophyisological therapy and so on.
3.Clinical Application of Simple Coaxial Microtube Methods for Selective Salpingography and Fallopian Tube Recanalization.

Objective To evaluate the clinical value of selective salpingography(SSG)and fallopian tube recanalization(FTR) for diagnosis and treatment of oviduct obstructive infertility with self-made simple coaxial microtube.MethodsSSG and FTR with self-made simple Coaxial Microtube were performed for 76 cases with 134 tubes of obstructive oviduct through wire-guided tubal recanalization,anti-inflammatory and anti-adhesion effect of recanalization fluid,and the patients were treated with anti-bacterial and hydrotubation after operations.ResultsSSG displayed that 81 tubes obstruction in interstitial tubal,25 tubes obstruction in isthmus,16 tubes obstruction in ampulla and 12 tubes obstruction in fimbria.The total recanalization rate of FTR was 76.1%,with the recanalization rate 96.3% in interstitial,the recanalization rate 68.0% in isthmus and the recanalization rate 43.8% in ampulla,and no serious complications occurred.All cases were followed up for one to two years postoperatively.Normal pregnancy pationes were 31 cases(40.8%),and 29 women were normal parturition,2 cases(2.6%) were extrauterine pregnancy.ConclusionThe advantage of SSG and FTR with self-made simple coaxial microtube is low cost,simplicity and excellent efficacy.It is worth popularizing in grass-roots hospitals.

5.Protective Effect of Erythropoietin on Anoxia of Motoneurons after Spinal Cord Injury in Rats

Objective To observe the protective effect of erythropoietin (EPO) on anoxia of motoneurons after spinal cord injury in rats.Methods The motoneuron was dissociated from neonate Wistar rats and cultivated in vitro with deprivation of oxygen in the medium. The changes of morphology were observed by phase-contrast microscopy, and the expression of EPO receptor (EPOR) was detected with Western blotting. The survival rate of motoneuron was measured with methyl thiazolyl tetrazolium (MTT) assay, and the level of lactate dehydrogenase (LDH) was observed. Results Compared with the control group, the expression of EPOR and the level of LDH increased (P<0.01),and the survival rate of motoneuron decreased (P<0.01) in the anoxia group. Compared with the anoxia group, the expression of EPOR and the level of LDH decreased (P<0.05), and the survival rate of motoneuron increased (P<0.01) in the EPO group, which was related with the level of EPO. Conclusion EPO could relieve the damage of motoneuron caused by anoxia, especially by upregulating EPOR.
6.Clinical observation on combination therapy of Ninggan-Tiaozhong decoction and psychological intervention for 40 cases of chronic atrophic gastritis

Objective To survey the clinical effect of combination therapy of Ninggan-Tiaozhong decoction and psychological intervention for chronic atrophic gastritis (CAG).Methods 80 cases CAG were randomly divided into a treatment group (treated with Ninggan-Tiaozhong decoction and psychological intervention) and a control group (treated with Ninggan-Tiaozhong decoction),with 40 cases in each group and performed a therapy for 12 weeks.The clinical efficacy and the change of Symptom Checklist (SCL-90)and Self-Rating Anxiety Scale(SAS) before and after treatment were evaluated.Results After 12 weeks of treatment,the total effective rate was 90.0% in the treatment group,and 74.4% in the control group.The treatment group was better than the control group(x2=4.255,P<0.05).After psychological intervention,SCL-90 and SAS score (39.5±4.3,1.3± 0.6)was significantly reduced in the treatment group than the control group (51.4±3.2,1.5±0.3),(P<0.05).Conclusion The combination therapy of Ninggan-Tiaozhong decoction and psychological intervention used in CAG is effective to relieve symptoms and to improve life quality of patients.
7.Preliminary application of spectral CT in differential diagnosis of squamous cell carcinoma and adenocarcinoma of the esophagogastric junction

Objective To evaluate the value of spectral CT imaging for the differential diagnosis of squamous cell carcinoma (SQCC) and adenocarcinoma (ADC) of the esophagogastric junction. Methods Forty-five patients with a mass in the esophagogastric junction proved by pathology underwent enhanced scan with spectral CT, including 20 cases of SQCC and 25 cases of ADC. Iodine concentration (IC) , water concentration (WC) , effective atomic number (Eff-Z) and spectral curve slope (λHU) of arterial phase (AP) and venous phase (VP) in the ROI of the mass were measured with gemstone spectral imaging post-processing software. The independent samples t test was used to compare the quantitative parameters above between two groups on the premise of satisfying normal distribution. ROC curves were drawn for the parameters which showed statistical differences and area under the curve (AUC) was used to measure and compare their respective differential diagnostic performance as well as the best threshold value. Results In AP,the average IC, Eff-Z, andλHU of ADC were (1.75±0.40) mg/ml, 8.65±0.22, and 3.33±0.74, respectively. The corresponding parameters of SQCC were (1.40 ± 0.35) mg/ml, 8.50 ± 0.20, and 2.71 ± 0.66, respectively. These parameters of ADC were significantly higher than that of SQCC (t=-2.833,-2.879,-2.678;P<0.05) . In VP, the average IC, Eff-Z, and λHU of ADC were (2.17 ± 0.23) mg/ml, 8.87 ± 0.11, and 4.10 ± 0.44, respectively. The corresponding parameters of SQCC were (1.67 ± 0.20) mg/ml, 8.60 ± 0.11, and 3.19 ± 0.41, respectively. The difference between ADC and SQCC was statistically significant (t=-6.963,-7.218,-6.521;P<0.05). For the average WC, No difference between the two groups in AP and VP was found. ROC curve analysis showed that IC, Eff-Z, andλHU in VP had better differential diagnostic performances than IC, Eff-Z, and λHU in AP, especially Eff-Z in VP. The AUC for it was 0.97. Using 8.72 as a threshold value, the sensitivity and specificity for diagnosis were 88.9% and 94.7% , respectively. Conclusion Multi-parameters quantitative analysis with spectral CT could be useful in the differential diagnosis of SQCC and ADC of the esophagogastric junction.
8.Medical legal dispute in hospitals of different grades: a retrospective study on 206 cases.

OBJECTIVE:
To analyze the characteristics of medical malpractice from different grades of hospitals and to explore forensic investigation strategies in assessing medical dispute.
METHODS:
A total of 206 cases of medical dispute from 2009 to 2010 investigated by the Department of Forensic Medicine in Nanjing Medical University were selected and analyzed according to fault incidence, fault-prone part, and degree of causality in the treatment.
RESULTS:
Among the 206 cases analyzed, tertiary hospitals, secondary hospitals and primary hospitals showed medium, high and low error rate, respectively. A majority of medical malpractice cases were distributed in the departments of surgery, medicine and gynecology.
CONCLUSION
The frequency and severity of medical malpractice in primary hospitals were high, which were gradually reduced in tertiary and secondary hospitals.

9.Protective effect of arctigenin in GK rats combined with hypertension macroangiopathy.

To study the protective effect of Arctigenin in goto-kakizaki (GK) rats combined with hypertension macroangiopathy. Six-week-old GK rats were divided randomly according to blood glucose level into four groups: the model group and low, middle and high dose arctigenin groups (12.5, 25, 50 mg x kg(-1)), with Wistar rats as the normal group. All of GK rats were given high-glucose and high-fat diet. After 16 weeks, GK rats were orally administrated with 10 mg x kg(-1) x d(-1) N-Ω-nitro-L-arginine methyl ester for eight weeks. During the modeling, all of arctigenin groups were orally administrated with different dose of arctigenin twice a day; The model group and the normal group were given solvents. At the beginning, mid-term and end of the experiment, blood glucose was measured. At the end of the experiment, efforts were made to detect blood pressure, collect abdominal aortic blood after anesthesia, fix thoracic aorta after bloodletting to make paraffin sections, observe morphological characteristics and detect the expression of VEGF by immunohistochemistry. According to the results, the blood glucose rose in all GK rats, with no significant difference between the drug group and the model group. At the end of the experiment, the blood pressure significantly increased in GK rats, indicating that Arctigenin could notably reduce the blood pressure in GK rats in a dose-dependent manner. The blood routine test showed increases in both the total white blood cell count and differential blood count, MPV and PDW, abnormal blood platelet parameters and decrease in PLT in GK rats, suggesting that Arctigenin could remarkably reduce the total white blood cell count and differential blood count, MPV and PDW. The thoracic aortic morphological observation revealed obvious endangium lesions in GK rats, demonstrating that Arctigenin could ameliorate the lesion extent. VEGF immumohistochemical staining showed a higher VEGF expression in the model group but lower expression in Arctigenin groups. In conclusion, Arctigenin had a protective effect on aorta in GK rats. Its mechanism may be related to blood pressure lowering, anti-inflammation, improvement in blood platelet function and reduction of VEGF expression.
