1.Rural children's iodine-nutritional status in Qiannan state:an evaluation on 20-year salt iodization

Objective To investingate the iodine-nutritional status of the rural children form 8 to 10 years old after salt iodzation had been implemented in Qiannan State.Methods The size of thyroid gland waft measured by palpation in 421 rural children aged 8~10 years old in Qiannan State.Calorimetric eerie-arsenic assay and vitriolic ammonium assimilmion were used for testing urinary iodine,while Rsvens test was used to test the intelligent quotient (IQ).Results The rate of goiter was 5.0%(21/421).The median of urinary iodine Was 331.2μg/L The average IQ was 93.32±18.68 of the children aged 8~10.The IQ of children with different iodine-nutritional level was significantly different(P<0.05).Conclusions The children's iodine nutrition in Qiannan State has been improved after the salt adds the iodine.Their intelligence level is quite normally.Iodine dificiency disease has been eliminated according to the national standard in terms of 8~10 year old child ufinaty iodine level and goiter rate.
2.Pathogens in expressed prostatic secretion and their correlation with serum prostate specific antigen: analysis of 320 cases.

OBJECTIVETo investigate the pathogenic infection and its drug resistance in expressed prostatic secretion (EPS) and its correlation with serum PSA, and provide some evidence for the systematic and normalized diagnosis and treatment of prostatitis.
METHODSThree EPS swabs were collected from each of the 320 prostatis patients following measurement of the serum PSA level, 1 for bacterial culture and identification, 1 for detection of Mycoplasma and drug sensitivity, and the other for examination of Chlamydia trachomatis antigen by colloidal gold immunoblot.
RESULTSTotally 244 strains were isolated from the 320 EPS samples, including 188 bacterial strains (dominated by Staphylococcus and sensitive to vancomycin or linezolid) and 44 Mycoplasma and Chlamydia strains (mainly Ureaplasma urealyticum and susceptible to josamycin or doxycycline). The serum PSA level was significantly higher in the pathogen-positive than in the pathogen-negative group ([6.98 +/- 0.56] microg/L vs [2.32 +/- 0.12] microg/L, P < 0.05).
CONCLUSIONProstatitis may lead to the elevation of the serum PSA level and the pathogens involved vary in their resistance to different antibacterial spectrums. Therefore, appropriate and individualized antibiotic therapy should be selected according to etiological diagnosis and the results of drug sensitivity test.

3.Clinical study on the accuracy of dual-energy computed tomography colonography in colorectal neoplasia detection

[Abstract ] Objective To compare the accuracy between regular computed tomography colonography (CTC)and dual-energy CTC in lesion detection.Methods Twenty-eight patients with clinical suspicious space occupying lesions of the colon were selected.All patients were underwent dual-energy mode contrast-enhanced CT scan and the data were reconstructed with colonography and dual-energy iodine maps methods.The diameter,enhanced computed tomography (CT)value and iodine value were measured.The results of colonoscopy and pathology were taken as gold standard.The sensitivity, specificity,accuracy,positive predictive value and negative predictive value of regular CTC and dual-energy CTC were compared.Variance analysis was performed for measurement data comparison among groups and chi-square test was used for count data analysis.Results Among 28 patients,colorectal lesions were detected in 24 cases by regular CTC,of which four cases were false-positive and one case was false-negative confirmed by colonoscopy and pathology.Colorectal lesions were detected in 20 cases by dual-energy CTC,of which no false-positive and one case was false negative confirmed by colonoscopy and pathology.The contrast enhanced CT value of polyps,adenoma,adenocarcinoma and stool was (38.54± 6.82),(49.16±7.31 ),(52.61 ±5 .93 )and (34.00±1 .41 )Hu,respectively.The enhanced value of adenoma and adenocarcinoma was significantly higher than that of polyps and stool,the differences were statistically significant among groups (F = 10.760,P = 0.001 ).There was no significant difference between polyps and stool (t=1 .44,P =0.188).The sensitivity of regular CTC and dual-energy CTC in lesion detection was 95 .6% (95 %cofidence interval(CI ):77.9%-99.2%)and 95 .6% (95 %CI :77.9%-99.2%),respectively.The specificity was 42.8% (95 %CI :15 .4%-93.5 %)and 100.0% (95 %CI :47.9%-100.0%).Conclusion Compared with traditional CTC,dual-energy CTC would distinguish lesions from stool,help differentiate between benign and malignant tumors and further increase the accuracy of CTC diagnosis.
4.Investigation on iodine nourishment and growth of children before and after the implementation of iodine in Qiannan,Gulzhou Province

Objective To explore the relationship of dynamic changes between the iodine nutritional condition and growth of children born before and after the implementation of universal iodine.Methods By means of sampling investigation,urinary iodine and goiter,height,weight,intelligence quotient(IQ)were comprehensively evahated in children aged 7~14 years old born before and after the implementation of iodine in 10 counties and 2 cities of Qiannan.Results The median of urinary iodine of children Was 91.88μg/L in 1984,133.33μg/L in 1994,316.00μg/L in 2006;goiter rate was 21.40%(122/570)in 1984,12.86%(107/832)in 1994,5.28%(45/851)in 2006;the average IQ was 89.18±4.10 in 1994,94.26±0.94 in 2006;children in each age group in 2006 has a higher height and a heavier weight compared with those in 1984.Conclusion The physical development and intelligence of children in the areas of iodine deficiency have improved along with the correction of iodine deficiency.
5.Assessment of the quality of randomized controlled trials in treating coronary heart disease by Chinese patent medicine.

OBJECTIVETo assess the quality of randomized controlled trials (RCTs) on Compound Danshen Dripping Pill, Compound Danshen Tablet, Suxiao Jiuxin Pill, Tongxinluo Capsule, and isosorbide dinitrate (ISDN) in treating coronary heart disease angina.
METHODSRCT reports were retrieved from CNKI, Wanfang Data, PubMed, China Master Theses Full-text Database, Chinese Electronic Periodical Services, Social Sciences Citation Index, Science Direct, Cambridge Journals Online, and EBM Reviews. Qualities of RCTs were assessed according to Jadad scale, M scale, CONSORT 2010, and CONSORT for herbal medicine. Kendall correlations between basic study characteristics and qualities of included RCTs were analyzed using R statistical software.
RESULTSEighty-eight RCTs were included. The medians (means, 95% CIs) were 2.00 (2.09, [1.81,2.37]) for the Jadad scale, 4.00 (3.52,[2.95, 4.09]) for the M scale, 15.00 (15.39, [13.87, 16.91]) for the CONSORT 2010, and 19.00 (19.06,[17.16, 20.96]) for the CONSORT for herbal medicine. Only 9.09% (8/88) of RCTs were of high quality (Jadad score >2). These RCT quality measures were not correlated with individual items of reporting, such as sample sizes and follow-up periods.
CONCLUSIONSThe quality of RCTs on Chinese patent medicine compared with ISDN was not improved from 1997 to 2009. It is urgent to improve the design of RCTs and the report quality.

6.Establishment and Preliminary Application of the Method for Detecting Expression of Human CDK14 with Real-Time Quantitative PCR

Objective The method for detecting expression of human CDK14 gene with Real-time quantitative PCR was developed.Methods To establish a method for detecting expression of human CDK14 gene with Real-time quantitative PCR by designing and synthesis of the primers of CDK14 target gene andβ-Actin reference gene and extracting total RNA from different lung cancer cell lines.Then the specificity,detection range and repeatability of this method were evaluated.At last,the expression level of CDK14 gene in different cell lines,which were with or without siRNA interference,were carried out by using this method.Results The method for detecting expression of human CDK14 gene with Real-time quantitative PCR,which had good specificity,good repeatability (CV=7.3 %) and wide detection range (Ct value range of CDK14 and β-Actin amplification curve were 22.47~32.96 and 15.14~ 27.55 respectively,r2 =0.9844),was developed and it was verified by electrophoresis analysis,melting curve,PCR product sequencing.And CDK14 gene expression level,which was detected by this method,increased in HCC827 D5,H1650 and number 1 siRNA segment was effective interference segment.Conclusion The method for detecting expression of human CDK14 gene with Real-time quantitauve PCR was established successfully.
7.A meta-analysis of the long-term effects of chronic pancreatitis surgical treatments: duodenum-preserving pancreatic head resection versus pancreatoduodenectomy.

BACKGROUNDSurgery is regarded as the most effective treatment to relieve pain and reduce complications in chronic pancreatitis (CP). Two major strategies exist: duodenum-preserving pancreatic head resection (DPPHR) and pancreatoduodenectomy (PD). Many studies suggest that DPPHR offers advantages during surgery and in the short-term; however, the long-term effects have not been thoroughly investigated. We analyzed the long-term outcomes of DPPHR and PD, over follow-up times of at least 1 year, to determine the optimal surgical treatment for CP.
METHODSWe systemically reviewed all CP surgical treatment reports, and only included randomized controlled trials (RCT) comparing DPPHR and PD, excluding unqualified studies using several pre-specified criteria. When multiple publications of a single trial were found, the most comprehensive current data were selected. Characteristics of the study populations and long-term postoperative outcome parameters were collected. The quality of the studies and data was analyzed using RevMan 4.2 software.
RESULTSFive trials were qualified for meta-analysis, with 261 participants in total (114 in the DPPHR group and 147 in the PD group). There were no significant differences in the age, gender, or indications for surgery of each group. At the mean of 5.7-year (1 - 14 years) follow-up examination, DPPHR and PD resulted in equally effective pain relief, exocrine and endocrine function, and similar mortality rates (P > 0.05); however, DPPHR patients had improved global quality of life and weight gain, and reduced diarrhea and fatigue (P < 0.05).
CONCLUSIONDPPHR and PD result in equal pain relief, mortality, and pancreatic function; however, DPPHR provides superior long-term outcomes.

8. Advances in the research of Fournier gangrene

Fournier gangrene is a relatively rare clinical critical disease, and its clinical symptoms are not specific and easily unrecognized by some clinicians. It has the features of acute onset, quick development, severe illness, and often accompanied by infection shock which is seriously life-threatening. It is difficult in treatment with high medical costs and long length of hospitalization, which increases pain for patients and relatives and brings heavy economic and psychological burden on patients, society, and medical workers. By reviewing the literature home and abroad and combined with clinical practice, I summarize the researches on concept, epidemiology, clinical manifestation, diagnosis and treatment of Fournier gangrene, in order to provide reference for vast number of clinical workers.
9.Technical tips and pitfalls of total laparoscopic esophagojejunostomy

With the development of laparoscopic technology and the accumulation of experience, total laparoscopic total gastrectomy has been gradually popularized and applied. Among them, laparoscopic esophagojejunostomy is the biggest challenge in this surgery because of its high difficulty. More and more surgeons have been trying to find a more feasible anastomosis method. So far, there have been many kinds of esophagojejunostomy, and they are constantly being improved. The purpose of this article is to review the technical tips and pitfalls of common laparoscopic esophagojejunostomy.
10.Relationship between serum HER2 extracellular domain levels, tissue HER2 expression, and clinico-pathological parameters in early stage breast cancer.

BACKGROUNDMeasurement of human epidermal growth factor receptor 2 (HER2) protein in the serum of metastatic breast cancer patients has previously been reported, but there are no consistent data to support the clinical utility of serum HER2 extracellular domain for patients with early stage breast cancer. We aimed to evaluate the correlation between serum extracellular domain levels and tissue HER2 expression, and analyzed their relationship with clinico-pathological parameters in patients with early stage disease.
METHODSA prospective study was conducted on 232 breast cancer patients with stage I-III prior to treatment. Preoperative serum samples were measured by enzyme-linked immunosorbent assay. Tissue HER2 status was analyzed by immunohistochemistry and fluorescence in situ hybridization assays.
RESULTSThe median serum extracellular domain concentration was 6.8 ng/ml. The best diagnostic cut-off value was 7.4 ng/ml, with 62.9% sensitivity and 85.3% specificity. High serum extracellular domain levels were reported in 89 patients (38.3%), and HER2-positive expression was observed in 77 patients (33.2%). Multivariate analysis showed that elevated serum extracellular domain correlated with postmenopausal status (P < 0.001), high histological grade (P < 0.001), negativity of both estrogen (P = 0.012) and progesterone receptors (P < 0.001), and high levels of carcinoembryonic antigen 153 (P = 0.048).
CONCLUSIONSWe recommend that 7.4 ng/ml should be used as the cut-off value when evaluating serum extracellular domain levels in early stage of breast cancer. Patients with high serum extracellular domain levels have a certain clinico-pathological characteristics, may provide a basis for clinical practice.
