BACKGROUND:Ilizarov technique is an effective treatment method which can solve various complications that occurred in the process of treatment, especial y for the treatment of nonunion associated with soft tissue defects or severe infection. OBJECTIVE:To review the research progress of nonunion treatment with the Ilizarov technique. METHODS:A computer-based search was performed on the CNKI database and PubMed database from January 1992 to April 2013 for the articles related to the Ilizarov technology for the treatment of nonunion. The key words of“nonunion, external fixator, Ilizarov”in English and“nonunion, fracture nonunion, external fixation, Ilizarov”in Chinese were used to search the articles in the title and the abstract. Final y, a total of 37 articles were included to review. RESULTS AND CONCLUSION:Ilizarov technique is an effective method to treat limbs nonunion with the infection and severe bone defect. It has become an international y recognized gold standard. Due to the firmly fixation of the Ilizarov ring external fixator, it can not only eliminate the shear force and rotation stress, thus playing a role of stretch tension and compression, but also has the cyclical micro-axialmotion characteristic during weight-bearing walking which can better promote bone union. Based on the reviews of the research progress of Ilizarov technique in the treatment of nonunion in recent years, we further investigate the advantages and methods of Ilizarov technique in the treatment of limbs nonunion. The clinical application of the method should fol ow the individual principle, and then choose the best and suitable method based on the analysis of advantages and disadvantages about the patient’s condition.

Objective To explore a new surgical treatment for severe inverted nipple and to reduce the possibility of recurrence. Methods Two opposite long six-side polygon de-epithelialized areas were designed within the range of areola on both sides of the nipple. The four sides in lateral portion were dissected and two wedge-shaped dermal and glandular tissue flaps were created, completely releasing the contractive fiber causing nipple retraction while reserving breast ducts. The flaps were reversed 180 degrees through the tunnel bilateral sides of breast ducts to support the nipple. It was tracted for 3-4 weeks by series 7 needle penetrating the nipple basis. Results All the 14 patients were followed up for 3-18 months, and they were satisfied with the appearance and the aesthesia of the nipple and no recurrence was observed, breast-feed function reserved in 2 cases after childbirth. Conclusions This operation is easy to perform with satisfying aesthetic outcome and breast-feed function remaining. It is an ideal method for correcting serious inverted nipple.

Objecfive To analyze the relationship between aortic root dimension(ARD)and anterior circulation infarction(ACI)by measuring ARD of ACI patients. Methods The ARD of 110 ACI patients(56 men and 54 women)were measured with 2-dimensional uhrasonography during ventricular systole.66 control subjects(32 men and 34 women)without the clinical signs of cerebral infarction or organic heart disease and cardiac insufficiency were selected after carrying out detailed cage history inquiring.physical examination,CT or MRI and ultrasonic examination.ResultsARD in men and women Was respectively(29.80±2.35)mm and(28.88±3.06)mm in the ACI group. While it was(27.64±1.70)mm and(26.98±1.60)mm respectively in the control subjects.ARD in men and women was significantly higher in the ACI group as compared with the control subjects(P<0.01).Logistic regression analysis indicated that the odds ratio(confidence interval)associated with ARD Was 1.630(1.205-2.204)in men and 1.494(1.098-2.033)in women for ACI.Conclusions There is a close relationship between ARD and ACI.ARD is an important risk factor of ACI.

Seen from the development of the treatment for hepatocellular carcinoma (HCC),although great progress has been made on the optimization of the treatment, the patients' overall survival (OS) has not been greatly improved compared with other tumors of digestive system such as colorectal cancer. That is mainly because of the early microvascular invasion (MIV) and portal vein tumor thrombus (PVTT) which are among the most important reasons for poor prognosis in HCC.Thus we overviewed the recent research findings on the treatment of HCC with PVTT,which might help the clinicians select the optimal therapeutic strategy.

BACKGROUND:Many scholars researched the biomechanics of middle clavicle fracture plate fixation, but little researched the plate position. OBJECTIVE:To observe the biomechanical characteristics ofanterior and superior plates for clavicle fracture with three-dimensional finite element models. METHODS:Three-dimensional finite element models of clavicle fracture with anterior and superior plates were established. The stress after anterior and superior plate fixation was analyzed. The maximum stress and displacement of plate fixation for clavicular fracture were observed under compression, torsion and three-point bending. RESULTS AND CONCLUSION:(1) In the compressed condition, the maximum stressand maximum fracture displacement were similar between the superior and anterior plate fixation (P> 0.05). (2) Under clockwise twist condition, the maximum stress and maximum fracture displacement were smaler in the superior plate fixation group than inthe anterior plate fixation group (P< 0.05). (3) Under counterclockwise twist condition, the maximum stress and maximum fracture displacement were similar between the anterior and superior plate fixation groups (P> 0.05). (4) Under three-point bending condition, the maximum stress was similar between the superior and anterior plate fixation groups (P> 0.05). The maximum fracture displacement was bigger in the superior plate fixation group than in the anterior plate fixation group (P< 0.05). (5) These findings suggest that superior fixation of clavicle fracture reconstruction plate has more advantages than the anterior plate fixation.

Chinese Journal of General Practitioners 2014;(6):472-474
One hundred and thirty general practitioners ( GP ) who received standard training ( training group ) and 189 GPs who did not receive standard training ( non-training group ) were selected with cluster random sampling method from five districts in Beijing.A questionnaire survey was conducted from April to May 2013, including basic information , working condition , professional competency and continuing education of the interviewees. The survey showed that 72.3%(94/130) in training group had physician titles;89.3%(116/130 )had work experience <10 y;97.0%(126/130)of them had bachelor degree or above;82.3%(107/130) expressed their interest in scientific research , but the 95.4% of them had no research projects;and 96% had no teaching appointment.GPs in non-training group were in a favorite position of education level and professional competency over those in training groups ; however , the latter were highly enthusiastic in further training and scientific research , and had strong potential for career development.
