1.Research on advantage chooses of CT and MRI in diagnosing spine tuberculosis

Objective:To analyze manifestation of spine tuberculosis in CT and MRI, compare two kinds of techniques respectively of diagnosis advantage. Methods: Retrospective analysis of 18 cases confirmed by the surgical operation and pathology imaging of CT and MRI Material of spine tuberculosis. Results: The CT can nicely show that the bone destruction, the bone increase osteoslerosis, sequester and new-born formation and narrow of intervertebral disc, swelling paraspinal soft tissue and psoas major muscle abscess, spinal canal of bone is narrow, calcification. Although CT can show intervertebral disc involvement and terminal plate of vertebral body destruction in sagittal reconstruction, not equal to MRI sensitive, Evaluating intervertebral disc involvement and area of paraspinal soft tissue swells, changed in early days spine tuberculosis of valuation, MRI had obvious advantage. Conclusion: Combining CT and MRI can reflect the image of spine tuberculosis to learn a characteristic completely and be advantageous to diagnosing and discriminate diagnosis of spinal tuberculosis.
2.Expression and significance of E-cadherin in the serum and transplanted tumor of nude mouse model of low rectal cancer with lymph node metastasis

Objective To establish a nude mouse model of low rectal cancer with lymph node metastasis, to detect the expression of E-calcium (E-cad) in the serum and transplanted tumor tissue of the mice, and to analyze the role of E-cad in lymph node metastasis of rectal cancer.Methods Human colorectal adenocarcinoma cells (SW-480) were inoculated into the rectal submucosa of nude mice to establish a model of low rectal cancer with lymph node metastasis.120 nude mice were randomly divided into three groups (40 mice in each group).Group A (rectal cancer model group) was injected with dimethylhydrazine and implanted with SW480 cells.Group B (dimethylhydrazine control group) was injected with dimethylhydrazine alone.The group C (control group) received no treatment.Serum E-cad was detected by ELISA.The mRNA and protein expression of E-cad in the tumor tissue was detected by RT-PCR and western blot, respectively.Results The serum level of E-cad in group A was 19.48±1.25 mg/L, significantly higher than those in groups B (2.36±0.18 mg/L) and C (2.15±0.12 mg/L) (t =8.28, 9.01, P < 0.05).The serum levels of E-cad were significantly higher in the nude mice with lateral lymph node metastasis than those without lymph node metastasis (t =10.28, P < 0.05).The protein expression of E-cad in the group A was lower than that in the groups B and C (t =9.81, 7.69, P < 0.05).In the group A, the protein expression of E-cad in the nude mice with lateral lymph node metastasis was significantly lower than that without lateral lymph node metastasis (t =9.36, P < 0.05).The mRNA expression of E-cad in the group A was significantly lower than that in the groups B and C (P < 0.05), and the mRNA expression of E-cad in the nude mice with lateral lymph node metastasis was significantly lower than that in the mice without lateral lymph node metastasis (t =7.85, P < 0.05).Conclusions The serum level of E-cad may be closely associated with the lymphatic metastasis of rectal cancer, and the mRNA and protein expressions of E-cad in colorectal cancer tissue were weak or absent, leading to a decrease of adhesion ability between cancer cells, and promote the invasion and metastasis of cancer cells.
3.Analysis of the relationship between serum HBV-DNA And hepatic fibrosis

Objective To analyze the relationship between serum HBV-DNA and hepatic fibrosis in patients with hepatitis B. Methods Serum HBV-DNA load and hepatic fibrosis hyaluronic acid (HA),Ⅲ procollagen N terminal peptide(PⅢNP),laminin (LN),Ⅳ collagen(Ⅳ-C)of 154 cases were detected by real-time fluorescence quantitative PCR and chemiluminescence method. Statistical analysis of the correlation between HBV-DNA load and hepatic fibrosis indicator was performed.Results The data of HBV-DNA load after taking lg value was 5.49±1.39;The serum concentration of hepatic fibrosis HA was (166.95±148.20)ng/mL,PⅢNP was (12.14±7.37)ng/mL,Ⅳ-C was (65.29±45.48)ng/mL,LN was (59.52±35.67)ng/mL.The difference between experimental group and control group in hepatic fibrosis was significant(P <0.05).Serum HBV-DNA only correlated with LN(r=0.284,P =0.015).Conclusion There is no significant correlation between serum HBV-DNA and hepatic fibrosis in hepatitis B pa-tients.Anti-fibrosis treatment should also be strengthened for hepatitis B patients in the course of antiviral treatment.
4.The application of large flat-panel multifunction digital X-ray machine in digestive system

Objective: To evaluate the application value of large flat-panel multifunction digital machine in digestive system. Methods: Exam the gastrointestinal tract and biliary with dynamic detector, and then observe its advantages and disadvantages. Results: All the 13 gastrointestinal cancer lesions and 15 inflammatory lesions were shown clearly, including mucosa. Biliary tomography found two small stones, which were undetected by the general X-ray photography. The excellent rate of the photo was improved to 98%. Conclusion: The advantages of the flat panel detector include the wide range of display, clear image of digital pulse fluoroscopy spot film, the improvement of the diagnostic accuracy and the reduction in radiation dose. The disadvantage is that it is more inconvenient than the remote cradle gastrointestinal machine in rotating the position of the patient.
5.Diagnosis and treatment of abdominal compartment syndrome aftergiantabdominal incisional her-nia surgery

Objective To investigate the diagnosis and treatment of abdominal compartment syn-drome (ACS)aftergiantabdominal incisional hernia surgery.Methods The diagnosis and treatment of eight cases of ACS after giantabdominal incisional hernia surgery in our hospital from June 201 0 to June 201 4 were retrospectively analyzed.Results Seven cases of ACS were cured,with a cure rate of 87.5%. One case with chronic pulmonary heart diseasedied ofsevere lung infection caused by ARDS,with a mor-tality of 1 2.5%.The cured patients discharged and werefollowedup for threeto six months,without any signs of recurrence and organ dysfunction.Conclusion Abdominal compartment syndrome is a serious complication aftergiantabdominal incisional hernia surgery,with treatment difficult and high mortality.The keysinreducingmortalityare early diagnosis and comprehensive treatment.
6.Transcatheter selective hepatic arteriography and embolization in treatment of hepatic hemangioma

Objective To investigate the features of transcatheter selective hepatic arteriography and efficacy of embolization in hepatic hemangioma. Methods Twenty-six patients of hepatic hemangioma underwent transcatheter selective hepatic arteriography and embolization with Lipiodol-ultrafluida as well as Pingyangmycin, and were followed-up for 3 - 12 months after treatment. Results The popcorn-like tumor stain sign and early fill-in and persistant staining sign of hepatic arterial angiography were observed in all patients before embolization. After one time of embolization, the tumor size decreased more than 25% in 22 patients (84.62%), less than 25% in 4 patients. No severe complication occurred. Conclusion Transcatheter selective hepatic arteriography and embolization is a favorable method of diagnosis and treatment for hepatic hemangioma.
7.Preliminary study of spectral CT in benign and malignant lung lesions

Objective To investigate the value of spectral CT for the discrimination of benign and malignant lung lesions through multi-parameter quantitative measurement.Methods Eighty-one patients who had lung nodule or mass on X-ray or plain CT were scanned using spectral CT,generating iodine concentration,water concentration and 101 sets of monochromatic images.Iodine concentration,water concentration,spectrum energy curve and CT value at 40 keV were measured with spectral post-processing software.70 eligible patients were divided into pneumonia group and tumor group.ANOVA analysis was used for the statistic analysis in two groups.Results The iodine concentration in pneumonia group and tumor group were (1.47 ± 0.17) and (0.77 ± 0.19) g/L (F =203.97,P < 0.05).The slope rates of spectrum energy curve in pneumonia group and tumor group were 1.65 ± 0.27 and 0.88 ± 0.22 (F =161.59,P <0.05).The CT values at 40 keV in pneumonia group and tumor group were (139.00 ± 16.25) and (83.29 ± 17.44) HU (F =160.45,P < 0.05),there were significant differences in iodine concentration,slope rates and CT values at 40 keV between two groups.The water concentration in pneumonia group and tumor group were (1021.56 ± 11.68) and (1022.62 ±8.56) g/L (F=0.19,P>0.05),there was no significant difference.Conclusion Spectral CT technique has very important value for the discrimination of benign and malignant lung lesions.
9.Application value of percutaneous puncture biopsy technique under CT guide in clinical ;pathological changes of breast

Objective: To evaluate the diagnosis value of percutaneous puncture biopsy technique under CT guiding in clinical pathological changes of breast. Methods:46 patients with breast lumps or nodules, including six cases of pleural disease, were carried out percutaneous puncture biopsy under CT guide, where a coaxial Angiotech 17G trocar together with BiopincelSG were used in the puncture process. Results: 44 cases of pathological findings were acquired and the diagnosis ratio was 93.4%, including 9 cases of squamous cell carcinoma, 24 cases of adenocarcinoma, 3 cases of small cell carcinoma, 1 case of metastatic tumor, 1 case of tuberculosis cases, 5 cases of inflammation and 1 case of normal reflexed pleura. 2 cases have no clear diagnosis. Conclusion: The percutaneous lung biopsy technique under CT guide has the merits of less diagnosis trauma, high diagnosis accuracy and high clinical application value.
10. Meta-analysis of the effect of acupoint injection on pain improvement and joint function in patients with knee osteoarthritis

BACKGROUND: A large number of clinical research data show that acupoint injection has the advantages of good therapeutic effect and few adverse reactions in the treatment of knee osteoarthritis, but there is a lack of related multicenter and large-sample clinical trial. OBJECTIVE: To evaluate the effect of acupoint injection on the pain improvement and joint function in patients with knee osteoarthritis by meta-analysis. METHODS: CNKI, Wanfang, VIP, CBM, PubMed, Embase and Cochrane Library databases were searched for the randomized controlled trials concerning the acupoint injection for treating knee osteoarthritis published before May 2019. The observation group was acupoint injection or acupoint injection combined with other treatments. The control group was the treatments different from acupoint injection. The language was either in Chinese or English. After screening, the data included in the study were extracted by Review Manager 5.3 software for meta-analysis. RESULTS AND CONCLUSION: (1) Sixteen randomized controlled trials were included, involving 1 287 patients (648 cases in the observation group, and 639 cases in the control group). (2) Meta-analysis results showed that the overall effective rate, cure rate, Lysholm score and Japanese Orthopaedic Association scores in the observation group were significantly higher than those in the control group [OR=3.23, 95%CI (2.19, 4.76), Z=5.91, P < 0.000 01; OR=1.86, 95%CI (1.35, 2.58), Z=3.74, P=0.000 2; MD=7.87, 95%CI (2.66, 13.08), Z=2.96, P=0.003; MD=9.16, 95%CI (4.18, 14.13), Z=3.61, P=0.000 3]. The Visual Analogue Scale score, WOMAC score, Lequesne index score, and the incidence of adverse reactions between two groups showed no significant difference [MD=-1.11, 95%CI (-2.29, 0.07), Z=1.84, P=0.07; MD=-7.56, 95%CI (-17.26, 2.14), Z=1.53, P=0.13; MD=-0.46, 95%C I(-1.62, 0.71), Z=0.77, P=0.44; OR=1.31, 95%CI (0.31, 5.57), Z=0.37, P=0.71]. (3) These results indicate that the efficacy of acupoint injection for treating knee osteoarthritis is accurate. Because the literature quality included in the study is not high and bias may exist, more high-quality randomized controlled trials are still needed to verify the conclusions.
