2.EST Combined with LC in Treating Cholecystolithiasis with Extraheptatic Bile Duct Stones

Objective To explore the value of the combination of endoscopic sphincterotomy (EST) and laparoscopic cholecystestomy (LC) in the treatment of Cholecystolithiasis with extraheptatic bile duct stones. Methods 62 cases with cholecystolithiasis combined with extraheptatic bile duct stones were underwent LC following EST. Results Two cases were treated with common bile duct exploration after the failure of EST. 60 cases were executed sucessfully by EST and LC. No case occured serious complications. 8 patients with transient hyperamylasemia and 4 cases with intestinal bleeding after operation were cured by means of conservative therapy. Conclusion Combination of EST and LC is a safe and effective method in the treatment of cholecystolithiasis with extraheptatic bile duct stones.

4.Femoral head replacement and clover titanium plate fixation in the repair of unstable intertrochanteric fractures in elderly patients

BACKGROUND:The unstable intertrochanteric fractures are common in the elderly patients. Artificial femoral head replacement is a common treatment method, and different ways can be selected for reconstruction. OBJECTIVE:To explore the clinical effect of femoral head replacement combined with clover titanium plate fixation for unstable intertrochanteric fractures of the elderly. METHODS:A retrospective analysis was performed in 64 elderly patients with unstable intertrochanteric bone in the Jiangyou 903 Hospital from December 2011 to December 2012. They were given femoral head replacement treatment, and were divided into control group (32 cases) and experimental group (32 cases) according to reconstruction method. They received steel wire tension band and reconstruction and clover titanium plate fixation. RESULTS AND CONCLUSION:There were no significant differences in operation time and bleeding volume in operation process, postoperative recovery time of joint activities, time of out-of-bed activity, and bone calus formation time between the two groups (P > 0.05). However, compared with the control group, therapeutic effect was better and the incidence of complications was low in the experimental group (P < 0.05). These data suggest that femoral head replacement combined with the implementation of clover titanium plate fixation treatment can obtain good results for treatment of unstable intertrochanteric fracture in elderly patients.
5.Mr. Wang Youren's Experience of Treating Scagulohumeral Periarthritis by Spinning Neck and Massage of Shoulders

Objective] Summarize clinical experience about treatment of scapulohumeral periarthritis with neck rotation and shoulder movement proposed by the well-known doctor Wang Youren from Beijing Massage Hospital. [Method] Observe the clinical treatment of scapulohumeral periarthrit, organize the related medical records and draw on the treatment thoughts and operational method.[Result] Mr. Wang thinks that there is close connection between the scapulohumeral periarthriti and neck in the perspective of pathogenesis and anatomy and physiology. It will have better treatment effect if giving the fine-tuning to the neck while treating the scapulohumeral periarthritis. [Conclusion] Mr. Wang Youren has unique clinical treatment experience in the area of scapulohumeral periarthritis, and this treatment has significant effect. Thus it is worth of spreading.

CBX protein(chromoboxin protein homolog)is an important member of PcG(polycombe group proteins)family protein,and PcG protein is an epigenetic regulatory complex that exists in the form of polymeric transcriptional inhibitory complexes PRCs.PcG protein can modify the target gene to transcript chromatin and plays an important role in stem cell differentiation and tumorigenesis and metastasis.The CBX protein family is an important member of the classic PRC1.Recent studies have focused on the different functions of CBX in stem cells,further demonstrating the composition and function of CBX.From more evidence,CBX plays an important role in tumorigenesis and development.This review summarizes the current advances in the study of CBX family members in tumors.
7.Clinical Observation of Formoterol Combined with Budesonide in the Treatment of Chronic Bronchial Asth-ma

OBJECTIVE:To observe the efficacy and safety of formoterol combined with budesonide in the treatment of chron-ic bronchial asthma. METHODS:Totally 80 patients with chronic asthmatic patients were randomly divided into control group and test group. Control group was given corticosteroids,antibacterial drugs or doxofylline and other conventional treatment;based on the treatment of control group,test group was treated with formoterol 1 suck+budesonide 1 suck by aerosol inhalation,once-twice a day. The treatment course was 2 weeks. The clinic data was observed,including clinical efficacy,IL-17,IL-33,FEV1/FVC,PEF before and after treatment,daytime symptom scores,nighttime symptom scores and incidence of adverse reactions. RESULTS:The total effective rate in test group was significantly higher than control group,the difference was statistically significant(P<0.05). Af-ter treatment,IL-17,IL-33,daytime symptom scores and nighttime symptom scores in 2 groups were significantly lower than be-fore,and test group was lower than control group,FEV1/FVC and PEF were significantly higher than before,and test group was higher than control group,the differences were statistically significant(P<0.05 or P<0.01). There was no significant difference in the total incidence of adverse reactions(P>0.05). CONCLUSIONS:Based on the conventional treatment,formoterol combined with budesonide has good efficacy and safety in the treatment of chronic bronchial asthma.
8.Development of Novel Antiviral Therapies for Hepatitis C Virus

Over 170 million people worldwide are infected with hepatitis C virus (HCV), a major cause of liver diseases. Current interferon-based therapy is of limited efficacy and has significant side effects and more effective and better tolerated therapies are urgently needed. HCV is a positive, single-stranded RNA virus with a 9.6 kb genome that encodes ten viral proteins. Among them, the NS3 protease and the NSSB polymerase are essential for viral replication and have been the main focus of drug discovery efforts. Aided by structure-based drug design,potent and specific inhibitors of NS3 and NSSB have been identified, some of which are in late stage clinical trials and may significantly improve current HCV treatment. Inhibitors of other viral targets such as NSSA are also being pursued. However, HCV is an RNA virus characterized by high replication and mutation rates and consequently, resistance emerges quickly in patients treated with specific antivirals as monotherapy. A complementary approach is to target host factors such as cyclophilins that are also essential for viral replication and may present a higher genetic barrier to resistance. Combinations of these inhibitors of different mechanism are likely to become the essential components of future HCV therapies in order to maximize antiviral efficacy and prevent the emergence of resistance.

Autophagy is a kind of ubiquitous physiological mechanism in normal and abnormal cells.Autophagy plays a key role in protein equilibrium and cleaning impaired organelle to maintain homeostasis in cells.It plays different roles at different stages of carcinogenesis by promoting tumor development or inhibiting tumor growth.It has potential applications for tumor therapy by up-regulating or down-regulating the level of au-tophagy in tumor cells.
10.Detection of Brucella by loop-mediated isothermal amplification

Objective A loop-mediated isothermal amplification (LAMP)assay was developed for rapid detection of Brucella.Methods Using Primer 4.0,we designed four specific primers at brookings coli outer membrane protein (OMP31) gene conservative area,under the action of Bst polymerase larger pieces to realize the step-like isothermal amplification of DNA.In this method,on the basis of optimal amplification conditions,the specificity and sensitivity of this method was compared with conventional PCR,and LAMP visualization experiment was conducted.Results The detection results of Brucella abortus (B.abortus) strain 544,B.abortus strain 104M,Brucella melitensis (B.melitensis) strain Rev-1,B.melitensis strain 16M,Brucella suis (B.suis) strain S2,B.suis strainand 1330S,Brucella canis (B.canis) strain RM6/66,Brucellá ovis (B.ovis) strain 63/290,and Brucella neotomae.(B.neotomae) strain 5K33 were positive,and Yersinia enterocolitica (Y.enterocolitica) O ∶ 9,Escherichia coli (E.coli) O157 ∶H7,and (Salmonella typhimurium,S.typhimurium) 47729 were negative.The minimum detection limit was 8.5 × 10-8 mg/L.LAMP was more sensitive than PCR.The results can be determined by electrophoresis or through visual judgment.Conclusions LAMP is a simple,specific and sensitive method.LAMP assay is a useful tool for rapid detection of Brucella.
