1.Progress in synthetic biology of pinocembrin.

Pinocembrin, belonging to flavanons, was isolated from various plants. Pinocembrin has a variety of pharmacological activities, such as neuroprotective effect, antimicrobial activity, and antioxidant efficacy. Pinocembrin was approved as class I drugs to its phase II clinical trial by CFDA in 2009, mainly used for the treatment of ischemic stroke. As a promising compound, the manufacturing technologies of pinocembrin, including chemical synthesis, extraction from plant and synthetic biology, have attracted many attentions. Compared with the first two technologies, synthetic biology has many advantages, such as environment-friendly and low-cost. Construction of biosynthetic pathway in microorganism offers promising results for large scale pinocembrin production by fermentation after taking lots of effective strategies. This article reviews some of recent strategies in microorganisms to improve the yield, with focus on the selection of appropriate the key enzyme sources, the supply of precursors and cofactors by microorganisms, the choice of substance and the level of the key enzyme expression.

2.Analysis of the risk factors of portal vein thrombosis in liver cirrhosis patients

To research the risk factors of portal vein thrombosis( PVT) in liver cirrhosis patients. 110 cirrhotic pa-tients with ( n =39 ) or without ( n =71 ) PVT diagnosed were enrolled. 12 common ultrasonographic and serum markers were analyzed. SPSS software was used for statistic analysis. Single factor analysis and Logistic regression model analysis showed that fibrinogen was the independent risk factor of portal vein thrombosis( PVT) in liver cir-rhosis patients. Fibrinogen should be monitored in patients with cirrhosis to prevent and find the occurrence of portal vein thrombosis.

4.Effect of inhaled arsenic trioxide and moxa leaf oil on eosinophil in asthmatic guinea pigs

Objective The experiment was designed to observe the effect of inhaled arsenic trioxide(As2O3)and in combination with inhaled moxa leaf oil on eosinophils(EOS)and lung tissue's morphology in asthmatic guinea pigs, and to explore its mechanism of relieving wheezing. Methods The ovalbumin(OVA)-induced asthmatic guinea pig model was established. Sixty guinea pigs were randomly divided into As2O3 atomization inhalation group B1 As2O3 low dose group:2.0 mg/(kg·d), B2 As2O3 high dose group:4.0 mg/(kg·d), combined treatment group C1 low dose group:2.0 mg/(kg·d)+moxa leaf oil 0.05 ml, C2 high dose group: 4.0 mg/(kg·d)+moxa leaf oil 0.10 ml, saline aerosol inhalation group(group A) asthma model group)and normal control group(group D)according to random permutation table method. After 7 days medicated, the EOS in bronchoalveolar lavage fluid(BALF)were compared. Guinea pig lung, heart, liver and kidney pathological specimens were prepared and the change of the lung tissue's inflammation and heart, liver and kidney tissue were investigated. Lung tissue’s electron microscopy specimens were prepared and the apoptosis of acidophilic cells and repair of alveolar epithelial cells were observed. Results Compared with asthma model group's EOS number[(58.08±19.01)×105], the difference in As2O3 low dose group、As2O3 high dose group、combined treatment low dose group、combined treatment high dose group and normal control group[(26.37±1.12)×105, (11.50±1.61)×105, (14.16±4.88)×105, (5.03±1.66)×105 and(0.35±0.16)×105, respectively] were statistically significant(P<0.01); there was statistical difference between As2O3 low dose group and As2O3 high dose group(P<0.01);There was statistical difference between As2O3 low dose group and combined treatment low dose group(P<0.01);There was statistical difference between As2O3 high dose group and combined treatment high dose group(P<0.05). Compared with combined treatment high dose group, difference in As2O3 low dose group、As2O3 high dose group and combined treatment low dose group was statistically significant(P<0.05). The difference in B1, B2, C1, C2 and D group was statistically significant compared with group A (P<0.01). The difference in B1 and B2, B1 and C1 group was statistically significant (P<0.01). The difference between B2 and C2 group was statistically significant(P<0.05). The difference in C2 group was statistically significant compared with group B1, B2 and C1(P<0.05). Conclusion EOS abnormal apoptosis was an important pathogenesis of bronchial asthma. The mechanism of arsenic trioxide anti-asthma was that arsenic trioxide could reduce acidophil infiltration and promote acidophilic cell apoptosis. The mechanism of moxa leaf oil anti-asthma was it could reduce acidophil infiltration. Smaller doses of the combination of inhaled As2O3 and moxa leaf oil was safe and effective to achieve the effect of relieving wheezing and the combined use had synergy.
5.Allograft cancellous bone combined with autologous red marrow for treatment of periarticular fractures

BACKGROUND:Bone defects often occur after the reduction of periarticuar fractures, and bone grafting is required to fil bone defects, thereby to make the early support of articular surface and prevent the col apse and shift of the articular surface. Al ograft cancellous bone is the transplant material for the treatment of bone defects, but its osteogenesis ability is poor. Autologous red marrow has osteogenesis ability. Therapeutic efficiency of al ograft cancellous bone combined with autologous red marrow for periarticular fractures is stil yet to be
assessed.
OBJECTIVE:To study clinical effect of locking plate fixation and al ograft cancellous bone combined with autologous red marrow in the treatment of periarticular fractures.
METHODS:Forty-three cases of periarticular fractures were incorporated into the Orthopaedics Department of the Third Hospital of Hebei Medical University. After cutting the articular surface anatomical y, composite particles of the red marrow and al ograft cancellous bone were implanted into bone defects, and then, an anatomic locking plate was used. Medial lateral or bilateral locking plate was used for tibial plateau fractures. Dorsal or volar locking plate was used for distal radius fractures, and distal tibial medial or lateral locking plate was used for distal tibial fractures.
RESULTS AND CONCLUSION:Forty-three patients were fol owed up for 12 months to 6 years, an average of 4.3 years. X-ray films and CT scans review showed that 43 patients’ col apse fracture al reached bony union. Fresh fracture healing time was 2-6 months, an average of 4 months;the healing time for old fracture was 3-7 months, an average of 5.5 months. After bone grafting, 43 patients al had no significant immune rejection, two cases showed more wound exudates, and the wound was healed by dressing after 2 weeks. One case had wound infection, the wound was healed after 4 weeks of draining and dressing, and the infection had no recurrence fol owing up for 4 years and 1 month. Forty patients were satisfied with bone grafting, accounting for 93%, and three cases dissatisfied, accounting for 7%, based on Mankin and Komender’s standard assessment. The results confirmed that al ograft cancellous bone combined with autologous red marrow transplantation can play a supporting role in treating periarticular fractures to prevent the col apse of the articular surface and fracture displacement, and to provide reconstruction materials for periarticular defects. Its long-term goal is fracture healing.
6.Evaluation of Plasma Concentration Monitoring Method for Sodium Valproate by Fluorescence Polarization Immunoassay

OBJECTIVE:To explore quality control of plasma concentration monitoring for sodium valproate and to improve the quality of pharmaceutical monitoring.METHODS:The statistical and continuous analysis of sodium valproate control data from our hospital of 2008 were conducted using fluorescence polarization immunoassay(FPIA).RESULTS:Mean recovery and RSD were 98.37% and 1.94% for low control samples,99.33% and 2.88% for medium control samples,98.17% and 4.10% for high control samples.The RSD of sample was lower than 5% and in line with the requirement for biological sample determination in Chinese Pharmacopoeia.CONCLUSION:FPIA is a comparatively accurate and stable method for plasma concentration monitoring of sodium valproate.
7.Drug Resistance of Pathogens in Pediatric Intensive Care Unit

OBJECTIVE To investigate the drug resistance of pathogens in pediatric ICU and discuss how to treat infections caused by these resistant strains.METHODS After genus identification,bacterial susceptibility test was carried out using Kirby-Bauer disk method.RESULTS A total of 270 clinical isolates were analyzed including 194(72%) strains of Gram-negative bacilli and 76(28%) strains of Gram-positive cocci.From them 182(67%) strains were isolated from sputum,78(29%) strains from blood and 10(4%) strains from secretion.The most common bacilli were Klebsiella spp,coagulase negative Staphylococcus,Escherichia coli,Enterococcus spp and Pseudomonas aeruginosa.About 66.7% of E.coli and 91.1% of Klebsiella spp isolates produced ESBLs,the isolating rate of MRCNS was 83.9%.CONCLUSIONS The characteristics of pathogen distribution and drug resistance surveillance must be understood in order to use antibiotics rationally,to control the increasing trend of drug resistance pathogens and to forbid the break out of nosocomial infections.

A 73-year-old man developed a skin-colored granule-sized papule on the scrotum 10 years prior to the presentation,which had increased in size and number without obvious symptoms.Histopathological examination showed multiple enlarged intradermal cysts.The cyst wall consisted of one or two layers of flat or columnar cells.Decapitation secretion was seen in the inner layer cells,and the out layer of cysts was formed by flat or cubic myoepithelial cells.A diagnosis of apocrine hidrocystoma of the scrotum is made.
10.An overview of effects of traditional medicine on pharmacokinetics of western medicine.

Traditional medicine (herb medicine) began to prevail again over last two decades, and it is about 70% of the world population taking herb medicine as supplement or alternative medicine according to a recent survey. The consumption of herb medicine increased exponentially in Canada, Australia and Europe during last 10 years. Since concomitant administration of herbal and western medicine has become a trend, it requires paying close attention to the problem. Herb-drug interactions have been extensively investigated worldwide, and there is an increasing concern about the clinical herb-drug interaction. In this review we introduced the current progress in the herb-drug interactions including evidence-based clinical studies and establishment of levels of evidence for herb-drug interaction; and in the related mechanisms including the induction and inhibition of metabolic enzymes, inhibition and induction of transport and efflux proteins, alteration of gastrointestinal functions, and alteration in renal elimination. We also analyzed both the achievements and the challenges faced in the concomitant administration of traditional Chinese medicine and western medicine.
