3.X-linked adrenoleukodystrophy: a retrospective analysis of 11 cases with review of literature

Objective X-linked adrenoleukodystrophy (X-ALD) is a genetically determined disorder that is characterized by demyelination of central nervous system,and impaired adrenal cortex and abnormal accumulation of very long chain fatty acids in body fluid and tissue.The clinical manifestation,biochemical change,and magnetic resonance imaging were analyzed.Methods Clinical data of 11 cases with X-ALD were summarized and analyzed,including symptoms,signs,and inspection result.Relevant literature was reviewed.Results All cases were males,whose average onset age was (7.2 ± 4.7) year-old.It was (2.4 ± 1.9) years that the mean interval appears from onset to diagnosis.Six cases were with onset of adrenal insufficiency (AI),remaining 5 onset neural symptoms,where plasma very-long-chain fatty acids (VLCFA) was tested in 6 patients,all with abnormally high levels and brain magnetic resonance imaging(MRI) showed demyelination of cerebral white matter in 9 ones.Conclusions ALD is a X-linked genetically determined disorder that mainly affects the nervous system and adrenal gland.Plasma VLCFA test,ALD gene test,and cerebral MRI are reliable diagnostic methods.Early diagnosis and appropriate therapy would improve survival and neurological outcomes.
4.Glucocorticoid treatment of children with mycoplasma pneumonia

Objective To observe the clinical effect of glucocorticoid treatment in children with Mycoplasma pneumo-nia. Methods A total of 45 cases of children with mycoplasma pneumonia were randomly divided into two groups, 25 cases in the control group were given azithromycin injection continuous intravenous infusion of 5D. And then changed to Azithromycin for Suspension oral(3D stop 4D), 2-3 courses. The observation group of 20 cases based on the control group were treated with injection of methylprednisolone 1-2 mg/(kg od), intravenous infusion of 3 d, in the two 7 d af-ter treatment groups, the fever, cough relief time time, the improvement of chest X-ray film and duration of hospitaliza-tion were compared. Results Hormone treatment group 13 cases were cured, 6 cases effective, 1 cases ineffective, the total effective rate was 95.0%. Non hormone treatment group 7 cases were cured, 11 cases effective, 7 cases ineffective, the total efficiency of 72.0%. Cure rate of hormone treatment group was significantly higher than that of non steroid group, there was statistically significant differenc(P<0.05). Conclusion Early application of low-dose, short term glucocor-ticoid treatment in children with Mycoplasma pneumoniae pneumonia,it can effectively improve the clinical symptoms, reduce or avoidthe occurrence of lung, pulmonary complications, improve the cure rate, to avoid the occurrence of seri-ous complications.
5.Clinical Study on Anti-aging Action of Herbal Cake-partition Moxibustion

Objective:To investigate the clinical efficacy and immunological mechanism of herbal cake-partitioned moxibustion for aging process.Method:The herbal cake-partitioned moxibustion was adopted for 223 cases to observe the aging scores before and after the treatment.Apart from that,the T-lymphocyte subsets and changes of IL-2 and 3-EP were also detected.Results:After treatment,the aging scores of 223 cases were all substantially reduced,along with an improvement of clinical symptoms,a strengthened cellular immune function,and an increase of total T-lymphocyte count.In addition,the CD4+/CD8+ ratio Was restored normal,the synthesis or secretion of IL-2 was increased and the β-EP(as the neurotransmitter to modulate immune function)was substantially improved.Conclusion:The aging process is closely associated with the immune function.Moxibustion Can modulate abnormal immune function and stabilize homeostasis and thus delay the aging process.

Objective:To establish a disease risk prediction model for the newborn screening system of inherited metabolic diseases by artificial intelligence technology.Methods:This was a retrospectively study. Newborn screening data ( n=5 907 547) from February 2010 to May 2019 from 31 hospitals in China and verified data ( n=3 028) from 34 hospitals of the same period were collected to establish the artificial intelligence model for the prediction of inherited metabolic diseases in neonates. The validity of the artificial intelligence disease risk prediction model was verified by 360 814 newborns ' screening data from January 2018 to September 2018 through a single-blind experiment. The effectiveness of the artificial intelligence disease risk prediction model was verified by comparing the detection rate of clinically confirmed cases, the positive rate of initial screening and the positive predictive value between the clinicians and the artificial intelligence prediction model of inherited metabolic diseases. Results:A total of 3 665 697 newborns ' screening data were collected including 3 019 cases ' positive data to establish the 16 artificial intelligence models for 32 inherited metabolic diseases. The single-blind experiment ( n=360 814) showed that 45 clinically diagnosed infants were detected by both artificial intelligence model and clinicians. A total of 2 684 cases were positive in tandem mass spectrometry screening and 1 694 cases were with high risk in artificial intelligence prediction model of inherited metabolic diseases, with the positive rates of tandem 0.74% (2 684/360 814)and 0.46% (1 694/360 814), respectively. Compared to clinicians, the positive rate of newborns was reduced by 36.89% (990/2 684) after the application of the artificial intelligence model, and the positive predictive values of clinicians and artificial intelligence prediction model of inherited metabolic diseases were 1.68% (45/2 684) and 2.66% (45/1 694) respectively. Conclusion:An accurate, fast, and the lower false positive rate auxiliary diagnosis system for neonatal inherited metabolic diseases by artificial intelligence technology has been established, which may have an important clinical value.
