1.Application of quality care model in perioperative nursing in thyroid surgery patients
Journal of Clinical Medicine in Practice 2015;(20):60-62,69
ABSTRACT:Objective To investigate application effect of quality care model in periopera-tive nursing in thyroid surgery patients. Methods A total of 50 cases with thyroid surgery in our hospital were divided into two groups with 25 people per group according to the willingness to re-ceive quality care.Control group implemented routine perioperative care while the experimental group applied quality care.Psychological situation, complications, relapse and satisfaction evalua-tion were evaluated. Results The SAS and SDS scores decreased significantly in the experimental group after comparison after treatment, there was significant difference (P <0.01);After treat-ment and care,SAS and SDS scores between the two groups was significant difference statistically significant (P <0.01);Wound infection, bleeding and breathing difficulties in the experimental group were significantly lower than that in the control group, there was statistically significant dif-ference (P <0.05);Patient care satisfaction rate in the experimental group was significantly better than that in the control group, there was a significant difference (P <0.01).Conclusion The ef-fect of quality care model in thyroid surgery in perioperative nursing is remarkable, and it is far-reaching significance.

Objective:To explore the effect of applying the wound dressing training based on Thorndike's learning theory (WCTT) in clinical nursing education. Methods:The study conducted controlled trial among nursing interns selected from Thyroid Surgery Department of West China Hospital of Sichuan University, and the nursing students were divided into the experimental group ( n=51) and the control group ( n=49). Nursing students in the experimental group were trained with WCTT, while the control group adopted the traditional teaching. Two groups were investigated by the ability of wound dressing questionnaire and were examined on the practical wound dressing operation to evaluate the teaching effect. SPSS 20.0 was used for statistical analysis. Results:Nursing students in the experimental group showed a higher self-evaluation on such aspects as understanding, interests, theoretical mastery, evaluation of wounds, practical skills, independence, fulfillment and model assessment. Meanwhile, the experimental group had a higher score of practical wound dressing operation than the control group [(93.25±2.11) points vs. (90.06±2.43) points] ( P<0.05). Conclusion:In clinical nursing teaching, WCTT could improve the wound dressing ability, independence and enthusiasm of nursing students, and this training mode had a positive effect on the clinical nursing teaching efficiency, which is worth further promotion.

Objective To understand the rehabilitation of postoperative patients with breast diseases, and to find out the common problems and caring key points,in order to provide a reference for the continuous nursing for breast disease patients after the discharge.MethodsA total of 1583 patients who underwent surgical treatment in thyroid and breast surgical department in West China Hospital of Sichuan University from June 2015 to July 2016 were selected by purposive sampling method. The all patients were followed up on telephone at the time point of 1 month, 3 months, 6 months and 12 months after discharge. The follow-up was about patients' rehabilitation (wound, functional exercise, self-care ability and drainage), mental state, satisfaction with follow-up and so on.Results Female accounted for 1574 of the follow-up cases (99.4%). The average age of the patients was (47.20±11.97) years old. The proportion of malignant tumors was 74.6%. The average body mass index (BMI) was (22.66±2.5)kg/m2. The average surgery waiting days and the average hospitalization days were (4.12±2.40) and (8.54±3.40) respectively. The follow-up loss rate at 1 month, 3 months, 6 months and 12 months after discharge was 2.27% (36/1583), 1.90% (22/1158), 2.93% (26/888) and 4.89% (9/184) respectively. The differences of benign and malignant diseases between patients with different BMI and age were statistically significant (P<0.05).ConclusionsTelephone follow-up is a simple and effective way to understand the rehabilitation of patients, and is worthy of clinical use. Follow-up results suggests that caregivers should concern about postoperative wound healing, affected upper limb functional rehabilitation, and mental health in patients with breast diseases, so as to take effective measures to improve patients life quality according to actual problems.
