1.Development of a grading diagnostic model for schistosomiasis-induced liver fibrosis based on radiomics and clinical laboratory indicators

Objective To investigate the feasibility of developing a grading diagnostic model for schistosomiasis-induced liver fibrosis based on B-mode ultrasonographic images and clinical laboratory indicators. Methods Ultrasound images and clinical laboratory testing data were captured from schistosomiasis patients admitted to the Second People’s Hospital of Duchang County, Jiangxi Province from 2018 to 2022. Patients with grade I schistosomiasis-induced liver fibrosis were enrolled in Group 1, and patients with grade II and III schistosomiasis-induced liver fibrosis were enrolled in Group 2. The machine learning binary classification tasks were created based on patients’radiomics and clinical laboratory data from 2018 to 2021 as the training set, and patients’radiomics and clinical laboratory data in 2022 as the validation set. The features of ultrasonographic images were labeled with the ITK-SNAP software, and the features of ultrasonographic images were extracted using the Python 3.7 package and PyRadiomics toolkit. The difference in the features of ultrasonographic images was compared between groups with t test or Mann-Whitney U test, and the key imaging features were selected with the least absolute shrinkage and selection operator (LASSO) regression algorithm. Four machine learning models were created using the Scikit-learn repository, including the support vector machine (SVM), random forest (RF), linear regression (LR) and extreme gradient boosting (XGBoost). The optimal machine learning model was screened with the receiver operating characteristic curve (ROC), and features with the greatest contributions to the differentiation features of ultrasound images in machine learning models with the SHapley Additive exPlanations (SHAP) method. Results The ultrasonographic imaging data and clinical laboratory testing data from 491 schistosomiasis patients from 2019 to 2022 were included in the study, and a total of 851 radiomics features and 54 clinical laboratory indicators were captured. Following statistical tests (t = −5.98 to 4.80, U = 6 550 to 20 994, all P values < 0.05) and screening of key features with LASSO regression, 44 features or indicators were included for the subsequent modeling. The areas under ROC curve (AUCs) were 0.763 and 0.611 for the training and validation sets of the SVM model based on clinical laboratory indicators, 0.951 and 0.892 for the training and validation sets of the SVM model based on radiomics, and 0.960 and 0.913 for the training and validation sets of the multimodal SVM model. The 10 greatest contributing features or indicators in machine learning models included 2 clinical laboratory indicators and 8 radiomics features. Conclusions The multimodal machine learning models created based on ultrasound-based radiomics and clinical laboratory indicators are feasible for intelligent identification of schistosomiasis-induced liver fibrosis, and are effective to improve the classification effect of one-class data models.
2.Interpretation of Expert consensus on community screening of diabetic retinopathy

Diabetes retinopathy (DR) is listed as one of the chronic diseases that should be focused on in the "14th Five-Year" National Eye Health Plan (2021-2025). Early screening is one of the effective measures to reduce blindness caused by DR. Establishing an efficient and practical community screening model is a powerful guarantee for completing early screening. The Ocular Fundus Diseases Group of the Ophthalmology Branch of the Chinese Medical Association has led the development of Expert consensus on community screening of diabetic retinopathy among DR community screening experts that is suitable for the current national situation, in order to guide and promote the further improvement of DR community screening work in China. This Expert Consensus provides detailed specifications on the current domestic trend of DR, the necessity of screening, the role of artificial intelligence grading, screening process, and quality control. This interpretation further emphasizes the importance of DR community screening, while emphasizing the responsibilities of different departments in the screening process. Finally, recommendations are provided for the sustainability of DR community screening. It is hoped that the screening rate of DR in China can be improved and blindness can be reduced by DR through Expert consensus on community screening of diabetic retinopathy and interpretation of the content.
3.The relationship between blood cell-related inflammatory markers and diabetic retinopathy: a study from Beichen Eye Study

Objective:To observe the correlation between blood cell-related inflammatory markers and diabetic retinopathy (DR).Methods:A cross-sectional study. From June 2020 to February 2022, the phase Ⅰ data of Beichen Eye Study in Tianjin Medical University Eye Hospital were included in the study. The research contents included questionnaires, routine systemic and ocular examinations, and laboratory blood cell-related indicators including mean platelet volume (MPV), platelet distribution width (PDW), neutrophils, and lymphocytes were performed. Neutrophil/lymphocyte ratio (NLR) and platelet/lymphocyte ratio (PLR) were calculated. The diagnosis and classification of DR referred to the international clinical classification standard of DR. Monocular or binocular DR was defined as DR patients. Participants were categorized into different groups based on whether they had diabetes and whether they had DR. The groups included the no-diabetes group, the diabetes without DR group, and the DR group. The Kruskal-Wallis H test was used for the comparison of quantitative data among multiple groups. Wilcoxon test was used for comparison between the two groups. The χ2 test was used to compare the categorical variables between groups. The variables was adjusted step by step, an unadjusted univariate model was built and the different parameters of the model Ⅰ, Ⅱ, Ⅲ were adjusted. The correlation between MPV, PDW, NLR, PLR, and DR in different models was analyzed by logistic regression. The receiver operating characteristic (ROC) curve was used to analyze the diagnostic efficacy of different NLR models for DR. Results:A total of 3 328 subjects were recruited. Among them, 1 121 (33.68 %, 1 121/3 328) were males and 2 207 (66.32 %, 2 207/3 328) were females. The median age of the included participants was 61.84 (6.05) years. The no-diabetes group, the diabetes without DR group, and the DR group were 2 679, 476, and 173, respectively. There was no significant difference in MPV and PLR among the three groups ( H=5.98, 1.94; P=0.051, 0.379). However, compared with no-diabetes group and the diabetes without DR group, PDW and NLR in the DR group showed an upward trend. In model Ⅲ with completely adjusted related factors, NLR was an independent risk factor for DR in no-diabetes group and DR group [odds ratio ( OR)=1.440, 95% confidence interval ( CI) 1.087-1.920, P=0.041], diabetes without DR group and DR group [ OR=1.990, 95% CI 1.440-2.749, P<0.001]. The results of ROC curve analysis showed that the diagnostic efficiency of NLR model Ⅲ was the highest, the area under the curve was 0.751 (95% CI 0.706-0.796, P<0.001), the optimal cutoff value was 0.390, and the sensitivity and specificity were 74.3% and 64.8%, respectively. Conclusions:The NLR of the DR group is significantly higher than that of the no-diabetes group and diabetes without DR group. NLR is an independent risk factor for DR.
4.Researches related to pain in Parkinson's disease:a bibliometrics analysis

Objective To analyze the hotspots and frontiers of researches related to pain in Parkinson's disease. Methods The literatures on pain in Parkinson's disease were retrieved from CNKI,VIP,Wanfang data,CBM and Web of Science Core Collection from inception to November,2023,and were analyzed with CiteSpace 6.1.R6. Results A total of 926 literatures were included with 293 in Chinese and 633 in English,respectively.Chinese high-fre-quency keywords were quality of life,sleep disorders and depression,while English high-frequency keywords were nonmotor symptom,quality of life and levodopa.The latest bursting word in Chinese was pathogenesis,while the latest bursting words in English were exercise and management. Conclusion Number of researches related to pain in Parkinson's disease is gradually rising,and the characteristics,patho-genesis,quality of life,rehabilitation interventions and clinical efficacy have become research hotspots.The mechanism of pain in Parkinson's disease and rehabilitation management program will be the main research top-ics in the future.
5.Analysis on screening results for chronic obstructive pulmonary disease among residents over 40 years in Shanghai Jiading town

Objective:To analyze the screening results for chronic obstructive pulmonary disease (COPD) among residents aged 40 years and above in Shanghai Jiading Town, and to explore the influencing factors of COPD.Methods:The study was a cross-sectional study. Four hundred residents aged 40 years and above in Jiading Town were selected to attend COPD screening by multi-stage stratified random sampling method from 10 to 18 July, 2023. Information on demographic characteristics, disease history and family history, disease behavior and risk factors were collected by a questionnaire survey. Physical examination and pulmonary function tests were performed for all participants. Patients showing<70% FEV 1/FVC in pulmonary function and excluded of other cardiorespiratory diseases were diagnosed as the COPD. The SPSS 25.0 software was used for statistical analysis; and logistic regression was used to analyze the related factors of COPD. Results:There were 166 (41.5%) males and 234 (58.5%) females in the participants. The results of screening showed that 181 (45.3%) individuals were identified as high risk of COPD and 32 cases of COPD was diagnosed with a prevalence rate of 8.0%. The prevalence rate of COPD in males were significantly higher than that in females ( P<0.05). There were no significant differences in the age ( P=0.993), education level ( P=0.158), body mass index ( P=0.776), and waist-to-hip ratio ( P=0.833) between COPD patients and non-COPD participants. Logistic regression analysis showed that frequent coughing at 14 years of age and before ( OR=7.763, 95% CI: 2.898-20.791, P<0.05), Hospitalization for pneumonia or bronchitis at or before 17 years of age ( OR=4.359, 95% CI: 1.343-14.1462, P<0.05), asthma ( OR=11.800, 95% CI: 2.001-69.573, P<0.05), chronic bronchitis ( OR=72.748, 95% CI: 20.501-258.144, P<0.05), emphysema ( OR=23.600, 95% CI: 1.407-395.756, P<0.05),and other respiratory diseases, immediate family members with chronic bronchitis ( OR=6.112, 95% CI: 1.960-19.058, P<0.05) and family history of COPD ( OR=100.920, 95% CI: 14.625-696.390, P<0.05), smoking behavior ( OR=7.017, 95% CI: 2.605-18.906, P<0.05), living with daily smokers ( OR=35.481, 95% CI: 1.609-782.310, P<0.05), and second-hand smoking ( OR=3.448, 95% CI: 1.271-9.352, P<0.05) were independent risk factors for the development of COPD. Conclusion:The study shows the prevalence of COPD and related risk factors in residents over the age of 40 in Shanghai Jiading town, indicating that more attention should be paid for high risk population in COPD screening.

Objective:To systematically review the efficacy of preoperative corticosteroids use as an adjunctive treatment for rhegmatogenous retinal detachment associated with choroidal detachment (RRDCD).Methods:A evidence-based medicine study. The National Library of Medicine's PubMed, Web of Science, CNKI, and WanFang database were searched. Clinical controlled studies were selected the study object was RRDCD patients and the interventions were preoperative corticosteroids used as an adjunctive treatment. The search was conducted from January 2000 to January 2022. Duplicated, incomplete, or irrelevant articles were excluded. The conventional meta-analysis was used to evaluate the efficacy of corticosteroids used before surgery. The network meta-analysis was used to directly or indirectly compare the efficacy of oral corticosteroids or intravenous dexamethasone, peribulbar injection of glucocorticoids, prednisolone acetate eye-drops, intravitreal injection of triamcinolone acetonide (TA) and posterior sub-tenon injection of triamcinolone acetonide. Publication bias was evaluated by funnel plot.Results:According to the search strategy, 43 articles were initially retrieved, and 929 eyes of 13 articles were finally included for analysis; 6 and 10 articles were included in the traditional meta-analysis and the network meta-analysis. Among the 6 studies included in the conventional meta-analysis, 5 studies were retrospective and 1 study was a randomized controlled trial, involving a total of 575 eyes. The analysis results showed that there was no significant difference in the primary retinal reattachment rate between the corticosteroids group and the control group [odds ratio ( OR)= 1.53, 95% confidence interval ( CI) 0.67-3.53, P=0.314]. Among the 10 studies included in the network meta-analysis, 7 studies were retrospective trials, 2 studies were randomized controlled trials, and 1 study was prospective trial, involving a total of 575 eyes. The analysis results showed that there were significant differences in the primary retinal reattachment rate between the triamcinolone acetonide intravitreal injection group and the no corticosteroid treatment group ( OR=4.09, 95% CI 1.06-15.79). Sub-tenon injection triamcinolone acetonide had a higher incidence rate of ocular hypertension than oral glucocorticoid or intravenous dexamethasone ( OR= 4.47, 95% CI 1.42-14.13). Conclusions:Triamcinolone acetonide intravitreal injection before surgery can improve the primary retinal reattachment rate in RRDCD patients. Patients with the posterior sub-tenon injection of triamcinolone acetonide should be alert to elevated intraocular pressure.
8.Combined analysis and verification of differential gene transcriptome and Gene Expression Omnibus database in peripheral blood of patients with proliferative diabetic retinopathy

Objective:To screening differentially expressed genes (DEGs) in proliferative diabetic retinopathy (DR) patients to provide new biological therapeutic targets for proliferative DR (PDR) therapy.Methods:A basic research. A total of 3 PDR patients (group PDR) and 3 non-diabetic patients (control group) were enrolled in the study in Tianjin Medical University Eye Hospital in October 2020. In addition, 40 cases of PDR and non-diabetic patients were selected and divided into PDR validation group and control validation group. Peripheral blood validation test was performed in PDR validation group and control validation group; RNA sequencing was performed in PDR group and control group. Transcriptomics (RNAseq) sequencing technology was used to screen DEG in PDR group and control group. The selected DEGs were analyzed by gene ontology (GO) function enrichment analysis, signal pathway enrichment analysis of Kyoto Encyclopedia of Genes and Genomes (KEGG) and protein-protein interaction network (PPI). The gene expression database was used to find the high-throughput data related to PDR, and multi queue comparison analysis was carried out. The target genes of differentially expressed miRNAs were predicted through targetscan platform, so as to clearly screen the correlation between DEG and PDR. Reverse transcription polymerase chain reaction and Western blot were used to verify the expression of DEG mRNA and protein related to PDR. The relative expression of PDR related DEG mRNA and protein between PDR validation group and control validation group were compared by paired t-test. Results:A total of 1 337 DEGs were screened by RNAseq sequencing in the peripheral blood of patients with PDR, of which 419 genes were up-regulated and 918 down-regulated. Among them, direct inhibitor of apoptosis protein-binding protein with low isoelectric point ( DIABLO), zinc finger and BTB domain containing 10 ( ZBTB10), polo-like kinases 3 ( PLK3), regulatory subunit 1 ( PIK3R1) and B cell translocation gene 3 (BTG3) were differentially expressed in PDR patients. The function of GO was enriched from the analysis of molecular function, biological process and cellular composition. The results showed that DIABLO, ZBTB10, PLK3, PIK3R1, BTG3 were involved in the pathological process related to PDR. KEGG enrichment analysis showed that glucose metabolic pathways such as extracellular matrix receptors, cytokine regulatory pathway, p53 signal pathway and galactose metabolism may be involved in the process of differential genes. The analysis of PPI protein interaction network showed that the larger the DEG-associated protein node, the greater the number of associated nodes. Among them, DIABLO, ZBTB10, PLK3, PIK3R1 and BTG3 played significant roles in the formation of the action network. By comparing and analyzing the existing high-throughput data related to diabetic retinopathy in Gene Expression Omnibus database and predicting by Targetscan platform, it was found that some significant differences in miRNA reported in aqueous humor, vitreous fluid and plasma of DR patients can be regulated by the differential genes found in this study. Compared with the control verification group, the relative expressions of DIABLO, ZBTB10, PLK3, PIK3R1 mRNA and protein in peripheral blood of the PDR verification group were up-regulated, and the relative expression of BTG3 mRNA and protein was down-regulated. Conclusion:DIABLO, ZBTB10, PLK3, PIK3R1 and BTG3 are DEGs in patients with PDR, and they can participate in the disease process by regulating the biological processes of cell proliferation, fibrosis and oxidative stress.
9.Value of the level of peripheral blood ionized calcium on the prognosis of elderly patients with sepsis

Objective:To explore the value of peripheral blood ionized calcium on the prognosis of elderly patients with sepsis.Methods:The clinical data of 73 elderly patients with sepsis from January 2018 to June 2019 in Shanghai Minhang Hospital, Fudan University, were retrospectively analyzed. Among them, 19 patients died within 28 days (death group), and 54 patients survived (survival group). The peripheral blood ionized calcium, procalcitonin (PCT), acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) and arterial lactic acid were compared between 2 groups. The risk factors affecting prognosis in elderly patients with sepsis was analyzed by multivariate Logistic regression. The receiver operating characteristic (ROC) curve was used to analyze the ability of each indicator in evaluating the prognosis.Results:The peripheral blood ionized calcium in death group was significantly lower than that in survival group: (0.93 ± 0.07) mmol/L vs. (1.05 ± 0.16) mmol/L, the PCT and APACHE Ⅱ were significantly higher than those in survival group: 2.43 (1.89, 3.76) μg/L vs. 1.34 (1.05, 2.72) μg/L and (20.20 ± 2.86) scores vs. (17.40 ± 3.95) scores, and there were statistical differences ( P<0.01); there was no statistical difference in arterial lactic acid between 2 groups ( P>0.05). Multivariate Logistic regression analysis result showed that the peripheral blood ionized calcium and PCT were independent risk factors affecting the prognosis in elderly patients with sepsis ( OR = 1.634 and 1.876, P<0.05). The ROC curve analysis result showed that the area under curve (AUC) of peripheral blood ionized calcium for the prognosis in elderly patients with sepsis was 0.844 (95% CI 0.740 to 0.918), the optimal cut -off value was 0.89 mmol/L, with a sensitivity of 83.3% and a specificity of 79.0%; the AUC of PCT for the prognosis in elderly patients with sepsis was 0.880 (95% CI 0.783 to 0.944), the optimal cut -off value was 2.79 μg/L, with a sensitivity of 81.4% and a specificity of 89.5%. Conclusions:The level of peripheral blood ionized calcium can predict the clinical outcome of elderly patients with sepsis.
10.The clinical experience of diagnosis and treatment on adrenal metastases (report of 55 cases)

Objective To discuss the clinical characteristics of adrenal metastases,and summarize the experience of diagnosis and treatment.Methods From January 2008 to June 2018,the clinical data of 55 patients with adrenal metastases treated in our hospital were analyzed retrospectively.This study included 34 male patients and 21 female patients and the median age was 60 years old (ranged 55 to 84 years old).The median value of maximum diameter of adrenal metastases was 3 cm (ranged 1.9 to 10.3 cm);with 35 cases on the left side,13 cases right and 7 cases bilateral.The primary sites of malignant tumors were pancreas (18 cases,32.7%),lung (12 cases,21.8%),liver (6 cases,10.9%) and colorectum (6 cases,10.9%),respectively.Thirty-four cases were confirmed by pathology after adrenalectomy and 21 cases were confirmed by needle biopsy.Thirty cases were diagnosed synchronously with the primary tumor and 25 cases were metachronous.The median time from diagnosis of primary tumors was 13.3 months (ranged 2.0 to 97.4 months).42 cases of these 55 cases were diagnosed within one year.Treatment options for adrenal metastatic lesions included single adrenalectomy in 18 cases,adrenalectomy combined with radiotherapy 16 cases,single intravenous chemotherapy 10 cases,intravenous chemotherapy combined with non-operative treatment 10 cases and single radiotherapy 1 case.Results The main pathological types were adenocarcinoma (19 cases,34.5%),ductal adenocarcinoma (10 cases,18.2%),hepatocellular carcinoma (6 cases,10.9%) and clear cell carcinoma (4 cases,7.3%).Two cases were lost follow-up and the follow-up rate was 96%,the median follow-up time was 8 months (ranged 1 to 135 months).The median overall survival (OS) time of 55 patients was 5.3 months (ranged 1 to 134 months).The one-year survival rate was 23.6% (13/55),the two-year survival rate was 12.7% (7/55),the three-year survival rate was 9.1% (5/55) and the five-year survival rate was 1.8% (1/55).Conclusions Pancreatic cancer was the most common type of malignant tumor for adrenal metastases in our hospital.Most primary tumors and adrenal metastases were diagnosed synchronously or within one year.Comprehensive treatment with retroperitoneal adrenalectomy may improve the OS,however the overall prognosis is poor.
