1.Configuring Anti-earthquake Medical Resource and Improving Medical Insurance

Some basic knowledge of the outbreak and direction of earthquake, and shows that in some areas the medical resources are wasted severely are briefly introduced. A long-term mechanism and emergency draft be set up for antiearthquake and rescue is suggested, which should aim at different areas and districts manage in large scale and reasonably distribute the limited medical resources. In this way, the indemnification can be raised, the victims' life and property can be saved, and the disaster areas can obtain better service.
2.Clinical Study of Acupoint Catgut-embedding Therapy for Prevention and Treatment of Bone Marrow Depression in Breast Cancer Patients Induced by FEC Chemotherapy

Objective To compare the clinical efficacy of acupoint catgut-embedding therapy plus fundamental treatment versus fundamental treatment alone for the prevention and treatment of bone marrow depression in breast cancer patients induced by FEC(fluorouracil, epirubicin and cyclophosphamide) chemotherapy. Methods A total of 46 post-operation breast cancer patients accepting the first cycle of FEC chemotherapy were randomly divided into treatment group and control group, 23 cases in each group. The control group was given fundamental treatment including oral use of Chinese medicine of modified Xiangsha Liujunzi Decoction for regulating stomach and arresting vomiting, modified Guipi Decoction and Gui Lu Erxian Decoction for nourishing blood and generating blood, and western medicine of Batilol Tablets and Vitamin B4 Tablets. The treatment group was given acupoint catgut-embedding therapy on bilateral Zusanli(ST36) and Shenshu(BL23) plus fundamental treatment. The effect on bone marrow depression in the two groups was observed after treatment. Results(1) On post-chemotherapy day 7 and 8, the count of white blood cells (WBC) and neutrophils (NE) in the treatment group was higher than that in the control group, the difference being significant(P < 0.05 or P < 0.01).(2) The utilization of granulocyte colony-stimulating factor(G-CSF) in the treatment group was less than that in the control group, the difference being significant (P < 0.05). (3) On post-chemotherapy day 10, the effect on improving bone marrow depression in the treatment group was superior to that in the control group, the difference being significant (P<0.01) . Conclusion The acupoint catgut-embedding therapy plus fundamental treatment is more effective and safe for the prevention and treatment of bone marrow depression in breast cancer patients induced by FEC chemotherapy than fundamental treatment alone.
3.Quantification of HA in H7N9 influenza vaccine using heterogeneous antiserum of the same HA sub-type virus

Objective To study the possibility of using heterogeneous antiserum in single radial immunodiffusion (SRID) for quantitative detection of HA in H7N9 influenza vaccine product when H7N9-specific antiserum is not available in order to establish a testing method for the detection of H 7N9 antigen in any urgent situation.Methods Antisera specific for H7N1, H7N2, H7N3 and H7N7 were obtained from NIBSC and used for SRID assay .Amino acid sequences of hemagglutinins were comparatively analyzed be-tween H7N9 virus and other viruses used to prepare heterogeneous antiserum .The titers of antisera against H7N9 and their homogenous antigens were detected by double immunodiffusion method .Based on the results of homology analysis and cross-reaction, a suitable antiserum was selected and its applicability was further validated by the SRID assay using H7N9 antigen.Results Influenza A virus subtype H7N3 that used for preparation of 07/278 antiserum showed the highest HA homology with H7N9 (97.14%).The titer of 07/278 antiserum against H7N9 antigen was 1 ∶8 as detected by double immunodiffusion assay .The H7N9 anti-gen and the 07/278 antiserum could form a clear precipitation line in SRID assay .The detection of H7N9 antigen in the range of 10 to 40μg/ml showed a good linearity in the standard curve .Conclusion The 07/278 antiserum from NIBSC can be used as an alternative reagent for the quantitative detection of hemaggluti -nin in H7N9 influenza virus vaccine .
4.Measurement of the amino acid sequence for the fusion protein FP3 with LC-MS/MS.

The amino acid sequence of the fusion protein FP3 was measured by two types of LC-MS/MS and its primary structure was confirmed. After reduction and alkylation, the protein was digested with trypsin and glycosyl groups in glycopeptide were removed by PNGase F. The mixed peptides were separated by LC, then Q-TOF and Ion trap tandem mass spectrometry were used to measure b, y fragment ions of each peptide to analyze the amino acid sequence of fusion protein FP3. Seventy-six percent of full amino acid sequence of the fusion protein FP3 was measured by LC-ESI-Q-TOF with the remaining 24% completed by LC-ESI-Trap. As LC-MS and tandem mass spectrometry are rapid, sensitive, accurate to measure the protein amino acid sequence, they are important approach to structure analysis and identification of recombinant protein.
5.Powder modification technology used for the preparation of the hydrophilic decoction pieces of indigo naturalis and the modification principle.

The hydrophilicity of the normal decoction pieces (NDP) of Indigo Naturalis is not good, therefore, it is not suit for decoctions. In this paper, powder modification technology is used and some NDP and alcohol are ground together in the vibromill to prepare the hydrophilic decoction pieces (HDP) of Indigo Naturalis. Initially, the properties of NDP, ultrafine decoction pieces (UDP) and HDP are compared, the hydrophilicity of UDP was promoted slightly, that of HDP is promoted dramatically. Then, three batches of Indigo Naturalis are prepared to HDP separately, but there is no obvious difference in the contact angle. Furthermore, the size distribution, surface area and micro-shape of HDP are bigger than that of UDP and smaller than NDP. The contents of indigo and indirubin in three decoction pieces are the same, as well as the species of inorganic substance, although there is a little difference in the proportion of five inorganic substances. The fact suggests the change of physical state and the qualitative and quantitative change of organism and inorganic substances are not the main factors to influence the hydrophilicity. In addition, hydroxyl, methylene and methyl can be identified at the wavenumber of 3 356 cm(-1) and 1 461 cm(-1) in infrared spectrum; the content of alcohol in HDP is 0.67% measured by gas chromatogram. The stability of HDP in the heating condition is studied, the fact suggests the hydrophilic effect of HDP at 40 degrees C is relatively stable. All above research suggests that the alcohol is the main factor to influence the hydrophilicity and maybe the intermolecular force which fixed alcohol molecule on the surface of Indigo Naturalis is the basic principle to produce the hydrophilicity.
6.Rapid preparation of antiserum against influenza virus (H7N9) hemagglutinin for single radial immunodiffusion assay

Objective To establish a method for rapid preparation of antiserum against influenza virus (H7N9) hemagglutinin,and to study the possibility of using it in single radial immunodiffusion (SRID) assay for quantitative detection of antigen in H7N9 influenza vaccine.Methods Hemagglutinin proteins expressed in eukaryotic cells were used to immunize sheep.Serum samples were collected to detect antibody titers by ELISA and double immunodiffusion assay.Different concentrations of antiserum were used in SRID assay to get the optimized concentration.Results After 4 times of immunization,the antiserum titers achieved 1 ∶ 1 000 000 and 1 ∶ 32 as indicated by ELISA and double immunodiffusion assay,respectively.The antiserum could form a clear precipitation line in SRID assay.The detection of antigen in the range of 10 to 40 μg/ml showed good linearity in the standard curve.The antigen titers in six batches of H7N9 vaccine detected by this SRID assay were identical with those by SDS-PAGE assay.Conclusion The antiserum against H7N9 hemagglutinin for SRID assay was developed successfully,and could be used as a reagent for the quantitative detection of antigen in H7N9 influenza vaccine.
7.Establishment and identification of universal antibody against all subtypes of the influenza A viral hemagglutinins

Objective To establish the universal antibody against all subtypes of the influenza A viral hemagglutinins(HA). Methods All HA sequences from influenza viruses in common domains were analyzed to search for the most conservative re,on of the HA gene. The peptides conjugated to carrier proteins were used to immunize rabbits in order to obtain high titer antibody. Results The most conservative region among all influenza viruses HA genes were 14 amino acids located at the N-terminal of HA2. The antibody titer reached to 1 : 80 000 after 4 injections of the coupled peptides. The achieved antibody was demonstrated by Western blot to bind HA proteins of influenza virus subtypes H1-H13 specifically. Conclusion The universal antibody has been successfully established by immunizing the rabbits with most conservative peptides of HA protein coupled to carrier protein.
9.Determination of drug antibody ratio in an antibody-drug conjugate.

This paper reports the determination of the drug antibody ratio in an antibody-drug conjugate with two methods, i.e. LC-MS and UV/VIS, and to provide a reliable method to scientifically evaluate and effectively control the drug antibody ratio. Deglycosylated sample was analyzed with C4 column followed by MS, and the number of conjugated drugs in the antibody was determined by the molecular weight increase due to the addition of different number of drugs to the antibody, and then drug antibody ratio was calculated by weighted average of different number of drugs conjugated to the antibody. Optical density at 252 and 280 nm was measured with UV/VIS, and due to the difference of extinction coefficients between the antibody and the drug, the drug antibody ratio was calculated from linear equation with two unknowns. The drug antibody ratio was 3.21 and 3.25 respectively measured by the two methods, and the results were similar with the two methods. Our study indicated that both methods, LC-MS and UV/VIS, could be applied to the analysis of drug antibody ratio of the antibody drug conjugate.
10.Prognostic value of immunoglobulin G and fibrinogen for patients with non-small cell lung cancer

Objective:To investigate the clinical value of immunoglobulin G and fibrinogen in prognostic prediction for patients with non-small cell lung cancer.Methods:The clinical data of 108 patients with non-small cell lung cancer treated at Lishui City People's Hospital from March 2018 to March 2021 were retrospectively analyzed. Based on their prognosis evaluated over 3 years of follow-up, the patients were categorized into a good prognosis group ( n = 62) and a poor prognosis group ( n = 46). Multivariate logistic regression analysis was conducted to identify the factors that influence the prognosis of patients with non-small cell lung cancer. Receiver operating characteristic curves were generated to evaluate the predictive value of immunoglobulin G, fibrinogen, and the predictive model for the prognosis of non-small cell lung cancer. Results:The binary logistic regression model indicated that clinical stage (Ⅱ-Ⅲ), the presence of lymph node metastasis, immunoglobulin G, and fibrinogen are significant factors influencing the prognosis of patients with non-small cell lung cancer ( OR = 11.709, 302.058, 14.079, 6.352, 4.262, all P < 0.05). Receiver operating characteristic curve analysis showed that the area under the curve for predicting the prognosis of non-small cell lung cancer based on immunoglobulin G and fibrinogen levels was 0.860 and 0.833, respectively. Incorporating immunoglobulin G and fibrinogen levels into the regression equation produced a predictive model with an area under the curve value of 0.905, a sensitivity of 0.804, and a specificity of 0.903. Conclusion:Clinical stage (Ⅱ-Ⅲ), the presence of lymph node metastasis, immunoglobulin G, and fibrinogen are the factors that influence the prognosis of non-small cell lung cancer. The predictive model established based on immunoglobulin G and fibrinogen levels exhibits substantial value in predicting the prognosis of patients with non-small cell lung cancer.
