1.Endoscopic mucosal resection for sub-pedicle or non gastrointestinal tract polyps in primary hospital

Objective To evaluate the efficacy of endoscopic mucosal resection (EMR) for sub-pedicle or non gastrointestinal tract polyps in primary hospital.Methods Retrospectively analyzed the data of 43 patients of EMR from November 2011 to December 2013.Results In 43 patients,84 lesions were resected,2 cases of hemorrhage during EMR immediately,1 case delaying hemorrhage,no complications such as perforation or infection occurred.Length of operation time was (6.73 ± 1.24) min per focus,hospital stay was (5.50 ± 0.61) d,there was no recurrence in any patient by endoscope during a follow-up of 6-24 months.Conclusions The technique of EMR is simple,less trauma,no pain,fast recovery for patients.As long as selecting suitable patient,EMR can be carried out widely in primary hospital.
2.Application and nursing care of bronchoalveolar lavage by fiberoptic bronchoscopy in patients with mechanical ventilation

Objective To investigate the application and nursing care of bronchoalveolar lavage (BAL)by fiberoptic bronchoscopy in patients with mechanical ventilation. Methods 64 patients with mechanical ventilation in RICU were randomly divided into the control group and the observation group with 32 patients in each group. The patients in both groups were accepted anti-infection, atomization inhalation and other supportive treatments. The patients in the observation group were treated with BAL, and the patients in the control group accepted routine treatment. The therapeutic effects in the two groups were evaluated and compared. Results There was significant difference in the recovery rate and effective rate, the blood gas analysis compared with the control group was significantly improved, the time of pulmonary infection control window, mechanical ventilation, length of stay were shorter than the control group. The case number to remove ventilator successfully, organs failure and hospital mortality were less than the control group. Conclusions Fiberoptic bronchoscopy with BAL treatment could be safety and efficacy for patients with mechanical ventilation. Nursing intervention plays an important role and a good therapeutic effect for improving achievement ratio of BAL.
3.Diagnosis and operative treatment of occult osteochondral avulsion fracture of the ankle joint

[Objective] To disuse the traumatic mechanism,clinical feature and treatment of the occult osteochondral avulsion of the posterior ankle joint.[Method]The data of 6 patients had been reviewed.From 2003.9 to 2006.6,6 patients had been treated,including 4 male and 2 female,with the average ages of 31.7 years(27~38 years).They all had injury history of their ankle but no fracture or dislocation had been found in X-ray film immediatey after the trauma.Four of them had been treated by plastic cast fixation of their ankle for 3 weeks,2 received treatment without fixation,but all the patients suffered pain and tenderness in the medial-posterior or the lateral-posterior of the ankle with leg or/and food radiation.The symptoms were persistent even when they were rest.The diagnosis of osteochondrl avulsion fracture had not been made until they were admitted in 4 mouth to 1 year after the injury.The fractured bone debris were showed by X-ray film and CT scan.All patients had been treated by operation of ablating the fractured fragment.[Result]All symptoms in the ankle had disappeared 2~3 days after the operation and no recurrence during 6 mouth to 2 years follow up.[Conclusion]It shoud be attaching importance to the occult osteochondral avulsion in early time after ankle injury,and when the diagnosis is made,operation is the appropriate treatment.
4.Effects of individualized nursing on the negative mood and quality of life of patients with hemoptysis

Objective To study the effects of individualized nursing on the anxiety, depression mood, quality of life of patients with hemoptysis. Methods Sixty patients with hemoptysis were randomized into the intervention group and the control group with 25 cases in each. The control group received single conventional therapy while the intervention group was given individualized nursing intervention besides the conventional therapy. Using SDS, SAS and SF-36 Life Quality Measuring Scale to evaluate the effects of the patients in both groups on admission and prior to discharge. Results The scores of anxiety and depression among the intervention group were lower than those of the control group after intervention.The dimensions of quality of life besides the physical function and bodily pain in the intervention group were significantly different after intervention. Conclusions The individualized nursing intervention can improve quality of care, the state of anxiety, depression and the quality of life of patients with hemoptysis.
5.Observation on the effect of smoking cessation education combined with lung function test

Objective To evaluate the effect of health education combined with lung function test on smoking cessation in smoking sanitation workers. Methods Sixty-nine sanitation workers who smoke were divided into two groups by using the table of random number, routine group (n=35) and lung function test group (n=34). The routine group was given health education. According to the lung function, those in the lung function test group were divided into normal lung function group (19 cases) and abnomal lung function group (15 cases) who received both health education and lung function test. Success rate of smoking cessation and the relapse rate were compared at 2 and 6 months follow-up. Results At 2 and 6 months follow-up, the smoking cessation rates of routine group were [10(29%), 4(11%)], which were lower than those of the normal lung function group [7(37%), 3(16%)], the differences were not statistically significant (P>0.05). The smoking cessation rates of routine group were lower than those in abnormal lung function group[11 (73%), 10(67%)] (P<0.05). The smoking cessation rates of the normal lung function group were significantly lower than those of abnormal lung function group (P<0.05). The relapse rate of routine group[6(17%) and the normal lung function group[4(21%)]at 6 months follow-up were not significantly different. The relapse rate of the normal lung function group and that of abnormal lung function group were not significantly different. Conclusion Health education combined with lung function test could help smokers with abnormal lung function to give up smoking successfully.
6.Determination of hesperidin and salvianolic acid B in Xiaopikang Capsule by HPLC

AIM: To determine the contents of hesperidin and salvianolic acid B in Xiaopikang Capsule(Folium Citri reticulatae,Radix et Rhizoma Salviae miltiorrhizae,etc.) by HPLC. METHODS: The HPLC system consisted of Diamonsil C_(18) column(250 mm?4.6 mm,5 ?m), metanol-acetonitrile-formicacid-water(30∶10∶1∶59) mixture as mobile phase,detection wavelength at 284 nm,flow rate of 1.0 mL/min. RESULTS: The ca-(libration) curves of two ingredients were linear within the ranges of 0.22 ?g-2.2 ?g(r=0.999 6,hesperidin),(1.62 ?g-10.2 ?g)(r=0.999 9,salvianolic acid B),respectively.The average recoveries of hesperidin was(98.9%)(RSD=1.60%),salvianolic acid B for 99.02%(RSD=0.97%). CONCLUSION: The method is rapid,sensitive and accurate.
7.The effect of the preoperative oral intake of 10% glucose solution on postoperative insulin resistance in patients undergoing gastric cancer resection

Objective To investigate the effect of oral intake of 10%glucose solution before surgery on the perioperative safety and postoperative insulin resistance in patients undergoing resection for gastric cancer. Methods Between March 2012 and December 2012,36 patients undergoing elective resection for gastric cancer were enrolled and randomized into three groups. Patients in group A were given 500ml of 10%glucose solution for oral intake two hours before surgery. Patients in the group B were given distilled water instead of glucose. Patients in the group C were asked to fast for 8 hours before operation. Patient′s wellbeing scores on a visual analogue scales (VAS) were recorded. Blood samples were collected to measure. Homeostasis model assessment (HOMA) was applied to assess the status of insulin resistance (HOMA-IR) and insulin sensitivity index (ISI) according to the level of blood glucose and the serum concentration of insulin. The gastric residues before anesthesia and the time of exhaustion and defecation were also recorded. Results The gastric residues were not increased significantly after the intake of 10%glucose solution (P > 0.05). Intake of 10% glucose solution before surgery relieved thirst and hunger (P < 0.01). The time of exhaustion and defecation in three groups was not significantly different (P > 0.05). The HOMA-IR levels postoperative immediately, on the first day and the third day after surgery were lower in group A than that of group B and group C (P < 0.05), but the ISI levels were higher significantly (P < 0.01). Conclusion Intake of 10%glucose solution 2 hours before surgery for gastric cancer is be safe , which can relieve preoperative thirst and hunger, and improve postoperative insulin resistance.
8.Effect of cluster-based care for bronchial asthma patients

Objective To observe the nursing efficacy of cluster-based care for bronchial asthma.Methods 100 cases with bronchial asthma were randomly divided into the study group and the control group with 50 patients in each group.The control group received usual care,and the study group received cluster-based care model.Curative effect,the total of the cases received inhalation therapy correctly,compliance of inhalation therapy,degree of satisfaction to nursing services in the two groups were compared.Results The curative effect in the study group were better significantly than the control group (86% vs 58%).The cases received inhalation therapy correctly in the study group were more than the control group (92% vs 72%).The compliance of inhalation therapy in the study group was significantly higher than the control group(90% vs 80%).The degree of satisfaction to nursing services in the study group were significantly higher than the control group (98% vs 86%).Conclusions Cluster-based care had good effect on patients with bronchial asthma.
9.Recovery law of hind limb function following complete spinal cord transection injury in rats

Objective To observe the recovery process of hind limb function following complete spinal cord transection injury and discuss corresponding mechanism. Methods The spinal cords of 15 SD rats were transected at T9 plane and a 3 mm spinal cord removed for a complete transection. The functional recovery of the hind limb was evaluated using CBS (combined behavioral score) and Basso Beattle Bresnahan (BBB) 1, 2, 4 and 6 weeks after injury. At the 6th week after injury, also experiment was done on histology, immunohistochemistry, motor evoked potential (MEP) of spinal cord and repeated transection. Results The hind limb function of the injured animals recovered at different degrees, most obvious at the 4th week and BBB was up to 12 at the 6th week. MEP result was related to the site of the stimulating electrode. No MEP was recorded when the stimulating electrode was placed above the injured site. But when the stimulating electrode was placed below the injury level, MEP was normal. The again transection of the spinal cord above the injury level took no effect on the recovered hind limb. However, again transection below the injury level resulted in complete paralysis. Histologic results showed that the injured site was filled with glial scars without axons. There existed a little scattered positive fibers of neurofilament protein 200 (NF200). Conclusions After complete spinal cord injury, there is a significant spontaneous recovery of the hind limb function that is a kind of autonomous functions of the injured distal spinal cord unrelated to repair of descending conducting fasciculus of the spinal cord. The distal spinal cord is intact in function.
10.Clinical research of repeat transurethral resection for treating stage T1 of non-muscle invasive bladder cancer

Objective To investigate the significance and safety of repeat transurethral resection(Re‐TUR) for treating stage T1 of non‐muscle invasive bladder cancer .Methods The clinical data were retrospectively analyzed on 41 cases of stage T1 of non‐muscle invasive bladder cancer in this department of our hospital from January 2013 to November 2014 .All cases underwent Re‐TUR at 4-6 weeks after primary surgery .Among them ,33 cases were male and 8 cases were female ,24 cases were single tumor and 17 cases were multiple tumors at first operation .The maximal tumor diameter was ≥ 3 cm in 13 cases and <3 cm in 28 cases . The first treatment was transurethral resection of bladder tumor(TURB‐t) .The pathological report was the stage T1 of urothelium cancer .Results All 41 cases were completed the operation smoothly ,and no serious complication occurred .In the postoperative pathological examination ,7 cases(17 .07% ) had tumor residue or tumor recurrence ,among them ,3 case had residue f tumor base and 4 cases were new onset tumor;the pathological grade at Re‐TUR in 1 case was increased from G2 to G3 .The follow up lasted for 3―27 months(average 13 .2 months) ,9 cases relapsed ,3 cases (42 .86% ,3/7) were positive at Re‐TUR and 6 cases(17 .65% , 6/34) were negative at Re‐TUR .Conclusion Re‐TUR for treating stage T1 of non‐muscle invasive bladder cancer is safe and feasi‐ble ,its significance to pick out high‐risk patient for conducting further active treatment ,which may have certain effect for reducing the recurrence rate of non‐muscle invasive bladder cancer .
