1.Progress in treatment of peripheral T-cell lymphoma

Peripheral T-cell lymphoma (PTCL) is a kind of highly aggressive lymphoma which contains a collection of subgroups. The patients with PTCL have poor prognosis under current chemotherapy regimens, so the treatment of PTCL remains a big challenge. Brentuximab vedotin combined with CHP regimen, pralatrexate in combination with romidepsin and other regimens have set up more options for patients with PTCL. Novel agents, such as PI3K inhibitors, which entered into clinical trials, bring good news to PTCL patients.
2.Imaging-guided pulsed-radiofrequency stimulation of medial branch of lumbar spinal nerve for the treatment of lumbar facet joint syndrome

Objective To assess the therapeutic effect of imaging-guided pulsed-radiofrequency stimulation of medial branch of lumbar spinal nerve in treating lumbar facet joint syndrome. Methods A total of 48 patients with clinically-diagnosed lumbar facet syndrome were randomly divided into group A (anti-inflammatory and analgesic group, n=26) and group B (pulsed-radiofrequency group, n=22). The patients of group A received oral meloxicam dispersible tablets , while the patients of group B were treated with DSA-guided pulsed-radiofrequency stimulation of dorsal medial branch of lumbar spinal nerve corresponding to the diseased lumbar facet. Using visual analogue scale (VSA) the severity of pain at the time of admission, discharge and 6 month after discharge was separately evaluated , and the therapeutic effect was assessed based on the improvement of VSA score. Results The VAS scores of group A and group B at the time of discharge and 6 months after discharge were significantly lower than those determined at the time of admission (P<0.01). At the time of discharge, the VSA score of group B was significantly lower than that of group A (P<0.05). Six months after discharge, the VAS score of group B was significantly lower than that of group A (P<0.05). The total effective rates at 6 months after di scharge of group A and group B were 46.5% and 87.6% respectively;the excellent effect rates of group A and group B were 26.4% and 76.2% respectively. Both the total effective rate and excellent effect rate of group B were significantly higher than those of group A (P<0.01). Conclusion Imaging-guided pulsed-radiofrequency stimulation of dorsal medial branch of lumbar spinal nerve can effectively relieve chronic lower back pain caused by lumbar facet joint syndrome , and its therapeutic effect is superior to oral anti-inflammatory and analgesic medication.
3.Intercostal nerve radiofrequency coagulation treatment for postherpetic neuralgia:clinical observation of 20 cases

Objective To evaluate the efficacy of fluoroscopy-guided intercostal nerve radiofrequency coagulation in treating postherpetic neuralgia. Methods A total of 40 patients with postherpetic neuralgia (VAS ≥ 4.0) were randomly and equally divided into two groups. Patients in group A (control group) were treated with gabapentin, amitriptyline and oxycodone, while patients in group B (study group) were treated with intercostal nerve radiofrequency coagulation together with the same medicines used in group A. VAS score and oxycodone dosage were determined before the treatment and one, 7, 30, 90 and 180 days after the treatment. The side effects and complications during the treatment were recorded. The results were analyzed and compared between the two groups. Results The VAS scores of patients in group A and group B before the treatment and one, 7, 30, 90 and 180 days after the treatment were (8.2 ± 1.6) vs (8.1 ± 1.1), (6.1 ± 1.7) vs (4.5 ± 1.2), (4.2 ± 1.4) vs (2.7 ± 1.4), (3.6 ± 1.1) vs (2.3 ± 1.3), (3.3 ± 1.3) vs (1.6 ± 0.9), and (2.7 ± 1.2) vs (1.3 ± 1.0), respectively (P<0.05 in all), while the oxycodone dosages in group A and group B were (28.5 ± 5.4) mg vs (28.7 ± 5.8) mg, (35.2 ± 8.5) mg vs (17.3 ± 5.4) mg, (1.6 ± 8.5) mg vs (12.3 ± 3.8) mg, (18.6 ± 4.4) mg vs (5.1 ± 1.7) mg, (10.4 ± 2.3) mg vs (5.6 ± 1.1) mg, and (8.4 ± 1.6)mg vs (4.0 ± 1.1)mg, respectively (P < 0.05 in all). The drug side effects in group B were obviously very mild, and no serious complications occurred. Conclusion Fluoroscopy-guided intercostal nerve radiofrequency coagulation can rapidly relieve the pain in patients with postherpetic neuralgia, and reduce the oxycodone dosage and the occurrence of drug-related side effects as well. Therefore this technique is a safe and effective method for postherpetic neuralgia.
4.Analysis on clinical results of hemiarthroplasty for femoral neck fractures in the elderly

Objective To summarize effect of hemiarthroplasty in treatment of femoral neck fractures in the elderly.Methods From January 1998 to January 2008,hemiarthroplaaties were performed in 84 patients with femoral neck fractures,including 24 males and 60 females,at age range of 75-97 years (mean 80 years).Of all,40 patients were treated with unipolar hemiarthroplasty and 44 with bipolar hemiarthroplasty.In the meantime,29 patients were treated with cement fixation and 15 with biological fixation with old femoral stems,and 40 with biological fixation with contemporary femoral stem.Results There were 50 patients(24 in unipolar and 26 in bipolar group)were followed up for average five years (1-10 years).X-ray showed total acetabular erosion rate of 50%(25/50),with 83%(20/24)in unipolar group and 19%(5/26)in bipolar group.Total definite loosening rate was 40%(20/50),which was distributed in six patients treated with cement fixation and 14 with biological fixation.Total revision rate was 24%(12/50),with 37.5%(9/24)in unipolar group and 12%(3/26)in bipolar group.Statistical analysis showed that acetabular erosion rate and revision rate in bipolar group were lower than those in unipolar group(P<0.01).Definite loosening rate in cement fixation group was significantly lower than biological fixation group,with no statistical difference compared with biological fixation of contemporary stem(P>0.05).Conclusions Bipolar replacement is significantly better than unipolar replacement for femoral neck fracture in the elderly.Cement fixation with old femoral stem is significantly better than biological fixation with old femoral stem,which is similar to biological fixation with contemporary femoral stem.
5.Research progress on hepatitis C virus-related cryoglobulinemia

Chronic hepatitis C virus ( HCV) infection has been reported in association with several extrahepatic complications, among which mixed cryoglobulinemia is the most documented. Clinical manifestations of HCV related cryoglobulinemia include skin lesions, glomerulonephritis, peripheral neuropathy, and widespread vasculitis.The therapy may be directed to eradicate HCV infections, suppress B-cell clonal expansion and cryoglobulin production, and ameliorate symptoms.This paper reviews the research progress on pathogenesis, clinical manifestations, diagnosis and treatment of HCV related cryoglobulinemia.
6.Factors affecting the prognosis for children in a persistent vegetative state

Objective To explore the factors affecting the prognosis for a child in a persistent vegetative state (PVS).Methods Forty-three PVS children were selected and provided with comprehensive rehabilitation training.It included physical therapy such as Rood approach and range of joint movement training,medication and hydro-acupuncture at the head points.After three months,each patient's state of consciousness was quantified,and multiple-factor unconditional logistic regression was conducted to identify factors relating to any changes.Results By the end of the intervention,29 of the patients had regained consciousness,a recovery rate of 67%.Logistic regression revealed that age,brainstem auditory evoked potentials (BAEP) and PVS score at admission all were closely related with whether a child regained consciousness.Gender,type of damage,EEG score,and the initial time of intervention had no significant relationship with regaining consciousness.Conclusions Older PVS patients with higher BAEPs and PVS scores at admission are more likely to regain consciousness.

It is considered that attention should be paid to acupoint location, postures of patients, manipulation methods, types of needling sensations, transmission lines and duration time of needling sensations, etc.

8.The effects of conductive education on gross motor function and Gesell development test results in children with cerebral palsy

Objective To observe the effects of conductive education on gross motor function and Gesell development test results in children with cerebral palsy.Methods One hundred and thirty-eight children with cerebral palsy were stratified randomly according to the gross motor function classification system (GMFCS) and sexes into two groups:the conductive education group received conductive education combined with general comprehensive rehabilitation,the control group received general comprehensive rehabilitation only.After 4 months of training,the results of both groups in terms of gross motor function and Gesell development test results were compared.Results The GMFCS evaluation results of the 2 groups were compared through covariance analysis (F =4.479,P =0.036 <0.05),the result of conductive education group was better than that of control group; in both groups the result after training was superior to that before training.For Gesell development test results comparison,the differences between conductive education group and control group were statistically significant (P < 0.05) ; the result of conductive education group was better than before the training (t =24.93,P =0.00 < 0.05) ; but in control group the difference between the results before and after training was not significant (t =13.34,P > 0.05).Conclusions Conductive education could improve gross motor function and whole body development.
9.Indirubin inhibits ATP-induced phagocytosis attenuation, ROS production and cell death of macrophages.

This study is to investigate the effects of indirubin on ATP-induced immune responses of macrophages. For this, neutral red dye uptake method was used to test phagocytosis, MTT assay was used for measuring cell death, and reactive oxygen species (ROS) was tested with fluorescent probe DHE. The data showed that extracellular ATP attenuated phagocytosis, induced cell death and increased ROS production, and these effects were restored by pre-treating with indirubin. This result suggested that indirubin blockade the effects of ATP on macrophages, because extracellular ATP-induced effects are dependent on P2 receptors, in particular P2X7 receptors. Furthermore, the effects of indirubin on the activation of P2 receptors were tested, in particular P2X7 receptors. The data showed that indirubin significantly decreased ATP-induced, P2 receptors mediated intracellular Ca2+ concentration ([Ca2+]i) rise and inhibited P2X7 receptor-based ethidium bromide (EB) dye uptake. These results suggested the inhibitory effects of indirubin on the activation of P2X7 receptors, which may underlying the effects on ATP induced ROS production, phagocytosis attenuation and cell death of macrophages.
10.The clinical efficacy observation of fluoroscopy-guided foam sclerotherapy with lauromacrogol for varicose veins of the lower extremities

Objective To assess the clinical efficacy of foam sclerotherapy with lauromacrogol for varicose veins of the lower extremities.Methods During the period from February to July 2011,we retrospectively analyzed 20 patients with varicose veins of the lower extremities,who were treated with lauromacrogol foam sclerosing agent injected directly at varicose veins,and in 5 extremities injected at the great saphenous vein(GSV) through a catheter at the same time.Elastic bandages were applied at the site of the injected varicosities after the therapy.The clogging of the varicose veins,the improvement of the clinical symptoms and the adverse reaction were observed. Results Lauromacrogol foam sclerosing agent was successfully injected under the guidance of fluoroscopy in 20 patients with 28 affected extremities. The average volume of foam sclerosing agent in each affected extremity was 7.8 ml. All patients presented selflimiting minor complications,including swelling and pain,which was obviously alleviated by the externallyapplied Mucopolysaccharide Polysulfate Cream.Two patients presented cough that was relieved after inhaling oxygen. Most of treated varicosities demonstrated pigmentation after the first week, which gradually disappeared after the four-month use of vitamin E capsule.A second foam sclerotherapy was carried out for 3 affected extremities of 3 patients two months after the first therapy. Two cases of leg venous ulcer were healed within a few weeks.All patients could walk immediately with no deep vein thrombosis,pulmonary embolism,anaphylaxis,or transient visual disturbance. Obvious abnormal varicose veins as well as the soreness and fatigue of the lower extremities disappeared in all patients at the 10th-month follow-up.The grading of the disease was significantly improved by the treatment (Z =5.103,P < 0.01 ).Conclusions The efficacy of lauromacrogol foam sclerosing agent in the treatment of varicose veins of the lower extremities is confirmed,with advantages of lower complication,ease of treatment,repeatability,and outpatient treatment.the filling-defects technique under fluoroscopy is a method for tracing the sclerosing foam,and can effectively prevent the decp vcin thrombosis.
