1.The concept, clinical design and transfer application of target restoration space.

The target restoration space (TRS) is a required tooth space that should be occupied by an esthetically formed and optimally functioned crown, veneer, or other fixed restoration. It can be either internal of or external of a tooth. Since minimal invasive can't be achieved without a minimal TRS, a precise preparation of slightest damage should conform to the transfer guide based on TRS. This paper introduces the concept, design of TRS, explains how it can be applied in tooth preparation, and helps to better understand a precise and minimal invasive tooth preparation technique.

2.The application of thyroid remnant measuring tape in endoscopic thyroidectomy

Objective To investigate a practical scientific method for measurement of thyroid remnant in endoscopic thyroidectomy via breast approach.Methods The clinical data of 95 hyperthyroid patients were reviewed and analyzed.The patients were submitted to endoscopic thyroidectomy via breast approach and bilateral subtotal thyroid resection were performed.Thyroid remnant were assessed by naked eyes in 32 cases,thyroid remnant measuring tape in 63 cases.The therapeutic effect was observed.Results All the operations were successfully performed.There was no dyspnea,bleeding,recurrent laryngeal nerve and thyroid gland injury,postoperative recovery was well.The function of thyroid gland was reviewed after operation every 2-3 months,and reviewed by color ultrasound after operation every 6 months.Followed up for 1.8-6.0 years.In 32 patients whose thyroid remnant were assessed by naked eyes,3 patients showed relapse after operation 3 months,2 patients developed hypothyroidism.In 63 patients whose thyroid remnant were assessed by thyroid remnant measuring tape,there had no relapse and hypothyroidism.Conclusion Using thyroid remnant measuring tape in endoscopic thyroidectomy,according to 3.0,2.0,0.5 measurement method,make 1.06 g/cm3 as standard,is scientific,practical and clinical feasible.
3.Integrated backscatter of hypertensive retinopathy by ultrasonic tissue characterization

Objective To investigate the integrated backscatter(IBS) changes of retina in hypertensive disease patients to offer the reference to clinician for ocular fundus changes of hypertensive disease.Methods One hundred chronic hypertension patients (100 eyes) were divided into 4 groups by clinical stages:① gradeⅠ,28 persons (28 eyes);② grade Ⅱ,27 persons (27 eyes);③ grade Ⅲ,24 persons (24 eyes);④ grade Ⅳ,21 persons (21 eyes).Thirty normal persons of 30 eyes were the control group.Their IBS values were measured on nose side of retina,and the correction IBS values(IBS%) were calculated.Results With the deterioration of hypertensive disease,the IBS(29.48±0.09,32.50±0.04,34.62±0.39,36.48±0.46,37.52±0.43) and IBS%[(68.50±2.11)%,(72.03±0.57)%,(74.29±0.77)%,(76.06±1.43)%,(77.45±0.75)%] of retina were gradually increased.The statistical difference were found among various groups(P=0.000).Conclusions With the deterioration of hypertensive retinopathy,the IBS and IBS% increased gradually.The IBS technique is a useful method to assess the retinopathy of hypertension.
4.Investigation and analysis for impact factors of distress in patients with first diag-nosed lung can-cer

Objective:To study the impact factors of psychological distress in patients with first diag-nosed lung cancer.Methods:The cross-sectional study was applied to newly diagnosed lung cancer pa-tients who received treatments in Cancer Center of West China Hospital of Sichuan University in Chengdu from June 201 3 to March 201 5 by distributed questionnaires.The general information of the patients,the hospital anxiety and depression scale (HADS)and distress management screening measure (DMSM) were included in the questionnaire to evaluate the states of distress,pain and the factors related to the distress of the patients.Results:The survey investigated a total of 390 patients with first diagnosed lung cancer,including 291 male patients and 99 female patients.The proportion of the patients with positive anxiety symptom was 26.7% (1 04 /390),with positive depression symptom was 27.7% (1 08 /390), and with positive distress symptom was 30.0% (1 1 7 /390).On the top five problem list of DMSMwere worry,disease treatment,breathing,pain and sleep.The Spearman correlation analysis showed that an-xiety score,depression score,and the pain intensity were positively correlated with the distress.Multiple linear regression analysis showed that gender (β=-0.209,P =0.003),age (β=-0.098,P =0.042),chronic disease (β=0.378,P <0.001 ),and pain score (β=0.1 00,P =0.029)could affect the distress of the patients.Smoking (β=0.1 1 1 ,P =0.041 )could affect the anxiety of newly diagnosed lung cancer patients.Conclusion:The gender,age,chronic disease and pain score of the patients are the independent factors of the psychological distress of first diagnosed lung cancer patients.Smoking is the predictive factors of the patients with anxiety.Controlling the pain intensity and the clinical process of chronic disease of the patients actively,solving the sleep and breathing problem and helping the patients to quit smoking progressively could alleviate the psychological distress of the patients.
5.Interaction between P2X3 and TRPV1 in the mechanism of pain

Pain, as one of the most common clinical symptoms, seriously affects the quality of life of patients.Transient Re-ceptor Potential Vanilloid Type 1 (TRPV1) and Purinergic receptor(P2X3)are both involved in the transmission of a variety of patho-logical pain signals.Their interaction is less reported.This article reviews the interaction between TRPV1 and P2X3 in the pain signal transduction, and provides a new way for the treatment of pain.
6.Curative effect and influencing factors of capecitabine in treatment of advanced breast cancer

Objective To explore and analyze the curative effect of capecitabine in the treatment of advanced breast cancer and its influcing factors. Methods 100 cases of advanced breast cancer were selected,all patients were treated with combined or individual therapy, one period was 21-28 days, and the course of treatment was more than two cycles. Before and after treatment, the Eastern Cooperative Oncology Group (ECGO) score, receptor status, chemotherapy cycles, tumor metastasis and other factors of the data were collected and recorded. Results The short-term efficacy of the patients was: stable disease (SD) 39 cases, progressive disease (PD) 8 cases, complete remission (CR) 1 cases, partial remission (PR) 52 cases. 53.0%(53/100) for objective response rate (ORR), 92.0%(92/100) for disease control rate (DCR). All patients who received treatment from the end of the follow-up period were: 1 to 24 months, the median survival period was 6.5 months. The independent influencing factors of progress free survival (PFS) were the number of cycles of combination chemotherapy (P<0.05). During chemotherapy, the adverse reactions of patients was not much, respectively: 4 cases of hand foot syndrome, 2 cases of grade Ⅰ, 1 case of grade Ⅱ, 1 case of grade Ⅲ; 7 cases of gastrointestinal reaction, 4 cases of grade Ⅰ, 3 cases of grade Ⅱ; 3 cases of hematologic toxicity, 1 case of grade Ⅰ, 2 cases of grade Ⅱ. The lower ECGO score, ≥4 times of cycles of treatment, ER/PR+ improved the short-term efficacy(P<0.05), while the tumor metastasis and HER-2 receptor could not effect the short-term efficacy. Conclusion The short-term efficacy of capecitabine is related to ECGO score, receptor status and chemotherapy cycles, while has no relation with tumor metastasis and HER-2 receptor.

[Objective]To determine the content of Polygonum multiflorum thunb. Polysaccharide and the refined. [Methods] The sulfuric acid-phenol method was used. [Results] ?_ max = 486nm. The linear rang was 10.5~94.5?g?L -1 .And the average recovery was 101.23%(n=5,RSD=3.03%). The content of Polygonum multiflorum thunb. Polysaccharide was 14.28% and the refined was 19.03%. [Conclusion] The method was simple,rapid and reliable.
8.Vascular ultrasound combined with Seldinger technology improves the success rate of peripherally inserted central catheter and reduces potential complications

ObjectiveTo investigate the applicability and superiority of modified Seldinger technique (MST).MethodsTotally 108 patients receiving peripherally inserted central catheter (PICC) through vascular ultrasound combined with MST technology were enrolled as MST group and another 108 patients receiving PICC using traditional direct vision method as control group.The first puncture success rate,PICC succesa rate,catheterization time required,and early complication rate were compared between these two groups.ResultsThe first puncture success rate was 88.89% in MST group and 72.22% in control group (P=0.002) ; the catheterization success rate was 82.41% in MST group and 57.41% in control group ( P =0.000).In the MST group,there were two cases where the catheter within neck was ectopic ( P =0.003 ),two cases experienced bleeding at the puncture sites (P =0.017),three cases had phlebitis (P =0.001 ),and one case suffered from catheter blockage (P=0.010).In the eontrol group,the corresponding numbers were 13,10,18,and 9.However,the time required for PICC showed no significant difference between these two groups (P=0.685).In the MST group,15 cases were catheterized in less than 15 minutes,72 cases between 15 and 25 minutes,and 21 cases for more than 25 minutes.In the control group,12 cases were catheterized in less than 15 minutes,70 cases between 15 and 25 minutes,and 26 cases for more than 25 minutes.ConclusionPICC through MST can be more effective in positioning the targeting blood vessels,increasing the success rate of catheter insertion,reducing various tissue damages,and lowering potential complications.
9.Analysis of related factors of tonsil postoperative pain.

OBJECTIVE:
To explore the related factors of tonsil postoperative pain.
METHOD:
After founding databank a statistic analysis was performed on 90 cases with tonsillectomy who admitted in our hospital. Chi-square test were used to investigate the related factors for postoperative pain in those patients.
RESULT:
After Chi-square test, the important effect factors in the postoperative pain were gender, age, surgical methods, preemptive analgesia, physical analgesia and preoperative anxiety level.
CONCLUSION
The pain after tonsillectomy is influenced by not only surgery itself, but also by preemptive analgesia, preoperative anxiety level and so on.

10.Effects of sleeping body posture on sleeping structure and respiratory events in patients with OSAHS.

OBJECTIVE:
To observe the effects of sleeping body posture on sleeping structure and respiratory events in patients with OSAHS.
METHOD:
We assessed the sleeping body position, the sleeping structure, position specific AHI and the Epworth Sleepiness Scale (ESS) in a total of 80 patients with Positional OSAHS. The patients were grouped according to AHI: mild OSAHS (5 < or = AHI <15), moderate (15 < or = AHI < 30) and severe (AHI > or = 30). The polysomnography data and clinical characteristics were compared between each group.
RESULT:
The severe OSAHS group, when compared with the mild and the moderate ones, had a significant different in REM%, NREM%, NREM LSaO2, RDI, S-AHI and L-AHI due to posture (P < 0.05). The severe and the mild OSAHS groups had significant different in MSaO2, LSaO2 due to posture (P < 0.05). The moderate and the mild OSAHS group had significant different in LSaO2, REM LSaO2 and RDI (P < 0.05). In all mild, moderate and severe groups, the LT% were higher than ST%, but the difference was not significant (P > 0.05). For mild-to-moderate groups, there was no correlation between the ESS and the AHI for any position different (P > 0.05). For severe group, the ESS was significantly correlated with L-AHI (r = 0.551; P < 0.01); the REM L-AHI and NREM L-AHI was also significantly correlated with ESS of severe group (r were 0.516 and 0.528, P < 0.01).
CONCLUSION
The L-AHI, NREM L-AHI, REM L-AHI and NREM LSaO2 were considered to monitor the stability of OS-AHS, while REM LSaO2 were consider to clarify the severity of OSAHS.
