1.Screw placement in repair of ankylosing spondylitis complicated by severe wheel-like kyphosis:stress distribution in multiple segments

BACKGROUND:Kyphotic deformity in ankylosing spondylitis is the flexion deformity of spine sagittal plane in the late lesion. Spinal osteotomy is the only treatment method in patients with severe wheel-like kyphosis. For thoracic and thoracolumbar ankylosing spondylitis patients with lumbar severe wheel-like kyphosis, osteotomy at a single site cannot obtain safe and effective orthopedic effect. OBJECTIVE:To observe the orthopedic effect of total spine osteotomy combined with V-shaped osteotomy for correction of severe wheel-like kyphosis deformity due to ankylosing spondylitis, and to analyze stress distribution. METHODS:From May 2003 to October 2012, total spine osteotomy combined with V-shaped osteotomy and pedicle screw fixation were adopted for repair of concurrent thoracic, thoracolumbar and lumbar severe wheel-like kyphosis deformity due to ankylosing spondylitis in 36 male cases in the Sixth Affiliated Hospital of Xinjiang Medical University. Spinal convex angle, chin-brow vertical angle and C 7 plumb line were measured to evaluate orthopedic effect. RESULTS AND CONCLUSION:The whole spine convex Cobb angle was corrected from preoperatively (89.6±9.8)° to (32.2±6.7)° at 1 week after treatment, showing significant difference (P<0.05), with an average correction rate of 64%. The chin-brow vertical angle was 9.6° averagely after correction (P<0.05), with an average correction rate of 76%. The C 7 plumb line was 4.4 cm averagely after correction (P<0.05), with an average correction rate of 81%. After fol ow-up of 24-48 months, no significant difference in above indexes was detected during final fol ow-up and 1 week postoperatively (P>0.05). Radiographs demonstrated that fixation position was good in al patients. These results confirmed that in patient with severe wheel-like kyphosis deformity due to ankylosing spondylitis, the application of total spine osteotomy combined with V-shaped osteotomy is a safe and effective method, can better correct the spinal sagittal curvature and reduce the risk of sagittal angle, result in the stress distribution in many segments and the shortening of the spine and epidural buckling in relatively long segment, can avoid nerve damage induced by spinal cord shortening and epidural excessive buckling within short segment.
2.Study on antibacterial property of silver loaded titanium dioxide antibacterial coated endotracheal intubation tube

Objective To study the antibacterial property of silver loaded titanium dioxide (TiO2) antibacterial coated endotracheal intubation tube,and to determine the minimum effective antibacterial concentration.Methods Intubation tubes coated with different concentrations of antibacterial agents were prepared with sol gel method.Polyethylene endotracheal intubation tubes were used as substrate,and silver loaded TiO2 was used as the antibacterial agent.According to the different antibacterial concentrations of the antibacterial agent,the tubes were divided into nine groups:10.0% group,5.0% group,2.0% group,1.5% group,1.0% group,0.8% group,0.6% group,0.2% group,and conrol group.They were respectively immersed in three standard bacteria suspensions with 1.0 × l05 cfu/mL:Pseudomonas aeruginosa,Staphylococcus aureus,and Escherichia coll Together with standard bacteria liquid group,there were 10 experimental groups.They were kept overnight for 24 hours.10 μL of respective culture medium was smeared on blood agar culture medium.After being cultured overnight in 35 ℃,the number of bacteria colonies was respectively counted.Results In 1.0 × 105 cfu/mL of three standard bacteria liquids with antibacterial agent concentration≥ 1.0%,three bacterial colonies had un-obviously growth rate.Almost the same strong antibacterial effects to achieve sterilizing rates of more than 98% was shown in each group of the antibacterial coating endotracheal intubation tubes (all P>0.05).As the antibacterial agent concentration decreased,three bacterial colonies were increasing gradually.Intermediate antibacterial effects were shown in tubes of 0.8% group,with significant statistic difference as compared with 1.0% and 0.6% groups [Pseudomonas aeruginosa:7.300 (4.050,8.350) vs.0.200 (0.050,1.200),9.700 (9.000,10.000); Staphylococcus aureus:4.100 (3.300,4.650) vs.0.000 (0.000,0.150),5.800 (5.350,7.650); Escherichia coli:1.400 (0.750,3.750) vs.0.050 (0.025,0.050),9.500 (8.500,9.800),all P<0.01].Conclusions Silver loaded TiO2 antibacterial coated endotracheal intubation tube had definite antibacterial properties,which were related to the antibacterial concentration.Strong antibacterial effects were shown when antibacterial concentration was above 1.0%,with bacteria almost completely killed in the immersing liquid.
3.Clinical observation of capecitabine as first-line monotherapy in elderly patients with Ⅱa breast cancer

Objective:To observe the clinical effect and adverse reaction of capecitabine as first-line monotherapy in elderly patients with stage Ⅱa breast cancer.Methods:From June 2002 to June 2005,71 elderly patients with stage Ⅱa breast cancer received chemotherapy(different scheme:capecitabine group and CEF group) after operation.The efficacies and adverse reactions were evaluated and compared between the two groups.Results:The 3-year and 5-year survival rates of patients in capecitabine group were 97.06% and 94.12%,respectively; the relapse rate was 5.88%; all were comparable to those of CEF group. One of the advantages of capecitabine was its oral administration. The adverse effect of capecitabine was mainly hand-foot syndrome,with an incidence of 82.35%,but was tolerable. Objective: To optimize and establish the best hydrolysis method of diethyl ester 4-amino-N5-formyl-N8,N10-dideazatetrahydrofolate through the optimization of simple compound of diethyl N-(4-aminobenzoyl)-L-glutamate.Methods: To increase the low yield of hydrolysis reaction of diethyl ester 4-amino-N5-formyl-N8,N10-dideazatetrahydrofolate due to the by-products and difficult purification, we studied the effect of NaOH and KOH, two kinds of alkalis, three concentrations between 0.175-1 mol/L and five types of reaction time involved in 20, 30, 60, 120 and 180 min on the common side chain diethyl N-(4-aminobenzoyl)-L-glutamate.A high performance liquid chromatography was established for measuring the target product and the by-products in reaction liquid in different reaction conditions.Finally, on the basis of the best hydrolysis method of diethyl ester 4-amino-N5-formyl-N8,N10-dideazatetrahydrofolate, we completed the optimization of the hydrolysis reaction conditions of diethyl ester 4-amino-N5-formyl-N8,N10-dideazatetrahydrofolate.Results: We developed the best reaction condition for the hydrolysis of diethyl ester 4-amino-N5-formyl-N8,N10-dideazatetrahydrofolate, which could be carried out easily and efficiently.The results indicated that treated with the optimized condition of 0.3 mol/L KOH in 60 min at the room temperature, diethyl ester 4-amino-N5-formyl-N8,N10-dideazatetrahydrofolate was converted into its diacid derivative in 95.6 % yield, which turned to be a better reaction condition compared with the previous reaction condition.The structures of those compounds were identified to be correct by 1H nuclear magnetic resonance(1H NMR), 13C nuclear magnetic resonance(13C NMR) and electrospray ionization time of flight mass spectrometry (ESI-MS).The purity of the diacid derivative of the compound was determined to be 96% by high performance liquid chromatography(HPLC).The new hydrolysis reaction condition could not only avoid the formation of single ester hydrolysis product and amide bond hydrolysis product, but also improve the yield of the hydrolysis reaction.Conclusion: We have developed an efficient reaction for the hydrolysis of diethyl ester 4-amino-N5-formyl-N8,N10-dideazatetrahydro.Since the final step of the synthesis of classical folic acid antagonists is always the catalyzed hydrolysis of the side chain glutamate, the reaction also has great significance for anti-folic acid anti-tumor inhibitors synthesis.
5.Establishment of a three-dimensional finite element model of ankylosing spondylitis kyphosis

BACKGROUND: The full three-dimensional finite element model can not only establish a realistic three-dimensionalmodel, but also can simulate the osteotomy on the model. Analysis of biomechanics guides the clinical operation.OBJECTIVE: To establish a three-dimensional finite element model of ankylosing spondylitis kyphosis and provide an effective digital platform for further studies.METHODS: A 30-year-old male patient with ankylosing spondylitis kyphosis participated voluntarily in the current study. CTimages obtained from CT transverse scanning from C1 to the sacrococcyx were imported into Mimics 17.0 software toestablish a three-dimensional geometric model of the posterior spine. The geometric model was then imported into Studio Geomagic 2013 software. For the subsequent optimization of image processing, the posterior spine convex geometry wasestablished on the three-dimensional geometric model. We used Unigraphics NX 8.5 to establish the spinal kyphosis surfacemodel, then added modeling of calcification of the ligaments, partial resection of useless sacral bone, and finally, imported themodel into ANSYS 15.0 finite element analysis software, then added the ligaments and set the parameters of the material,generating a complete three-dimensional finite element model of ankylosing spondylitis.RESULTS AND CONCLUSION: A three-dimensional finite element model of complete ankylosing spondylitis wassuccessfully established. Using the ten-node approach, we generated 398370 tetrahedral elements, and 668538 nodes.This will provide a reliable digital platform for the next step of biomechanical analysis.
6.Prestin Expression in HEI-OC1 Cells With Oxidative Stress Damage

Objective To examine the effects of oxidative stress induced damage to the Prestin expression in HEI-OC1 cells,and to study the mechanism of sensory deafness.Methods We used different concentrations (50μM,100μM,200μM)of hydrogen peroxide canister to cultivate HEI-OC1 cells,and to detect the activity of su-peroxide dismutase(SOD).The quantitative real-time PCR and immunofluorescence were used to detect the prestin expression of mRNA.Results The SOD activity decreased in the HEI-OC1 cells damaged by oxidative stress.The high concentration of the infected group decreased more significantly(F= 9926.293,P<0.01).The expressions of Prestin mRNA and Prestin protein were decreased obviously in the HEI-OC1 cells.The high concentration of in-fected group decreased more significantly (F= 4065.046and7657.217,P<0.01).Conclusion Oxidative stress in-ducing damage inhibits the expression of prestin.Prestin protein may be used as a molecular marker of sensory deafness.
7.Analysis of bacterial distribution and resistance from exacerbation of COPD patients in C and D Classification in our emergency department

Objective To analyze the bacterial distribution and resistance of clinical isolates from exacerbation of COPD patients in C and D classification in our emergency department.Methods The sputums were taken to culture and antimicrobial susceptibility test.VITEK-2 compact automicrobe system was used to identify bacteria.Disc diffusion test was used to determine the antimicrobial resistance.Results Of 222 clinical isolates,27.0% were Gram positive bacteria,73.0% were Gram negative bacteria.The Gram positive bacteria mainly was Staphylococcus aureus and Streptococcus pneumoniae.The detection rates of methicillin-resistant Staphylococcus aureus (MRSA) were 50.00%.The MRSA were more resistant than methicillin-sensitive Staphylococcus aureus (MSSA),and they were all sensitive to vancomycin,linezolid or tigecycline.The Gram negative bacteria mainly was Pseudomonas aeruginosa,Escherichia coli,Klebsiella spp and Acinetobacter baumannii.Pseudomonas aeruginosa and Acinetobacter baumannii had high resistant rates.The extended spectrum β-lactamases (ESBLs) producing strains in Escherichia coli were higher than in Klebsiella spp,which had multiple drug resistance.Conclusions The bacterial resistance situation is very severely in exacerbation of COPD patients in C and D classification in our emergency department.The first isolating rate was Pseudomonas aeruginosa.Surveillance of bacterial resistance is very important and effective control measures to reduce resistance is very necessary.
8.Expression of cyclooxygenase-2 and c-erbB-2 in human breast cancer and their correlationship

Objective:To investigate the expression and correlation of cyclooxygenase-2(COX-2) and c-erbB-2 in human breast cancer.Methods: The expressions of COX-2 and c-erbB-2 in 48 breast cancer patients with breast cancer were detected with immunohistochemistry examination and was analyzed.The relationship between their expression and the clinicopathologic characters of breast cancer was also analyzed.Results: The positive rate of COX-2 was 60.4%(29/48) in these patients;the expression was higher in patients with tumor size larger than 2 cm,lymph node metastasis,and negative expression of ER or PR,but was not related with the ages,the pathologic types and the TNM stages.Expression of c-erbB-2 was identified in(39.6%)(19/48) of these patients;the expression was significantly higher in patients with lymph node metastasis or negative expression of ER and PR,but was not related with the ages,the sizes and the pathologic types.COX-2 was positive in 84.2%(16/19) of patients positive of c-erbB-2 and in 44.8%(13/29) of patients negative of c-erbB-2,with significant difference found between the 2 figures(P=0.006).Conclusion: The high expression of COX-2 in breast cancer is related to the expression of c-erbB-2,suggesting that they may regulate each other and jointly contribute to the tumorigenesis and progression of breast cancer.

Objective To establish a sensitive HPLC method with UV detection for the determination of tanshinone Ⅱ_A(TSⅡ_A) and cryptotanshinone(CT) in rat plasma and to study pharmacokinetics of Radix Salviae Miltiorrhizae(RSM) liposoluble components in rat in vivo. Methods TS Ⅱ_A and CT in plasma of rat at different sampling time were determined by pretreatment with Gemifibrozil as internal standard,the protein was precipitated in methanol solution after iv administration of alcohol extract of RSM 5 mg/per animal.The mean plasma concentration-time curve was protracted and pharmacokinetic parameters were calculated by 3p87.Results The main pharmacokinetic parameters of TS Ⅱ_A were as follows: t_(1/2?) was(0.088?0.024) h;t_(1/2?) was(2.1?0.7) h;V_C was(0.8?0.6) L;CL was(2.0?1.1) L/h~(-1);AUC_(0-4) and AUC_(0-∞) were(2.2?0.9) mg?h/L and(3.4?1.7) mg?h/L,respectively.Conclusion The concentration-time curve of TS Ⅱ_A can be fitted to two-compartment model after ethanol extract of RSM iv administrated.Concentration of TS Ⅱ_A in rat plasma declines rapidly with large apparent volume of distribution.CT decreases also rapidly and it is difficult to acquire its pharmacokinetic parameters in rat.
