1.Design and Implement of the ECLSS Adaptive Learning Intelligent Decision Support System

Objective To monitor state of environment control and life support system (ECLSS), and to diagnose faults and provide decision support in manned space flight. Methods According to the principle of decision support system, neural networks and the expert system, the foundational constructional structure and work flow of the ECLSS decision support system(EDSS) were designed,fault tree analyze(FTA) method was used to sort all of the faults and to form the expert knowledge framework, expert system and neural networks were integrated to adaptive learning from experience. Results In the two-men-five-days SZ-6 manned space flight, this software system worked well, and had reduced the difficulty and intensity of ground flight control and had provided good technological support for the consummation of this flight. Conclusion For the first time, the ECLSS decision support system is designed and implemented by using database and expert system technology. It can judge parameters state and diagnose faults exactly and rapidly, and the data models are designed correctly to give the rational result of parameter prediction.

The paper introduces the system design of internet platform in novelty assessment and selective dissemination of informa-tion of medical science and technology project in Zhejiang province, including the exploitation of environment , processing of novelty as-sessment and selective dissemination of information as well as main function design. This system has noticeable advantages, such as com-prehensive functions, multiple participation, transparent service process, which could finish the relevant work on the internet, improve the quality of information service and enhance the efficiency.
3.Determination of Phospholipid Content in 5-FUR Prodrug Liposome

OBJECTIVE: To determine the concentration of phospholipid in 5-FUR prodrug liposome. METHODS: The sample was fragmented by ethanol and extracted by chloroform. The concentration of phospholipid in 5-FUR prodrug liposome was determined by ammonium ferrothiocyarate spectrophometry at a wavelength 488 nm. RESULTS: The linear range of phospholipid was 0.01~0.10 mg?mL-1(r=0.999 6, n=3) with an average recovery rate of 99.66% (RSD=3.17%). CONCLUSION: This method is simple, convenient and sensitive, and it can be used for the quality control of 5-FUR prodrug liposome.
4.Preparation and Stability of Tetrandrine Liposomes

OBJECTIVE: To draw a comparison between the two preparative technologies of tetrandrine liposomes. METHODS: Tetrandrine liposomes were prepared with ammonium sulfate transmembrane gradient method and film dispersion method respectively. Leaking ratio,particle size,and content of phospholipid of liposomes were selected as the parameters to indicate the stability of liposomes,and the entrapment efficiency was taken as quality index to compare the two preparation technologies. RESULTS: Tetrandrine liposomes prepared with ammonium sulfate transmembrane gradient technology achieved high entrapment efficiency of 81.1% and good stability,while those prepared by film dispersion method had an entrapment efficiency of only 32.9% with poor stability. CONCLUSION: Ammonium sulfate transmembrane gradient method is feasible and superior to the film dispersion method in the preparation of tetrandrine liposome.
5.Clinical observation of capecitabine as first-line monotherapy in elderly patients with Ⅱa breast cancer

Objective:To observe the clinical effect and adverse reaction of capecitabine as first-line monotherapy in elderly patients with stage Ⅱa breast cancer.Methods:From June 2002 to June 2005,71 elderly patients with stage Ⅱa breast cancer received chemotherapy(different scheme:capecitabine group and CEF group) after operation.The efficacies and adverse reactions were evaluated and compared between the two groups.Results:The 3-year and 5-year survival rates of patients in capecitabine group were 97.06% and 94.12%,respectively; the relapse rate was 5.88%; all were comparable to those of CEF group. One of the advantages of capecitabine was its oral administration. The adverse effect of capecitabine was mainly hand-foot syndrome,with an incidence of 82.35%,but was tolerable. The gastrointestinal reaction and bone marrow repression in capecitabine group were significantly lower than those in the CEF group(P
6.Clinical efficacy of domestic porous tantalum rod in treatment of early avascular necrosis of femoral head

Objective To investigate the clinical efficacy of domestic porous tantalum rod in treatment of early avascular necrosis of femoral head (ANFH).Methods A prospective study was made on 18 cases (19 hips) diagnosed as early ANFH treated by domestic porous tantalum rod from July 2014 to December 2015.There were 14 males and four females, with a mean age of 44.2 years (range, 30-62 years).According to the modified Ficat staging, four cases were identified as stage Ⅱa and 14 stageⅡb.Efficacy of the treatment was evaluated by Harris score, Womac score, radiological images of the hip, complications and bone growth.Results All cases were followed up for 8-24 months (mean, 16 months).No complications such as infection, wound healing problems, immunological rejection, tantalum rod breakage, loosening or displacement were observed at last follow-up.Harris score was (82.7±9.0)points, (84.5±10.8)points and (87.2±10.0)points at postoperative 3, 6 and 12 months respectively, significantly higher than that pre-operation [(75.5±11.9)points] (P<0.05).Harris score was rated excellent in 10 cases, good in three, fair in five and poor in one at the last follow-up, with the excellent and good rate of 68%.Womac score was (17.4±9.4)points, (12.4±7.3)points, and (11.1±8.4)points at postoperative 3, 6 and 12 months respectively, significantly lower than that pre-operation [(28.3±13.1)points] (P<0.05).Seventeen cases (18 hips) showed no obvious deterioration in femoral head necrosis, with femoral head survival rate of 95%.One case underwent total hip arthroplasty after operation because of progressive hip pain and collapsed femoral head.Bone ingrowth was detected in the porous tantalum biomaterial after operation.Conclusion Domestic porous tantalum rod can effectively promote bone ingrowth, relieve pain, prevent the collapse of the femoral head, delay total hip arthroplasty time and finally improve hip function in treatment of early avascular necrosis of femoral head.
7.Transmembrane transport activity of paclitaxel regulated by fangchinoline in MDR1-mDCK II cells.

OBJECTIVETo research the regulation of transmembrane transport activity of paclitaxel influenced by fangchinoline in MDR1-MDCK II cells.
METHODPaclitaxel, one of the substrate of P-gp, was selected as the model drug. Verapamil hydrochloride was adopted as the active control to investigate the bilateral transport activity of paclitaxel regulated by fangchinoline in MDR1-MDCK II cells. RP-HPLC was applied to determine the concentration of paclitaxel in the transporting medium, which was used to calculate apparent permeability coefficient of paclitaxel across MDR1-MDCK I1 monolayer cells.
RESULTThe efflux rate of paclitaxel was faster than the absorption rates across the MDR1-MDCK II monolayer cells with highly expressed P-gp. The absorption rates of paclitaxel combinated with fangchinoline and verapamil hydrochloride respectively were remarkably increased and the efflux rate was decreased. The reversal effect of the fangchinoline was stronger than the verapamil hydrochloride with the same molar concention.
CONCLUSIONFangchinoline can apparently decrease the efflux of paclitaxel and inhibit the multidrug resistance of antitumor drug mediated by P-gp.

8.Short and Mid-term Efficacy of Device Closure of Patent Foramen Ovale for Treating the Patients With Patent Foramen Ovale Combining Cryptogenic Stroke and Transient Ischemic Attack

Objective: To explore the short and mid-term efficacy of device closure of patent foramen ovale (PFO) for treating the patients with PFO combining cryptogenic stroke (CS) and transient ischemic attack (TIA). Methods: A total of 56 PFO patients with CS and TIA receiving device closure in our hospital from 2009-05 to 2015-12 were retrospectively studied. Transthoracic echocardiography (TTE), electrocardiogram (ECG), chest X-ray were examined at 24h, 1 month, 3 and 6 months after theoperation; telephone visit was conducted every 6 months thereafter. Results: There were 54/56 PFO patients combining CS and 2 combining TIA; 53 (94.6%)patients received PFO occluder from Starway medical technology. Aspirin was used for 6 months after the operation. The patients were followed-up for the average of (34.67±23.24) months. No body suffered from post-operative stroke and TIA; no residual shunt was observed. Conclusion: The short and mid-term efficacy of device closure has been satisfactory for treating the patients with PFO combining CS and TIA; its overall clinical value should be further investigated in large population and long-term study.
9.Role of dentritic epidermal T lymphocytes in immune rejection of skin allograft in mice and its mechanism.

To explore the role of dentritic epidermal T lymphocytes ( DETCs) in immune rejection of skin allograft in mice and its related mechanism. Methods (1) Full-thickness skin was harvested from back of one male wild type (WT) C57BL/6 mouse. Epithelial cells were isolated for detection of the expression of DETCs and their phenotype with flow cytometer. Another male WT C57BL/6 mouse was used to harvest full-thickness skin from the back. Epidermis was isolated for observation of the morphological characteristics of DETCs with immunofluorescence technology. (2) Four male green fluorescence protein (GFP)-marked C57BL/6 mice, 7 female WT C57BL/6 mice (group WT), and 7 female ybT lymphocytes 8 gene knock-out (GK) C57BL/6 mice (group GK) were used. Full-thickness skin in the size of 1.4 cm x 1.4 cm on the back of mice in groups WT and GK were excised, and the wounds were transplanted with full-thickness skin in the size of 1.2 cm x 1.2 cm obtained from male GFP-marked C57BL/6 mice. The survival time of skin grafts was affirmed with small animal in vivo imager and naked eyes and recorded. (3) Two male WT C57BL/6 mice were used to isolate epithelial cells. Cells were inoculated into 48-well plate and divided into activation group (A) and control group (C) according to the random number table, with 4 wells in each group. Cells in group A were treated with 10 pL concanavalin A in the concentration of 2 microg/mL for 24 hours, while those in group C with PBS in the same volume as that in group A. The expression of interferon y in DETCs was detected with flow cytometer. (4) Four male GFP-marked C57BL/6 mice were used as donors. Fourteen female WT C57BL/6 mice were used as receptors and divided into interferon gamma neutralizing group (IN) and control group (C) according to the random number table, with 7 mice in each group. The skin transplantation model of C57BL/6 male to C57BL/6 female was established as in part (2). Before surgery and 72 hours after, mice in group IN were intraperitoneally injected with 200 pL interferon y neutralizing antibody in the concentration of 1 mg/mL, and those in group C with normal saline in the same volume as that in group IN. The survival time of skin grafts was observed and recorded using the methods in part (2), and the result of group IN was compared with that of group GK in part (2). The survival curve of skin grafts was processed with Log-rank ( Mantel-Cox) test. Results (1) The positive expression rate of DETCs in epithelial cells of skin in mouse was 7.27%, and they were all CD3 cells. DETCs were found to be scattered in the epidermis of skin in mouse with dendritic morphology. (2) The survival time of skin grafts of mice in group GK was 22-35 d, obviously longer than that in group WT (12-16 d, y2 = 14. 10 , P < 0.001). (3) Expression of interferon gamma was detected in 22. 70% DETCs in group A, which was obviously higher than that in group C (0.51%). (4) The survival time of skin grafts of mice in group IN was 19-24 d, which was obviously longer than that in group C (12-16 d, chi 2 = 13.60, P < 0.001) but close to that in group GK as in part (2) (chi2 = 0.06, P = 0.810). Conclusions DETCs are involved in promotion of immune rejection of skin allograft probably by secretinf interferon gamma.

10.Infrastructure and sanitation practice of Chinese primary and middle schools canteens

Objective:
To understand the current situation of infrastructure construction and sanitation practice of school canteens in China and to provide a reference for improving food safety in school canteen and sanitation management level.
Methods:
Stratified sampling method was adopted to select the surveyed schools. Information regarding school canteen infrastructure and sanitation practice was collected through questionnaires. Chi-square test was used to compare the difference in passing rate on the survey indices among different school types.
Results:
In environmental sanitation, 91.0% of the school canteen was rated as good and in personal hygiene, 92.8% of the employees were rated as good. The overall health level of urban schools was better than that of township schools, and non-boarding schools was better than that of boarding schools, with statistically significant differences(P<0.01). 67.1% of school canteens provided with independent food outlets while 98.7% of school canteens without independent food stores. The qualified rate of washing facilities in the dining area was 94.7%, and the qualified rate of distance between the canteen and the pollution source was 92.5%. Nearly all the employees held canteen catering service license (99.0%) and the health qualification certificate (99.5%). Most of the school canteens (98.7%) established the safety rules and regulations, including the food retention sample system. Nearly 61.5% schools initiated the principal meal program.
Conclusion
Sanitation management in the canteens of primary and secondary schools in China is generally satisfactory, with some canteens still fail to meet the standard requirements. More supervision from schools and relevant government departments (market supervision, health and education departments) is needed to provide students with a clean and heathy campus canteen.
