1.Antineoplastic Medicine-induced Adverse Drug Reaction Reported in Our Hospital: Analysis of 162 Cases

OBJECTIVE: To investigate the characteristics and regularity of antineoplastic medicine-induced adverse drug reaction (ADR) occurred in our hospital. METHODS: 162 antineoplastic medicine-induced ADR cases reported in our hospital from 2005 to 2008 were analyzed statistically in respect of patients’age and sex, categories of drugs, route of administration, drug combination,organs and systems involved in ADR and clinical manifestation, etc. RESULTS: Of the total 162 ADR cases, proportion of men was similar to women. 68 cases (41.98%) were old persons aged older than 60. 123 cases(75.93%) were single use of drugs, 39 cases(24.07%) were combined use of drugs. 39.51% of ADR cases were induced by antineoplastic medicine made from plants and took up the first place in terms of incidence of ADR. 36.42% of ADR cases dominantly manifested as lesion of skin and its appendants. CONCLUSION: ADR monitoring of antineoplastic medicine should be strengthened to ensure rational and standardized use of antineoplastic medicine, avoid or reduce the occurrence of severe ADR.
2.Doppler tissue imaging of porcine chronic ischemic myocardium before and after gene transfer

Objective To investigate the value of Doppl er tissue imaging (DTI) in detecting chronic ischemic myocardium before and after gene transfer. Methods An Ameroid constrictor was placed around the left circumflex artery of each open-chest pig. After 4 weeks, groupⅠandⅡwere intramyocardially administrated recombinant adenovirus coding for the vascular endothelial growth factors(Ad.VEGF-B) or phosphate buffer(PB). After 4 weeks, their hearts were removed for pathological investigation. Color M-mode, two-dimensional and spectrum DTI of lateral-posterior wall before operation, 2 weeks and 4 weeks after Ameroid placement, 2 weeks and 4 weeks after administration were studied. Results Color of normal myocardium was bright, filling and distinct stratified, and of ischemic myocardium faint, local lost and cloudy stratified. After gene transfer, color filling became better and stratification distinct. It has been reported that angiogenic growth factors may be useful in the treatment of ischemic heart disease. In our experiment a recombinant deficient adenovirus vector coding for aFGF was intramyocardially administrated into swines with chronic ischemic myocardium to investigate its effect on the improvement of myocardial function. In the present study, experimental minipigs underwent thoracotomy and placement of an Ameroid constrictor on the left circumflex branch coronary artery. Four weeks later, Ad.aFGF( n =7), Ad.Null( n =5) or PBS( n =6), was administrated directly into the myocardium at 10 sites in the circumflex branch distribution area (10 9 pfu or 100?l). Another four weeks later, cardiac function was examined by echocardiography and the results showed significant improvement of myocardial function in Ad.aFGF animals compared with Ad.Null and PBS animals. So such a strategy of gene therapy can be used in patients with ischemic heart disease.

5.Study of myocardial perfusion of porcine chronic ischemic myocardium before and after gene transfer

Objective To observe the perfusion changes of porcine chronic ischemic myocardium before and after gene transfer by myocardial contrast. Methods Ten normal pigs were divided into two groups: groupⅠfor gene transfer(6 pigs) and groupⅡ for control(4 pigs). Myocardial contrasts were performed respectively in basic status, 4 weeks after an Ameroid constrictor was placed around the proximal left circumflex artery (LCX) and another 4 weeks after the replication deficient recombinant adenovirus coding for the vascular endothelial growth factors (Ad. VEGF)was administrated into the regional ischemic myocardium in groupⅠ, and the phosphate buffer(PB) was administrated into similar situs in groupⅡ. The perfusion of above mentioned different blood supply myocardium was observed with HP Sonos 5500 and S4 transducer by myocardial contrast. Results The normal left ventricular myocardial wall was uniformly thick and its perfusion was bright and well filling. The chronic ischemic myocardium was thinned and its echo showed the contrast defect. The echo of the myocardium for gene transfer returned to good, but the echo of the control group remained bad including some bright bits and pieces. Conclusions The blood supply of the porcine chronic ischemic myocardium was ameliorated by gene transfer. The perfusion characters of the different blood supply myocardium can objectively reflect the blood supply status of the myocardium.
6.Evaluation of chronic ischemic myocardium before and after gene transfer by combination of ultrasonic integrated backscatter and dobutamine stress test

Objective: To establish porcine chronic ischemic myocardium model and to evaluate ultrasonic integrated backscatter(IBS) and Doppler tissue imaging(DTI) in detecting this model. Methods: An Ameroid constrictor was placed around the porcine proximal left circumflex artery. The calibrated average image intensity (%AⅡ), cyclic variation of IBS (CVIB), the subendo-epicardical gradient index (TGI) and the spectrum of left ventricular papillary muscle level short axis view (LVPM-SAM) and apical four chamber view (AP-4CV) were measured at normal state, 2 w, 4 w, 6 w and 8 w after Ameroid placement. Results: Normal %AⅡ, CVIB and TGI were 2.29?0.32,(9.69?2.22)dB and 0.22? 0.08 respectively. After Ameroid placement, the %AII increased gradually, the CVIB decreased gradually, and the decrease was higher in subepicardium than that in subendocardium. ObjectiveTo assess the effects of constraint-induced movement therapy(CIMT) on sub-acute stroke patients with upper extremity motor dysfunction.Methods63 patients with hemiplegia after stroke were divided into two groups: control group(31 cases) and CIMT group(32 casese). The two groups received the regular rehabilitation training for 14 d. Then the control group went on the the regular rehabilitation training, while the CIMT group received CIMT for 14 d. All patients were assessed by the Action Research Arm Test(ARAT) and Fugl-Meyer(FMA) on the first day, the 15th day and the 30th day after the treatment.ResultsARAT score and FMA scores were higher in CIMT group than in the control group 30 d after the treatment and in CIMT group 15 days after the treatment(P<0.001).ConclusionCIMT is more effective to improve the upper limb motor function of the sub-acute stroke patients than the regular rehabilitation training.
9.Color Doppler ultrasonographic findings in active ankylosing sacroiliitis

Objective To evaluate the color Doppler ultrasonographic findings of active ankylosing sacroiliitis.Methods Thirty three patients (66 sacroiliac joints in total) with active ankylosing sacroiliitis identified by MRI and twenty eight healthy volunteers (56 sacroiliac joints) underwent color Doppler ultrasound examinations of sacroiliac joints.Degree of blood flow within and around the sacroiliac joints was evaluated by semi-quantitative scale (0 - 4 grade).For arteries, resistive index was also measured.Statistical analyses were performed to compare blood flow demonstration rate, blood flow signal scale distribution,degree of blow flow and RI value between the two groups.Results Higher blood flow demonstration rate was found in patients with active ankylosing sacroiliitis(60/66,90.1%) than control group (25/56,44.6%)( P = 0.000).The scale distribution and degree of blow flow between these two groups were statistically different (all P = 0.000) ,with mainly 2~3 grade for the patients and 0~1 grade for the control.Mean value of RI in the patients was 0.57 ± 0.07, which was lower than the control (0.66 ± 0.04, P = 0.000).Conclusions Patients with active ankylosing sacroiliitis tend to have higher degree of blood flow in sacroiliac joints and lower RI values of artery.Color Doppler ultrasound may help to evaluate the activity of ankylosing sacroiliitis.
10. Preliminary study of the relationship between novel coronavirus pneumonia and liver function damage: a multicenter study

Objective:
To analyze the clinical characteristics of cases of novel coronavirus pneumonia and a preliminary study to explore the relationship between different clinical classification and liver damage.
Methods:
Consecutively confirmed novel coronavirus infection cases admitted to seven designated hospitals during January 23, 2020 to February 8, 2020 were included. Clinical classification (mild, moderate, severe, and critical) was carried out according to the diagnosis and treatment program of novel coronavirus pneumonia (Trial Fifth Edition) issued by the National Health Commission. The research data were analyzed using SPSS19.0 statistical software. Quantitative data were expressed as median (interquartile range), and qualitative data were expressed as frequency and rate.
Results:
32 confirmed cases that met the inclusion criteria were included. 28 cases were of mild or moderate type (87.50%), and four cases (12.50%) of severe or critical type. Four cases (12.5%) were combined with one underlying disease (bronchial asthma, coronary heart disease, malignant tumor, chronic kidney disease), and one case (3.13%) was simultaneously combined with high blood pressure and malignant tumor. The results of laboratory examination showed that the alanine aminotransferase (ALT), aspartate aminotransferase (AST), albumin (ALB), and total bilirubin (TBil) for entire cohort were 26.98 (16.88 ~ 46.09) U/L and 24.75 (18.71 ~ 31.79) U/L, 39.00 (36.20 ~ 44.20) g/L and 16.40 (11.34- ~ 21.15) mmol/L, respectively. ALT, AST, ALB and TBil of the mild or moderate subgroups were 22.75 (16.31- ~ 37.25) U/L, 23.63 (18.71 ~ 26.50) U/L, 39.70 (36.50 ~ 46.10) g/L, and 15.95 (11.34 ~ 20.83) mmol/L, respectively. ALT, AST, ALB and TBil of the severe or critical subgroups were 60.25 (40.88 ~ 68.90) U/L, 37.00 (20.88 ~ 64.45) U/L, 35.75 (28.68 ~ 42.00) g/L, and 20.50 (11.28 ~ 25.00) mmol/L, respectively.
Conclusion
The results of this multicenter retrospective study suggests that novel coronavirus pneumonia combined with liver damage is more likely to be caused by adverse drug reactions and systemic inflammation in severe patients receiving medical treatment. Therefore, liver function monitoring and evaluation should be strengthened during the treatment of such patients.
