2.Choice of modus operandi for patients with vaginal vault prolapse

Objective To explore the perfect operation method for patients with vaginal vault prolapse.Methods Twenty six patients with vaginal vault prolapse who underwent transvaginal mesh repair by Prolift from January 2008 to June 2010 were analyzed retrospectively.Results The interval time between the first operation and this morbidity of ten patients who had underwent total hysterectomy for none FPFD ( ie:hysteromyoma et al) was ( 115.2 ±51.67) months,but the interval of sixteen patients who had underwent traditional repair operation (ie:transvaginal hysteromyoma and/or colporrhaphia anterior-posterior) for FPFD was (24.38 ± 13.43) months,which was markedly shorter ( t =6.75,P <0.01).The interval time between the first operation and the 2nd operation of the ten patients was ( 142.8 ±59.04) months,but that of the sixteen patients was (62.13 ± 44.51 ) months,which was significantly shorter ( t =3.97,P <0.01 ).Their age and POP-Q staging had no difference ( P >0.05).All operations were successfully performed by one gynecologist.The average operation time was ( 60.96 ± 7.88 ) minutes,the average blood loss was ( 119.23 ±27.53) ml,and there was no concomitant injury.All patients were followed up of 3 ~24 months.No vaginal erosion and prolapse recurrence happened,and the cure rate was 100%.Conclusions The Prolift system appeared to be a relatively safe and effective alternative to conventional surgeries for the treatment of vaginal vault prolapse.However,long-term follow-up still need to be performed.
3.Observation of effect of comprehensive community nursing on patients with diabetes mellitus

Objective To establish nursing intervention for patients with diabetes mellitus and discuss in-fluence of community nursing on rehabilitation of patients with diabetes mellitus. Methods Patients with con-finned diagnosis of type two diabetes mellitus (61 cases) were selected,individual health records were established and health education was given to them,including diet,exercises,psychological nursing,medication instruction and regular blood glucose monitoring. Patients compliance,control of fasting blood glucose and glycosylated hemoglobin were compared before and after nursing intervention. Results Patients compliance such as regular physical ex-amination,diet control,regular blood glucose monitoring and exercises treatment improved after intervention com-pared with those before community intervention (P<0.05). The control of fasting blood glucose and glycosylated hemoglobin was better after intervention compared with those before community intervention (P<0.01). Con-clusion Community nursing intervention for patients with diabetes mellitus proved to have good effect.
4.Development of an effective store permeability evaluation scale about chemotherapy drugs based on Delphi method

Objective To develop a scale of assessing inpatient′s risk factors of chemotherapy drugs permeability. Methods Designed a questionnaire for expert information collecting based on literature review,and employed the Delphi method to develop a set of indicators to measure inpatient′s effective store seepage risk factors. Fifteen experts finished the 2-round survey. Results Expert's enthusiasm was high, the two rounds of enquiry for effective questionnaire recovery rate was 100%. Expert's authority was very high, the average duration of clinical practice was 23.8 years, the average duration of nursing management was 13.1 years, the rate of familiarity about this kind of research was 100%, the recognition degree in the accumulation of theoretical knowledge, the clinical practice and the home or abroad research degree was 93%, 100%and 73%respectively. Two rounds of enquiry for expert advice of Cronbach's alpha coefficient was 0.96 and 0.98 respectively, experts tend to agree that the scale had good consistency, high credibility, delivery system eventually formed contains, vein, the static drop time (static) chemotherapy, more routine chemotherapy, liquid type, drug factors, social support, cooperation degree, liquid volume, mission 11 dimensions such as level of understanding, the nurse puncture store effective seepage risk factors assessment of 42 items. Conclusion The inpatient′s effective store seepage risk factor assessment scale developed by using the Delphi method is reliable for assessing effective store seepage risk factor of hospitalized patients .
5.Discussion on effect of comprehensive community nursing on patients with primary hypertension

Objective The optimal community nursing intervention methods for community patients with primary hypertension was discussed in order to promote their health. Methods System management and community intervention were applied to 140 community patients with primary hypertension for 2 years, including strengthing community communication, popularizing health knowledge, improving cognition of danger of hypertension, hypertensive disease project management, tracking and monitoring the patients with primary hypertension and medication supervision.The cognition rate of prevention and control knowledge for hypertension, hypertension-related cardiovascular complication and formation rote of hypertension control behavior were compared before and after intervention. Results The prevalence rate of the knowledge on hypertension was increased, accompanied with promoting regular medication in patients with hypertension, decreasing cardiovascular complication caused by hypertension and heightening formation rate of controlling behavior in hypertension patients.Conclusions Applying community nursing intervention is practically effective in prevention and treatment of primary hypertension, deserving promotion and application in community medical institute.
7.Effect of anti-platelet antibodies on the efficacy of immunoglobulin therapy and Treg/Th17 ratio in children with immune thrombocytopenia

【Objective】 To investigate the relationship between anti-platelet antibodies, therapeutic effect of intravenous immunoglobulin (IVIG) and Treg/Th17 cells imbalance in children with immune thrombocytopenia (ITP). 【Methods】 The changes and correlation of platelet count and Treg/Th17 ratio before and after IVIG treatment in 60 newly diagnosed ITP children with anti-platelet antibodies and 60 children with primary ITP without anti-platelet antibodies were analyzed. 【Results】 1) Compared with the control group, the efficacy of IVIG treatment was better in children with ITP in the case group(CR+ R cases: 50 vs 32) (P<0.01). 2) After IVIG treatment, platelet count(×109/L)(case: 4.5±2.9 vs 327.4±69.5, control: 4.1±3.2 vs 304.7±75.9), Treg cell level(%)(case: 2.15±1.08 vs 5.09±1.37, control: 2.41±0.92 vs 4.98±1.10), Treg/Th17 ratio(case: 1.10±0.19 vs 7.75±1.11, control: 1.27±0.21 vs 4.69±0.81)significantly increased while Th17 cell level(%) significantly decreased in the 2 groups of children(case: 2.07±1.31 vs 1.37±0.92, control: 2.13±1.18 vs 1.48±1.01); compared with the control group, there was no significant change in Treg, Th17 and Treg/Th17 ratio before and after treatment in the case group (P>0.05), but platelet count increased more significantly (P<0.05). 3) There were 3 positive cases in the control group and 12 negative cases in the case group after IVIG treatment, and IVIG treatment probably had no effect on the positive rate of anti-platelet antibodies in children with ITP (P>0.05). 4) The change in platelet count after IVIG treatment was significantly positively correlated with Treg levels (r=0.49 in the case group and r=0.441 in the control group) and negatively correlated with Th17 cell levels (r=-0.390 in the case group and r=-0.364 in the control group). 【Conclusion】 Anti-platelet antibodies can be used as a predictor of the efficacy of IVIG therapy in children with ITP, but they are not associated with changes in the Treg/Th17 ratio.
8.Determination of mequindox and its metabolite in pork luncheon meat by ultra-performance liquid chromatography/triple qudrupole tandem mass spectrometry

Objective To establish a reliable pretreatment method for the detection of mequindox and its metabolite in pork luncheon meat by ultra-performance liquid chromatography/triple qudrupole tandem mass spectrometry. Methods Samples were extracted with ethyl acetate, and the results of purification and enrichment by PAX and PEP solid-phase extraction columns were analyzed. Acetonitrile/methanol (3:11) - 0.1% formic acid water was used as the mobile phase, and Shimadzu Inertsil ODS-3-column (3µm, 2.1 × 100mm) chromatographic columns were used for qualitative and quantitative analysis using the multi-reaction detection positive ion mode. Results The results showed that PEP cartridge had good recovery rate. The detection limit of mequindox was 0.10µg/kg, and limit of quantitation was 0.30µg/kg. The average recoveries for spiked levels of 0.33, 0.83, and 1.65µg/kg were 127%, 72.0%, and 60.1%, respectively. The detection limit of 2-quinoxalinecarboxylic acid was 0.10µg/kg, and limit of quantitation was 0.40µg/kg. The average recoveries for spiked levels of 0.42, 1.05, and 2.1µg/kg were 125%, 99.0%, and 60.9%, respectively. Conclusion This method is suitable for the determination of mequindox and its metabolite 2-quinoxalinecarboxylic acid in luncheon meat.
9.Effect of cerebral hemodynamic changes on white matter damage in premature infant with patent ductus arteriosus.

OBJECTIVE:
To investigate the effect of cerebral hemodynamic changes on white matter damage in premature infant with patent ductus arteriosus(PDA).
METHODS:
A total of 106 premature infants were enrolled in the study, including 35 PDA infants with hemodynamic changes (hsPDA group), 35 PDA infants without hemodynamic changes (non-hsPDA group) and 36 non-PDA infants (control group). Serum level of neuron-specific enolase (NES) was detected and craniocerebral ultrasound examination was performed on d3, d7 and d14 after birth. The correlation between blood flow rate of PDA and gray scale value of lateral ventricle was analyzed.
RESULTS:
Gray scale values of lateral ventricle and serum levels of NES in hsPDA group were higher than those in control group on d3, d7 and d14 (P<0.01), but no significant difference was observed between non-hsPDA group and control group (P>0.05). There was a positive correlation between the blood flow rate of PDA and gray scale value of lateral ventricle (r=0.876, P<0.01) in premature infants.
CONCLUSION
Patent ductus arteriosus with hemodynamic changes is closely related to white matter damage in premature infants, and early intervention is necessary.
