1.Biological characteristics of Chinese hamster infected with Babesia

Objective To establish a Chinese hamster model of babesia infection, to find the changing pattern of organs and biochemical parameters in Chinese hamster infected with Babesia, and to promote the detection and treatment of babesiosis.Methods Healthy 5-week old Chinese hamsters were infected by intraperitoneal injection of blood containing Babesia.Blood samples were collected at 0, 2, 4, 6, 8, 10, 12, 14, 16, 23, 30, and 37 days after infection from 5 hamsters at each time point.Blood smears were prepared to detect the parasites using Giemsa staining.ELISA assay was employed to test the IL-2 concentration.The blood biochemical indexes were detected using an automatic biochemical analyzer.DNA was extracted from the whole blood and REAL-TIME RCR was performed to determine the reproduction of Babesia.Aftert the animals were sacrificed, the heart, lung, spleen, liver, and kidney were taken to analyze the changes of organ coefficients.Results The highest level of Babesia in the hamsters occurred on day 4 after the Babesia injection, and then showing a decreasing tendency.However, there was a transient increase on the 12th day after infection.The liver and spleen displayed most extensive response to the infection showing hepatomegaly and splenomegaly, but the variation of heart and kidneys coefficients was within the norm.There were prominent changes of blood cells, especially leucocytes, with two peaks at day 10 and 23 after the Babesia infection.The peak changes of blood biochemical indexes occurred at day 12 after infection.The concentration of serum IL-2 reached a peak on the 10th day after infection.Conclusions The Chinese hamsters display typical characteristics of tick-borne diseases such as hepatomegaly and splenomegaly.The immunological system is activated along with the infection and reaches a highest stage in the second week.Afterwards the Babesia can live in the hamster body for a long period of time.The results of this study provide useful information supporting further studies on the detection, treatment and prevention of Babesiosis.
2.Prokaryotic expression and characterization of truncated mutant capsid protein of genotype IV hepatitis E virus.

A truncated mutant of the Open Reading Frame 2 (ORF2, aa384-606) was amplified from cDNA of genotype IV hepatitis E virus (HEV) by polymerase chain reaction (PCR), subcloned to expression plasmid pTO-T7, and expressed in Escherichia coli. SDS-PAGE and Western blotting were used to detect and identify the recombinant protein, namely rP24. After washing of inclusion bodies, dissolving in denaturing agents, refoldeding by dialysis, ion exchange chromatography and gel chromatography, dynamic light scatter was used to study the hydrated radius of rP24. Western blotting was applied to detect the immunoreactivity of rP24, and mouse immunity test and indirect enzyme linked immunosorbent assay (ELISA) were applied to evaluate the immunogenicity and the detection rate of HEV positive and negative serum. SDS-PAGE and Western blotting show that rP24 was highly expressed in the form of inclusion bodies after induction, and had strong immunoreactivity to monoclonal antibody (McAb) 15B2. After a multi-step purification of rP24, Western blotting indicated that the purified rP24 also had strong immunoreactivity to neutralizing McAb 8C11 and HEV positive serum, suggesting that rP24 simulated the nature structure of HEV capsid protein. Dynamic light scatter demonstrated that the average hydration radius of purified rP24 was 7.48 nm. The mouse immunity test showed that the purified rP24 also had good immunogenicity, and the period of serum antibodies converted from negative to positive was very short, but the antibodies maintained more than 20 weeks. Indirect ELISA tests showed that the detection rate of was the same as anti-HEV-IgG diagnostic kit (Wan Tai corporation). Taken together, the rP24 simulated the neutralizing epitopes of natural HEV, and had strong immunoreactivity and immunogenicity. It provided a basis for the further investigation of the difference of infection mechanism between genotype I and genotype IV HEV.

3.Prevention and treatment of complications after open wedge high tibial osteotomy for knee arthritis

Open wedge high tibial osteotomy (OWHTO) is one of the effective treatments of medial knee arthritis and an important step in the treatment of knee arthritis. However, with the popularization of OWHTO, more and more of its related complications have been reported, including under- and over-correction of the alignment, tilted joint line, fracture of the cortical hinge, and changes in the posterior tilt of the tibial plateau. This paper reviewed the domestic and foreign research literature on the OWHTO treatment of knee arthritis to analyze the incidence of intraoperative and postoperative complications, treatment strategies and prevention methods concerning the complications in the treatment of OWHTO. A comprehensive understanding of the prevention and treatment methods of the complications can help avoid and reduce some of the complications so that OWHTO can be popularized more widely.
4.Effect of electroacupuncture on expression of V2R,AQP2 and ENaC-α in cochlea of guinea pigs with endolymphatic hydrops

Objective To investigate the effect of electroacupuncture(EA)on the expression of vasopressin type 2 receptor(V2R),aquaporin-2(AQP2)and epithelial sodium channel-α(ENaC-α)in guinea pigs with arginine vasopressin(AVP)-induced endolymphatic hydrops(EH).Methods Male albino guinea pigs were randomly divided into normal control,EH model and EA groups(n=12 guinea pigs in each group).The EH model was established by continuous injection intraperitoneally of AVP,4μg/(kg·d)for 7 days,and then 6μg/(kg·d)for 3 days,totally 10 days.EA(100Hz,1mA)was applied to Baihui(GV 20)and unilateral Tinggong(SI 19)for 20 min,once a day for continuous 10 days.Cochlear hydrops severity was measured using hematoxylin-eosin(HE)staining,and then the ratio of scala media(SM)area to SM + scala vestibuli(SV)area(R value)was calculated.The expression level of V2R,AQP2 and ENaC-α in the cochlea was detected by western blot.Results Following modeling,R value,the V2R,AQP2 and ENaC-α protein expression were significantly increased in comparison with the normal group(all P<0.01).After EA intervention,R value,the V2R,AQP2 and ENaC-α protein expression were notably down-regulated in comparison with the model group(all P<0.01).Conclusion EA can relieve the cochlear hydrops of EH guinea pigs,which may be related to its effect in down-regulating the expression of V2R,AQP2 and ENaC-α protein in the cochlea.
5.Upper arm vein versus subclavian vein for totally implantable venous access ports for patients with gastrointestinal malignancy: a retrospective comparison of complications.

OBJECTIVETo compare two different routes of totally implantable venous access ports (TIVPs) from the upper arm vein and the subclavian vein in terms of complications for patients with gastrointestinal malignancy.
METHODSPatients who underwent implantations of TIVPs from September 2013 to January 2015 were retrospectively evaluated. The outcome measurements were rates and types of postprocedural early-stage and long-term complications.
RESULTSA total of 208 patients(upper arm vein group, 86; subclavian vein group, 122) were included in this study. All TIVPs were implanted successfully. The rate of catheter displacement was higher in upper arm vein group(14.0% vs 5.7%, P=0.04), while other postprocedural early-stage complications had no significant difference between the two groups. The occurrence of transfusion obstacle and rates of overall postprocedural long-term complications were significantly lower in upper arm vein group than that in subclavian vein group(1.2% vs. 9.8%, P=0.02; 7.0% vs. 27.0%, P=0.01, respectively).
CONCLUSIONCompared with subclavian vein group, upper arm vein group has lower postprocedural long-term complication rates and is recommended as a safe and comfortable choice for port implantation.

To screen and evaluate the TCM in the treatment of bronchial asthma. Methods Retrieved the journals from China National Knowledge Infrastructure, the Vip Database, Wanfang Data and the Chinese Biomedical Literature Database (Sino Med) . The time limitation ran from January 1, 1987 to December 31, 2016. Extracted the indicators of the assessment based on the evaluating and screening system of country feasible TCM technology. Results a total of 150 treatment techniques were screened.They mostly were Chinese compound formula or point-application therapy. The score was between 24.857 and 34.304. Application of evaluating and screening system of country feasible TCM technology can screen and evaluate the feasible technologiesin the treatment of remission phase of bronchial asthma.
7.Study on the Medication Regularity of Traditional Chinese Medicine in the Treatment of Urinary Tract Infection Based on the Ancient and Modern Consilia Cloud Platform

Objective: To research the medication regularity of Chinese medicine on treating urinary tract infection based on the Ancient and Modern Consilia Cloud Platform to provide references for clinical treatment of urinary tract infection.Methods: The Ancient and Modern Consilia Cloud Platform was used to screen the consilia of Traditional Chinese Medicine for the treatment of urinary tract infection, the materials mainly come from books, journal articles, famous medical studios in the modern Chinese medicine case database. Screening, cleaning, standardizing consilia, and the standardized consilia database was set up. Statistical analysis, cluster analysis and complex networks were used to analysed the core prescription, the herb medicine frequency, the herb medicine four gas, the herb medicine five flavors, the herb medicine channel tropism and other regularity of Chinese medicine treatment of urinary tract infection, and displayed visually. Results: A total of 501 prescriptions were included, a total of 245 herb medicine were involved, and liquorice, poria, dianthus superbus and rehmannia were used more frequently. The tastes are cold, neutral, sweet taste, and bitter taste. Their channel tropisms mainly belong to lung and liver. The core prescriptions are based on Bazhengsan and Zhibaidihuangwan. Conclusion: Heat, dampness and diuretic herbs are used in the treatment of urinary tract infections in Chinese medicine, and nourishing yin and kidney, and relieving the stagnation of liver at the same time.Asthenic and sthenia syndrome, appearance and essence are also taken into account.
8.Effects of different treatment methods on renal anemia in maintenance hemodialysis patients with hyperparathyroidism secondary to uremia

Objective To observe the effects of three treatment methods on renal anemia in maintenance hemodialysis patients with hyperparathyroidism secondary to uremia and analyze the influencing factors of erythropoietin (EPO) dosage. Methods A total of 55 maintenance hemodialysis patients with secondary hyperparathyroidism at the hemodialysis center of Huashan Hospital affiliated to Fudan University from January 2015 to December 2016 were retrospectively divided into three groups according to different treatment methods, parathyroidectomy +forearm transplantation group (surgery group, n=16), cinacalcet treatment group (n=6), and calcitriol treatment group (n=33), respectively. The hemoglobin level and erythropoietin dosage were measured before treatment and in the 3rd month, the 6th month and the 12th month after treatment. The changes of hemoglobin and erythropoietin dosage in the three groups before and after treatment were observed, and the mixed effect model was used to analyze the difference of the change of hemoglobin and erythropoietin dosage among three groups. Multiple linear regression analysis was used to analyze the influencing factors of EPO dosage after one year. Results The levels of intact parathyroid hormone (iPTH) in the surgery group and the cinacalcet group before treatment were significantly higher than that in the calcitriol group (both P<0.05). In the 12th month after treatment, the levels of iPTH decreased significantly in the patients of surgery group and the cinacalcet group compared with those before treatment (both P<0.05). The levels of serum alkaline phosphatase, serum calcium and serum phosphorus in the surgery group also decreased significantly compared with those before treatment (all P<0.05). The mixed effect model analysis showed that the hemoglobin level of surgery group was on an upward trend after the treatment, and the overall level was significantly higher than cinacalcet and calcitriol treatment group (P=0.007). There was no significant difference in the dosage change of erythropoietin (EPO) in the three groups over time. However, the intra-group comparison of the mixed effect model showed that the dosage of EPO in the 12th month was significantly lower than that of before the treatment in surgery group (P=0.007). Multiple linear regression analysis showed that dialysis vintage (B=-0.064, P=0.012) and ferritin ≥ 500 μg/L (B=0.645, P=0.032) were independent influencing factors of EPO dosage. The longer the dialysis vintage, the less EPO dosage, and more EPO dosage were observed in patients with ferritin ≥ 500 μg/L. Conclusions Parathyroidectomy and forearm transplantation is more effective in reducing EPO dosage and improving renal anemia in maintenance hemodialysis patients with secondary hyperparathyroidism. Dialysis vintage and ferritin are independent influencing factors for the dosage of EPO.

Chronic refractory wound (CRW) is one of the most challengeable issues in clinic due to complex pathogenesis, long course of disease and poor prognosis. Experts need to conduct systematic summary for the diagnosis and treatment of CRW due to complex pathogenesis and poor prognosis, and standard guidelines for the diagnosis and treatment of CRW should be created. The Guideline forthe diagnosis and treatment of chronic refractory wounds in orthopedic trauma patients ( version 2023) was created by the expert group organized by the Chinese Association of Orthopedic Surgeons, Chinese Orthopedic Association, Chinese Society of Traumatology, and Trauma Orthopedics and Multiple Traumatology Group of Emergency Resuscitation Committee of Chinese Medical Doctor Association after the clinical problems were chosen based on demand-driven principles and principles of evidence-based medicine. The guideline systematically elaborated CRW from aspects of the epidemiology, diagnosis, treatment, postoperative management, complication prevention and comorbidity management, and rehabilitation and health education, and 9 recommendations were finally proposed to provide a reliable clinical reference for the diagnosis and treatment of CRW.
