Nowadays,with the ever-tenser physician-patient relationship,construction of medical professionalism has turned out to be a heated topic.This essay tries to explore the construction of medical professionalism from the perspective of cultivating a humanistic culture among medical students.Based on the comparison of medical humanistic education between China and Britain,this article arrives at the conclusion that defective humanistic education is one of important reasons that cause the inadequacy of medical professionalism among Chinese physicians,and points out some effectual measures to improve the medical professionalism in our country by the inspiration from our British counterparts.
2.A Study on the Effects and Mechanism of Arsenic Trioxide in Inducing Apoptosis of Colonic Cancer Cell Line

Objective To explore the effects and molecular mechanisms of arsenic trioxide(As 2O 3) in inducing apoptosis of colonic cancer cell line SW480. Methods The morphologic changes and apoptotic rate of SW480 cells induced by As 2O 3 were observed with fluorescent microscopy and flow cytometry. The bcl-2 and Fas expressions induced by As 2O 3 in SW480 cells were examined by immunohistochemical method. Results 24 hours after exposed to As 2O 3, SW480 cells exhibited typical morphologic changes of apoptosis, the apoptotic rate of which was 2.1%~10.6%. A typical subdiploid peak before G 1 phase was observed by flow cytometry, and As 2O 3 mainly acted in G 2/M phase. As 2O 3 decreased bcl-2 expression and increased Fas expression in SW480 cells. Conclusion As 2O 3 could induce apoptosis of SW480 cells. The molecular mechanism of As 2O 3-induced apoptosis of SW480 cells might be down-regulating the bcl-2 expression and up-regulating the Fas expression.

Based on the comparison of medical humanistic education among The Mainland,Hong Kong and Taiwan,this article arrives at the conclusion that the gap in the field of humanity education in the Mainland is one of the important reasons that cause the ever-tenser physician-patient relationship in nowadays.So we propose the countermeasures actively.By increasing the humanity courses,changing the teaching method,reinforcing the construction of talented staff in humanity,creating the environment for the humanity education and so on,we reinforce humanity quality of medical students in order to improve our medical education.
4.Pre-operation and/or Pre-transfusion Testing on Patients to Avoid Risk ofOccupational Infection Exposure among Medical Staff and Its Significance in Prevention of Nosocomial Infection

OBJECTIVE To investigate the pre-operation and/or pre-transfusion testing on patients to avoid risk of(occupational) infection exposure among medical staff and its significance in prevention of nosocomial infection.METHODS Serum hepatitis B(virus surface) antigen(HBsAg),antibody against hepatitis C virus(anti-HCV),(antibody) against human(immunodeficiency) virus(anti-HIV) and toluidine red unheatedserum test(TRUST) were performed on samples from 10 745 cases of pre-operation and/or pre-transfusion from 2003 to 2004.RESULTS The number and(percentage) of positive reaction for HBsAg,anti-HCV,anti-HIV and TRUST were 1163,(10.81%);21,0.20%;1,0.01%(confirmed by HIV Reference Laboratory,Center for Disease Prevention and(Control) of Zhejiang Province) and 74,(0.69%),respectively.TRUST positivity was further confirmed with TPHA.(Overlapping) infections were mostly observed between HBV and HCV,or,to a less scale,between HBV and syphilis.For HBV infection the(department) of digestive medicine was prevalent(?~2=4.77,P
5.The effects of typical and atypical antipsychotics on P50 sensory gating in first-episode schizophrenics

Objective To compare the effects of typical and atypical antipsychotics treatment on P50 sensory ga-ting in first-episode schizophrenics.Methods Using conditioning-testing stimulus paradigm and stimulus train paradigm to record the P50 auditory evoked potential in 36 normal controls and in 61 first-episode schizophrenics before and after treat-ment.Patients were categorized into two groups:the typical antipsychotic treatment group(typical group)and the atypical antipsychotic treatment group(atypical group).Results Before treatment,both of the typical and atypical groups had low-er levels of S2-P50 amplitude,P50 suppression in conditioning-testing stimulus paradigm and P50 amplitude as well as P50 suppression evoked by high frequency stimuli in stimulus train paradigm in comparison with controls(P<0.05).After treatment,the typical antipsychotic treatment significantly improved the levels of P50 suppression in the stimulus train para-digm but not the levels of S2-P50 amplitude,P50 suppression in the conditioning-testing stimulus paradigm(P<0.05)whereas the atypical antipsychotic treatment improve the levels of P50 amplitude,P50 suppression in both stimulus train paradigm and the conditioning-testing stimulus paradigm(P<0.05). Conclusions The typical antipsychotic treatment can ameliorate the P50 suppression in stimulus train paradigm,but not in the conditioning-testing stimulus paradigm,whereas atypical antipsychotic treatment can ameliorate P50 suppression in both paradigms.
6.The effects of olanzapine,clozapine,risperidone and quetiapine treatment on P50 sensory gating in first-ep-isode schizophrenics

Objective To compare the effects of atypical antipsyehoties treatment on PS0 sensory gating in first-episode schizophrenics. Methods The P50 auditory evoked potential was recorded by using conditioning-testing stimulus paradigm and stimulus train paradigm in 36 normal controls and 53 first-episode schizophrenics be-fore and after treatment,and compare the difference of P50 sensory gating after treatment. Results Before treat-ment, compared with control group, the atypical groups both had statistic difference of T-P50 amplitude ((1.01±0.88)μV, (0.68±0.64)μV, (0.58±0.47)μV), P50 suppression ((0.61±0.27), (0.54±0.22, (0. 59± 0.19)) in conditioning-testing stimulus paradigm and P50 amplitude,P50 suppression evoked by high frequency stimuli in stimulus train paradigm(P < 0.05), but no difference among the atypical groups (P > 0.05). After treat-ment,compared with control group, there was no statistic difference in olanzapine and elozapine groups of T-P50 amplitude and P50 suppression in conditioning-testing stimulus paradigm, but the difference in risperidone and que-tiapine groups still obviously(P<0.05). In stimulus train paradigm, there was no statistic difference of P50 ampli-tude, P50 suppression evoked by high frequency stimuli in every groups (P>0.05). Compared within atypical groups, the difference of P50 amplitude and P50 suppression were both obviously(P<0.05). Conclusion Each a-typical antipsychotic has different effect on P50 sensory gating;and the conditioning-testing stimulus paradigm and stimulus train paradigm P50 sensory gating may reflect different central neuron mechanism.
7.A comparative study of the clinical efficiency of escitalopram and paroxetine in treatment of obsessive-compulsive disorder

Objective To investigate the difference of clinical efficiency and safety in obsessive-compulsive disorders (OCD) treatment with escitalopram or paroxetine.Methods A total of 156 OCD patients were randomly divided into escitalopram group (ESC group) and paroxetine group (PAR group).Yale-Brown Obsessive Compulsive Scale (Y-BOCS),Hamilton Depression Scale (HAMD) and Treatment Emergent Symptom Scale (TESS)were used to evaluate the clinical efficiency and safety before and after1,2,4,6,8 weeks treatment.Results The cure rate(21.79% vs 17.95%) and effective rate(70.51% vs 71.79%) had no statistically difference between ESC group and PAR group,and incidence of side effect had no significant difference between two groups(x2 =1.99,P>0.05).Compared with the group before treatment,HAMD scores were significantly decreased from the first weekend in ESC group,but in PAR group HAMD scores did not decrease until the second weekend,and the differences were also significant(P<0.05).Conclusion Escitalopram is a safety,effective and well-tolerated drug in the treatment of obsessive-compulsive disorder.
8.Sleep characteristics in first-episode schizophrenics and the effects of olanzapine on body weight and sleep-breathing disorder

Objective To explore the sleep characteristics in first-episode schizophrenics and the effects of olanzapine on body weight and sleep-breathing disorder.Methods 36 first-episode schizophrenics (patient group) and 33 normal controls (control group) were tested with polysomnography(PSG),and compared the difference of PSG,sleep-breathing index and body mass index(BMI) before and after treatment in patient group.Results Before treatment,compared with control group,the patient group had significantly prolonged sleep latency((83.64± 10.62) min vs (29.41 ± 10.05) min),shortened total sleep time ((286.43 ± 17.04) min vs (343.66 ± 16.38)min),decreased sleep efficiency((65.73 ±11.47) vs (86.13 ± 8.15)),increased awake time and arousal number((65.70 ± 10.33) min vs (25.93 ± 9.60) min ; (38.26 ± 6.88) vs (14.40 ± 2.72)) in sleep continuity ; and increased N1 stage ((87.43 ± 11.35) min vs (36.55 ± 6.40) min),decreased N2,N3 stage ((100.53 ± 10.42)minvs (143.35±13.52)min;(49.83±7.51)minvs (87.52±9.74)min) in sleep structure (P < 0.05).After treatment,sleep continuity and sleep structure in patient group were improved,compared with control group,only BMI,arousal index and hypopnea index had statistic difference (P < 0.05).Conclusion The first-episode schizophrenics have both sleep continuity and sleep structure deficits.Although olanzapine treatment can improve sleep quality,long-term use of it may cause overweight and sleep-breathing disorder.
9.Clinical study on acupuncture and massage combined with anterior cervical decompression and bone graft internal fixation in treatment of cervical spondylotic myelopathy

Objective To evaluate the effect of acupuncture and massage combined with anterior cervical decompression and bone grafting for the treatment of cervical spondylotic myelopathy, and to analyze the mechanism of action and provide guidance for clinical prevention and treatment.Methods A total of 62 patients with cervical spondylotic myelopathy were enrolled in our hospital from October 2013 to October 2014.The patients were divided into treatment group and control group according to random number table.The control group was treated with anterior decompression and bone grafting, and the treatment group received acupuncture and massage combined with anterior cervical decompression and bone grafting and internal fixation.The changes of JOA score, TCM syndrome score, VAS score and quality of life score before and after treatment were observed and the prognosis was evaluated.Results After treatment,the JOA score, quality of life score of the two groups were significantly increased than those before treatment, while the TCM symptom scores, VAS scores were significantly decreased.The improvement effect of the treatment group was better than that of the control group(P<0.05);The prognosis of the treatment group is better than that of the control group(P<0.05).Conclusion The treatment of cervical spondylotic myelopathy with acupuncture and massage combined with anterior decompression and bone grafting and internal fixation can significantly improve the clinical efficacy and prognosis, and the safety is higher.
10.Diagnosis and treatment of gastric cancer with ovarian metastasis:a report of 17 cases

Objective To investigate the clinical and pathological characteristics, diagnosis and treatment of gastric cancer with ovarian metastasis. Methods The clinical data of 17 cases of gastric cancer with ovarian metastasis, confirmed by surgery and pathology, were analyzed retrospectively. Results The average age of the patients was 48.41 years, and the first appearance of symptoms and signs often were of metastatic ovarian cancer. The main ultrasonographic findings were either a complex type of mass with both solid and cystic characteristics or only solid, and most of them were accompanied by intraperitoneal fluid accumulation. Bilateral metastatic ovarian cancer was more common(13 cases). The preoperative accurate diagnosis of this disease was difficult, so that the misdiagnostic rate was 64.7% in this series. Operation was done in all the patients, but prognosis was poor. The median survival time was only 11.6 months. Conclusions The prognosis of gastric cancer with ovarian metastasis is poor. It is of importance to inspect the stomach in cases of bilateral ovarian cancer. Radical resection of the primary disease focus together with hysterectomy and bilateral adnexectomy should be performed. Postoperative comprehensive therapy is conducive to improve the prognosis of gastric cancer with ovarian metastasis.
