1.Extra-articular arthroscopic treatment of rotator cuff calcifying tendonitis

[Objective]To evaluate the surgical technique and effectiveness of extra-articular arthroscopic debridement in patients with rotator cuff calcifying tendinitis unresponsive to conservative treatment.[Methods]Arthroscopic debridement was performed in 18 patients(5 males,13 females;mean age 56.4,range 34 to 78) in whom pain and functional disability persisted for more than 3 months despite conservative therapy for rotator cuff calcifying tendinitis.Arthroscopy was inserted into subacromial interspace and bursectomy was done.Calcified plaques in supraspinatus tendon were explored and eliminated outside shoulder joint under arthroscopy.VAS pain score,Constant-Murley score and plain radiograph was adopted for evaluation before and after surgery.[Results]The mean follow-up period was 9 months(6 to 15 months)[KG-58x].Pain and functional disturbance of the shoulder disappeared or obviously improved.The average VAS pain score was(7.8?0.6) preoperatively and(1.7?0.4) postoperatively.The average Constant-Murley score was(61?7)preoperatively and(91?4)at the last follow-up.Radiograph showed little residual deposits in 2 cases but without preoperative symptoms.No case need rotator cuff repair.[Conclusion]Extra-articular arthroscopic removal of calcium deposits together with bursectomy is effective and reliable in patients with chronic calcifying tendinitis unresponsive to conservative treatment.
2.3M-Z350 Nano-resin and 3M-Z100 composite resin for cosmetic restoration of anterior teeth with vital pulp: Evaluation on postoperative sensitivity and pain

BACKGROUND: Resin filling, full crown and veneer are used in cosmetic restoration of anterior teeth. Doctors prefer resin because of its perfect clinical effect and small hurt. Some patients complain of sensitivity after rsein filling and we are interest in choosing a better resin with less sensitivity.OBJECTIVE: To compare the incidence of postoperative sensitivity and the difference of the patients' self sensitivity alter cosmetic restoration of anterior teeth with vital pulp by 3M-Z350 Nano-resin and 3M-Z100 composite resin. METHODS: A total of 112 teeth with vital pulp because of caries or traumatic tooth fractures of 68 patients demanding cosmetic restoration were selected from the Department of Endodontics, Stomatological Hospital of Luzhou Medical College from August to December 2006. The teeth were random divided into 3M-Z350 group and 3M-Z100 group, with 56 teeth in each group. 3M-Z350 nano-rasin and 3M-Z100 composite resin were used for cosmetic restoration of anterior teeth with vital pulp. Ryge criteria were used to evaluate the sensitivity to the cold stimulation following cosmetic restoration, and the level of pain was detected by the pain ruler.RESULTS AND CONCLUSION: Compared with 3M-Z100 group, the incidence of sensitivity and self-pain level were decreased in the 3M-Z350 group at 1 day, 1 week, and 1 month after restoration (P < 0.01 or P < 0.05). There was no significant difference in the sensitivity and self-pain level between the two groups (P > 0.05). Anterior teeth with vital pulp were well restored by 3M-Z350 nano-resin which might reduce the incidence of postoperative sensitivity.
3.Utilization of Uracil-DNA glycosylase for combining reverse transcription and anti-contamination with polymerase chain reaction in hepatitis C virus

Objective: To develop a hepatitis C virus(HCV) reverse transcription-polymerase chain reaction (RT-PCR) assay using Uracil-DNA glycosylase (UDG) for amplicon contamination control and evaluate the temperature and UDG concentrations for anti-contamination. Methods: In this new HCV RT-PCR assay, reverse transcription, UDG anti-contamination and the first PCR were carried out at the same time. The layer candles were used to prevent the contamination in the second PCR. dU-DNA was used as quality control for UDG anti-contamination and templates to determine the sensitivity of the new HCV RT-PCR assay. HCV cDNA was detected by DNA enzyme immunoassay (DNA-EIA). Results: Complete degradation of amplicon DNA was observed on the conditions of 0.2 u UDG per reaction volume respectively at 37 ℃ and 42 ℃ for 40 min. The anti-contamination condition also could eliminate all detectible dU-DNA, including the highest concentration of amplicon DNA.The 1∶104 dilution of the HCV RNA sample containing 2.110?105copies/mL copies of RNA could be detected. Conclusion: Our results indicate that this new RT-PCR assay can control the contamination stringently and is sensitive as well.
4.Nano-cellulose proteinversusacellular matrixinoral mucosarepair

BACKGROUND:The sensitivity andmucus secretion of theoral mucosamake oral soft tissues difficult to repair, so patients cannot achieve satisfactory outcomes after treatment. Nano-celulose protein mainly composed of glycine, alanine and serine has good histocompati bility. However, there is a lack of comparative study about the effect of nano-celulose protein and acelular matrix in oral mucosa repair, and the clinical effects of the two materials are stil under discussion.
OBJECTIVE:To investigate the effects of nano-celulose proteinver susacelular matrix in oral mucosa repair.
METHODS:Oral mucosadefect models were preparedinrats, and these rat models were randomly divided into four groups:oral mucosa defectswere repaired by vaseline (control group), nano-celulose protein, bovine skin acelular matrix and human skin acelular matrix, respectively. Repair effects were compared among different materials within 2 months after surgery.
RESULTS AND CONCLUSION:The diameter of oral mucosa defect measured using a vernier caliper, had no significant differences among groups at 1 day after surgery (P> 0.05); the diameter of oral mucosa defect in the nano-celulose protein group was significantly lower than that in the other groups at 3, 5 and 7 days after surgery (P<0.05); the diameter of oral mucosa defect in the bovine and human skin acelular matrix groups was significantly lower than that in the control group at 5 and 7 days after surgery (P< 0.05). Morphological observationoftheoral mucosa under light microscope showed: the number of newborn capilary endothelialcels in the defect region had no significant differences among nano-celulose protein, bovine acelular matrix and human acelular matrixgroupsat 1, 3, 5 and 7 weeks after surgery (P> 0.05);but there were significant differences in the number of newborn capilary endothelialcels between the control group and the other three groups (P< 0.05). Furthermore, at 21 days after surgery, closely aligned and thicker new epithelialtissuecould be found in the nano-celulose protein group; in the bovine acelular matrix group,thedefect regionwasrepaired wel, new epithelialtissueappeared andthe number of inflammatory cels decreased; in the human acelular matrix group, inflammatory cels appearedobviously, and new epithelialtissueformed with the normal thickness. In contrast,abundant inflammatory cels and thinner epithelial tissues appeared in the control group. To conclude,bothnano-celulose protein and acelular matrix can accelerate wound healing by promoting oral mucosal epithelial hyperplasia.
5.Silk fibroin scaffold for repair of buccal mucosa defects

BACKGROUND:Fibroin is a natural macromolecular material with Arg-Gly-Asp peptide structure that is a special tripeptide structure closely related to cel adhesion, and it can promote cel migration, adhesion, and proliferation and influence cel morphology and function. OBJECTIVE:To compare the effects of different silk fibroin scaffolds to repair buccal mucosa defects in rats. METHODS:Ninety Sprague-Dawley rats were selected to make unilateral buccal mucosa defect models, and randomly divided into three groups, 30 rats in each group: porous silk fibroin scaffold was implanted into the buccal mucosa defect in experimental group, multi-layered crosslinked silk fibroin film was implanted into the buccal mucosa defect in control group, and vaseline gauze was used to cover the buccal mucosa defect folowed by suturing in blank control group. After 15 days, wound diameter was detected; after 30 days, bone defect tissues were taken for hematoxylin-eosin staining. RESULTS AND CONCLUSION:At postoperative 15 days, the wound diameter was significantly smaler in the experimental group than the control and blank control groups (P < 0.05), as wel as smaler in the control group than the blank control group (P < 0.05). Hematoxylin-eosin staining showed that at 30 days after operation, there were more epithelial spikes and fibroblasts, but less inflammatory cels in the experimental group than the other two groups (P < 0.05), and fibroin fibers were partialy absorbed and degraded in the experimental group. These findings indicate that porous silk fibroin scaffold for buccal mucosa defect repair can accelerate epithelialization and wound healing.
6.Relationship of quadriceps tendon and the stability of external fixator in rabbit femur fracture model

BACKGROUND:In rabbit femur fracture model experiment, there are a few studies on whether quadriceps tendon wil affect the stability of the fixed device. OBJECTIVE:To analyze the effects of quadriceps tendon to the stability of external fixator in rabbit models of femur fracture. METHODS:The 48 New Zealand white rabbits were randomly divided into two groups. External fixator was used to make fracture model. In the experiment group, quadriceps tendon was cut. In the control group, quadriceps tendon was retained. 2 weeks later, X-ray examination was utilized to observe the number of failed external fixator in both groups from modeling to success. RESULTS AND CONCLUSION:(1) X-ray examination:2 weeks after model establishment, external fixation frame screw was free from femur in one rabbit of the experiment group, and seven rabbits in the control group. Of them, one screw was broken. Threaded portion retained in the femur. (2) External fixation failure rate:failed external fixator was found in one rabbit in the experiment group, resulting in a failure rate of 4%, and 12 rabbits in the control group, resulting in a failure rate of 50%(P<0.05). These findings suggested that cutting off the rabbit quadriceps tendon can effectively ensure the stability of the external fixator, and indirectly improve success rate of the experiment.
7.Application of intelligent simulator (SimMan) in comprehensive skill training of doctors and nurses

Objective To examine the application effect of intelligent simulator(SimMan) in comprehensive skill training among doctors and nurses.Methods Totally 168 doctors and nurses were chosen as the training object,6 people (4 doctors and 2 nurses) formed a work team.Comprehensive training course was in accordance with 2010 American Heart Association guidelines for cardiopulmonary resuscitation and emergency cardiovascular care science.Real clinical situation was designed by using the third generation SimMan of Norway laerdal company.Trainees were administered two tests before and after training,in which there were two parts containing specialized operation ability and non-technical ability.Overall qualification rate,average mark and treatment time of qualified teams were compared before and after the training.SPSS 16.0 software was used to analyze the data and P <0.05 signifies statistically significance.Results Overall qualification rate,average mark and treatment time of qualified teams were 71.43%,(84.70 ± 3.15),and (449.25 ± 77.0) s respectively before training while 100%,(93.55 ± 3.86) and (323.71 ± 42.26) s after training.There were significant differences in results of two groups before and after training (P =0.002,0.000,0.000).Conclusions Application of the third generation intelligent simulator (SimMan) in comprehensive training will obviously improve training effectiveness and cultivate the non-technical ability.
8.Clinical evaluation of immediate complete denture with base reconstruction

23 cases were treated with immediate full denture.The second base-remodeling or re-prosthodontic treatment was given 3 months later based on the oral condition.Masticatory efficiency (ME)was measured at each stage.About 30% patients participated in the second base-remodeling procedure with ME of 52.63%.
9.Analysis and state of HCV genotype 6a infection

Objective: To investigate the infection state of hepatitis C virus genotype 6a in China.Methods: Three(95,126,150)HCV genotype 6a serum samples were identified by digesting 5′NCR with compound enzyme method.Then,HCV 5′NCR and NS5B fragments were amplified from these samples by RT-PCR assay and sequenced.The phylogenetic trees of the samples were analyzed and compared with 24 HCV complete gene sequences from GenBank.Results: The sequencing reports on 5′NCR showed "CA" bases in 3 serum samples(95,126,150) were inserted into-145 site,and the sequences of 3 serum samples had the highest homology with sequence Y12083(0.934,0.930,and 0.926,respectively).The results of the phylogenetic trees suggested these 3 serum samples belonged to HCV genotype 6a.The sequencing reports on NS5B showed the 3 serum samples also had the highest homology with HC-J4(0.934,0.930,and 0.926,respectively),and the results of the phylogenetic trees suggested these 3 serum samples belonged to HCV genotype 1b.To exclude the influence of amplification efficiency of primers,NS5B fragments were amplified by HCV genotype 6a specific primers and no amplification products appeared.Conclusion: There are different results of HCV genotype by analyzing 5′NCR and NS5B in 3 samples infected with HCV genotype 6a.It may be related with gene recombination.It suggests HCV genotype should be analyzed on more than two regions.
10.Detection of circulating tumor cells from peripheral blood in patients with esophageal squamous cell carcinoma and its clinical significance

ObjectiveTo explore the relationship between circulating tumor cells(CTCs) in the peripheral blood of patients with esophageal squamous cell carcinoma(ESCC) and the physiopathological characteristics of esophageal neoplasms.MethodsUsing negative selection system,we depleted red blood cells(RBCs) in red blood cell lysis buffer,depleted white blood cells (WBCs) with Miltenyi magnetic beads and enriched the rare cells from ESCC patients'peripheral blood.Immunofluorence staining (IF) was adopted to identify CTCs.ResultsCirculating tumor cells in the peripheral blood of patients with esophageal squamous cell carcinoma was closely related to cell differentiation grade,the invasion of primary cancer,lymph node status,P-TNM stages,and was rarely related to the sex,age or the location of tumor.ConclusionThe results suggest that circulating tumor cells in the peripheral blood of patients with esophageal squamous cell carcinoma may express the development of esophageal cancer and may be served as a tumor marker to evaluate the biological behavior of esophageal cancer.
