1.Association between VDR gene FokI polymorphism and colorectal cancer susceptibility:a meta-analysis

Objective To discuss the relationship between FokI gene polymorphism and colorectal canc-er.Method The references that met the inclusive criteria were assessed .The extracted data were summarized in the form of tables .Data were analyzed by heterogeneity test .Meta-analysis was conducted by using fixed effects model or random effects model .Results Fifteen experimental studies were included in this study ,including 7, 859 colorectal cancer patients and 9,933 controls.The association between VDR gene FokI and colorectal cancer was expressed as the odds ratio(OR)and 95% confidence interval(CI).FokI recessive genetic model,ff versus FF+Ff,ORrecessive=1.01(95% CI=0.87~1.17,I2 =54%,P=0.91),ORdominant =0.94(95% CI=0.83~1.06,I2 =64%,P=0.30);allele model,fversus F,OR=0.98(95% CI=0.89~1.08,I2 =71%,P=0.72).In subgroup analysis,recessive genetic model,ORCaucasian race =0.95(95% CI=0.86~1.05,I2 =0%,P=0.33);ORyellow race=1.12(95%CI=0.77~1.63,I2 =72%,P=0.54).In subgroup analysis,dom-inant gene model,ORCaucasian race =0.95(95% CI=0.86~1.04,I2 =27%,P=0.29);ORyellow race =0.89(95%CI=0.65~1.22,I2 =78%,P=0.47).In subgroup analysis,allele model,ORCaucasian race=0.97 (95%CI=0.90~1.04,I2 =31%,P=0.38);ORyellow race =1(95% CI =0.79 ~1.27,I2 =82%,P=1.00).Conclusion Our results suggested that in VDR gene FokI ,F and f alleles were not obviously directly correlated with colorectal cancer .No difference was found between Asian and Caucasian .The conclusions in other populations should be further identified by extensive experimental studies .

Scientific researchers , medical and nursing staff in Academy of Military Medical Sciences and Chinese PLA General Hospital are the main readers of Medical Library of Chinese PLA.The papers they published in CMCI-covered journals from 2010 to 2012 and their citations were analyzed by citation analysis in order to find the de-mand of users and the use of literature in militarymedical journals , and discover the demand for biological literature information in scientific researchers and nursing staff in Academy of Military Medical Sciences and Chinese PLA General Hospital.The authors proposed that library holding journals should be evaluated with the support rate of highly cited journals and zero citation rate of library holding journals to guide the development of journal resources , provide evidence for the adjustment of resource acquisition , and perfect the library holding resources ofuser-ori-ented demand.
3.Application of probiotics intestinal preparation combined with early enteral nutrition in perioperative period of laparoscopic rectal cancer radical proctectomy

Objective:To study the safety and practicability of probiotics intestinal preparation combined with early enteral nutrition in perioperative period of laparoscopic rectal cancer radical proctectomy.Methods:The clinical data of 131 patients who underwent selective laparoscopic rectal cancer radical proctectomy in Shengjing Hospital of China Medical University from January to June 2018 were retrospectively analyzed. Among them, 80 patients received probiotics intestinal preparation combined with early enteral nutrition (experimental group), and 51 patients received traditional intestinal preparation and postoperative treatment (control group). The bleeding volume during operation, hospitalization expenses, first exhaust time, postoperative hospitalization time, indwelling catheter time and complications were compared between 2 groups.Results:The bleeding volume during operation, hospitalization expenses, postoperative hospitalization time in experimental group were significantly less than those in control group: (166.33 ± 15.18) ml vs. (226.47 ± 16.46) ml, (42 365 ± 3 921) yuan vs. (51 048 ± 4 772) yuan and (13.32 ± 1.03) d vs. (13.98 ± 1.29) d, and there were statistical differences ( P<0.01). There were no statistical difference in the first exhaust time, indwelling catheter time and incidence of complications between 2 groups ( P>0.05). Conclusions:The probiotics intestinal preparation combined with early enteral nutrition in perioperative period can reduce the hospitalization expenses and shorten the hospitalization time in patients undergoing laparoscopic rectal cancer radical proctectomy, which is safe and feasible.
4.Optimization of the Extraction Technology of Total Saponins from Coreopsis tinctoria by Orthogonal Test

OBJECTIVE:To optimize the extraction technology of total saponins from Coreopsis tinctoria. METHODS:Etha-nol leaching technology was adopted. Based on single factor test,the extraction technology was optimized by orthogonal test using extraction temperature,ethanol volume fraction,extraction time,solid-liquid ratio as factors,extraction rate of total saponins as in-dex. The optimized technology was validated. RESULTS:The optimal technology was that ratio of solid to liquid was 1:30 (C. tinctoria-60% ethanol),extracting for 2 h at 50 ℃. Validation test showed that average extraction rate of total saponins was 6.8%(RSD=0.85%,n=3). CONCLUSIONS:The optimized technology can be used for the extraction of total saponins from C. tinc-toria and keep stable.
5.Expression of variants of CD44 molecules in polymorphic adenoma of salivary glands

Objective To investigate the expression of CD44v3 and CD44v6 in the polymorphic adenoma of salivary glands. Methods Using immunohistochemistry technology to detect the expression of CD44v3 and CD44v6 in 30 cases of polymorphic adenoma of salivary glands. Results The expression of CD44v3 was positive in 66.7 %cases within 30 paraffin-embedded sections, while CD44v6,86.7 % was positive. Conclusion The high expression of CD44v3 and CD44v6 maybe the reason of the low invasive ability and migration of polymorphic adenoma of salivary glands.
6.The medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular for the flatfoot related with accessory navicular

Objective To investigate the clinical outcomes of the medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular for the flatfoot related with accessory navicular.Methods From March 2009 to October 2011,13 patients (16 feet) with flatfoot related with accessory navicular received treatment by the medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular.There were 4 males and 9 females,with an average age of 41.3 years (range,18-64 years).All patients had obvious valgus calcaneus,the angle of which was 11.3°± 1.4°.According to AOFAS ankle-hindfoot scores,the arch height,calcaneus inclination angle (CI),talocalcaneal angle (TC),talar first metatarsal angle (TMT) on the lateral weight-bearing radiograph of foot,and the talocalcaneal angle (TC),talar first metatarsal angle (TMT) on the AP view of the weight-bearing radiograph of foot,and the heel valgus alignment on axial radiographs of the hindfoot were measured on the X-ray film.Results All patients were followed up for 12 to 31 months,with the average of 16.8 months.Eleven patients (13 feet) felt no pain 6 months after operation,while 2(3 feet) felt pain after long walking.There was no complication,including infection,nerve injury,un-union,and so on.The average AOFAS ankle-hindfoot score improved from 56.4-±6.4 preoperatively to 88.1±2.8 at the last follow-up.Radio graphically,all parameters were statistically significant between pre-operation and the last follow-up,including the arch height,CI,TC,TMT modifying from 3.8±0.3 mm,9.5°±1.1°,47.3°±2.5°,17.6°±1.6° to 12.0±1.1 mm,20.1°±1.5°,32.3°±2.5°,6.8°±1.0° respectively on the lateral weight-bearing view; TC improving from 39.5°±2.3° to 26.2°±2.0°and TMT improving from 15.2°±1.7° to 6.3°±1.0° on the AP weight-bearing view.Conclusion The medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular is a good choice for the treatment of flatfoot related with accessory navicular with excellent clinical outcomes.
7.Effects of six kinds of Chinese herb extracts on the activities of rat liver microsomes in vitro.

Effects of six kinds of Chinese herb extracts, including Folium Crataegi extract, Herba Epimedii extract, Folium Acanthopanacis Senticosi extract, Trifolium pratense L. extract, Folium Ginkgo extract and Radix Puerariae extract, on the activities of CYP450 isozymes (CYP1A2, CYP2C, CYP2E1, CYP2D, CYP3A) in rat hepatic microsomals were studied by using a UPLC-MS/MS (MRM) and cocktail probe substrates method. The results showed that effects of six kinds of Chinese herb extracts on each CYP450 isozyme activity were inhibitory. The IC50 of Folium Crataegi extract for the inhibition of rat microsomal CYP2D activity was only for 4.04 microg x mL(-1), which showed the highest inhibition; Trifolium pratense L. extract had strong inhibitory action to CYP2D, the IC50 value was 5.73 microg x mL(-1); Folium Crataegi extract also had strong inhibitory action on CYP2E1, the IC50 value was 10.91 microg x mL(-1). Furthermore, the IC50 of Folium Ginkgo extract for the inhibition of rat microsomal CYP3A, 2D, 2E1 activities were 45.12, 35.45 and 22.41 microg x mL(-1), respectively, and the IC50 of Folium Acanthopanacis Senticosi extract on the inhibition of rat microsomal CYP2E1 activity was 32.89 microg x mL(-1). In addition, mechanism of inhibition experimental results showed that the inhibiting abilities of Folium Crataegi extract and Radix Puerariae extract on each CYP450 isozyme increased with the increasing of the preincubation time, therefore, the inhibitory effects were a mechanism-based inhibition.
8.Study on Preparation of Anti-tumor Drug H6 Polymeric Micelles and Their in vitro Anti-tumor Effects

OBJECTIVE:To prepare anti-tumor drug H6(lactone compound)polymeric micelles,and to investigate its in vitro anti-tumor effects. METHODS:Using mPEG2000-PCL4000 as carrier,H6/mPEG2000-PCL4000 micelles were prepared. Using particle size, PDI and 48 h whether to produce precipitation as indexes,feeding ratio,H6 concentration,volume ratio of organic solvent were screened. The encapsulation efficiency and drug-loading amount of micelle were all detected. MTT assay was used to detect the tox-icity of micelles and H6 solution to human non-small cell lung cancer cell A549 and human lung cancer cell H460. RESULTS:The screened formulation was as follows as feeding ratio of 1∶25,H6 concentration of 2 mg/mL,the ratio of ethanol to chloroform of 1∶1(V/V). The parameters of prepared H6/mPEG2000-PCL4000 micelles were as follows as particle size of(40.74±0.116 3)nm,PDI of(0.101±0.006),encapsulation efficiency of(94.87±0.016 3)%,drug-loading amount of(7.07±0.001 5)%(n=3). IC50 of mi-celles and H6 solution to A549 cell were 15.62 and 12.57 nmol/L;IC50 of micelles and H6 solution to H460 cell were 27.68 and 15.19 nmol/L. CONCLUSIONS:H6/mPEG2000-PCL4000 micelles are prepared successfully and show in vitro anti-tumor effects.
9.Intraoperative complications of endovascular management for intracranial aneurysms

Objective To summarize the clinical experience of endovascular intervention for intra-cranial aneurysms, especially in the respect of the technique and management of intra-operative complications. Method The clinical data of 60 patients with intra-cranial aneurysms treated with endovascular intervention in the past 3 years were analyzed. The relevant literature especially with regard to the practical technique described was reviewed so as to potentiall minimize and properly manage the intra-operative complications. Results A total of 69 sacciform aneurysms and one dissecting aneurysm located at left vertebral artery (VA) were detected by using digital subtraction angiography (DSA) in 60 patients. There were 65 saccular aneurysms obliterated with constructive approach, and five of them treated with stent-assisted technique and four of them treated with ballon-assisted technique. The VA dissecting aneurysm was obliterated with coils by deconstructive approach with complete occlusion of its parent vessel. There were a total of 53 complete occlusions of aneurysms accounted for 76.81% of 69 sacciform aneurysms in 51 patients ( 85 % ) and eight subtotal occlusion of aneurysms (95 % ~ 99 % occlusion) accounted for 13.56% of total sacciform aneurysms in five patients (8.47%) and four incomplete occlusion of aneurysms ( < 95% occlusion) accounted for 6.78% of total sacciform aneurysms in three patients (5.08%), and one was failure in operation. The rupture of aneurysms occurred during operation in4 patients (6.78%). Two senile patients suffered from intra-opeartive symptomatic thromboembolisn. One patient had stent shifting and spring coil dislocated and moved into the M3 segment of the ipsilateral MCA. Vasospasm occurred in 15 patients during operation, and most of them received endovascular intervention 3 days after the initial ictus. The rate of good recovery was 93.3% at discharge from hospital (the modified Rankin Scales, mRS< 2). There were 55 patients followed up for up 24 months after discharge, and excellent recovery rate was found in 51 patients ( mRS < 2), and 3 died.During the follow-up period, no aneurismal recanalization or rupture was noticed in all patients. Conclusions The endovascular intervention is a safe and effective approach to the intra = cranial aneurysms. Advances in the skill of technique and proper management will decrease the complications during operation and improve the prognosis of patients.
10.CT manifestations of patients with swine-origin influenza A H1N1 QI

Objective To explore the manifestations of chest multi-slice spiral CT in patients with initial infection of swine-origin influenza A (H1N1) virus (S-OIV). Methods The chest multi-slices spirals CT images of 19 firstly diagnosed patients with swine-origin influenza A (H1N1) in our institution were retrospectively studied. CT manifestations were evaluated by three experienced radiologists. Location, appearance of lung abnormalities, abnormal distribution, pleural effusion and others (pericadiaum, lymphadenopathy and pleural thickening) were observed and quantitatively analyzed. The correlation of ground-glass and consolidation CT scores with the fever time was studied. Results The abnormal CT findings were observed bilaterally in 18 of 19 subjects including ground-glass (n= 3), consolidation (n=3 ), consolidation accompanied with ground-glass (n=12). Most of these lesions were distributed diffusively (n=14) while the others located in the middle and low lobes (n= 4). Unilateral (n=3) or bilateral (n=2) pleural effusion were observed. Lymphadenopathy (n=2), effusion of pericadium (n=1), pleural thickening (n=1) and cardiac enlargement (n=2) were also found in patients with H1N1. CT scores of ground-glass were 4. 25(n=2),3.75 (n=1),2.25(n=1),1.75(n=1),1.00(n=6),0.75(n=2), 0.50(n=2),0(n=4).CT scores of consolidation were4.25(n=1),4.00(n=1),3.75 (n=1), 2.75(n=1),1.25(n=3),1.00(n=2),0.75(n=2),0.50(n=1),0.25(n=3),0(n=4). CT scores of ground-glass were significantly correlated with the fever time (r= 0.776, P < 0.01), CT scores of consolidation had no correlation with the fever time(r=0.322,P > 0.01). Conclusions The most common CT findings in patients with S-OIV infection are diffuse distribution of bilateral ground-glass opacities with or without associated focal or multifocal areas of consolidation. The increasing of ground-glass's range could be the marker of progression of H1N1 pulmonary infection at initial stage.
