1.Protective effects of sea buckthorn polysaccharide extracts on LPS/D-GalN-induced liver injury in mice and modulation on TLR4,SOCS3 expression

Objective:To study the protective effects of sea buckthorn polysaccharide extracts on lipopolysaccharide( LPS)/D-galactosa mine ( D-GalN )-induced liver injury in mice and investigate the regulation on hepatic TLR4 and SOCS3 expression.Methods:C57BL/6 male mice were randomly divided into six groups:control group,model group,dexamethasone positive control group, low, medium and high dose group of sea buckthorn polysaccharide.Mice in the sea buckthorn polysaccharides low, medium and high dose group were gavaged with 50, 100 and 200 mg/kg sea buckthorn polysaccharide extracts for 14 days respectively.Acute liver injury model were established by intraperitoneal injection of LPS(10 μg/kg) and D-GalN (700 mg/kg).The mice in the dexamethasone positive control group were intraperitoneally injected with dexamethasone (10 mg/kg) before model estab-lishment.Serum and liver samples were collected after model establishment for 4 h .Serum levels of ALT and AST were detected.Histological changes of liver tissue were observed by HE staining.Hepatic expression of TLR4 and SOCS3 was detected by Western blot.Results:Sea buckthorn polysaccharide significantly inhibited LPS/D-GalN-induced elevation in serum levels of ALT and AST.It also alleviated liver cell injury and inflammatory infiltration.Western blot results showed that sea buckthorn polysaccharide inhibited LPS/D-GalN-induced TLR4 expression.SOCS3 expression was not dramatically influenced by sea buckthorn polysaccharide supplementation.Conclusion:Sea buckthorn polysaccharide protects against LPS/D-GalN-induced liver injury.This protective effects may be achieved by inhibiting the expression of TLR4 but not associated with modulation on SOCS3 expression.
2.Inhibitory effects of seabuckthorn polysaccharide on oxidative stress in mice with actue liver injury and modulatory effect on BCL-2/Bax and PPAR-γexpression

Objective:To explore the inhibitory effects of seabuckthorn polysaccharide on hepatic oxidative stress in a mice model of acute liver injury induced by intraperitoneal injection of LPS and D -GalN and detect the expression on hepatic BCL-2/Bax and PPAR-γ.Methods: C57BL/6 male mice were randomly divided into six groups:control group ( CTRL), model group ( L/G), dexamethasone positive control group ( DXM ) , low ( SPL ) , medium ( SPM ) and high dose group ( SPH ) of seabuckthorn polysaccharide.Mice in the SPL,SPM and SPH group were gavaged with 50,100 and 200 mg/kg seabuckthorn polysaccharide for 14 days respectively.Acute liver injury model were established by intraperitoneal injection of LPS (10 μg/kg) and D-GalN (700 mg/kg) .Serum and liver samples were collected 4 h after model establishment .Serum levels of ALT and AST and the content of MDA were de-tected.Hepatic expression of SOD 2 BCL-2 and Bax was determined by Western blot and the expression of PPAR-γwas detected by im-munohistochemistry .Results:ALT and AST levels significantly increased in the model group and decreased dose-dependently after pre-treatment with seabuckthorn polysaccharide .The level of MDA in the model group increased significantly as compared with the control group and decreased in seabuckthorn polysaccharide groups ,while the level of SOD 2 decreased in the model group and recovered in sea-buckthorn polysaccharide groups .The expression of Bax decreased after pretreatment with seabuckthorn polysaccharide .There was no obvious effect on BCL-2 expression after sea buckthorn polysaccharide supplementation .The expression of PPAR-γreduced in the sea-buckthorn polysaccharide group as compared with the model group .Conclusion:Seabuckthorn polysaccharide protects against LPS /D-GalN-induced liver injury.The effect is associated with an upregulation of SOD 2 and downregulation of Bax .
3.Rab5a promotes expression of pro-inflammatory cytokines and type Ⅰ IFN in CpG induced macrophages

Objective:Using the macrophage cell lines RAW264.7 stably expressing Rab5a and its dominant negative mutant Rab5aN133I as models to analyze the effect and the mechanism of Rab 5a,Rab5aN133I on CpG-induced production of pro-inflammatory cytokines and type Ⅰ IFN.Methods: The eukaryotic expression vectors of Rab5a and Rab5aN133I were transfected into RAW264.7 cells by liposome,and screened with G418.The G418-resistant colonies were obtained and amplified.The transformed cell lines were i-dentified by RT-PCR,Real time-PCR and Western blot.The production of cytokines were measured after transformed cell lines of Rab5a and Rab5aN133I was stimulation with CpG for 8 h.Results: Rab5a expression in transfected cells was significantly higher than the control cell group (P<0.05).Overexpression of Rab5a significantly promoted the production of TNF -α,IL1-β(P<0.01) and IFN-β( P<0.05) in CpG stimulated RAW264.7.The production of cytokines was restored in Rab 5aN133I transfected cell line.Conclusion:Rab 5a promotes CpG-induced pro-inflammatory cytokines and typeⅠIFN in macrophages,it may be act as a positive regulator in TLR9 signaling pathway.
4.The value of using B-type natriuretic peptide and D-dimer in preliminary recognition of cardioembolic stroke patients

Objective To determine the value of using B-type natriuretic peptide (BNP) and D-dimer in preliminary recognition of cardioembolic stroke patients.Methods A mutilple-center study was conducted in Foshan Hospital of traditional Chinese Medicine (TCM) and its affiliated hospitals from July 2015 to July 2016.In the emergency departments (EDs),emergency physicians prospectively assessed consecutive adult patients with acute cardioembolic stroke and measured plasma BNP by POCT platform on admission,then followed up.Stroke neurologists evaluated patients' functional outcome at hospital discharge and also made discharge diagnosis and stroke etiologic subtypes according to the TOAST criteria.Results In this study,290 acute ischemic stroke patients met the study criteria [mean age (68.41 ± 12.06) years;53.8% female].Of the enrolled patients,28.3% were diagnosed with LAA at discharge,17.9% with CE,42.8% with SAO,11.0% with SOE or SUE.And the mean BNP concentration was significantly higher in the CE group than that in other three subtypes (P < 0.001).After adjustment for multiple clinical predictors like gender,age,coronary artery disease,atrial fibrillation and renal function,BNP and D-dimer were associated with CE [BNP OR:1.044 (95% CI 1.025,1.064),P < 0.001;D-dimer OR:1.511(95% CI 1.020,2.238),P =0.039,respectively].Conclusion Through POCT technique in the EDs,cardioembolic stroke patients can be differentiated from other TOAST subtypes.BNP with/without D-dimer has good but different corresponding diagnostic performance in preliminary recognition of cardioembolic stroke patients.
5.Application of epinephrine saline rinsing solution in hemostasis of cleft palate repair

Objective:To investigate the hemostatic effect of epinephrine saline rinse solution in cleft palate repair.Methods:A total of 100 children who underwent cleft palate repair in the operating room of the Second Affiliated Hospital of Shantou University Medical College from 2018 to 2020 were selected, Among them, 51 were males and 49 females, aged from 6 months to 12 years, with an average (2.5±2.49) years. The patients were divided into two groups according to whether to use epinephrine saline flushing fluid: in group A, 43 cases were treated with adrenaline saline irrigation solution to wash the incision during the operation; gauze soaked in rinse solution was used to fill the oral cavity before endotracheal intubation and extubation after operation; in B group of 57 cases, no intraoperative rinses were used. The intraoperative blood loss and operation duration were compared between the two groups.Results:Intraoperative use in group A after adrenaline saline rinses showed that the intraoperative blood loss of children (16.23±4.88) ml was significantly lower than that of group B (19.26±4.13) ml. The duration of operation in group A (109.79±40.27) min was significantly shorter than that in group B (137.16±50.47) min, The difference was statistically significant ( t=2.92, P<0.05). Conclusions:The incision is rinsed with epinephrine saline solution during cleft palate repair. In addition, before endotracheal intubation and extubation after operation, gauze soaked in rinsing solution is used to fill the oral cavity, which could significantly reduce the amount of bleeding and shorten the operation time.
6.Analysis of slef-perceived burden and its influencing factors among non-dialysis chronic kidney disease patients

Objective To describe the level of slef-perceived burden among non-dialysis chronic kidney disease(CKD) patients and analyze its influencing factors .Methods Adopting sampling survey method , 148 cases of non-dialysis CKD patients were included .General condition questionnaire , slef-perceived burden scale (SPBS) and self-rating anxiety scale (SAS) were employed to investigate.Results The score of SPBS was (3.08 ±1.01) in non-dialysis patients with CKD, which was in mild-to-moderate levels, item of economic had the highest scores.In addition, mono factor analysis and multi-factor analysis showed the course of disease (χ2 =38.69,P<0.001), number of complications (χ2 =4.30,P=0.04), and anxiety state (χ2 =4.61,P=0.03) were three of the major influences on self-perceived burden .Conclusions Non-dialysis CKD patients′self-perceived burden is in mild-to-moderate levels, economic burden generally heavier .The course of disease, number of complications and anxiety state constitute the main influencing factors .For this reason , medical personnel should strengthen the attention to these aspects to lighten the patients psychological burden and delay the process of kidney disease .
7.Retrospective analysis of 350 cases with dissection of lymph nodes posterior to right recurrent laryngeal nerve in endoscopic thyroidectomy through gasless axillary posterior approach

Objective:To evaluated the safety and feasibility of dissection of lymph nodes posterior to right recurrent laryngeal nerve (ⅥB compartment) in endoscopic thyroidectomy through gasless axillary posterior approach.Methods:A total of 350 cases with right lobe papillary thyroid carcinoma (PTC) who underwent endoscopic lobectomy, isthmusectomy and central compartment neck dissection via gasless axillary posterior approach based at the Department of General Surgery, Nanfang Hospital, Southern Medical University from June 2020 to December 2022 were retrospectively analyzed. Summarize the clinical, pathological characteristics, and postoperative complications of the patients. SPSS 25.0 was used for statistical analysis of the data.Results:All 350 patients underwent endoscopic surgery successfully, with no conversion to open surgery. There were 303 females and 47 males, with an average age of (36.3±9.2) years. Of those, 287 patients were in pT1a stage, 62 in pT1b stage, and one patient in pT2 stage. There was no T3 or T4 stage patient. The mean numbers of yielded lymph nodes in right central compartment and ⅥB compartment were 8.11±4.65 (range, 1-31) and 2.62±1.86 (range, 1-12), respectively. ⅥB compartment metastasis was detected in 52 (14.86%) of 350 patients. The incidence of transient recurrent laryngeal nerve injury was 0.86%(3/350). Postoperative hematoma occurred in three patients (0.86%).Conclusion:The dissection of ⅥB compartment in endoscopic thyroidectomy through gasless axillary posterior approach is safe and feasible in selected PTC patients

In clinical dentistry,addressing unique conditions such as tilted,elongated,and torsion teeth during prepa-ration can be effectively managed through digital tooth morphology design.The production of a multi-stage 3D-printed guide offered a more efficient and accurate solution.This article presented a case of significant inclination,elongation,and torsion in the maxillary and mandibular canines that were successfully treated using crown restoration modification.A crown preparation guide was fabricated based on the final form design of the restoration using the target restorative space analysis technique to ensure precise tooth preparation.A tooth preparation guide was also designed and utilized fur-ther to enhance accuracy and efficiency during complex tooth preparation.The combined application of these multi-stage guides demonstrated promising clinical prospects.
9.Self-management types and characteristics of patients with chronic kidney disease: A cluster analysis

Objective? To explore the potential types of patients with chronic kidney disease (CKD) based on their self-management ability and hope level. Methods? A total of 133 patients who were selected as subjects from the follow-up cases in the Chronic Kidney Disease Management Center of Xinqiao Hospital Army Medical University from January to June 2017 using the convenient sampling method. CKD Self-Management Instrument and Herth Hope Index were used to assess the subjects and cluster analysis was used to identify patient types. Comparison of general information was conducted across different patient types. Results? A total of 121 valid surveys were collected. The 121 cases of CKD patients can be characterized into 3 types: Type I patients have high self-management ability with a high hope level; Type II patients have medium self-management ability with a high hope level; Type III patients have low self-management ability with a medium hope level. There were statistical differences in age, educational level, medical payment method, and CKD staging across patients of the three different types (P< 0.05). Conclusions? There are 3 types of CKD patients in this study. If health professionals can effectively identify patient types and offer individualized care accordingly, the health condition of CKD patients could greatly improve.
10.Da Chaihutang for Treatment of Sepsis with Yang Syndrome:A Randomized Controlled Trial

ObjectiveTo explore the clinical efficacy and safety of Da Chaihutang (DCH) for the treatment of sepsis with Yang syndrome. MethodsA total of 70 patients suffering from sepsis with Yang syndrome were randomly divided into an observation group and a control group, with 35 cases in each group. They both received standard Western medicine treatment. The observation group was additionally given a dose of DCH, which was boiled into 100 mL and taken twice. The control group was additionally given an equal volume and dosage of warm water. The intervention lasted for three days. The 28-day all-cause mortality and the changes in the following indicators before and after intervention were compared between the two groups, including sequential organ failure assessment (SOFA), acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score,white blood cell (WBC),the percentage of neutrophils (NEU%),C-reactive protein (CRP),procalcitonin (PCT),alanine transaminase (ALT),aspartate transaminase (AST),total bilirubin (TBil),creatinine (Cr),blood urea nitrogen (BUN),acute gastrointestinal injury (AGI) grade,gastrointestinal dysfunction score (GDS),serum intestinal fatty acid-binding protein (iFABP), citrulline (CR),platelet (PLT),prothrombin time(PT),activated partial thromboplastin time (APTT),fibrinogen (Fib),international normalized ratio (INR),and D-dimer (D-D). ResultsThere was no significant difference between the two groups regarding 28-day all-cause mortality. After the intervention,SOFA,WBC,PCT,and Cr were significantly decreased, and PLT was significantly increased in the control group (P<0.05). SOFA,APACHE Ⅱ,NEU%,CRP,PCT,ALT,AST,Cr,BUN,AGI grade,GDS,and serum iFABP and CR were significantly improved in the observation group (P<0.05). After the intervention,APACHE Ⅱ,PCT,AGI grade,GDS,and serum iFABP in the observation group were significantly lower than those in the control group ,while CR and PLT were higher (P<0.05,P<0.01). There were significant differences regarding the gap of SOFA,APACHE Ⅱ,AST,TBil,AGI grade,GDS,iFABP,CR, and PLT between the two groups (P<0.05,P<0.01). There were slight differences regarding PT,APTT,Fib,INR,and D-D between the two groups,which were in the clinical normal range. ConclusionOn the basis of Western medicine, DCH helped to reduce sepsis severity and improved multiple organ dysfunction with high clinical efficacy and safety, but further research on its impact on the prognosis of patients with sepsis is still required.
