Objective:To forecast Urban Employees' Basic Medical Insurance ( UEBMI) in the next 85 years based on population size, age and gender distribution, and to calculate its sustainability gap. Methods:According to the population policy of Beijing and economic development of the region;based on the data of the sixth census in Bei-jing, the estimation model was constructed, and its parameters were defined and selected. Based on the period of 2010, the future of UEBMI in the next 85 year was predicted based on the population size, age and gender distribu-tion. Results:The population of UEBMI in Beijing will reach the maximum value in 2020, and then the downward trend will increase. The aging rate of the insured population will accelerate year after year and will reach the peak in the middle of this 21st century. The sustainability gap is predicted to reach at least 9541. 35 Billion dollars, and the Beijing municipal government is expected to level an average of at least 44 . 693 billion yuan per year to set aside the financial funds in order to fill the gap, so sustainable development of the fund is ensured. Conclusion:From the point of view of the insured population, there is a huge financial sustainability risk in the future of UEBMI in Beijing.

Objective To investigate the expression of serum interleukin-2(IL-2),tumor necrosis factor-α(TNF-α)in the pa-tients with pulmonary tuberculosis and diabetes,and prelininary explore its clinical significance.Methods Experiment were divided into control group(health check-up outpatient,n=10),pulmonary tuberculosis group(n=20),diabetes group(n=20),tuberculosis combine diabetes group(n=20),tuberculosis combine diabetes group was divided into improved group,diabetes group and deterio-rated group after 2 months with therapy according to disease outcome,ELISA assay was used to measured the level of serum IL-2 and TNF-α.Results Before treatment:serum IL-2 levels of pulmonary tuberculosis group,diabetes group and pulmonary tuberculo-sis combine diabetes group were reduced (P<0.05),and the pulmonary tuberculosis group was lower than the pulmonary tubercu-losis group and diabetes group (P<0.05);serum TNF-αlevels of pulmonary tuberculosis combine diabetes group,pulmonary tu-berculosis group and diabetes group increased (P<0.05),and that of pulmonary tuberculosis combine diabetes group was higher than that of diabetes group (P<0.05).After treatment:the level of serum IL-2 increased and TNF-αsignificantly decreased in pul-monary tuberculosis combine diabetes improved group (P<0.05),the deterioration group had no change(P>0.05).Conclusion IL-2 and TNF-αplay a role in the immune pathogenesis of tuberculosis combine diabetes,which dynamic changes can reflect the progression and outcome of pulmonary tuberculosis combine diabetes.

Objective To construct the vector pcDNA3.1 containing VEGF165 gene and examine of pcDNA3.1-VEGF165 vector to the angiopoiesis, composition of collagen in rabbit bone defect model. Methods Extract total RNA from the rabbit tissue. Prepare cDNA by inverse transcription and clone the gene by PCR. Clone plasmid pMD18-T/VEGF165 combined with pcDNA3.1 to reconstruct pcDNA3.1- VEGF165. 28 New Zealand white rabbits weighted (2.0?0.130) kg were made bone defect model for 10 mm length in the bilateral radii. Cut down the skin, resect the bone of 10 mm in length in the middle of radius. The pcDNA3.1-VEGF165 0.2 ml (200 ng) plasmid was injected at one defect side randomly. The defect in the other side was served as control group, and injected with absorbable gelatin sponge and sodium chloride 0.2 ml. After the examination by X-ray the local specimens were obtained at 1,2,4,6,8,and 12 weeks respectively. The expression of collagen type Ⅰ and Ⅲ was examined at 4, 12 weeks by immunohistochemical staining techniques. Results The pcDNA3.1-VEGF165 vector was constructed successfully. The roentgenography: there was no difference between the two sides after 1 week operation; 2 weeks after the operation, there were some callus in the experimental group; there was nothing in the control group. After 4 weeks, there were much callus, synostosis and others in the experimental group, all of these were late in the control group. Two groups were healed after 12 weeks, the bone density was lower in the control group. Inflammatory cell infiltrate, cellular interstitialis, fibroblast, collagen and osteoblast were no difference between the two groups only at the first week, but the density of angiogenesis was much more in the experimental group at the following times. Expression of collagen type Ⅰ, Ⅲ were more intensity in the experimental group than the control one at 4,12 weeks. Conclusion Transcription to local bone cells at defect position with pcDNA3.1-VEGF165 plasmid can enhance quantity of the angiopoiesis, extra cellular matrix and the healing of bone defect.

Objective To explore the clinical diagnositic code(CDC)in ovarian endometrial cysts (OEC).Methods 218 cases of pelvic masses were investigated by use of symptoms,signs,B mode sonography and CA125.172 cases were diagnosed with OEC out of 218 cases,While 46 cases were ruled out by combination exploratory laparotomy and laparoscopy.Meanwhile we compared it with revisional american fertility society(r-AVS)including diagnosis and staging.Results The operation results of 218 cases were compared with the results of CDC and its Diagnositic Sensitiveness;Specificness;Positive predictive value;Negative predictive value;Diagnositic accordance rate as well as Diagnositic index of CDC were 96.4%,79.6%.93.0%,90.0%,91.7% and 73.4% respectively(χ2=127.818,P＜0.001).Further more,172 cases of OEC which diagnosed with CDC were further subdivided into three stages as 1,2,3 according to its score,mild;moderate;heavy and this stage category were compared with the category of AFS(Ⅰ-Ⅱ,Ⅲ,Ⅳ),the Diagnositic accordance rate of two categories were 95.6%,95.6% and 83.0% respectively,No significant difference were found in Var.mitis and medium-sized(χ2=0,P＞0.05).but the Medium-sized and Gravis tape was significance(χ2=4.472,P＜0.05).Conclusion CDC has a better coincidence with operation in the diagnosis of OEC which can be used in the non-traumatic diagnosis and staging of OEC in clinic.

Objective To investigate the early comprehensive intervention on neonatal hypoxic ischemic encephalopathy (HIE) effects and the biological mechanisms.Methods 120 cases of HIE were randomly divided into intervention group and control group.Under the basis of drug treatment,the intervention group were given the different perceptual stimuli,audiovisual stimulation,active and passive limb training early comprehensive intervention,the control group only to normal drug treatment.The mental development index(MDI) and psychomotor develo pmental index (PDI) were applied after 3,6,12,and 18 months.S100B protein levels were detected using enzyme immunoassay.Results MDI score in the intervention group with HIE,from the beginning of the 12 month,was higher than that in the control group,there was significant differences (12th month:(96.6±9.4)vs (86.8±9.5),P<0.01).PDI score from the beginning of the 6 month,was higher in the intervention group with HIE than that in the control group,there were significant differences (6th month:(91.6±9.2)vs (82.2±11.0),P<0.01).Serum S100B protein level has a tendency to fall for all HIE patients with the old growth.Born in two groups of serum S100B protein levels were higher in neonate period,but there was no difference(P>0.05).From the beginning of the 3rd month,serum S100B protein levels in the intervention group were reduced significandy,compared with the control group at each time point level(3rd month:(1.29±0.15)ng/L vs(1.40±0.14)ns/L;6th month:(1.06±0.21)ng/L vs(1.26±0.22)ng/L,P<0.01).Conclusion Early intervention can improve the mental and motor development of children with HIE,and serum S100B protein is involved in the mechanisms.

Objective To assess the human β-defensin-2 ( hBD2 ) gene therapeutic efficacy in a rat urinary tract infection (UTI) model via intravesical liposome-mediated gene transfer.Methods Fifty-six female Wistar rats (class SPF) weighting 1 80 -220 were randomly divided into an experiment group and a control group ( each n =28 ).The animals were administered either 250 μl recombinant pCAGG-hBD2 or control vector pCAGG intravesically.After 48 h,rats in both groups were infected via intravesical inoculation with 200 μl of the bacterial suspension of UTI89 ( 1 × 108 CFU/ml).The rats were sacrificed at 4,24,48,and 72 h post-inoculation.The bladders were aseptically removed and bisected.One half was fixed in neutral buffered formalin for histological analysis.The remaining half-bladders were homogenized and titered for surviving bacteria.The clean-catch urine sample from each rat was collected in sterile before they were killed for bacterial titers determined and WBC counted.Results Numbers of bacterial colony-forming unit in urine and bladders from hBD2 gene treated UTI rats were significantly lower than those from the control vector administered UTI rats at 24,36,and 72 h after infection ( P ＜ 0.05 ).The amount of WBC in urine was significantly less in the defensin group than in the control group.In addition,in vivo expression of hBD2 could reduce mucosa damage,interstitium edema and inflammatory cells infiltration in UTI animals.Conclusions The successful inhibition of UTI progression could be obtained with hBD2 gene therapy.Recombinant beta-defensin-2 could kill UTI89 in vivo and suppress the subsequent infection-induced inflammatory responses.

BACKGROUND: Hydroxyapatite has favorable biocompatibility, can protect pulp tissue and promote the formation of osteoid dentin and the concrescence of pulp tissue. However, hydroxyapatite has no capability of anti-infection. Increasing scholars deem that antibacterials should be added with the hydroxyapatite during the treatment of vital pulp conservation to elevate curative effects.OBJECTIVE: To observe the curative effects of self-solidifying hydroxyapatite/norvancomycin composite as pulp capping material and to make a comparison with calcium hydroxide.DESIGN, TIME AND SETTING: The present randomized controlled observation experiment was performed at the General Dental Clinic, the Ninth People's Hospital, Shanghai Jiao Tong University Medical College between January 2004 and June 2005.PARTICIPANTS AND MATERIALS: A total of 60 patients (60 teeth) that suffered from deep caries or pulp exposure, could return visit on time, and agreed to sign informed consents were selected for this study. These patients comprised of 28 males and 32 females, aged 9-40 years. Diagnosis criteria: Teeth with food impaction pain and/or caloric stimulation pain which can relieve after stimulation removal; in addition, with no spontaneous pain, radiating pain, and hypnalgia. Self-solidifying hydroxyapatite/2.5% norvancomycin composite were prepared in the Department of Dental Material, Shanghai Second Hospital. Self-hardening calcium hydroxide was provided by Dentsply Company, USA.METHODS: All 60 patients were randomly divided into an experimental group and a control group, with 30 patients (30 teeth) per group. Self-solidifying hydroxyapatite/2.5% norvancomycin composite and self-hardening calcium hydroxide were applied as pulp capping agents in the experimental and control groups, respectively. Pulpal tissue reactions were assessed after 8 weeks. Teeth with normal response were restored permanently, and the others were given root canal treatment.MAIN OUTCOME MEASURES: Pulpal tissue reaction of patients.RESULTS: In the experimental group, one case showed vague pain after 1-week pulp capping and received root canal therapy, with success rate of 97%. In the control group, two cases presented with vague pain after 3-day and 1-week pulp capping, respectively, and also received root canal therapy, with success rate of 93%.CONCLUSION: Self-solidifying hydroxyapatite/norvancomycin composite acquires a high success rate of pulp capping, with curative effects similar to calcium hydroxide, it is a good pulp capping agent for vital pulp preservation.

Objective To investigate the changes of collagen types Ⅰ and Ⅲ, the effects of Oleum Curcumae Aromaticae oil injection on injuried rat arteries and collagen types Ⅰ and Ⅲ after balloon angioplasty Methods The left common carotid artery in rats was denuded with a balloon catheter The rats were killed 14 days after injury Cross sections stained with picrosirius red from the injuried segments were processed for distribution, arrangememt, amount, proportion of collagen type Ⅰ and type Ⅲ by polarizing microscope, and those stained with hematoxylin eosin for histological and morphological changes studied by computer assisted picture analysis system Results Fourteen days after balloon injury, collagen type Ⅰ and type Ⅲ of the model group were found mainly in adventitia, much more, thicker, brighter and more disordered than those of the sham operation group Compared with the model group, collagen type Ⅰ and type Ⅲ of the E Zhu You group were found mainly in adventitia, less, thinner, dimmer and more orderly Lumen area, EEL perimeter, IEL perimeter of the model group were decreased and the areas of collagen types Ⅰ and Ⅲ increased more significantly than those of the sham operation group Compared with the model group, maximal intimal thickness, neointima area, the areas of collagen type Ⅰ and Ⅲ of the E Zhu You group were decreased significantly which was not different from those of the sham operation group Lumen area, EEL perimeter and IEL perimeter were increased markedly up to those of the sham operation group The ratio of collagen subtypes Ⅰ/Ⅲ was not significantly different between the two of the three groups Conclusion The accumulation and derangement of collagen types Ⅰ and Ⅲ may be more important in restenosis than the ratio of collagen subtypes Ⅰ/Ⅲ We concluded that Oleum Curcumae Aromaticae oil injection inhibits neointimal formation and vascular remodeling after arterial injury by mechanisms involving collagen types Ⅰ and Ⅲ production and their arrangement

Objective To investigate the changes in the levels of motilin in the gastric body in a rat model of incisional pain.Methods Eighty-four healthy male Sprague-Dawley rats,weighing 180-220 g,were randomized into 2 groups (n =42 each):normal saline group (NS group) and incisional pain group (P group).In group P an 1 cm long incision was made in the plantar surface of right hindpaw under sevotlurane anesthesia.Six rats were chosen from each group and paw withdrawal threshold to mechanical stimulation (MWT) and paw withdrawal latency to nociceptive thermal stimulation (TWL) were measured at 24 h before operation (To) and 1,6,24,48 and 72 h after operation (T1-5).Six rats were chosen from each group at T0-5 and sacrificed and the gastric mucosal tissues were prepared for measurement of motilin levels by ELISA.Results Compared with group NS,MWT,TWL and motilin levels were significantly decreased at T1-4 (P ＜ 0.05) and no significant change was found at T0 and T5 in P group (P ＞ 0.05).Compared with the baseline value at To,MWT,TWL and motilin levels were significantly decreased at T1-4,and no significant change was found at T5 in group P (P ＞ 0.05).MWT,TWL and motilin levels were significantly higher at T3-T5 than at T1 in group P (P ＜ 0.05).MWT,TWL and motilin levels were increased gradually at T3-T5 in P group (P ＜ 0.05).The motilin levels were positively correlated with MWT and TWL (r =0.9597 and 0.9231,respectively,P ＜ 0.01) in group P.Conclusion The levels of motilin in the gastric body is significantly decreased in a rat model of incisional pain and the decreasing range is positively corre lated with the degree of incisional pain.

Objective To investigate effects of intrathecal injection of morphine and fentanyl combined with low-dose naloxone on the pain behavior and the expression of blood motilin (MTL) in the rat model of incisional pain.Meth?ods A total of 72 healthy male Sprague-Dawley rats (weight 180-220 g), successfully intrathecally catheterized, were ran?domly divided into 6 groups (n=12 ):normal saline group (NS group), incisional pain group (P group), morphine (5μg/kg)+fentanyl (0.25μg/kg) group (MFP group), morphine+fentanyl+naloxone (0.2 ng/kg, 1 ng/kg, 5 ng/kg) group (MFPN1, MF?PN2 and MFPN3 groups). All groups except NS group were made the model of incisional pain on the right plantar surface. At 24-hours before intrathecal cathetherization (T0), 24-hours before modelling (T1), 1-hours (T2), 3-hours (T3) , 6-hours (T4), 24-hours (T5) , 48-hours (T6) and 72-hours (T7) after modelling respectively, paw withdrawal mechanical threshold (PWMT) and paw withdrawal thermal latency (PWTL) were detected in right hind paw in 6 rats of each group. The other 6 rats in each group were sacrificed 6-hour after operation. The plasma expression of motilin was detected by ELISA. Re?sults Compared with NS group, the PWMT was not significantly different in all time points in MFPN2 group. The values of PWTL were significantly longer at T2 and T5 in MFPN2 group than those of NS group (P<0.05). The PWMT and the PWTL were significantly decreased at T2, T3 and T4 in P and MFPN3 groups than those of NS group (P<0.05).Compared with P group, the PWMT at T6, and PWTL at T3 and T4 were significantly decreased in MFPN3 group (P<0.05). The MTL at T6 was significantly decreased in P, MFP, MFPN1 and MFPN3 groups compared with that of NS group (P<0.05). There was no significant difference in MTL between MFPN2 group and NS group (P>0.05).Conclusion In the rat model of incision?al pain, intrathecal injection of naloxone at 1 ng/kg can inhibit the down-regulation of blood motillin caused by morphine and fentanyl, and which can up-regulate the PWTL, enhancing the analgesic effects of opioids.
