1.Clinical study of sunitinib as first line treatment in 40 patients with metastatic renal cell carcinoma

Objective To evaluate the efficacy and safety of sunitinib as first line treatment in patients with metastatic renal cell carcinoma (mRCC).Methods A total of 40 patients with mRCC were treated with sunitinib between January 2009 and December 2011,including 27 males and 13 females.The median age was 59 (31-75) years old.All patients received a pathologic diagnosis of renal clear cell carcinomas with radical nephrectomy.All cases were treated with first line therapy.Sunitinib monotherapy was administered in repeated 6-week cycles of daily oral therapy for 4 weeks,followed by 2 weeks off in 34 patients,while another 6 patients received 37.5 mg/d continuously until disease progression or unacceptable toxicities occurred.Overall response rate and safety were evaluated.Results The median follow up was 14 months (range 4-36 months).All patients could be evaluated the efficacy.17.5 % (7/40) patients achieved partial responses,77.5 % (31/40) atients were in stable and 2 (5.0 %) patients were with disease progression including 1 death.The objective response rate was 17.5 % (7/40),and the disease control rate was 95.0 % (38/40).The most common side effects included fatigue 12 cases (30.0 %),alopecia 3 cases (7.5 %),thrombocytopenia 23 cases (57.5 %),thyroid dysfunction 26 cases (65.0 %),neutropenia 20 cases (50.0 %),hypertension 18 cases (45.0 %),hand-foot syndrome 17 cases (42.5 %) and diarrhea 4 cases (10.0 %).The major grade 3 adverse events included thrombocytopenia 2 cases (5.0 %) and edema 1 case (2.5 %).Most adverse events were ameliorated by treatment interruption.Conclusion Sunitinib has definitive efficacy for patients with mRCC with slight side effects.

Because of the particularity of the interventional therapy,that is,the interventional management covers a large scope in clinical application and involves the diagnosis and treatment of various diseases of multiple systems and organs,the clinical interventional practice has really brought an unprecedented challenge for the administration of the nursing care in the intervention ward.In our hospital,independent nursing group for the intervention ward was established two years ago.For the past two years,we have constantly groped and summarized the reasonable and effect administration of interventional nursing care.Pertinent administrative measures,such as nurse training,strengthening of communication with physicians and focusing on key links in nursing care and promptly finding out the weak points in clinical work,have effectively improved the quality of clinical nursing,in this way the clinical nursing practice has been integrated into the interventional therapy and the safe and high-quality nursing service has been provided to the patients.
3.Analysis of biological features and prognostic factors of mantal cell lymphoma

Objective To study the biological features and prognostic factors of mantle ceil lymphoma (MCL).Methods The clinical data of 39 cases of MCL were analyzed prospectively.The fellow-up informations were also studied.Age,sex,B symptoms,Ann-Arbor staging,bone marrow and lympho node biopsies were assessed.Serum lactate dehydrogenates(LDH) and β2-microglobulin (β2-MG) level,Cyclin D1 expression and immunophenotype were investigated.Results The median age of patients was 62 years old (range 43-80).The male-to-female ratio was 5.5:1,a clear predominante of male patients.Thirty-five (89.7 %)cases presented with advanced stage disease (Ann Arbor stage Ⅲ to Ⅳ) at initial diagnosis.CD20 was positive expressed in 39 patients,that all cases expressed B-cell markers.Majority of cases were positive for Cyclin D1 (84.7 %) and CD5 (64.1%).Conclusion MCL is a special type of B-cell originated non-Hodgkin lymphoma (NHL).The clinical data suggest considerable clinical heterogeneity and it has characteristics of both indolent and aggressive NHL that some patients show a chronic/indolent course,while others have a more fulminant course and short survival.LDH,β2-MG and Cyclin D1 arc the important prognostic factors.
4.Expression and clinical significance of POKemon and p14ARF in colorectal carcinoma cells lines

Objective To study the expression of POKemon and pl4ARF genes in colorectal cancer cell lines. Methods The expressions of POKemon and pl4ARF were detected in 5 human colorectal cancer cell lines by RT-PCR and immucytochemistry method. Results The expressions of POKemon were positive in cell lines of SW480, SW480/M5, SW620 and LOVO, and were negative in HCT116. The expression of pl4ARF was positive in HCT116 and negative in other cell lines. Conclusion POkemon is expressed positively in colorectal cancer cell lines and its function is negatively correlated with pl4ARF.
5.Study on Method of Expanding and Culturing Rat Mesenchymal Stem Cells in vitro and Its Biological Characteristics

[Objective] To establish a method to seperate rat mesenchymal stem cells(rMSCs) from rat marrow, culture rMSCs in vitro, and study its biological characteristics. [Methods] rMSCs were seperated from rat marrow with wall-sticking method and expanding cultured in vitro. To draw the growth curves of cells at 1th, 3th, 5th passage, detect the surface antigens with flow cytometry. To evaluate the effect of fetal calf serum with different concentrations to rMSCs. [Results] There were approximately (5~6)?105 cells after primary culture and over (2~3)?108 rMSCs at 5th passage. The appearance of rMSCs is like shape of spear and its growth curves are like shape of S. The rMSCs were positive for CD90 and negative for CD45. [Conclusions] Cells seperated and cultured by the method established in this experiment were rMSCs. They have the biological characteristics of rMSCs, and are good resources for study in tissue engineering.
6.Mechanism of exogenous nitric oxide in attenuating endotoxin-induced increase of rat pulmonary microvascular permeability

AIM and METHODS: The animal model of acute lung injury (ALI) caused by intratracheal instillation of lipopolysaccharides(LPS) in vivo and human peripheral blood polymorphonuclear neutrophil (PMN) in vitro were used to study the effects of sodium nitroprusside (SNP), nitric oxide (NO) donor, on LPS-induced PMN accumulation, microvascular permeability and PMN apoptosis. RESULTS: ① In vivo , PMN accumulation in lung, the protein content in bronchoalveolar lavage fluid (BALF) and the Evans blue dye and monastral blue dye extravasation in lung tissue of LPS group were markedly higher than those of both sham operation group and LPS+SNP group. ② In vitro, the apoptotic percentage of SNP group was much higher than that of control group, while compared with LPS group, SNP+LPS group has significantly higher apoptotic percentage. CONCLUSIONS: SNP intratracheal instillation attenuated LPS-induced microvascular permeability and alleviated ALI. PMN apoptosis induced by SNP may be one of the potential mechanisms underlying the decrease of PMN accumulation in lung tissue.
7.Comparison of the diagnostic value of MRI diffusion weighted imaging with ultrasound and mammography in the breast disease

Objective To explore the clinical value of diffusion weighted imaging (DWI) in the diagnosis of breast disease ,and compared with ultrasound and mammography .Methods The clinical data of 28 patients who were pathologically confirmed with breast cancer were retrospectively reviewed .The diagnostic accuracy of DWI ,mammogra-phy and ultrasound , the surface diffusion coefficients ( ADC value ) were statistically analyzed .Results ( 1 ) DWI scan:the diagnostic accuracy of malignant tumor was 90.9%(20/22),the ADC value of malignant tumor was (0.955 ±0.199) ×10 -3mm2/s;the diagnostic accuracy of benign lesions was 83.3%(5/6),the average ADC value of benign lesions was (1.660 ±0.339) ×10 -3 mm2/s,there were statistically significant differences ( t=2.371,P<0.05).(2)Mammography:the diagnostic accuracy of malignant tumor was 81.8%(18/22),the diagnostic accuracy of benign lesions was 33.3%(2/6).(3) Ultrasound:the diagnostic accuracy of malignant tumor was 86.4%(19/22),the diagnostic accuracy of benign lesions was 50.0%(3/6).The diagnostic accuracy of malignant tumor had no significant difference among three techniques (χ2 =0.752,P>0.05),but for the diagnostic accuracy of benign lesions,DWI was better than ultrasound and mammography ,the difference was statistically significant (χ2 =6.146, P<0.05).Conclusion For the diagnosis of malignant breast tumors and benign lesions ,DWI is better than ultra-sound and mammography ,which has high clinical application value .

9.Effect of interleukin - 10 on attenuating endotoxin - induced acute lung injury

AIM: To study the role of polymorphonuclear neutrophile(PMN) in lipopolysaccharide (LPS)- induced acute lung injury (ALI) and the protective effect of interleukin - 10(IL - 10) on ALI. METHODS: LPS alone (100 ?g) or LPS+ IL-10 (l ug) was instilled intratracheally into rats. PMN numbers, protein content and malondialdehyde (MDA) content in bronchoalveolar lavage fluid (BALF) were measured. Histological change of lung was also observed. RESULTS: LPS increased significantly PMN numbers, protein content and MDA content in BALF. Histological finding shows PMN accumulation in lung. IL - 10+LPS reduced remarkably PMN numbers ,pro- tein content and MDA content in BALF than those caused by LPS. PMN decreasing was also identified by light microscopy. CONCLUSION: LPS instilled intratracheally causes PMN accumulation in lung and ALI, while IL - 10 could alleviate ALI through reducing PMN accumulation.
10.Expression and methylation status of IGFBP-rP1 gene in laryngocarcinoma.

OBJECTIVE:
To investigate the relationship between the promoter methylation and protein expression of insulin-like growth factor binding protein-related protein 1(IGFBP-rP1) in laryngeal squamous cell carcinoma (LSCC).
METHOD:
Methylation specific PCR (MSP) approach and immunohistochemistry methods were used to examine the methylation status and protein expression of IGFBP-rP1 in 45 samples of laryngeal carcinoma and 18 samples of tissues beside cancer.
RESULT:
For the promoter site, methylation frequency of IGFBP-rP1 in tumor specimens (33.3%, 15/45) was significantly higher than that in corresponding normal tissues (5.6%, 1/18) (P < 0.05). The protein expression of IGFBP-rP1 in tumor tissues was significantly higher than that in corresponding normal tissues (P < 0.05) and was inversely correlated with its methylation status of promoter.
CONCLUSION
Epigenetic silencing of IGFBP-rP1 gene expression by promoter hypermethylation may play an important role in LSCC.
