1.Establish and optimize the model of experimental allergic encephalomyelitis in mice

Objective To explore the appropriate dosage of drugs inducing experimental allergic en cephalomyelitis (EAE) in mice,and evaluate the modified model mice.Methods Different doses of myelin oligodendrocyte glycoprotein (MOG35-55:200 μg,100 μg,50 μg,25 μg),together with different doses of inactivation of mycobacterium tuberculosis (H37RA:800 μg,250 μg,100 μg) and pertussis toxin (500 ng,200 ng),were used to induce the EAE model.After immunized,the clinical disease severity of EAE mice was measured by the standard EAE grading clinical score daily.The open field test was used to detect the locomotion of mice.The Western blot and immunofluorescence were used to detect the level of myelin basic proteins (MBP) in different brain regions of mice.Results Compared with the EAE mice induced with high-dose drugs,the mice with low-dose drugs (25 μg MOG35-55,100 μg H37RA,200 ng pertussis toxin) had low neu rological scores.And they displayed normal locomotion compared with the control mice (day 16:group EAE (8.885±0.772) cm/s vs control group (8.933±0.567) cm/s,P>0.05;day 31:group EAE (11.130±0.630) cm/s vs control group (10.670±0.959) cm/s,P>0.05;day 55:group EAE (7.686±0.428) cm/s vs control group (8.313±0.918) cm/s,P>0.05).Moreover,there was a significant decrease of MBP in the parahippocampal cortex (PHC) and fimbria-fornix of EAE mice induced with low-dose of drugs (PHC:group EAE (0.369±0.096) vs control group (1.000±0.163),P<0.05;fimbria-fomix:group EAE (0.494±0.071) vs control group (1.000±0.143),P<0.05).Conclusion The EAE mice induced with low-dose drugs(25 μg MOG35-55,100 μg H37RA,200 ng pertussis toxin) have low neurological scores,normal locomotion,and myelin impairment in the central neuronal system.And it can be used in the cognitive behavioral research of demyelination disease,such as multiple sclerosis.
2.X-ray Diagnosis of Solitary Globular Lesion of Lung

Objective To promote the understanding of X-ray examination in the diagnosis of solitary globular lesion of lung.Methods Of 402 cases with solitary globular lung lesion, 398 were proved by operation,pathology and 4 cases of lung abscess were proved by clinical therapy. Anteroposterior radiogrphs were taken in all cases;lateral radiogrphs were taken in 368 cases.Results Of these cases ,there were peripheral lung cancer 96,tuberculoma 33,lung echinococcosis 253, lung abscess 9, pulmonary metastatic carcinoma 3.There were no characteristic signs in the rare diseases. Therefore , it's difficult to diagnose these lesions. The rare diseases include inflammatory pseudotumor 2, pulmonary hamartoma 2 , lung sarcoma 2 , pneumoblastoma 1 and lung fibroma 1. Conclusion X-ray examination is important in the diagnosis of solitary globular lung lesion.
3.Influence of Low Molecular Weight Heparin on Perioperative Bleeding after Rear Operation of Thoracolumbar Spinal Bursting Fracture

Objective To study the influence of bleeding after using low molecular weight heparin (LMWH) to prevent deep vein thrombosis (DVT) in patients with thoracolumbar spinal bursting fracture and spinal cord injury operated by rear internal fixation. Methods 120 cases were divided into the observation group (64 cases) and control group (56 cases). The observation group accepted hypodermic injection of LMWH to prevent DVT, while the control group did not. Results The volume of bleeding and blood transfusion, the change of red blood cell count (RBC) and hemoglobin were compared between two groups before and after operation. There is no statistical significance. Conclusion During the period of the operation of rear internal fixation, low molecular weight heparin cannot increase the bleeding and is safe to prevent deep vein thrombosis.
4.X-ray and CT Diagnosis of Unilateral Dislocation of the Lower Cervical Vertebra

Objective To study X-ray and CT methods of diagnosis of unilateral dislocation of the lower cervical vertebra(C_(3~7)).Methods 15 cases of the injury of unilateral dislocation of the lower cervical vertebra were reviewed from August 2002 to June 2008,X-ray and CT findings were observed.Results Lateral projection of X-ray showed:the anterior displacement of dislocated vertebra was less than 25% in 15 cases,the distance of posterior border of lateral mass of below vertebral body to line of neural scute changed rapidly in 13 cases,sign of "bow tie" of processus articularis superior in 12 cases.Antero-posterior projection showed that spinal process was lateral deviation.Oblique projection showed that intervertebral joints were dislocation or interlocking. CT showed:the "hamburger" sign were destroyed in 7 cases,fracture of unilateral articular process in 8 cases,rotation of vertebral body of dislocation and its superior vertebral body in 13 cases, "double ring" sign in 12 cases.Conclusion The signs of X-ray and CT in combination with the mechanism of injury and clinical situation,the unilateral dislocation of the lower cervical vertebra can be diagnosed accurately.
5.Impact of combined therapy on quality of life in patients with rheumatoid arthritis

One hundred patients with rheumatoid arthritis (RA) were randomly assigned to the medication group (n=38) or combined therapy group (n=62). The quality of life was investigated through the questionnaire before and after 6 months' therapy. Data showed that there was statistically significant difference between the two groups in physical, social, and emotional functions, and self-recognized health status (19.0±9.1 vs 6.6±4.8, 10.0±6.9 vs 3.8±3.3, 6.6±3.2 vs 3.0±1.8, and 11.4±7.6 vs 5.3±3.4) ( all P<0.05 ). Combined therapy could significantly improve the quality of life in RA patients.
6.Relationship between activated STAT3 protein and epithelial-mesenchymal transition in papillary thyroid carcinoma.

OBJECTIVE:
To investigate the expression of signal transducer and activator of transcription 3 (STAT3) and phosphorylated STAT3 (p-STAT3) protein in papillary thyroid carcinoma (PTC), and to explore the correlation and significance between the expression of STAT3, p-STAT3 and epithelial-mesenchymal transition (EMT).
METHOD:
The expression of STATS3. p-STAT3, E-cadherin and Vimentin protein in 56 cases of PTC specimens and adjacent normal tissues specimens ware detected by immunohistochemistry. The correlation of the expression of STATS, p-STAT3, E-cadherin and Vimentin protein in PTC with clinicopathological characteristics was analyzed.
RESULT:
The positive rates of STAT3, p-STAT3 in PTC tissue were significantly higher than those in adjacent normal tissues specimens respectively (P < 0.01). The positive rates of E-cadherin in PTC tissues were remarkably lower, compared to adjacent normal tissues specimens (P < 0.01), however the positive rates of Vimentin in PTC tissues were remarkably higher, compared to adjacent normal tissues specimens (P < 0.01). The expression of STAT3, p-STAT3, E-cadherin and Vimentin protein were associated with lymph node metastasis and clinical stage (all P < 0.05). The expression of STAT3 and p-STAT3 was negatively correlated with E-cadherin expression (r = -0.494 and r = -0.364, P < 0.01, respectively), but positively with Vimentin expression (r = 0.533 and r = 0.377, P < 0.01, respectively) in PTC tissues.
CONCLUSION
PTC tissues have STAT3 protein activation and EMT phenotype, as were all correlated significantly with PTC invasion and metastasis. STAT3 signaling pathway activation might mediate EMT and then promote PTC invasion and metastasis.

7.Value of ultrasound monitoring of endometrial thickness in assessing pregnancy rate improvement after estrogen administration following a missed abortion

Objective:To analyze the clinical value of ultrasound monitoring of endometrial thickness in assessing pregnancy rate improvement after estrogen administration following a missed abortion.Methods:A retrospective study was conducted on 86 patients who underwent surgical abortion at Cixi Maternal and Child Health Hospital from January 2022 to June 2023. Based on the treatment received, the patients were divided into two groups: a control group and an observation group, with 43 patients in each group. The control group received only routine postoperative care without any medication, while the observation group received estrogen treatment after surgery. The clinical efficacy, endometrial thickness, intrauterine adhesion and re-pregnancy rate were compared between the two groups.Results:In the observation group, there was significant difference in endometrial thickness in terms of intergroup, group-by-time interaction, and time effect ( Fintergroup =129.49, Finteraction =14.25, Ftime =146.64, all P < 0.001). Intrauterine adhesions were less severe in the observation group compared with the control group ( χ2 = 4.34, P < 0.05). The clinical effective rate was significantly higher in the observation group than in the control group [88.37% (38/43) vs. 69.76% (30/43), Z = 2.35, P = 0.019]. Additionally, the rate of re-pregnancy was significantly higher in the observation group than in the control group [46.54% (20/43) vs. 13.95% (6/43), χ2 = 10.81, P < 0.05]. Conclusion:Patients who have retained abortion can benefit from ultrasound examination to assess endometrial thickness after estrogen administration. This approach significantly enhances endometrial thickness, promotes menstrual recovery, and reduces intrauterine adhesions. It also improves the rate of re-pregnancy and is highly valuable in clinical settings.
8.Influencing factors of delirium after cardiac valve replacement under cardiopulmonary bypass with propofol sedation

Objective To investigate the influencing factors of delirium after cardiac valve replacement went under car-diopulmonary bypass(CPB)with propofol sedation.Methods A total of 152 patients underwent cardiac valve replacement under CPB in Nanyang Central Hospital from January 2020 to December 2022 were selected as research objects,and they were randomly divided into observation group A[50 ≤bispectral index(BIS)<60]and observation group B(35≤BIS<45)according to the depth of propofol sedation,with 76 cases in each group.The clinical data such as age,gender,body mass index(BMI),diabetes,hypertension,coronary heart disease,chronic obstructive pulmonary disease,sleep disorder,nutritional disorder,anxiety,depression,smoking history,drinking history,preoperative cardiac insufficiency,intraoperative hypoxemia,intraoperative hypoproteinemia,postoperative acute renal injury,secondary intubation,massive blood transfusion,excessive pain,postoperative left ventricular ejection fraction(LVEF),surgical method and CPB time were collected,and the incidence of postoperative delirium of patients was evaluated by the confusion assessment method of intensive care unit(CAM-ICU)method.The incidence of postoperative delirium of patients between observation group A and observation group B was compared.The influencing factors of postoperative delirium occurrence was analyzed by using univariate and multivariate logis-tic regression analysis.Results Among the 152 patients underwent heart valve replacement,36 patients experienced postoperative delirium,with an incidence of 23.68%.The incidence of postoperative delirium of patients in the observation group A and the observation group B was 38.16%(29/76),9.21%(7/76),respectively;the incidence of postoperative delirium of patients in the observation group A was significantly higher than that in the observation group B(x2=17.617,P<0.05).The gender,BMI,diabetes,hypertension,coronary heart disease,cognitive disorder,sleep disorder,nutritional disorder,anxiety,depression,smoking history,drinking history,intraoperative hypoxemia,intraoperative hypoproteinemia,postoperative acute renal injury,secondary intubation,massive blood transfusion,and surgical method were not related to postoperative delirium(P>0.05);the age,chronic obstructive pulmonary disease,preoperative heart failure,excessive pain,postoperative LVEF,and CPB time were associated with postoperative delirium(P<0.05).Multivariate logistic regression analysis showed that age 60 years,preoperative cardiac dysfunction,excessive pain,and CPB time≥100 minutes were risk factors for postoperative delirium(P<0.05),while postoperative LVEF≤50%and propofol sedation depth of 35≤BIS<45 were protective factors for postoperative delirium(P<0.05).Conclusion Propofol sedation depth of 35≤BIS<45,postoperative LVEF ≥50%can effectively reduce the risk of postoperative delirium after cardiac valve replacement under CPB.Age≥60 years old,preoperative cardiac insufficiency,excessive pain,and CPB time≥100 min can increase the risk of postoperative delirium.

The dynamic changes of cognitive function has been paid more and more attention by foreign scholars.Dynamic assessment based on ecological momentary assessment(EMA)can capture subtle changes in cognitive function and provide more comprehensive information for early identification and timely intervention of people with cognitive impairment, which is an effective supplement to traditional cognitive assessment.This paper reviewed the concept of ecological momentary assessment, its advantages in cognitive assessment, its feasibility and effectiveness, and its application status in the evaluation of cognitive function in the elderly, so as to provide a reference for making ecological assessment of the cognitive function for older adults that is in line with China's national conditions.
10.Effect of bilateral greater splanchnic nerve transection on hepatic injury in dogs with acute necrotizing pancreatitis

ObjectiveTo investigate the effect of bilateral greater splanchnic nerve transection on hepatic injury in dogs with acute necrotizing pancreatitis (ANP). MethodsA total of 24 healthy adult mongrel dogs were randomly divided into sham-operation group (SO group with 8 dogs), ANP model group (ANP group with 8 dogs), and ANP+bilateral greater splanchnic nerve transection group (GSNT group with 8 dogs). Peripheral venous blood samples were collected at 2 hours before surgery and at 12 and 24 hours and 3, 5, and 7 days after surgery to measure the serum levels of amylase (AMY), alanine aminotransferase (ALT), and aspartate aminotransferase (AST). The dogs were sacrificed at 7 days after surgery, and the pancreatic and hepatic tissues were harvested to observe their pathological changes by visual inspection and under a light microscope. Western blot was used to measure the protein expression of p-NF-κB p65 in liver tissue. A one-way analysis of variance and a repeated-measures analysis of variance were used for comparison between groups, and the least significant difference t-test and wilcoxon rank sum test were used for further comparison between two groups. ResultsAt 2 hours before surgery, there were no significant differences in serum levels of AMY, ALT, and AST between the three groups (P>0.05). Compared with the SO group, the ANP group and the GSNT group had significant increases in the serum levels of AMY, ALT, and AST, pancreatic and liver pathological scores, and the protein expression of p-NF-κB p65 in liver tissue after surgery (all P<0.05). After surgery, the GSNT group had significantly lower serum levels of ALT and AST, pancreatic and liver pathological scores, and protein expression of p-NF-κB p65 in liver tissue than the ANP group (all P<0.05), while there was no significant difference in the serum level of AMY between these two groups (P>0.05). ConclusionThe activation of NF-κB plays an important role in the progression of ANP in dogs. Bilateral greater splanchnic nerve transection can alleviate liver injury in dogs with ANP, and such a protective effect may be associated with the downregulation of NF-κB activity in the liver.
